Brachial artery access for endovascular procedures: how safe is it?

PURPOSE: While femoral and radial artery access are routine for endovascular procedures, brachial artery access remains an important alternative in selected cases. This study aimed to evaluate the complication rate of percutaneous brachial artery access at a tertiary centre and identify potential risk factors for access-site complications. MATERIALS AND METHODS: A retrospective review was performed of all procedures utilising percutaneous brachial artery access over a 12.5-year period. Patient demographics, sheath size, closure technique and complications were recorded. Complications were cla

Covered Stent Assisted Coil Embolisation of a Rare Splenomesenteric Artery Aneurysm - A Case Report and Literature Review.

BackgroundSplenomesenteric artery aneurysms are a rare clinical entity occurring in <1% of the population, where the splenic artery originates from the superior mesenteric artery (SMA). These aneurysms carry a high risk of rupture due to exposure to high-flow hemodynamic stress near the SMA origin.Case PresentationA 76-year-old female was incidentally diagnosed with a 1.6 cm saccular splenomesenteric aneurysm during investigation for sepsis. The patient underwent successful endovascular exclusion using a combination of a microvascular plug and coil embolisation to address the aneurysm 'front'

Adult intestinal malrotation: A rare incidental finding in trauma imaging.

Intestinal malrotation is a congenital anomaly resulting from abnormal rotation and fixation of the midgut during embryonic development. It is typically diagnosed in infancy, and its detection in adults-particularly in asymptomatic individuals-is rare. In adults, clinical presentation ranges from chronic, nonspecific symptoms to acute intestinal obstruction and midgut volvulus. A 36-year-old woman presented to the emergency department following a motor vehicle collision. She reported no gastrointestinal complaints and denied any history of recurrent abdominal pain, bilious vomiting, abdominal

Investigating the availability of new anatomical measurements in the diagnosis of anterior nutcracker syndrome.

BACKGROUND: Nutcracker Syndrome (NCS) results from compression of the left renal vein (LRV), most commonly between the superior mesenteric artery (SMA) and the abdominal aorta. Diagnosing NCS in children remains challenging due to nonspecific symptoms, anatomical variability, and lack of standardized criteria. OBJECTIVE: To observe the usability of new and more practical measurements in diagnosing anterior NCS in pediatric patients. METHODS: This retrospective study included 23 children with NCS and 45 controls who underwent abdominal CT for unrelated reasons. Eight anatomical parameters wer

Early and follow-up results of reinterventions for type 2 endoleak following endovascular repair of abdominal aortic aneurysm (EVAR): the multicenter REinterventions for Type 2 Endoleak (RETE) study.

BACKGROUND: Type II endoleak (T2EL) is frequent after endovascular abdominal aortic aneurysm repair (EVAR) and may require reintervention in selected patients. However, the optimal treatment strategy for durable resolution remains uncertain. The aim of this study is to retrospectively analyze perioperative and long-term results of T2Els' reinterventions, both endovascular and open surgical, in a large patient cohort from twenty high-volume academic centers. METHODS: A retrospective analysis of consecutive patients undergoing first reintervention for T2ELs was conducted across 20 centers over

Spontaneous Mesenteric Hematoma Following Severe Coughing in a Patient on Dual Antiplatelet Therapy: A Case Report.

Spontaneous mesenteric hematoma (SMH) is a rare cause of acute abdominal pain and may mimic more common intra-abdominal conditions, creating diagnostic challenges. A 67-year-old man with hypertension, hyperlipidemia, coronary artery disease status post four-vessel coronary artery bypass grafting, and chronic dual antiplatelet therapy with aspirin and clopidogrel presented with acute worsening lower abdominal pain. He reported a severe coughing episode during a recent bronchitis-like illness approximately two weeks before presentation, followed by lower abdominal pain in the same general region

Pancreaticoduodenectomy after Laparoscopic Sleeve Gastrectomy with Preservation of Gastric Arterial Inflow: A Case Report.

INTRODUCTION: Obesity is an established risk factor for pancreatic ductal adenocarcinoma. As laparoscopic sleeve gastrectomy (LSG) has become increasingly prevalent, pancreatic surgeons may encounter malignancies in patients with a surgically altered gastric anatomy. Pancreaticoduodenectomy (PD) after an LSG is rare and requires meticulous preoperative planning to prevent ischemic and anastomotic complications. CASE PRESENTATION: A 47-year-old man with morbid obesity (BMI 59.1 kg/m2) underwent LSG. Twenty-three days later, obstructive jaundice led to the diagnosis of pancreatic head adenocarc

Sjögren's Syndrome-Associated Medium- and Large-Vessel Vasculitis Presenting With Acute Abdominal Symptoms.

We report a case of Sjögren's syndrome-associated vasculitis involving the medium and large arteries in a patient who presented with acute abdominal pain, vomiting, and fever. Computed tomography angiography demonstrated vasculitic involvement of the superior mesenteric, inferior mesenteric, renal, and left external iliac arteries. Immunological evaluation revealed strongly positive antinuclear antibodies (ANA) and an extractable nuclear antigen (ENA) profile positive for anti-SSA, anti-SSB, and Ro-52 antibodies, while anti-double-stranded DNA (anti-dsDNA) antibodies were negative. The patient

3-[[4-(Benzylamino)quinazolin-2-yl] Amino] Benzenesulfonamide, a Quinazoline-Based PDE5 Inhibitor, Induces Vasorelaxation in Rat Mesenteric Arteries via Multiple Mechanisms.

We synthesised a series of N2,N4-diaminoquinazoline derivatives, among which 3-[[4-(benzylamino)quinazolin-2-yl]amino]benzenesulfonamide (compound 5) exhibited potent phosphodiesterase type 5 (PDE5) inhibitory activity. Increasing evidence suggests that PDE5 inhibition is a promising therapeutic strategy for peripheral vascular disease. This study aimed to investigate vasorelaxant effects of 5 and its underlying mechanisms in rat mesenteric arteries. Compound 5 was synthesised and purified, and evaluated its vascular activity using organ bath assay. Molecular docking was performed to identify

Clinical Application of Three-dimensional Laparoscopic Artery-first Pancreaticoduodenectomy in Treating Pancreatic Head Malignant Tumors.

Pancreaticoduodenectomy is a technically demanding procedure associated with substantial surgical risk. Conventional laparoscopic approaches have several limitations, including restricted visualization, which can make exposure of the superior mesenteric artery and identification of vessels and lymph nodes challenging. In patients with vascular adhesion or tumor invasion, surgical dissection is particularly difficult and may increase the risk of intraoperative vascular injury and hemorrhage. In addition, poor delineation of anatomical planes may result in incomplete tumor dissection, inadequate

High-Origin Visceral and Renal Arteries with a Symptomatic Right Renal Artery Aneurysm: A Case Report and Literature Review.

Background: Renal artery aneurysms (RAAs) are uncommon visceral arterial lesions whose management depends on symptoms, patient-specific rupture risk, and arterial anatomy. Markedly high origin renovisceral arteries may further complicate endovascular access and reconstruction. This report describes the management of a symptomatic RAA in a young woman with multifocal aneurysmal disease and unusual renovisceral arterial origins at the 11th and 12th thoracic vertebral levels (T11-T12). Case presentation: A 35-year-old woman presented with intermittent right loin pain. Computed tomography angiogra

Gastric Ischemia Secondary to Atherosclerotic Disease: A Case Report and Literature Review.

Gastric ischemia is an uncommon entity, as the stomach has an extensive collateral arterial blood supply; clinically significant injury usually requires multivessel mesenteric compromise. When it occurs, it closely mimics peptic ulcer disease and gastric malignancy, and diagnosis is often delayed. A 77-year-old female patient with poorly controlled type 2 diabetes mellitus and dyslipidemia (non-compliant with metformin and atorvastatin) presented with a one-month history of severe postprandial epigastric pain, progressive food avoidance, and 10 kg of unintentional weight loss. She was admitted

Catching Two Birds with One Stone: A Primary Open Revascularisation First Approach in a Case Report of Chronic Mesenteric Ischaemia.

INTRODUCTION: Symptomatic chronic mesenteric artery occlusive disease requires timely revascularisation. Although endovascular management is guideline endorsed as first line treatment, inferior mesenteric artery (IMA) revascularisation lacks evidence based guidance. This report presents a case in which an open repair first approach was selected with the IMA as the primary revascularisation target, aiming to provide long term benefit while enabling concomitant superior mesenteric artery (SMA) revascularisation. REPORT: A 60 year old woman, who was a smoker (40 pack/year), had a past medical hi

Celiac Disease: Waiting to Explode in Asia.

Celiac disease (CeD) is an acquired immune enteropathy for which the presence of two factors, i.e., predisposing HLA genes (HLA DQ2/DQ8) and gluten in diet are essential for induction of the pathogenic immune response. Acute symptoms/gut injury specifically depend on the amount of gluten ingested. The incidence and prevalence of medically diagnosed CeD is expected to rise in the coming decades, and most of these people are expected to be from Asia. Through publicly available datasets, we mapped the estimated proportion of population harboring HLA DQ2/DQ8 in different regions of Asia and the pe

Complex hybrid repair of multifocal visceral and aortoiliac pseudoaneurysms associated with neurosarcoidosis.

We report the case of a 36-year-old man with neurosarcoidosis who developed multiple pseudoaneurysms of the superior mesenteric artery (SMA), infrarenal aorta, and bilateral iliac arteries. Fluorodeoxyglucose-positron emission tomography demonstrated no significant uptake within the aneurysmal segments. Given the complex vascular anatomy, a staged hybrid approach was undertaken. Initial treatment consisted of a hepatic artery-to-distal SMA bypass followed by plug embolization of the SMA aneurysms. Subsequently, aortoiliac reconstruction was performed with a left aortouni-iliac endograft, femor

Rare origin of the right inferior phrenic artery from the anterior inferior pancreaticoduodenal artery in a patient with hepatocellular carcinoma.

The inferior phrenic arteries show considerable variation in their origins. We describe a rare origin of the right inferior phrenic artery (RIPA) from the anterior inferior pancreaticoduodenal artery (AIPDA) in a patient with ruptured hepatocellular carcinoma (HCC), identified by contrast-enhanced computed tomography (CT) and angiography, and discuss its anatomical and clinical significance. A woman in her 80s underwent emergency transcatheter arterial embolization for ruptured HCC. Preprocedural contrast-enhanced CT did not identify the origin of the RIPA at its usual sites but suggested that

Small Bowel Volvulus Diagnosed and Treated Using Intraoperative Angiography in a Hybrid Operating Room: A Case Report.

INTRODUCTION: Small bowel volvulus is a surgical emergency that may cause mesenteric ischemia; however, diagnosis may be difficult when bowel perfusion appears preserved. CASE PRESENTATION: An 84-year-old man presented with recurrent postprandial abdominal pain and melena 6 months after a total pharyngolaryngectomy with a jejunal free flap reconstruction. Extensive endoscopic evaluations, including capsule endoscopy, revealed no bleeding source. Contrast-enhanced CT revealed the whirl sign at the root of the small-bowel mesentery with the narrowing of the superior mesenteric artery (SMA), whi
