Successful minimal-invasive management of misplaced sacroiliac screw perforating the left external iliac vein: an interdisciplinary intervention by angiology and traumatology-a case report.

BACKGROUND: Percutaneous sacroiliac screw fixation is an effective option for unstable pelvic ring fractures. It is minimally invasive, causes little soft tissue damage, and reduces complication risk. Nevertheless, complications such as screw loosening or nerve root damage are reported. Although vascular injuries are also described, detailed management strategies are rarely found in the literature. CASE PRESENTATION: A 58-year-old white German female suffered polytrauma, including a type c posterior pelvic ring fracture, after a fall from great height. The fracture was treated with screw oste

Anomalous occluded vessels and the "Corona Mortis": A reappraisal of nomenclature origins, anatomical variations, and surgical risks.

BACKGROUND: The "corona mortis" (CMOR) typically refers to arterial or venous anastomotic branches located posterior to the superior ramus of the pubis within the Retzius space, connecting the obturator vascular system to the extrailiac or subabdominal vascular system. This anatomical variation is closely associated with inguinal hernia repair, pelvic and acetabular surgery, pelvic tumor lymph node dissection, and pelvic trauma hemorrhage. However, its nomenclature origin, terminological boundaries, and actual bleeding risk have long been controversial across disciplines. OBJECTIVE: To establ

Direct Anterior Approach to Hip Resurfacing Arthroplasty.

BACKGROUND: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate preparation of the femoral head and acetabulum is critical for successful outcomes3. In the present video article, we describe a reproducible technique utilizing preoperative 3D computed tomography (CT)-based planning and patient-specific 3D-printed jigs to optimize femoral head p

Avoiding Vascular Misadventure: Pre-emptive Contralateral Endovascular Access in Revision Total Hip Arthroplasty: A Case Report.

CASE: A 73-year-old woman with a custom triflange acetabular component (CTAC) underwent re-revision total hip arthroplasty (THA) with prophylactic contralateral vascular access. Severe acetabular bone loss led to medialization of the CTAC, putting the distal external iliac artery at risk during surgery. Endovascular access allowed for immediate control of hemorrhage if encountered. Patient was symptom-free at 1 year. CONCLUSION: This case demonstrates the feasibility and safety associated with obtaining pre-emptive vascular access for high-risk revision THAs. This methodology allows for poten

Variations in the length and anatomy of the common iliac vessels and their clinical consideration during anterior pelvic approaches.

The bifurcation and confluence of the common iliac vessels and their course are critical considerations for safe surgical acetabular fracture repair. Anatomical variations in the expected placement of these vessels have been associated with intraoperative complications, including hemorrhage. The purpose of this study was therefore to define vertebral levels of bifurcation and confluence of the common iliac vessels and trace their passage across the quadrilateral plate, a surgically significant landmark. The findings revealed 57% and 11% of bifurcation and confluence, respectively, were located

Endovascular management of iliac vessel injury complicating lumbar disc surgery: a case report and review of literature.

BACKGROUND: Lumbar disc surgery is one of the most commonly performed spinal procedures and is generally considered safe. However, rare complications such as major vascular injury can be life-threatening and are often difficult to recognize intraoperatively. These injuries most commonly involve the iliac vessels because of their close anatomical relationship to the L4-L5 disc space. Early diagnosis is challenging, and delayed recognition is associated with significant morbidity and mortality. With advances in endovascular techniques, minimally invasive management has emerged as an effective al

Intradural dorsal arteriovenous fistula: a case report and literature review.

BACKGROUND: Intradural dorsal arteriovenous fistula (AVF) is a rare spinal vascular malformation that primarily affects the blood-supplying arteries of the intradural dorsal. Its clinical manifestations are dominated by bilateral lower limb weakness and a lack of specificity in early symptoms, which can easily lead to underdiagnosis or misdiagnosis. Thus, the successful diagnosis of intradural dorsal AVF is difficult. CASE DESCRIPTION: A 56-year-old woman presented with lower limb weakness for 3 months without significant sensory deficits. The patient has a history of hyperthyroidism, but the

A retrospective study of catastrophic pelvic fractures in California Thoroughbred racehorses.

Here, we describe and characterize catastrophic pelvic fractures in Thoroughbred racehorses in California. We reviewed retrospectively racehorse autopsy reports from horses that died spontaneously or were euthanized because of complete pelvic fractures in California between 1992 and 2018. The exercise history of affected horses was compared with eventmatched control horses. Catastrophic pelvic fractures were sustained by 133 horses. Pelvic fractures were 2.7% of all musculoskeletal fatalities. Pelvic fractures occurred during training (48.0%), racing (35.0%), and non-exercise (17.0%). Horses ≤

A case of circumferential caval penetration caused by an inferior vena cava filter with asymptomatic duodenal perforation.

We report a case of asymptomatic duodenal perforation caused by an inferior vena cava (IVC) filter. A 43-year-old man presented with multiple filter struts projecting through the IVC wall and penetrating the duodenum, identified on esophagogastroduodenoscopy. The patient had undergone retrievable IVC filter placement one year earlier for left iliac vein thrombosis and was being followed up with computed tomography. Open surgical removal of the IVC filter and primary repair of the duodenum were performed to prevent potential hemorrhagic and infectious complications. The postoperative course was

Development and characterization of rat hindlimb ischemia models mimicking peripheral arterial disease severity.

Peripheral arterial disease (PAD) is a progressive atherosclerotic disorder that reduces blood flow to the lower extremities, leading to skeletal muscle degeneration, functional decline, and limb loss in advanced stages. To enable rigorous preclinical evaluation of regenerative therapies across disease stages, we developed and quantitatively characterized three rat hindlimb ischemia models representing graded arterial insufficiency: iliac artery ligation (Iliac), femoral artery ligation and excision (Femoral), and combined femoral plus popliteal artery ligation and excision (Fem/Pop). Laser Do

External Iliac artery injury in severe pelvic ring trauma: Limb salvage or early hemipelvectomy? A case series.

PURPOSE: External iliac artery (EIA) injury associated with pelvic ring injury (PRI) is rare but represents a particularly devastating pattern of pelvic trauma, combining life-threatening hemorrhage and critical limb ischemia. Although vascular reconstruction is generally recommended, limb salvage may be unrealistic when severe soft-tissue destruction and contamination coexist. Guidance on abandoning salvage attempts and proceeding to early amputation or hemipelvectomy remains limited. METHODS: We reviewed our institutional experience with patients sustaining combined PRI and EIA injury betwe

A Case of External Iliac Artery Injury during Simultaneous Pancreas-Kidney Transplantation Successfully Managed with Resuscitative Endovascular Balloon Occlusion of the Aorta.

INTRODUCTION: Simultaneous pancreas-kidney transplantation (SPK) is an established treatment for patients with type 1 diabetes mellitus complicated by end-stage renal disease. In retransplantation cases, removal of the existing graft may be required, and severe adhesions around the graft increase the risk of vascular injury. In particular, bleeding from deep vessels can be difficult to control because vascular exposure and clamping are often challenging. CASE PRESENTATION: A 45-year-old woman with end-stage renal disease secondary to type 1 diabetes mellitus had previously undergone living-do

Evaluating the safe application of the Bridging Infix method for anterior pelvic fixation in relation to neighbouring anatomical structures.

BACKGROUND: Numerous techniques for anterior pelvic fixation exist, however, a novel minimally invasive subcutaneous technique, the Bridging Infix, has been proposed. Although, little is known regarding the risk of injury to anatomical structures in close proximity to the modified implant site. The current study aimed to assess the implant's proximity to adjacent anatomical structures and palpable bony landmarks to ensure secure and safe fixation. METHODS: An anatomical study was conducted using 50 formalin-fixed cadavers and two fresh-frozen specimens. Dissections were performed to identify

Two-Year Comparative Outcomes of Four Early Clot Removal Techniques for Deep Vein Thrombosis.

BACKGROUND: Despite full adherence to treatment, patients with acute iliofemoral deep vein thrombosis (IFDVT) remain at high risk for developing postthrombotic syndrome (PTS). Consequently, endovascular strategies such as early thrombus removal and catheter-directed thrombolysis (CDT) have been developed. In this study, we compared the long-term effectiveness of endovascular thrombectomy techniques and CDT available at our institution. METHODS: We retrospectively analyzed 91 adults with acute IFDVT who underwent endovascular interventions between 2019 and 2023. Patients were stratified into f
