Optimal Surgical Approach for Peri-Portal Vein Pancreatic Body Cancer: From the Pancreatic Tail or Head Side?

OBJECTIVE: For peri-portal vein (PV) pancreatic body (Pb) cancer, we have often encountered difficulties in selecting the appropriate surgical approach-whether from the pancreatic tail side or the head side. This study aimed to clarify which procedure is more suitable for peri-PV Pb cancer. METHODS: We reviewed 30 cases of peri-PV Pb cancer. The resected status of lymph nodes, their positivity rates in each procedure, and postoperative recurrence patterns were analyzed. RESULTS: Among the 30 patients, 26 underwent distal pancreatectomy (DP), 3 underwent pancreatoduodenectomy, and 1 underwent

Acute Abdominal Pain in Pregnancy Revealing a Wandering Spleen With Infarction: A Case Report.

INTRODUCTION AND IMPORTANCE: Wandering spleen (WS) is a rare condition caused by the absence or laxity of splenic suspensory ligaments, predisposing the spleen to displacement and potential complications. While torsion is the most commonly reported issue, infarction due to vascular compromise is a serious and rare complication, particularly in pregnancy. We present a case of splenic infarction in a WS during pregnancy, emphasizing diagnostic challenges and management strategies. CASE PRESENTATION: A 19-year-old primigravida at 5 months of gestation presented with progressively worsening abdom

Neoadjuvant Oxaliplatin and Pancreaticoduodenectomy for Pancreatic Ductal Adenocarcinoma: CT Features of Chemotherapy-Associated Liver Injury and Postoperative Complications.

RATIONALE AND OBJECTIVES: Oxaliplatin-based neoadjuvant therapy (NAT) is increasingly being used in patients undergoing curative-intent surgery for pancreatic ductal adenocarcinoma (PDAC). Liver toxicity, notably through the development of chemotherapy-associated liver injury (CALI), has been shown in patients with colorectal liver metastases. However, its impact on postoperative complications after pancreaticoduodenectomy (PD) is not known. The study aimed to assess the hepatic changes on computed tomography (CT) imaging after oxaliplatin-based NAT and to identify markers predictive of postop

Therapeutic potential of tetrahydroxylated bile acids in reducing liver injury: Insights from the Zfyve19-/- mouse model.

IMPORTANCE: The production of tetrahydroxylated bile acids (THBAs) is associated with better prognosis in some cholestatic patients as well as in multidrug resistance protein 2 knockout (Mdr2-/- ) mice. However, it remains unclear whether this protective effect is specific to Mdr2-/- mice. OBJECTIVE: To evaluate the effects of THBA (3α,6α,7α,12α-Tetrahydroxy-10β,13β-pentanoic acid) in Zfyve19-/- mice, a newly developed mouse model characterized by cholestatic liver injury. METHODS: THBA was administered to Zfyve19-/- mice challenged with alpha-naphthyl isothiocyanate (ANIT). Serum biochemist

Development and validation of an interpretable machine learning model for predicting portal vein system thrombosis following splenectomy in Wilson's disease.

Post-splenectomy portal vein system thrombosis (PVST) is a serious complication that adversely affects patient prognosis. The aim of this study was to develop and validate a machine learning-based model for early risk prediction of PVST after splenectomy. A total of 594 patients who underwent splenectomy were included in this study. The data were randomly divided into a training set (n = 417) and a validation set (n = 177) in a 7:3 ratio. Clinical characteristics associated with PVST risk were screened using LASSO regression and univariate analysis. Seven machine learning algorithms were appli

Top 10 Technical Tips and Tricks for Safe Sciatic Nerve Management During Posterior Hip Endoscopy.

BACKGROUND: A thorough understanding of posterior hip anatomy is essential for safe and effective posterior hip endoscopy. The sciatic nerve runs laterally to the ischium and hamstring origin. While sciatic nerve injury during proximal hamstring repair is uncommon, the increasing use of endoscopic techniques has led to a rise in iatrogenic injuries. This highlights the need for a structured approach to identifying and protecting the sciatic nerve. INDICATIONS: Surgical intervention is indicated for complete proximal avulsions involving all 3 hamstring tendons, partial avulsions involving ≥2 t

Characterizing Heterogeneity in Brain Morphology in Traumatic Brain Injury Using Normative Modeling.

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) is heterogeneous, complicating efforts to develop standardized diagnostic and therapeutic approaches. Conventional group-based analyses often obscure individual differences by averaging across diverse injury patterns. By contrast, by comparing deviation from the expected distribution observed in healthy controls, normative modeling can capture patient-specific deviations from expected neuroanatomical norms. The aim of this research was to use normative modeling to capture individual variability in brain morphometry on individuals who have

Robotic versus laparoscopic spleen-preserving distal pancreatectomy: a propensity score-matched study of the "splenic artery and vein plane first" strategy.

This study aims to compare the intraoperative and short-term postoperative efficacies between robotic-assisted and conventional laparoscopic approaches for spleen-sparing left pancreatectomy. Both minimally invasive modalities were executed by adopting the Splenic Artery and Vein Anatomical Plane First (SAPP) surgical philosophy. A retrospective assessment was conducted on patients who received minimally invasive distal pancreatectomies with Kimura-style splenic preservation from January 2019 to September 2024. To mitigate potential selection bias and equilibrate baseline variables across the

The Optimal Timing of Portal and Hepatic Vein Ligation for Increasing Liver Remnant Volume in Rats.

INTRODUCTION: Portal vein embolization combined with hepatic vein embolization enhances future liver remnant hypertrophy in major hepatectomy. However, the optimal timing, simultaneous or sequential, remains undefined. This study evaluates the optimal timing of hepatic vein ligation (HVL) in combination with portal vein ligation (PVL) using a rat model. METHODS: PVL was performed for the middle and left lateral lobes, while HVL was applied to the hepatic vein of the left lateral lobe. Rats were allocated to the PVL only, PVL + HVL, or sham group. HVL was added at -3, 0, +3, +24, or +72 h rela

Imaging and genomics in stroke.

Imaging after ischemic and hemorrhagic stroke may allow measurement of key phenotypes of injury and recovery for which targeted therapies are still lacking. Such imaging endophenotypes provide quantifiable and heritable biomarkers that can represent mechanistic aspects of disease processes better than clinical measures. Artificial intelligence is allowing extraction of these imaging biomarkers in large cohorts, which can be paired with genomic and other omics data. This will allow the evaluation of what genetic and other biologic variations impact stroke injury and recovery. Integration of the

NEDD4 like E3 ubiquitin protein ligase-mediated ubiquitination and degradation of dipeptidyl peptidase 4 restrains Th2-linked inflammation in allergic rhinitis.

Allergic rhinitis (AR) is a chronic inflammatory disorder characterized by type 2 T helper (Th2) cell-dominant immune responses. NEDD4 like E3 ubiquitin protein ligase (NEDD4L) plays a role in regulating T cell activation and Th cell differentiation. Dipeptidyl peptidase 4 (DPP4) is involved in Th2 inflammatory responses in AR. In this study, NEDD4L was found to be downregulated in peripheral blood CD4+-enriched T cells from AR patients. In a BALB/c mouse model of AR, established by intraperitoneal sensitization with 25  μg ovalbumin (OVA) followed by intranasal challenge with 500  μg OVA, NED

Association of Changes in Portal Insulin with Immunometabolism During and After Hepatitis C Virus Infection.

BACKGROUND AND AIMS: Insulin resistance is a common extrahepatic manifestation of hepatitis C virus (HCV) infection (HCVi), but its mechanism is poorly understood. While systemic insulin resistance is documented, portal insulin dynamics, a key regulator of hepatic metabolism, remain unexplored. This study aimed to investigate the relationship between insulin, the gut-liver axis, and immunometabolic changes in patients with HCV. METHODS: HCV patients were evaluated before (HCVi; n = 29) and after sustained virologic response (SVR) achieved with sofosbuvir/velpatasvir treatment (SVR, n = 23) (N

Arthroscopic Anterior Labral Repair With Capsulorrhaphy and Interval Closure: Surgical Technique.

BACKGROUND: Traumatic anterior labral injuries (Bankart lesions) lead to anterior shoulder instability. Arthroscopic anterior labral repair (also known as Bankart repair) provides a soft tissue reconstruction to reduce the risk of dislocation. However, the described techniques vary significantly. We present our technique for enhancing the quality and volume of tissue incorporated in the repair and utilizing interval closure to further reduce recurrent instability rates. INDICATIONS: Non-throwing athletes or high-demand patients with recurrent instability after a traumatic event with <20% glen

Myeloproliferative neoplasm as a cause of splanchnic vein thrombosis.

Splanchnic vein thrombosis (SVT) is a rare, life-threatening condition that is often associated with cirrhosis, pancreatic malignancy and thrombophilia. Myeloproliferative neoplasms are under-recognised causes, posing diagnostic challenges. In this case, we present a male in his late 20s with a history of unprovoked pulmonary embolism and portal/splenic vein thrombosis, on indefinite anticoagulation therapy presenting 3 years later due to abdominal pain. Imaging revealed cirrhosis, cavernous transformation of the main portal vein and splenomegaly. These findings and a prior negative hypercoagu
