Selective Androgen Receptor Modulators (SARMs) in Sport: Patterns of Use, Product Contamination, Health Risks and Doping-Control Implications.

Selective androgen receptor modulators (SARMs) were developed to reproduce the anabolic effects of testosterone while sparing the prostate, and are marketed as a safer alternative to anabolic steroids. Tissue selectivity is reproducible in preclinical models but largely metabolic in origin and small in humans, and no SARM has been approved in more than 25 years. In randomised trials, lean body mass rises dose-dependently by roughly 1-1.5 kg, with suppression of testosterone, sex hormone-binding globulin and high-density-lipoprotein cholesterol and, with some compounds, reversible rises in amin

Investigation of the Mechanism of Lorlatinib-Induced Hepatotoxicity: Reactive Metabolites of Lorlatinib Activate Inflammasomes.

Lorlatinib, a third-generation anaplastic lymphoma kinase (ALK) tyrosine kinase inhibitor, is widely used to treat ALK-positive nonsmall cell lung cancer. However, lorlatinib has occasionally been associated with severe liver injury. Immune-mediated adverse events are frequently provoked by certain pharmacological agents or their reactive metabolites. However, the specific contribution of immune pathways to hepatotoxicity caused by lorlatinib is yet to be fully elucidated. In the present study, the potential for direct inflammasome stimulation by lorlatinib was examined in differentiated THP-1

When should a laparoscopic cholecystectomy be performed after ERCP for common bile duct stones combined with gallbladder stones? A meta-analysis of randomized controlled trials.

BACKGROUND: Gallstones combined with common bile duct stones are a common disease in biliary surgery. At present, for gallbladder stones with common bile duct stones, the globally recognized standard treatment is a step-by-step treatment of "endoscopy combined with surgery". First, more urgent and potentially life-threatening bile duct stones are treated, and then gallbladder stones are treated. However, after the common bile duct stones are successfully removed by ERCP, how long should laparoscopic cholecystectomy be performed at intervals? There is a dispute between "early" (defined as withi

Implementation of a Bail-out Strategy in Difficult Laparoscopic Cholecystectomy.

BACKGROUND AND OBJECTIVES: Laparoscopic cholecystectomy is the standard treatment for benign gallbladder disease; however, difficult cases may increase the risk of biliary and vascular injury. This study evaluated surgical approach patterns and major injury rates after adoption of a structured bail-out approach. METHODS: This retrospective cohort study included adult patients who underwent cholecystectomy between Sep 2012 and Jul 2025. Patients were divided into a preimplementation period before Jun 2022 and a bail-out period from Jun 2022 to Jul 2025. Demographic characteristics, surgical ap

Baveno VI criteria validation and development of a new non-invasive strategy for varices rule-out in patients with HCC (SAPHE-R: Stiffness, ALBI, Platelets, HEpatocellular carcinoma Rule).

BACKGROUND AND AIMS: Accuracy of Baveno criteria for avoiding endoscopy in patients with hepatocellular carcinoma (HCC) is uncertain, and systematic upper gastrointestinal endoscopy (UGE) may not be necessary. We evaluated Baveno VI criteria performance in this population and developed an alternative non-invasive rule to exclude esophagogastric varices of any grade, rather than high-risk varices (HRV). METHODS: Patients with BCLC 0-C HCC and compensated advanced chronic liver disease (cACLD) from Italian Liver Cancer (ITA.LI.CA) database, with available liver stiffness measurement (LSM), UGE,

Navigating the Pancreatic Duct: Up-to-Date Radiologic Expedition through Intraluminal Pathologic Entities.

Several congenital, traumatic, infectious, inflammatory, and neoplastic processes, as well as iatrogenic interventions, can alter intraluminal contents of the pancreatic duct, leading to adverse outcomes. Variant ductal anatomy may result in clinically significant complications such as duodenal obstruction, acute and chronic pancreatitis, and increased risk of developing gallbladder or biliary malignancies. The presence of calculi, blood, pus, and other unintended iatrogenic material in the pancreatic duct can be associated with a range of complications. Neoplastic involvement of the pancreati

Polystyrene nanoplastics trigger ferroptosis-dependent liver injury caused by ACSL4 overaccumulation.

Nanoplastics (NPs) are persistent environmental pollutants that can accumulate in the liver, but the mechanisms of their hepatotoxicity remain poorly understood. Here, using AML12 cells and a mouse model, we investigated how polystyrene nanoplastics (PS-NPs) cause liver injury. Exposure to PS-NPs induced dose-dependent cytotoxicity and ferroptosis, evidenced by elevated Fe²⁺ and reactive oxygen species (ROS) levels, enhanced lipid peroxidation, glutathione peroxidase 4 (GPX4) downregulation, and acyl-CoA synthetase long chain family member 4 (ACSL4) upregulation. PS-NPs exposure increased hepa

Histone acetyltransferases and deacetylase 6 (HDAC6) Promotes Hepatic Stellate Cell Activation and Liver Fibrosis.

Liver fibrosis results from excessive extracellular matrix (ECM) deposition following chronic hepatocyte injury. Hepatic stellate cell (HSC) activation is central to fibrogenesis; however, the regulatory mechanisms controlling this process remain unclear. Protein acetylation, regulated by histone acetyltransferases and deacetylases (HDACs), modulates gene expression and protein stability. To date, no studies have explored acetylation dynamics during HSC activation using proteomic approaches. In this study, we investigated the role of acetylation in HSC activation and liver fibrosis. Proteomic

Discovery of Potent, Selective DNMT1 PROTAC Degraders: Simultaneous Blockade of Catalytic and Non-Catalytic Functions for Liver Fibrosis Therapy.

Liver fibrosis is a progressive pathological condition driven by chronic hepatic injury and currently lacks effective therapies. DNA methyltransferase 1 (DNMT1) promotes fibrogenesis through both catalytic and non-catalytic mechanisms. To simultaneously target these dual functions, we designed and synthesized a series of DNMT1-targeting PROTAC degraders based on DC-517. Our optimization campaign culminated in compound B7, which emerged as the most potent agent, mediating ubiquitin-proteasomal degradation of DNMT1. In vitro, B7 reduced global DNA methylation and altered readouts associated with

The Gut-Liver Axis Mediates Liver Injury Induced by Low-Molecular-Weight Polycyclic Aromatic Hydrocarbons Through TLR4-Dependent Inflammation and Bile Acid Dysregulation.

Low-molecular-weight polycyclic aromatic hydrocarbons (LMW-PAHs) are ubiquitous dietary contaminants in grilled, smoked, and thermally processed foods, yet their hepatotoxic mechanisms remain poorly understood. We established a rat model co-exposed to fluorene (Flu) and phenanthrene (Phe) at low, medium, and high doses, validated by urinary metabolites and colonic AHR/CYP1A1. We assessed colonic barrier integrity, gut microbiota (GM) composition, lipopolysaccharide (LPS) translocation, hepatic inflammation, oxidative stress, lipid metabolism, and gut-liver metabolomic profiles. Flu/Phe co-expo

Lycium barbarum polysaccharides and melatonin alleviate cadmium-induced hepatic metabolic injury associated with alterations in PI3K/AKT signaling.

Lycium barbarum polysaccharides (LBP), bioactive macromolecules derived from Lycium barbarum, exhibit metabolic regulatory and hepatoprotective activities. Cadmium (Cd) exposure is a major environmental concern associated with hepatic metabolic dysfunction and liver injury; however, the potential protective role of LBP against Cd-induced hepatic dysfunction and the effects of its combined administration with melatonin (MT) remain incompletely understood. In this study, we investigated the protective effects of LBP, alone or in combination with MT, in a mouse model of chronic Cd exposure. Cd ex

Aqueous extract of Artemisia capillaris Thunb. herbal tea alleviates liver injury in MASLD mice and lipotoxicity in HepG2 cells by inhibiting apoptosis and promoting autophagy.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a chronic metabolic disorder characterized by diffuse hepatic steatosis. Excessive lipid accumulation induced by a high-fat diet can trigger hepatocyte apoptosis and impair autophagic flux, thereby aggravating liver injury. Artemisia capillaris Thunb. herbal tea contains multiple bioactive constituents with hepatoprotective, choleretic, and lipid-regulatory activities. Therefore, this study aimed to evaluate the protective effects of Artemisia capillaris Thunb. herbal tea aqueous extract (ACTE) against MASLD-associated hepatic

Stepwise Design of a High Signal-to-Background Ratio Dual-Emission Probe for Imaging Mitochondrial HClO and Microenvironmental Changes in Diabetic Liver and Kidney Injury.

Diabetic hepatorenal injury is linked to oxidative stress, mitochondrial dysfunction, and ferroptosis. Elevated hypochlorous acid (HClO) levels, together with alterations in mitochondrial polarity and viscosity, are key indicators of diabetic hepatorenal injury. Early, in situ, real-time monitoring of these signals is essential for disease assessment and therapeutic intervention. However, most existing fluorescent probes target only single analytes or organs, restricting multi-signal and dual-organ monitoring, while their imaging signal-to-background ratios (SBRs) remain suboptimal. Changes in the gut microbiome during fasting-induced molting and its effects on gut-liver function.

Fasting-induced molting (FIM) rejuvenates the laying cycle in hens; however, the fasting process may result in intestinal dysbiosis, which compromises host immunity and disrupts the intestine-liver function. Consequently, investigating the changes in gut microbiota during FIM and its impacts on gut-liver metabolic homeostasis is crucial for enhancing the efficiency of FIM in laying hens. Here, a total of 90 laying hens, aged 60 weeks, were selected for the FIM. Samples were collected at four time points: the day before fasting, the 15th day of fasting, the 5th day of refeeding, and the 47th da

Refinement of Structural Alerts for Hepatic Steatosis.

With the growing focus upon application of new approach methodologies (NAMs) within regulatory risk assessment of chemical substances, there is a clear need for human-relevant in silico models to predict toxicity associated with chemical exposure. This work describes the development of an in silico profiler for hepatic steatosis, a condition that results from the excess accumulation of fat within the liver. Toxicological data for chemicals were compiled from the literature, and expert judgments were made regarding their potential to induce steatosis. This list was used to test the 214 rules de

Ginsenoside Rb3 Combined with 20(S)-Protopanaxadiol Ameliorates Hypercholesterolemia via the Regulation of Lipid Metabolism and Gut Microbiota.

Hypercholesterolemia is a major contributor to cardiovascular disease and imposes a serious global disease burden. The stems and leaves of Panax notoginseng, as edible parts of the plant, are rich in ginsenoside Rb3. However, the lipid-lowering effects and mechanism of ginsenoside Rb3 remain unclear, which limits the utilization of Panax notoginseng stems and leaves. Our results showed that ginsenoside Rb3, particularly with PPD, inhibited cholesterol absorption, enhanced LDLR-mediated LDL uptake, reduced body weight, alleviated dyslipidemia, and attenuated liver injury and lipid accumulation.

Hyperoside Derived from Actinidia arguta Bee Pollen Alleviates Zearalenone-Induced Hepatotoxicity and Oxidative Stress in Piglets via the PI3K/AKT Signaling Pathway.

Zearalenone (ZEA) is a prevalent mycotoxin inducing hepatotoxic effects. Hyperoside (HYP), a flavonoid derived from Actinidia arguta bee pollen, exhibits hepatoprotective properties against ZEA; however, the molecular mechanisms underlying this protection remain incompletely characterized. This study integrated in vivo piglet models, in vitro porcine hepatocytes, network pharmacology, and molecular docking to characterize the molecular targets of HYP. The results demonstrated that HYP significantly attenuated ZEA-induced hepatic lesions and oxidative stress. Computational analyses pinpointed t
