Rationale: Metabolic remodeling occurs during partial hepatectomy (PHx)-induced liver regeneration. Phospholipid remodeling during this process and its subsequent impact on liver regeneration remain unknown. The remnant liver's ability to defend against injury is also essential for normal liver regeneration, although the underlying mechanisms remain unclear. Methods: Phospholipidomics was performed to describe phospholipid remodeling after 70% PHx. Phosphate cytidylyltransferase 2, ethanolamine (PCYT2) was overexpressed in hepatocytes using adeno-associated virus under the thyroxine-binding gl

BACKGROUND: Splanchnic vein thrombosis (SVT) is common in acute pancreatitis (AP) and often leads to intra-abdominal collaterals, increasing the risk of bleeding during interventions. However, data on its impact on intervention outcomes are limited. We prospectively assessed the effect of SVT on outcomes of interventions for symptomatic pancreatic fluid collections (PFCs).
METHODS: Fifty patients (mean age: 40.9 y; 62% male) with symptomatic PFCs were prospectively evaluated. Contrast-enhanced CT was used to assess PFCs, detect SVT, and identify intra-abdominal venous collaterals (IAVCs). Pat

AIMS: Inclisiran, an siRNA-targeting hepatic PCSK9 mRNA, reduces low-density lipoprotein cholesterol (LDL-C), but its effect on major adverse cardiovascular event (MACE) remains unconfirmed. The SIRIUS in-silico modelling programme aimed to predict the efficacy of inclisiran on MACE in virtual patients with atherosclerotic cardiovascular disease (ASCVD).
METHODS AND RESULTS: The SIRIUS simulation (NCT05974345) used a validated mechanistic model of ASCVD and lipid-lowering therapy (LLT) effects in a virtual population with established ASCVD and LDL-C ≥ 70 mg/dL. Each virtual patient served as

Chronic liver allograft rejection (CR) remains a significant cause of late graft dysfunction and failure despite advances in surgical techniques and immunosuppression. This chapter reviews the current understanding of CR, integrating diagnostic histopathological findings, atypical histological patterns of late rejection that are frequently associated with CR, and emerging diagnostic tools. CR is driven by overlapping T cell-mediated and antibody-mediated mechanisms, leading to progressive bile duct loss, vascular injury, and structural alterations. Diagnostic histopathological criteria include

BACKGROUND: Deep learning image reconstruction (DLIR) has gained recognition as a promising technique to improve image quality in low-dose CT imaging. However, its performance in dual-energy CT portal venography (DE-CTPV), particularly under reduced contrast medium volume and radiation dose (dual-low dose) conditions, remains underexplored.
OBJECTIVE: This study aims to compare the performance of DLIR and adaptive statistical iterative reconstruction (ASIR-V) in DE-CTPV, with a focus on image quality across multiple vascular segments of the portal venous (PV) system under dual-low dose protoc

Previous studies have confirmed that scald injuries can lead to disturbances in hepatic lipid metabolism, and bone marrow-derived mesenchymal stem cells (BMSCs) have emerged as a promising therapeutic strategy for alleviating such disorders. However, research focusing on the regulation and restoration of liver lipid metabolic processes remains limited. In this study, we investigated the effects of BMSCs on hepatic lipid metabolism disorders induced by scald injury in rats through integrated transcriptomic and metabolomic analyses. The results demonstrated that portal vein infusion of BMSCs mar

BACKGROUND Knowledge of the concordance between portal vein (PV) and bile duct (BD) anatomy is essential for planning living donor liver transplantation (LDLT) and hepatobiliary surgery. Unrecognized variants can result in bile leakage, vascular injury, or graft-related complications; therefore, accurate preoperative mapping is mandatory. Because the PV and BD systems develop in parallel during embryogenesis, an anatomical relationship between them has been proposed, but available data remain limited and inconsistent. This retrospective study of 423 living liver donors aimed to evaluate the as

Venous congestion is a major contributor to morbidity and mortality in several clinical settings. Relying on traditional methods for bedside evaluation of congestion often leads to inadequate decongestion and increased hospital readmissions. The Venous Excess Ultrasound Score (VExUS) was introduced by Beaubien-Souligny to improve assessment of systemic congestion by integrating inferior vena cava size and Doppler patterns from hepatic, portal, and intrarenal veins. This review aims to describe how to perform VExUS and summarize the current state of evidence based on original data studies eithe

OBJECTIVES: This study aims to characterize postoperative recurrence patterns and anatomical distribution in patients with pancreatic cancer within the framework of extrapancreatic neural tract anatomy. Based on the observed recurrence sites, we propose stratified clinical target volume (CTV) delineation recommendations for postoperative adjuvant radiotherapy. METHODS: We retrospectively analysed 352 patients who underwent resection for pancreatic adenocarcinoma and recorded the sites of initial postoperative recurrence. Recurrence foci were reconstructed according to adjacent anatomical struc

» Ramp lesions occur in up to 40% of anterior cruciate ligament (ACL) injuries and are easily missed without systematic posteromedial inspection.» Magnetic resonance imaging has 70% sensitivity; arthroscopic probing using the posteromedial portal remains the diagnostic gold standard.» Unstable ramp lesions require repair; stable lesions may heal spontaneously with ACL reconstruction.» Repair restores knee stability and yields excellent outcomes comparable with isolated ACL reconstruction.» Return-to-sport rates exceed 80%, with rehabilitation mirroring standard ACL reconstruction protocols.

The liver has a unique microarchitecture, with hepatic sinusoids receiving blood from the portal vein and hepatic artery and draining into the central vein. This flow establishes an oxygen gradient along the sinusoids critical for defining the liver zonation. In metabolic dysfunction-associated steatotic liver disease (MASLD), fat accumulation and fibrosis disrupt this architecture, contributing to localised hypoxia. Mounting evidence implicates hypoxia in MASLD, including the activation of canonical hypoxia sensors such as hypoxia-inducible factors. Moreover, chronic intermittent hypoxia, cha

BACKGROUND: In recent years, increasing evidence has emerged regarding the use of direct oral anticoagulants (DOACs) for the treatment of splanchnic vein thrombosis (SVT) in non-cirrhotic patients. However, available data are predominantly retrospective, derived from small and heterogeneous cohorts; consequently, the use of DOACs for this indication remains off-label.
METHODS: To further evaluate the safety and efficacy of DOACs in this setting, 21 consecutive patients with acute non-cirrhotic SVT were enrolled and treated with DOACs for a minimum of 6 months.
RESULTS: No cases of SVT progre

BACKGROUND: In locally advanced pancreatic adenocarcinomas (LA-PDAC) when both common hepatic artery and portal vein encasement coexist, classical right-to-left dissection with vascular resection is difficult to achieve.1 We describe an alternative approach named left-to-right pancreatoduodenectomy at splenic vessels in which the central part of the dissection is carried out last and the reversed splenic artery is used to reconstruct the hepatic artery.2 PATIENT AND METHODS: The patient was a 76-year-old woman with both transverse colonic cancer and LA-PDAC (common hepatic artery and portal ve

Hypoxia, which represents a key pathological feature of gastric mucosal disorders, exacerbates mitochondrial oxidative stress through dysregulated cellular responses. However, the underlying mechanism of mitochondrial oxidative stress under hypoxia during gastric mucosal epithelial injury in portal hypertensive gastropathy (PHG) is not fully understood. To assess the impact of mitochondrial oxidative stress under hypoxic conditions during gastric mucosal epithelial injury in the PHG, mucosal tissues from patients with PHG and healthy individuals were collected. Furthermore, portal hypertension

BACKGROUND: COVID-19 is caused by SARS-CoV2, which primarily produces respiratory disease. Although liver enzyme abnormalities are common in patients with COVID-19, little is known about histological changes in the liver. Most of the available information is based on autopsy studies, with limited liver biopsy studies in living patients. METHODS: Liver biopsies performed between March 2021 and September 2021 in patients with a history of COVID-19 were retrieved, and 17 biopsies were reviewed. Additionally, immunohistochemical staining for the SARS-CoV2 nucleocapsid protein was performed in all

BACKGROUND AND AIMS: Electron microscopy (EM) has provided key insights into hepatocellular alterations in MASLD, but most studies relied on percutaneous biopsies and primarily focused on selected features such as megamitochondria or autophagic vesicles. We aimed to evaluate the feasibility of ultrastructural analysis in transjugular biopsies and to provide a comprehensive description of hepatocellular changes in patients with MASLD.
METHODS: Patients with MASLD, well characterized by hepatic venous pressure gradient (HVPG) and stratified by fibrosis stage, portal hypertension, and MASH statu

BACKGROUND: Meniscal injuries are commonly observed in acute anterior cruciate ligament (ACL) injuries, with varying reports on the incidence and location of meniscal tears. In particular, the incidence of posterior menisco-capsular tears may have been underestimated in previous literature owing to the technical challenges associated with posterior arthroscopic evaluation. This study aimed to determine the true incidence and location of posterior menisco-capsular junction tears in patients undergoing anterior cruciate ligament reconstruction (ACLR) up to 6 weeks of injury, hypothesizing a high

BACKGROUND: Surgery for gastric cancer involves gastrectomy with lymph node dissection based on established arterial landmarks. While arterial variations around the celiac axis are well recognized, venous anomalies such as aberrant courses of the splenic vein have received limited attention, and their potential impact on the safety of standard lymphadenectomy remains unclear. We report a case of laparoscopic total gastrectomy complicated by an aberrant course of the splenic vein that posed a potential risk of vascular injury during standard lymph node dissection. CASE PRESENTATION: An 80-year-
