[Large vessel vasculitis without aortitis in giant cell arteritis: About 4 cases].

INTRODUCTION: Giant cell arteritis (GCA) is the most common vasculitis after the age of 50. Large vessel vasculitis (LVV) is frequently observed in 40 to 70% of cases, primarily affecting the aorta. Extracranial LVV is scarcely described. OBSERVATION: We present four cases of LVV-GCA without aortitis. LVV was found in 3 cases on PET-CT showing limb involvement, for the last case, CT scan revealed involvement of mesenteric artery. The diagnosis of GCA was confirmed in three of the four cases by temporal artery biopsy. In the last case, the diagnosis was made after PET-CT analysis and ophthalmo

Urolithin A alleviates vascular remodeling through mitochondrial SIRT3-mediated SOD2 deacetylation and antioxidation in hypertensive rats.

OBJECTIVES: Urolithin A (UA) is a natural polyphenolic compound produced by gut bacteria. Vascular remodeling contributes to hypertension, and vascular smooth muscle cells (VSMCs) proliferation and migration are important processes in vascular remodeling. METHODS: VSMCs were obtained from the thoracic aorta of Wistar-Kyoto rats (WKY) and spontaneously hypertensive rats (SHR). Intraperitoneal injections of UA (50 mg/kg, every 2 days for 4 weeks) were performed in SHR. RESULTS: UA attenuated proliferation and migration, reduced mitochondrial reactive oxygen species (mitoROS) levels, and increa

Carbon Dioxide Angiography in Endovascular Aneurysm Repair May Overestimate the Presence of Type II Endoleak.

BACKGROUND: To evaluate the utility of carbon dioxide (CO2) angiography in the detection of type II endoleak (T2EL) during endovascular treatment of infrarenal abdominal aortic aneurysms (AAAs). METHODS: A retrospective analysis was conducted on a prospectively enrolled 20 consecutive patients with infrarenal AAAs treated with endovascular aneurysm repair (EVAR) and intraoperative completion imaging using both contrast media and CO2. The primary end points were the early intraoperative detection of T2EL and 30-day reintervention and mortality. The secondary end points were the confirmatory di

Mesenteric Steal Syndrome Caused by Abdominal Aortic Stenosis due to Takayasu Arteritis.

INTRODUCTION: Chronic mesenteric ischemia (CMI) is usually caused by atherosclerotic stenosis of multiple mesenteric arteries. However, aortic stenosis secondary to Takayasu arteritis can rarely cause intestinal ischemia through hemodynamic steal from the mesenteric to lower limb circulation. CASE PRESENTATION: A 67-year-old woman with a history of Takayasu arteritis presented with postprandial and exertional abdominal pain and a 3-kg weight loss over 2 weeks. CTA revealed isolated severe stenosis of the infrarenal aorta with development of the arc of Riolan, and Doppler ultrasonography demon

[Multidisciplinary expert consensus on acute mesenteric arterial ischemia (2026 edition)].

Acute mesenteric arterial ischemia refers to the sudden obstruction of blood flow in the main trunk or branches of the mesenteric arteries, leading to intestinal ischemia and potentially necrosis. This is a critical abdominal emergency with high mortality, poor prognosis, and a significant risk of misdiagnosis. This consensus focuses on acute mesenteric arterial ischemia (excluding isolated superior mesenteric artery dissection and aortic dissection-related ischemia) and integrates the latest guidelines and evidence from domestic and international sources, including the European Society for Va

Metabolomic Signatures of Spontaneous Isolated Superior Mesenteric Artery Dissection: Indole Derivatives and Lysophospholipids as Novel Biomarkers.

Spontaneous isolated superior mesenteric artery dissection (SISMAD) is a rare but increasingly recognized vascular disorder that can lead to intestinal dysfunction with severe complications. Currently, no plasma biomarkers, particularly metabolomics-based markers, have been identified for SISMAD. To identify novel plasma biomarkers and potential therapeutic targets for SISMAD and its associated intestinal dysfunction, we analyzed plasma samples from 30 patients with SISMAD and 30 healthy controls. Blood was collected within 72 h of diagnosis. Ultra-high-performance liquid chromatography couple

Fatal delayed superior mesenteric artery thrombosis after frozen elephant trunk repair in acute type a aortic dissection with false lumen perfusion: a case report.

BACKGROUND: Frozen elephant trunk (FET) implantation is an established technique for acute Type A aortic dissection (ATAAD), promoting true lumen expansion and aortic remodeling. However, in patients with preoperative false lumen perfusion, FET carries a significant risk of postoperative false lumen thrombosis that may acutely obstruct branch vessel flow and trigger malperfusion syndrome. CASE PRESENTATION: A 74-year-old hypertensive female presented with acute chest pain. Computed tomography angiography confirmed ATAAD with false lumen perfusion supplying all visceral arteries. Emergency asc

Laparoscopic and robotic extravascular stenting of the left renal vein for anterior nutcracker syndrome in a single-center series.

BACKGROUND: Nutcracker syndrome (NCS) arises from extrinsic compression of the left renal vein (LRV) between the superior mesenteric artery and the abdominal aorta. Extravascular stenting (EVS) has emerged as a minimally invasive alternative to historical operations and endovascular stents. We report a single-center series spanning 2010 to 2025 and propose a standardized, reproducible framework that couples intraoperative process quality with objective postoperative hemodynamic targets. METHODS: We retrospectively analyzed 22 consecutive NCS patients treated with laparoscopic or robot-assiste

Impact of precise preoperative vascular assessment and different dorsal pancreatic artery variant subtypes on pancreatic surgery-related bleeding.

BACKGROUND: The variability of pancreatic vasculature, especially the dorsal pancreatic artery (DPA), increase surgical difficulty and may elevate the risk of intra- and postoperative bleeding. This study aimed to establish a precise preoperative vascular assessment protocol for pancreatic surgery, summarize DPA variant patterns, and evaluate their impact on pancreatic surgery-related bleeding. METHODS: In this prospective study, 206 patients undergoing pancreatic surgery were included and evaluated preoperatively using computed tomography (CT) imaging and Preoperative Accurate Assessment For

Laparoscopic-assisted management of a superior mesenteric artery pseudoaneurysm secondary to chronic pancreatitis: a rare case report.

Superior mesenteric artery (SMA) pseudoaneurysms are rare but potentially life-threatening complications of chronic pancreatitis. This case report describes a 68-year-old African American female with a history of chronic pancreatitis who presented with acute periumbilical pain. Imaging revealed a 1.2 cm SMA pseudoaneurysm. Given the distal location and high rupture risk, the pseudoaneurysm was surgically resected following laparoscopic localization with open ligation. The patient recovered uneventfully and was discharged on postoperative day 3. This case underscores the importance of early rec

Recanalization of a Severely Calcified Superior Mesenteric Artery in the Setting of Extensive Paravisceral Aortic Calcification Using Intravascular Lithotripsy: A Case Report and Focused Literature Review.

Calcified atherosclerotic plaques are a well-recognized cause of chronic mesenteric ischemia (CMI). We present a case where intravascular lithotripsy (IVL) was successfully used to treat a severely calcified and stenotic superior mesenteric artery (SMA). A 72-year-old male with diffuse atherosclerotic disease presented with acute-on-chronic abdominal pain and weight loss. Computed tomography angiography (CTA) demonstrated extensive calcified aortic atherosclerotic disease extending to the major branches, particularly the SMA, causing severe stenosis. Plain old balloon angioplasty (POBA) follow

Splenic artery arising from the superior mesenteric artery: analysis of morphological characteristics with contrast-enhanced CT images.

PURPOSE: To elucidate morphological characteristics of the splenic artery (SA) arising from the superior mesenteric artery (SMA). METHODS: Contrast-enhanced CT images of 13 patients with SA arising from SMA were retrospectively analyzed. Gross appearance of SA arising from SMA was observed on volume-rendering 3D images. Additionally, we measured the following lengths m each patient on the appropriate multiplanar reformation (MPR) images; (A) the distance from the origin of SMA to the origin of SA, (B) SA length from the origin of SA to the point which is at the shortest distance from the celi

Optimization of artery-first approach in right-sided colon cancer CME: preoperative CTA assessment of MCA branching and MCV-Henle's trunk relationship.

BACKGROUND: Vascular injury, particularly involving Henle's trunk, remains a major challenge during complete mesocolic excision (CME) for right-sided colon cancer. Due to the high anatomical variability of right-sided colonic vessels, the influence of tumor location, and heterogeneity in operative techniques, radical right hemicolectomy (RRC) continues to be a high-risk procedure. This study aims to identify preoperative CTA mapping to reduce vascular injury and enhance mesenteric margin integrity in standardized CME surgery. MATERIAL AND METHODS: A single-center, retrospective observational

Viscum album L. mother tinctures modulate Na+/K+ ATPase activity and expression, and promote endothelium-dependent vasodilation via SK channel and nitric oxide signalling.

INTRODUCTION: Ethanolic extracts of Viscum album L. (European mistletoe), known as mother tinctures (VAMTs), have been used in traditional medicine to treat hypertension and cardiovascular diseases. However, the underlying biological mechanisms remain poorly characterized. METHODS: This study investigated the cytotoxic, and cardiovascular effects of ten VAMTs prepared from fresh plants harvested in summer and winter. The extracts were derived from three subspecies (V. album subsp. album, abietis, and austriacum) growing five distinct host tree species. Cytotoxicity and oxidative stress were a

Sonographic Hallmarks of Nutcracker Syndrome.

Nutcracker syndrome (NCS) occurs when the left renal vein (LRV) is compressed between the aorta and superior mesenteric artery (SMA), which can manifest as hematuria, proteinuria, flank pain, and pelvic venous congestion as a sequela of renovenous hypertension. In the absence of symptoms, LRV compression is referred to as nutcracker anatomy or the nutcracker phenomenon. Though there are no currently formally accepted diagnostic pathways, US, CT, and MRI are frequently used to assess patients with NCS. The US is often the first imaging modality to be used in the workup of NCS. Doppler US is a w

A Case of Preserving the Collateral Arterial Arcade in the Head of the Pancreas in a Patient with Celiac Axis Stenosis.

An 80-year-old man with middle-third cholangiocarcinoma associated with celiac axis stenosis underwent pancreaticoduodenectomy. During surgery, the anterior arcade that had developed in the pancreatic head as a collateral pathway between the superior mesenteric artery and the hepatic artery was preserved as a collateral route to ensure adequate hepatic and gastric perfusion. This may be a useful option for pancreaticoduodenectomy in a patient with celiac axis stenosis.

Intraoperative positioning system-guided antegrade in situ laser fenestration in an aortic model.

OBJECTIVE: Fenestrated endovascular aneurysm repair (fEVAR) is routinely being used for visceral vessel incorporation in complex aneurysmal pathology; however, it is associated with prolonged procedure times, high radiation exposure, high contrast use, and multiple staged procedures. To combat these limitations, Centerline Biomedical developed the three-dimensional surgical navigation and visualization technology: intraoperative positioning system (IOPS). We previously presented our IOPS-guided antegrade in situ laser fenestration in a standard aortic model. This case report details our experi
