Integrated Bedside Hemodynamic Assessment and Monitoring of Venous Congestion With Venous Excess Ultrasound (VExUS): A Narrative Review.

This review evaluates the venous excess ultrasound (VExUS) score as a bedside tool for quantifying systemic venous congestion and predicting congestive end-organ injury, particularly acute kidney injury (AKI). By assessing flow patterns in the hepatic, portal, and renal veins alongside inferior vena cava (IVC) size, VExUS enables serial monitoring of congestion trajectories. While highly prognostic in heart failure, its predictive performance varies in more heterogeneous populations. Ultimately, integrating VExUS with cardiac output indices may help clinicians identify specific hemodynamic phe

Life-Threatening Aberrant Omental Variceal Bleeding with Interventional Treatment: A Case Report.

Ectopic varices resulting from portal hypertension can cause fatal bleeding. Intra-abdominal hemorrhage from omental varices is rare, and no previous reports have described a direct connection between the splenic and ovarian veins. We report a case of intra-abdominal hemorrhage caused by an omental varix in a 49-year-old women with liver cirrhosis. Because she was hemodynamically unstable, surgical treatment was not feasible; therefore, an interventional procedure was planned. The varices originated from the splenic vein and drained into the ovarian vein. Through percutaneous portal venous acc

Distinct collagen resolution dynamics in early versus advanced liver fibrosis: a quantitative SHG/TPEF study in mice.

BACKGROUND & AIMS: Liver fibrosis regression occurs after the source of injury is removed, but its dynamics and histologic features remain poorly characterized. Second-harmonic generation/two-photon excited fluorescence (SHG/TPEF) microscopy enables quantitative, high-resolution collagen assessment. We employed SHG/TPEF to characterize spatiotemporal collagen remodeling during spontaneous regression in carbon tetrachloride (CCl4)-induced mouse models. METHODS: Early- and advanced-stage liver fibrosis were established by intraperitoneal CCl4 injection for 8 or 12 weeks. After peak injury, mice

Changes in the apoptotic index, along with gene and protein expression levels, in splenomegaly due to portal vein stenosis in rats.

BACKGROUND: The spleen is an important peripheral immune organ, and splenomegaly (SM) is a common complication of portal hypertension. However, the regulatory mechanisms underlying splenic apoptosis remain poorly understood. MATERIALS AND METHODS: A portal vein stenosis-induced SM model was established in Sprague-Dawley rats, using a sham-operated (SO) group as the control. Apoptotic changes in splenic tissue cells were evaluated using the terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay, immunohistochemistry staining, quantitative real-time polymerase chain reaction

Baveno VI criteria validation and development of a new non-invasive strategy for varices rule-out in patients with HCC (SAPHE-R: Stiffness, ALBI, Platelets, HEpatocellular carcinoma Rule).

BACKGROUND AND AIMS: Accuracy of Baveno criteria for avoiding endoscopy in patients with hepatocellular carcinoma (HCC) is uncertain, and systematic upper gastrointestinal endoscopy (UGE) may not be necessary. We evaluated Baveno VI criteria performance in this population and developed an alternative non-invasive rule to exclude esophagogastric varices of any grade, rather than high-risk varices (HRV). METHODS: Patients with BCLC 0-C HCC and compensated advanced chronic liver disease (cACLD) from Italian Liver Cancer (ITA.LI.CA) database, with available liver stiffness measurement (LSM), UGE,

Venous Thromboembolism in Acute Necrotizing Pancreatitis Is Associated With Worse Outcomes, With Subtype-Specific Patterns: A Nationwide Analysis, 2016-2022.

OBJECTIVES: Venous thromboembolism (VTE) complicates acute necrotizing pancreatitis (ANP), but outcomes across extra-splanchnic deep vein thrombosis (DVT), portal vein thrombosis (PVT), and splenic vein thrombosis (SVT) remain poorly defined. We evaluated national trends and in-hospital outcomes in ANP with versus without VTE and across VTE subtypes. METHODS: Using the National Inpatient Sample (2016-2022), adults hospitalized with principal ANP were identified. Secondary diagnosis codes captured VTE subtypes. Primary analyses compared mortality and major complications in ANP with versus with

Assessment and Reversibility of Hepatic Abnormalities in Heart Failure: Liver in Heart Failure.

Hepatic abnormalities in non-Fontan heart failure result from congestive hepatopathy and hypoperfusion-related ischemic injury, compounded by neurohormonal activation and systemic inflammation. Assessment of liver disease severity is challenging because standard liver biochemistries, serum fibrosis markers, severity score models, elastography, and imaging modalities have limited diagnostic accuracy in congestive hepatopathy, and hepatic fibrosis is often heterogeneous on histology in this population. Management focuses on optimization of cardiac function through medical therapy or mechanical c

A gastric dieulafoy lesion presenting with life-threatening upper gastrointestinal hemorrhage during recovery from hypertriglyceridemic acute pancreatitis: a case report.

BACKGROUND: Dieulafoy lesions are an uncommon but potentially life-threatening cause of non-variceal upper gastrointestinal bleeding, caused by rupture of a large-caliber submucosal artery through a minute mucosal defect; diagnosis is difficult because bleeding is intermittent and the surrounding mucosa appears nearly normal. Hemorrhage from a gastric Dieulafoy lesion during recovery from hypertriglyceridemic acute pancreatitis (HTG-AP) has rarely been reported. CASE PRESENTATION: A man in his twenties with severe obesity presented with acute epigastric pain after alcohol and a high-fat meal.

Autoimmune hepatitis following long-term denosumab therapy in a patient with Hashimoto thyroiditis.

Denosumab, a receptor activator of nuclear factor-κB ligand (RANKL) inhibitor, is widely used to treat osteoporosis. Its role as a trigger for autoimmune hepatitis (AIH) remains uncertain. We report a 68-year-old woman with osteoporosis and Hashimoto thyroiditis who developed jaundice and severe hepatocellular liver injury after 3 years of denosumab therapy. Alternative viral, obstructive, and metabolic causes were excluded. Liver biopsy demonstrated interface hepatitis with a lymphoplasmacytic portal infiltrate consistent with AIH, and autoimmune serologies were positive. Hemophagocytic lymph

Genetic and functional characterization of a novel SERPIND1 variant in a family with isolated gastric varices and venous thrombosis.

BACKGROUND: Isolated gastric varices type I (IGV1) is a rare but severe manifestation of splenic vein thrombosis. While thrombophilia is a recognized cause, the underlying genetic determinants are often elusive. This study investigates the case of a proband presenting with IGV1, deep vein thrombosis (DVT), and Long QT syndrome Type 2 (LQT2), aiming to elucidate the genetic basis and functional impact of the identified SERPIND1 variants. METHODS: Whole-exome sequencing identified candidate variants in the proband, which were validated by Sanger sequencing. Functional assessment included quanti

Arthroscopic Reduction Techniques for Complex Metacarpophalangeal Joint Dislocation With Sesamoid Bone Entrapment: A Case Series.

PURPOSE: Complex metacarpophalangeal joint (MCPJ) dislocations typically require open reduction, but there is a paucity of literature exploring arthroscopic-assisted reduction. We present our experience in six cases of arthroscopic reduction of complex MCPJ dislocation, evaluating the success rate and clinical outcomes and delineating key technical pearls and intraoperative challenges. METHODS: This retrospective series included patients with complex dorsal MCPJ dislocation without fractures or open injuries who were treated at a single institution from 2004 to 2025. After closed reduction fa

Near-total splenic infarction with pseudocystic transformation due to combined splenic artery and splenic vein thrombosis: CT-based diagnosis.

Splenic infarction typically appears on imaging as a peripheral wedge-shaped region of non-enhancement within the splenic parenchyma. Near-total infarction with liquefied transformation is uncommon and may mimic other cystic splenic lesions. We report a 55-year-old woman who presented with fever, vomiting, and left-sided chest pain radiating to the back. The patient had initially been evaluated at an outside facility and subsequently referred to our institution with a diagnosis of splenic vein thrombosis. She was admitted from 22 February 2026 to 24 February 2026 and managed conservatively wit

Emphysematous gastritis with extensive portal venous gas and endoscopic mucosal necrosis: gastric preservation despite an alarming presentation.

Emphysematous gastritis (EG) is a rare, potentially life-threatening condition. Extensive gastric pneumatosis and portal venous gas may suggest irreversible ischemia, although radiological severity and mucosal injury do not necessarily indicate transmural necrosis. A 70-year-old man presented with acute epigastric pain and vomiting. Computed tomography (CT) demonstrated extensive gastric and duodenal pneumatosis with portomesenteric venous gas but patent visceral arteries. Immediately after CT, he developed cardiac arrest, with return of spontaneous circulation after approximately 14 min. Emer

Rethinking Pathogenesis of Primary Sclerosing Cholangitis.

Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease characterized by progressive fibroinflammatory injury of intrahepatic and extrahepatic bile ducts that may lead to cirrhosis, hepatobiliary malignancy, and death, with liver transplantation representing the only effective intervention in end-stage disease. Rather than a single dominant pathomechanism, the data support a model in which four axes - bile acid metabolism, the intestinal microbiome and gut barrier, biliary epithelial dysfunction, and Th17-skewed immune dysregulation - are reciprocally coupled and converge o

Development and a single-centre temporal validation of a machine learning-based model for early prediction of acute pancreatitis-associated liver injury.

BACKGROUND: Acute pancreatitis (AP) is a common acute abdominal inflammatory disorder characterized by premature activation of pancreatic enzymes, leading to pancreatic autodigestion, edema, hemorrhage, and even necrosis. The liver, as the first portal organ receiving pancreatic venous drainage, is one of the most frequently affected extra-pancreatic organs in AP. AP-associated liver injury (LI) is closely correlated with the intensity of systemic inflammation, the development of multiple organ dysfunction, and increased mortality. While machine learning (ML) models have been successfully appl

Safe Ultrasonography-Assisted Elbow Arthroscopy Technique.

Elbow synovectomy has traditionally been performed using direct visual observation. However, recent advancements in elbow arthroscopy have increased the popularity of arthroscopic synovectomies. This minimally invasive procedure has garnered attention because of its efficacy in halting joint deterioration while minimizing patient discomfort and invasiveness. Conversely, the technical demands of arthroscopic elbow synovectomy pose challenges, often resulting in notable complications, such as nerve damage. Considering the utility of real-time nerve visualization for injury prevention, we suggest

Coil Embolization and Open Splenectomy for a Massive Splenic Vein Aneurysm: A Case Report.

BACKGROUND: Splenic vein aneurysms (SVAs) are a rare vascular anomaly of the portal system that are often asymptomatic and found incidentally on imaging. Intervention is reserved for aneurysms that are enlarging, thrombosed, symptomatic, or ruptured to avoid catastrophic complications. This can range from endovascular to open, complex procedures. Here, we present the case of a massive, symptomatic, partially thrombosed SVA that required a hybrid approach involving both minimally invasive and open surgery. Successful conservative management of hepatic portal venous gas complicating severe immune-related gastrointestinal injury during pembrolizumab-based concurrent chemoradiotherapy for cervical cancer: a mechanistic case report.

Concurrent chemoradiotherapy (CCRT) combined with pembrolizumab has become the standard treatment for high-risk locally advanced cervical cancer, increasing the need for prompt recognition of immune-related adverse events (irAEs). We report a rare case of severe gastrointestinal irAE complicated by hepatic portal venous gas (HPVG) during pembrolizumab-based CCRT. A 72-year-old woman with FIGO 2018 stage IIIC1r cervical squamous cell carcinoma developed persistent diarrhea and anorexia during treatment, followed by sudden hematemesis after the first brachytherapy session. Integration of transplanted chemically induced liver progenitor cells into recipient bile ducts in a rat bile duct transection model.

BACKGROUND: Chemically induced liver progenitor cells (CLiPs) generated from mature hepatocytes are bipotent and represent a promising source for biliary repair. However, their therapeutic potential in cholangiopathy models remains unclear. In this study, we investigated rat CLiPs using a rat bile duct transection (BDT) model. METHODS: Rat CLiPs were induced using the Y-27632/A-83-01/CHIR99021 regimen, resulting in loss of the hepatocyte marker ALB and gain of the progenitor markers CD90 and EpCAM. GFP-labeled CLiPs (1 × 10^7) were transplanted into Sprague-Dawley rats following BDT via porta
