Streptococcus gallolyticus bacteremia is associated with colorectal malignancy and infective endocarditis, whereas infected superior mesenteric artery aneurysm (SMAA) is rare. A 94-year-old woman with cecal cancer developed S. gallolyticus sepsis that improved with intravenous antibiotics. Two months later, contrast-enhanced computed tomography revealed a newly developed 17-mm distal SMAA, suggesting an infected aneurysm secondary to the prior bacteremia. Because of the high risk of rupture, urgent aneurysmectomy and ileocecal resection were performed. This rare case highlights an infected SMA

Atypical spindle cell lipomatous tumor (ASCLT) is a rare benign adipocytic neoplasm, and primary involvement of the retroperitoneum is exceedingly uncommon. Because its imaging features overlap with those of other fat-containing tumors, particularly atypical lipomatous tumor/well-differentiated liposarcoma (ALT/WDL), preoperative diagnosis remains challenging. We report a case of primary retroperitoneal ASCLT in a 58-year-old woman whose lesion was incidentally detected during a routine physical examination. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a small, well-d

INTRODUCTION: Cytomegalovirus (CMV) enteritis is a potentially life-threatening gastrointestinal infection typically observed in immunocompromised individuals, such as transplant recipients or patients receiving chemotherapy or long-term corticosteroids. However, CMV enteritis can also develop in the absence of overt immunosuppression, particularly in elderly or critically ill patients under physiological stress, nutritional decline, or prolonged hospitalization. Diagnosis in these settings is challenging because clinical symptoms and imaging findings are nonspecific and may overlap with ische

BACKGROUND: Acute arterial occlusive mesenteric ischemia is associated with high mortality. Embolic and atherosclerosis-associated forms differ in vascular pathology, collateral circulation, and ischemic tolerance, which may contribute to differences in clinical presentation and outcomes. This study compared clinical presentation, treatment strategies, and outcomes between embolic acute mesenteric ischemia (EAMI) and atherosclerosis-associated acute mesenteric ischemia (AAMI).
METHODS: This retrospective single-center study included all consecutive patients diagnosed with acute arterial occlu

BACKGROUND: Complete mesocolic excision (CME) with central lymphadenectomy represents the oncologic standard for right colon cancer surgery. However, vascular anatomical variability particularly involving the superior mesenteric vessels and Henle's gastrocolic trunk may substantially increase operative complexity and hemorrhagic risk. This study aimed to evaluate mesenteric vascular variations systematically and to analyze their implications for the quality and safety of CME/D2 right hemicolectomy.
MATERIALS AND METHODS: A retrospective descriptive single-center study was conducted at a terti

BACKGROUND: Nutcracker syndrome is a rare vascular compression disorder caused by entrapment of the left renal vein, most commonly between the aorta and the superior mesenteric artery. Surgical treatment is reserved for patients with persistent and debilitating symptoms; however, evidence regarding minimally invasive approaches remains limited. This study aimed to evaluate the outcomes of laparoscopic left renal vein transposition in patients with symptomatic nutcracker syndrome.
METHODS: A retrospective analysis was conducted of nine consecutive patients who underwent laparoscopic left renal

BACKGROUND: Portal annular pancreas (PAP) is a rare congenital anatomical variation characterized by pancreatic parenchyma encircling the portal vein (PV) or superior mesenteric vein (SMV). An aberrant common hepatic artery (CHA) originating from the superior mesenteric artery (SMA) represents another uncommon vascular anomaly. The coexistence of these two variations with a distal common bile duct (CBD) malignancy is extremely rare and poses significant technical challenges for pancreaticoduodenectomy (PD).

INTRODUCTION: Acute mesenteric ischemia (AMI) is a surgical emergency caused by inadequate blood flow through the mesenteric vessels, resulting in intestinal ischemia and eventual gangrene of the bowel wall. The condition may result from arterial thrombosis, embolism, mesenteric venous thrombosis, or non-occlusive systemic hypoperfusion.
CASE PRESENTATION: We report a rare case of AMI in a 73-year-old female with a significant past surgical history of transabdominal hysterectomy with bilateral salpingo-oopherectomy. She presented with sudden, severe abdominal pain that was disproportionate to

INTRODUCTION AND IMPORTANCE: Superior mesenteric artery (SMA) syndrome is a rare condition (incidence of 0.013%-0.78%) that primarily affects young adults, resulting from duodenal compression between the SMA and the aorta following rapid weight loss and retroperitoneal fat pad depletion. Conservative management succeeds in 70-80% of cases; surgical intervention is required for refractory symptoms.
CASE PRESENTATION: A 21-year-old woman presented with severe abdominal pain, nausea, and bilious vomiting following a 36% weight reduction over 5 months (66-42 kg; body mass index [BMI] 17.48 kg/m2)

MR angiography (MRA) provides a versatile, radiation-free approach for evaluation of the mesenteric vasculature offering contrast-enhanced and noncontrast techniques tailored to clinical needs. Advances in hardware, pulse sequences, acceleration methods, and contrast agents have improved spatial and temporal resolution and diagnostic confidence. Mesenteric MRA can be used for vascular mapping, abdominal organ transplant evaluation and assessment of dynamic vascular compression syndromes, chronic mesenteric ischemia, vasculitis, vasculopathy, and mesenteric aneurysms. This article provides a co

The right hepatic artery (RHA) provides blood flow to the right lobe of the liver. In typical anatomy, the RHA branches from the proper hepatic artery, which branches from the common hepatic artery originating from the celiac trunk of the abdominal aorta. During a standard cadaveric dissection, a large accessory right hepatic artery (aRHA) was noted arising directly from the superior mesenteric artery. The lumen of the identified aRHA measured approximately 1 cm in diameter. The artery also appeared darker in color than the surrounding vasculature, with no evidence of prior surgery or procedur

INTRODUCTION AND IMPORTANCE: Superior mesenteric artery (SMA) syndrome is a rare cause of duodenal obstruction. Its diagnosis is often delayed due to nonspecific symptoms that mimic common functional gastrointestinal disorders. This case highlights the diagnostic challenge and the critical role of imaging, further complicated by an incidental, anatomically related finding.
PRESENTATION OF CASE: A 13-year-old male presented with a 2-month history of post-prandial vomiting, epigastric pain relieved by a prone position, and unintentional weight loss of 9.3% of total body weight, having previousl

Preeclampsia (PE) is characterized by placental ischemia-induced systemic inflammation, oxidative stress, and maternal vascular dysfunction. We previously reported that upregulated gastrin-releasing peptide (GRP) and its receptor (GRPR) within the hypothalamic paraventricular nucleus (PVN) of PE rats contribute to the progression of PE. However, whether such central upregulation contributes to the peripheral organs damage in PE remains unknown. This study investigated the protective effects of the GRPR antagonist ICI-216140, administered via PVN microinjection, on mesenteric arteries and place

Iatrogenic injury to the superior mesenteric artery (SMA) represents a rare but serious complication of nephrectomy, associated with significant morbidity and mortality. Repair can be technically challenging. We describe the case of a 71-year-old patient who had an SMA injury during an open nephrectomy for a large carcinoma. Her SMA was reconstructed successfully using a deceased-donor iliac artery interposition graft. Due to her high-risk pathology and progressive disease early in the post-operative period, she was commenced on lenvatinib and pembrolizumab. Despite the vascular allograft, imm

BACKGROUND: Postoperative major adverse events (MAEs) can occur after surgical repair of acute Stanford type A aortic dissection (ATAAD), and conventional clinical assessment may not fully capture the prognostic information provided by preoperative multimodal imaging. This study aimed to develop and externally validate a staged risk prediction model for postoperative MAEs and to evaluate the incremental value of preoperative multimodal imaging relative to conventional clinical and laboratory assessment in the prediction of MAEs.
METHODS: In this retrospective, two-center cohort study, patient

Median arcuate ligament syndrome (MALS) is atypical in elderly patients, in whom it frequently coexists with severe atherosclerotic disease. We report the case of a 77-year-old male with marked mitral regurgitation who presented with intestinal claudication and malnutrition. CT angiography demonstrated MALS associated with a fusiform aneurysm of the celiac trunk with dissection, along with 60% atherosclerotic stenosis of the superior mesenteric artery. Given the limiting ischemic presentation, the patient underwent laparoscopic release of the ligament. The procedure was complicated by avulsion

INTRODUCTION: The association of duodenal atresia with apple-peel (Christmas tree) configuration of the distal small bowel is exceptionally rare, with only a few cases reported in the literature. This unusual combination is of particular interest because it may provide evidence supporting a vascular etiology for selected cases of duodenal atresia. We report a neonate with type III duodenal atresia associated with apple-peel bowel and absent proximal superior mesenteric artery anatomy.
CASE PRESENTATION: A late-preterm (35 + 5 weeks) female neonate with antenatal suspicion of jejunal atresia p

Exogenous glucagon-like peptide 2 (GLP-2) administration increases small intestinal growth and mesenteric blood flow, although the vascular mechanisms involved remain unclear. The objective of this study was to characterize the effects of exogenous GLP-2 administration on glucagon-like peptide 2 (GLP-2)-, vasoactive intestinal peptide (VIP)-, serotonin (5-HT)-, and acetylcholine (ACh)-mediated vascular relaxation in isolated bovine mesenteric arteries (MA) and mesenteric veins (MV). Ten steers received repeated subcutaneous injections of bovine serum albumin or GLP-2 for 48 h before vessel col
