Cyclin-dependent kinase 4/6 (CDK4/6) inhibitor-induced hepatotoxicity and its resolution: a study of seven cases.

AIMS: Hepatotoxicity is an increasingly recognised and potentially dose-limiting adverse effect of CDK4/6 inhibitors, but their histopathological spectrum remains incompletely characterised. We aimed to describe the clinicopathological features of histologically characterised CDK4/6 inhibitor-induced liver injury. METHODS AND RESULTS: We retrospectively identified patients with metastatic breast cancer who developed liver enzyme elevation and underwent liver biopsy while receiving a CDK4/6 inhibitor at a single comprehensive cancer centre (2020-2024). Inclusion required a RUCAM score ≥6 and e

The impact of tranexamic acid on graft function in liver transplantation: A randomised controlled trial.

Tranexamic acid (TXA) has been reported to reduce transfusion requirements in liver transplantation (LT). However, clinical evidence regarding the impact of TXA on graft function and other outcomes in liver transplant recipients remains limited. This study aimed to evaluate the effect of TXA on early allograft function following LT. In this prospective, single-centre, randomised study, 89 adult participants scheduled for LT were randomised to receive either TXA or placebo. After anaesthesia induction, a continuous infusion of TXA (10 mg/kg/h) was maintained until portal vein unclamping in the

Clinical and pathophysiological perspectives on haemostasis, inflammation, and thrombo-haemorrhagic risk in cirrhosis.

Haemostasis in cirrhosis is a dynamic process characterised by a fragile equilibrium. Haemostatic alterations become more pronounced during acute decompensation and acute-on-chronic liver failure, conditions driven by increasing systemic inflammation. When compounded by bacterial infection or acute kidney injury, additional alterations may emerge, predisposing to bleeding and thrombosis. Recent data indicate an interplay between haemostasis and inflammation in chronic liver disease, with implications for disease progression and thrombo-haemorrhagic complications. In this review, we discuss evo

Routine CT reporting of vascular involvement in pancreatic ductal adenocarcinoma: completeness and association with margin-positive resection.

BACKGROUND: National Comprehensive Cancer Network (NCCN) guidelines emphasize detailed reporting of tumor-vessel relationships for pancreatic ductal adenocarcinoma (PDAC), but the completeness and prognostic significance of vascular findings documented in routine clinical CT reports remain uncertain in the contemporary era of neoadjuvant therapy (NAT). PURPOSE: To assess the completeness of NCCN-relevant vascular reporting in routine preoperative CT examinations and determine whether CT-reported vascular findings and derived NCCN resectability categories are associated with margin-positive (R

Data Access, User Engagement, and Interoperability Standards in Public Health Data Portals: Environmental Scan Study.

BACKGROUND: Rapid digitalization has led to the generation of vast volumes of health data across diverse sources, including public health surveillance, electronic health records from hospitals, primary care facilities, and telemedicine. However, it often remains fragmented across multiple domains and systems due to siloed data collection practices and a lack of interoperability standards, limiting its potential to generate actionable public health insights. OBJECTIVE: In this study, we examined publicly available health data portals and assessed their scope, user engagement features, and impl

Incidence and Risk Factors for Development of Portal Hypertension and Sinusoidal Liver Injury Associated with T-DM1 in HER2-Positive Breast Cancer.

PURPOSE: While an association between trastuzumab emtansine (T-DM1) and development of portal hypertension has been suggested, clinical studies on the incidence and risk factors remain limited. MATERIALS AND METHODS: HER2-positive breast cancer patients treated with T-DM1 as adjuvant or palliative therapy were retrospectively enrolled. The composite endpoint of portal hypertension and sinusoidal liver injury was defined as development of splenomegaly and at least one among gastroesophageal varices, spontaneous portosystemic shunt, or portal hypertensive ascites; or biopsy-confirmed sinusoidal

Hydrogen sulfide-mediated vasodilation requires heme oxygenase-derived carbon monoxide.

BACKGROUND: Hydrogen sulfide (H 2 S) is an important endothelial-derived vasodilator, but the signaling mechanism remains incompletely understood. We previously demonstrated that H 2 S-mediated vasodilation requires transient receptor potential vanilloid type 4 (TRPV4) channels. Because H 2 S has been reported to enhance heme oxygenase (HO) activity and HO-derived carbon monoxide (CO) regulates endothelial signaling, we hypothesized that H 2 S-mediated vasodilation requires HO-2-derived CO. METHODS: Pressure myography was performed in isolated rat mesenteric arteries to determine the contribu

Laparoscopic Right Hemicolectomy Using A Cranial-First Approach: Technical Description and Early Outcomes.

There are various surgical approaches for laparoscopic radical right hemicolectomy. However, the Henle's trunk and middle colic vessels often reveal anatomical variations, and improper handling may lead to vascular injury. This protocol presents a "cranial-first" approach combined with complete mesocolic excision (CME) to attempt to overcome the aforementioned technical difficulties. 86 cases were collected from September 2023 to April 2026. First, the gastrocolic ligament was opened to expose the right gastroepiploic vessels and the pancreatic head. The colic branches of Henle's trunk and the

The gut microbiota-inflammageing axis in cardiometabolic health of older adults and dietary intervention strategies.

Rapid global population ageing is accelerating the burden of cardiometabolic disease, and inflammageing-the chronic, low-grade, sterile inflammation that accompanies advancing age-has emerged as a shared pathological substrate linking cardiovascular disease, metabolic syndrome, and type 2 diabetes mellitus in older adults. Increasing evidence identifies the gut microbiota as an upstream driver of this process, forming a modifiable feedback loop with the intestinal barrier, the immune system, and cardiometabolic homeostasis. With advancing age, beneficial taxa such as Bifidobacterium and Faecal

Baseline low-level viremia in HBV-related hepatocellular carcinoma: prognostic significance, risk stratification, and clinical implications.

INTRODUCTION: The prognostic relevance of baseline low-level viremia (LLV) after hepatitis B virus-related hepatocellular carcinoma (HBV-HCC) diagnosis remains uncertain. We evaluated the association between baseline LLV and overall survival (OS), explored related biological features, and developed survival prediction models for patients with baseline LLV. METHODS: This retrospective study included 921 patients with HBV-HCC: 611 with baseline LLV and 310 with HBV DNA <20 IU/mL. Propensity score matching (PSM), inverse probability of treatment weighting (IPTW), and Cox regression were used to

Immune checkpoint blockade and thrombo-inflammatory remodeling in microsatellite instability-high intrahepatic cholangiocarcinoma with Trousseau syndrome: a case report.

BACKGROUND: Cancer-associated thrombosis (CAT) is a major cause of morbidity and mortality in patients with malignancy and is increasingly recognized as a manifestation of tumor-associated thrombo-inflammation. Although immune checkpoint inhibitors (ICIs) have transformed the treatment of microsatellite instability-high (MSI-H) malignancies, their impact on the interplay between antitumor immunity and cancer-associated thrombosis remains poorly understood, particularly in intrahepatic cholangiocarcinoma (ICC). CASE PRESENTATION: We report a 74-year-old woman with MSI-H intrahepatic cholangioc

Radical Antegrade Modular Pancreatosplenectomy Versus Standard Resection in T3 Body and Tail Pancreatic Ductal Adenocarcinoma: A Multicenter Retrospective Cohort Study.

BACKGROUND: The optimal operation for T3 body and tail pancreatic ductal adenocarcinoma (PDAC) remains uncertain. Radical antegrade modular pancreatosplenectomy (RAMPS) was designed to facilitate systematic posterior dissection and lymphadenectomy, but whether its survival benefit varies by anatomic or molecular risk is unclear. METHODS: This multicenter retrospective cohort study included patients with pathologically confirmed T3 body and tail PDAC who underwent upfront RAMPS or standard retrograde pancreatosplenectomy (SRPS) at three high-volume centers from January 2014 to December 2024. Sequential Flow-Based Assessment of a Suspected Inferior Pancreaticoduodenal Artery-Derived Pathway Entering the Right Hepatic Artery during Pancreaticoduodenectomy: A Case Report.

INTRODUCTION: During pancreaticoduodenectomy (PD), hepatic arterial ischemia is usually considered in patients with celiac artery stenosis or occlusion, in whom the gastroduodenal artery (GDA)-based pancreaticoduodenal arcade may serve as a critical collateral pathway. However, a similar but atypical pitfall may exist when the GDA and anterior superior pancreaticoduodenal artery (ASPDA) are absent: a prominent inferior pancreaticoduodenal artery (IPDA)-derived pathway may raise concern regarding hepatic inflow despite the anatomical presence of the common hepatic artery (CHA). Use of the superior mesenteric artery as an arterial interposition graft in horseshoe kidney transplantation: a case report.

Vascular reconstruction in kidney transplantation remains a technical challenge, especially in cases involving horseshoe kidneys, which often have anomalous vascular and ureteral anatomy. Proper selection of reconstruction technique and vessel graft source is crucial for long-term organ graft survival. This case report describes the successful use of a donor superior mesenteric artery segment as an interposition graft between a compromised donor renal artery and recipient external iliac artery in a horseshoe kidney transplant. To the best of our knowledge, this is the first reported case where

Stenting of the Inferior Mesenteric Artery as a Single Vessel Treatment for Chronic Mesenteric Ischemia.

PURPOSE: Angioplasty and stenting of the mesenteric arteries have increasingly become the first therapeutic option for CMI. The superior mesenteric artery (SMA) and the celiac artery (CA) have been the usual targets for revascularization. However, published data on stenting of only the inferior mesenteric artery (IMA) for this disease are limited. This study evaluated the results in 10 patients treated at our institution. STUDY DESIGN: A retrospective review of 10 patients who underwent angioplasty and stenting of only the IMA for symptomatic CMI from 2005 to 2025. Patient demographics, angio

A Two-Timescale Reduced-Order Model Coupling Acute Arterial Compliance with Stress- and Shear-Regulated Growth and Remodeling.

Arterial growth and remodeling (G\&R) models should distinguish immediate load-induced deformation from slower structural adaptation. A computationally light two-timescale model was therefore developed in which the reference-pressure structural geometry, described by inner radius $R_g$ and wall thickness $H_g$, is separated from the instantaneously loaded geometry $R_i$ and $h$. At every slow G\&R state, a nonlinear scalar equilibrium is solved for pressure-dependent dilation while annular wall area is conserved to represent acute wall incompressibility. The resulting loaded geometry determine

Failure Modes of Endovascular Therapy for Spontaneous Isolated Superior Mesenteric Artery Dissection.

BACKGROUND: Mechanisms of failure after endovascular stenting for symptomatic spontaneous isolated superior mesenteric artery dissection (SISMAD) remain poorly characterized. We analyzed device- and procedure-related failure modes in a 17-year single-center cohort. METHODS: We reviewed 117 consecutive patients with SISMAD (2008-2025). Patients stented for persistent or escalating pain despite conservative management formed the cohort; catheter-infusion and open-surgery patients were analyzed separately. Lesions were classified by Yun morphology. Failure modes were classified a priori as Type

Robot-assisted surgery for rectal cancer with persistent descending mesocolon: analysis of vascular anatomical features and short-term outcomes.

PURPOSE: Persistent descending mesocolon (PDM) is a rare congenital anomaly characterized by a medial shift of the descending colon and vascular variations that can complicate colorectal surgery. This study aimed to clarify the PDM-associated vascular anatomy and describe the short-term outcomes of robot-assisted surgery. METHODS: We retrospectively analyzed 541 patients with primary rectal cancer who underwent robot-assisted surgery (2017-2024) and compared the short-term outcomes between the PDM and non-PDM groups. Maresin 2 Attenuates Hypertensive Vascular Remodeling via Inhibiting Macrophage Infiltration and Pro-fibrotic Polarization.

Immune-inflammatory responses are pivotal regulators of hypertensive vascular remodeling. Specialized pro-resolving mediators (SPMs), including Maresin 2 (MaR2), are endogenous anti-inflammatory molecules. However, the specific role of MaR2 in regulating hypertension and vascular remodeling, as well as its underlying mechanisms, remains unclear. In this study, a murine model of hypertension was induced by continuous infusion of Angiotensin II (AngII) at 1 μg/kg/min. MaR2 intervention (2 μg/kg/day) was administered either prophylactically (before model induction) or therapeutically (after model
