Histological, immunohistochemical, and ultrastructural evaluation of mesenchymal stem cell-derived microvesicles versus granulocyte colony-stimulating factor in jejunal ischemia-reperfusion injury in albino rats.

Intestinal ischemia-reperfusion injury (IIRI) is a life-threatening vascular emergency with high mortality, commonly occurring as a complication of hemorrhagic shock or acute mesenteric arterial occlusion. When revascularization is performed after this issue, injury will occur. The influence of MSC-MVs and G-CSF on IIRI was detected in this study. Fifty-six male adult albino rats were allocated into five groups: control, ischemia-reperfusion, MSC-MVs, G-CSF, and recovery. Ischemic injury was induced for one hour using micro-vascular clamp across the superior mesenteric artery at its beginning

A multinational pilot survey of clinical practice patterns in tumor-specific mesocolic excision and complete lymph node dissection for colorectal cancer.

BACKGROUND: Uncertainties persist regarding the allocation of apical lymph nodes in colorectal cancer, the approaches to lymph node dissection and mesocolic excision, which may contribute to inconsistent surgical practices. The aim of this study is to assess surgeons' practices in lymph node dissection and mesocolic excision approaches and to identify areas lacking standardization. METHODS: A multinational pilot survey of 22 colorectal surgeons from 6 countries was conducted during the FICARE colorectal meeting. The survey consisted of 21 Likert-scale questions on surgical practices and lymph

Altered vasomotor function and endothelial impairment in a neuroendocrine tumor mouse model.

Neuroendocrine tumors (NETs) secrete vasoactive hormones that promote hemodynamic instability. This study investigated whether NETs alter vascular responsiveness using a murine model. J:Nu mice received intrasplenic injections of BON-1 NET cells (BON-1, n = 21) or PBS (VEH, n = 21) and monitored for 10 weeks. Liver metastases were identified by histological analysis, chromogranin A expression, and presence of ALDH1-positive cancer stem cells. Vasomotor function of isolated mesenteric arteries was assessed to acetylcholine, serotonin, vasopressin, and endothelin-1, and with nitric oxide synthas

A nomogram for predicting the disease-free survival of patients with pancreatic ductal adenocarcinoma.

BACKGROUND: Accurate prediction of disease-free survival after radical resection in patients with pancreatic ductal adenocarcinoma (PDAC) remains challenging. Therefore, this study establishes and validates a predictive model for disease-free survival (DFS) after radical resection in patients diagnosed with PDAC. METHODS: This study gathered clinical and preoperative imaging characteristics of 258 patients with PDAC who underwent radical resection at our institution. The patients were randomly allocated into a training set (n = 218) and a validation set (n = 40) at an 8:2 ratio. Survival outco

Simultaneous Myocardial and Mesenteric Ischemia Mimicking Each Other in an Older Patient.

BACKGROUND: The rare co-occurrence of acute mesenteric ischemia (AMI) and myocardial infarction (MI) poses significant diagnostic and therapeutic challenges. CASE SUMMARY: An 87-year-old woman with epigastric pain was initially diagnosed with an acute inferior ST-segment elevation MI and successfully treated with percutaneous coronary intervention. Persistent symptoms led to further investigation, revealing concurrent AMI. Her condition worsened with superior mesenteric artery occlusion and gastrointestinal bleeding due to excessive anticoagulation. A multidisciplinary conservative approach u

Gastroprotective Action of Adiponectin Against Gastric Mucosal Injury Induced by Ischemia and Reperfusion-Involvement of Nitric Oxide, Sensory Afferent Nerves, and Anti-Inflammatory Mediators.

Adiponectin is adipokine exhibiting beneficial metabolic action through lipid and carbohydrate metabolism stimulation, as well as anti-inflammatory action. We have determined the role of adiponectin in gastroprotection against the formation of acute gastric lesions induced by ischemia-reperfusion (I/R). Gastric lesions evoked by I/R are a serious clinical entity; however, the participation of reactive oxygen species (ROS) and lipid peroxidation products and the involvement of nitric oxide (NO), neuropeptides released from sensory afferent nerves, and the hormone gastrin in the potential gastro

Examination of Inferior Mesenteric Artery Node Metastasis in Left-Sided Colorectal Cancer: A Retrospective Cohort Analysis.

PURPOSE: Inferior mesenteric artery node (IMAN) metastasis is rare in left-sided colorectal cancer. Tumor depth is a risk factor for IMAN metastasis, but other risk factors are unclear. This study investigated the frequency of IMAN metastasis and examined clinicopathological risk factors associated with this metastasis in left-sided colorectal cancer. We also aimed to evaluate the clinical benefit of routine D3 lymphadenectomy in surgically treated advanced colorectal cancer. METHODS: We retrospectively studied patients with colorectal cancer who underwent D3 lymphadenectomy and R0 resection

Concurrent variations of the left renal and gonadal vessels: a case report.

A thorough knowledge of vascular variations in the vicinity of the kidney is essential due to the increased number of kidney transplants, more so on the left side because the donor's left kidney is preferred for transplantation. We found multiple vascular variations in the vicinity of the left kidney, which were noted during routine dissection classes. The left renal artery arose from the abdominal aorta and supplied the left kidney in a usual manner. There was an additional left renal artery, which took its origin from the abdominal aorta, below the origin of the inferior mesenteric artery. T

Endovascular Management of the Sutton-Kadir Syndrome: A Case Report.

Sutton-Kadir Syndrome (SKS) involves rare inferior pancreaticoduodenal artery (IPDA) aneurysms with celiac artery (CA) stenosis. These aneurysms carry a greater risk of rupture and therefore require treatment. Endovascular methods offer an alternative to open surgery. We present a 54-year-old woman with CA occlusion and a 35 mm IPDA aneurysm. Due to patient preference, a staged endovascular approach was used: first, superior mesenteric artery (SMA) angiography and CA stenting, then IPDA aneurysm coil embolization. Follow-up imaging showed successful aneurysm occlusion. Endovascular treatment i

Postoperative superior mesenteric artery stenting following pancreaticoduodenectomy in a patient with severe mesenteric atherosclerosis.

Pancreaticoduodenectomy (PD) in patients with severe mesenteric atherosclerosis carries a risk of hepatic and intestinal ischemia after ligation of the gastroduodenal artery (GDA). We report the case of a 69-year-old male with pancreatic head adenocarcinoma who underwent PD. Intraoperatively, hepatic artery flow was only pressure-dependent after GDA division. Postoperatively, he developed abdominal pain and emesis. Computerized tomography angiogram showed near-occlusive celiac and thrombosed superior mesenteric artery (SMA) with inferior mesenteric artery (IMA) collateralization. SMA stenting

Mesenteric arterial vasculature: Normal anatomy, variants, and its clinical implications.

Proper delineation of the mesenteric arterial vascular anatomy is crucial for pre-procedure planning. The abdominal aorta gives rise to three major mesenteric arteries: the celiac artery, the superior mesenteric artery, and inferior mesenteric artery. In this review, we highlight the normal mesenteric arterial supply, along with commonly encountered anatomical variants, which have important implications for both surgical and endovascular interventional procedures.

Cardiac arrest as the first presentation of Takayasu arteritis in a young adult: a case report.

BACKGROUND: Sudden cardiac arrest (SCA) is rare in young individuals, being typically caused by inherited cardiomyopathies or electrical disorders. However, less common aetiologies should not be overlooked. CASE SUMMARY: A male in his twenties suffered an aborted SCA. Initial investigations revealed significant triple-vessel coronary artery disease leading to urgent surgical revascularization. Macroscopically, diffuse coronary luminal narrowing due to marked wall thickening was observed. Histopathology of the right internal mammary artery showed a non-necrotizing vasculitis pattern, which, to

A redox sensor in protein kinase A regulatory subunit Iα regulates vasodilation and protects against hypertension.

The regulatory RIα subunit of type I protein kinase A (PKARIα) can be oxidized to an interprotein disulfide homodimer, linking cellular oxidant signals with downstream phosphorylation-mediated physiological regulation. Although PKA and oxidants each have well-established roles in vasodilation and blood pressure lowering, whether the oxidation of RIα to the disulfide state regulates these physiological processes is unknown. Here, we demonstrate that PKARIα contains a thiol redox sensor, the oxidation state of which is dynamic and modulates systemic arterial blood pressure by regulating oxidant-

Ultrasound-Guided Transmesenteric Retrograde Recanalization of Ostial Superior Mesenteric Artery Occlusion.

BACKGROUND: Chronic mesenteric ischemia caused by superior mesenteric artery (SMA) occlusion is treated with endovascular or open revascularization. However, complete ostial SMA occlusion without a stump or collaterals poses significant technical challenges, particularly in high-risk patients. CASE SUMMARY: A 64-year-old woman with multiple autoimmune disorders presented with postprandial abdominal pain and occult gastrointestinal bleeding. Computed tomography angiography demonstrated complete ostial SMA occlusion without a residual stump and absence of the inferior mesenteric artery. Antegra

Impact of Hypobaria and Hypoxia Exposure on Mortality, Inflammation, and Coagulopathy in an Animal Model of Polytrauma.

INTRODUCTION: Following traumatic injury, both the inflammatory and hemostatic systems are activated. Patients experience a systemic increase in circulating cytokines and coagulation leading to a greater risk of organ dysfunction. Post-trauma aeromedical evacuation exposes patients to hypobaria and potential hypoxia. We sought to assess the impact of hypobaria and hypoxic (H/H) on mortality, coagulation, and inflammation in a mouse model of polytrauma. MATERIALS AND METHODS: Eight- to 12-week-old male C57BL/6J mice were subjected to sham or polytrauma operation with the latter consisting of b
