One-year recovery after lateral retinaculum release combined with chondroplasty in patients with lateral patellar compression syndrome.

PURPOSE: Lateral patellar compression syndrome (LPCS) is characterized by a persistent abnormally high stress exerted on the lateral articular surface of the patella due to lateral patellar tilt without dislocation and lateral retinaculum contracture, leading to anterior knee pain. The purpose of this study is to evaluate the efficacy and prognosis of lateral retinaculum release (LRR) combined with chondroplasty in the treatment of LPCS. METHODS: This retrospective study evaluated 40 patients who underwent LRR combined with chondroplasty for LPCS between 2020 and 2021. The assessment included

Drug-induced liver injury secondary to anakinra in a patient with systemic juvenile idiopathic arthritis-Case report and literature overview.

BACKGROUND: Anakinra, an interleukin-1 receptor antagonist, is widely used to manage systemic juvenile idiopathic arthritis (sJIA) and related inflammatory conditions. While generally well tolerated, rare cases of drug-induced liver injury (DILI) have been reported. METHODS: In addition to a literature review, we present a pediatric case of severe DILI attributed to anakinra. The case is of a 14-year-old male with sJIA complicated by macrophage activation syndrome (MAS) who developed DILI following the initiation of anakrina. RESULTS: Anakrina was started after initial corticosteroid treatme

Andrographolide-induced PANoptosis underlies its multiple organ toxicity in mice.

Andrographolide (Andro), the major bioactive component of Andrographis paniculata, exhibits potent anti-inflammatory properties but has raised safety concerns due to reported organ toxicity. This study aimed to investigate the mechanisms underlying Andro's in vitro and in vivo toxicity. In mice, single dose (≤100 mg/kg) Andro administration showed no acute toxicity, with no overt histopathological organ injury. But repeated administration of the same dose of Andro triggered damage in lung, liver, uterus, and kidney, characterized by pulmonary alveolar disruption, renal tubular edema, and eleva

Consistent Adherence to Physical Activity Guidelines and Digestive System Cancer Risk and Mortality.

IMPORTANCE: Growing evidence suggests that physical activity protects against digestive system cancers (DSCs). However, it remains largely unknown whether consistently reaching the physical activity guidelines (≥7.5 metabolic equivalent task [MET]-hours/week) is associated with lower DSC risk or whether a much higher level is needed. OBJECTIVE: To examine the association between physical activity and DSC risk and mortality with a focus on the optimal amount and long-term consistency in performing the recommended level over a long period of time. DESIGN, SETTING, AND PARTICIPANTS: This popula

De novo formation of tuft and goblet cells protects the intrapancreatic biliary duct system from inflammatory injury.

The biliary tree is susceptible to inflammatory injury that can result in severe liver damage. However, the intrapancreatic biliary duct system is largely refractory to damage caused by pancreatitis; the mechanisms that protect these ducts from injury remain unknown. Here, we demonstrate that, although serine/threonine-protein kinase DCLK1 (DCLK1) + tuft cells and mucin-2 (MUC2) + goblet cells were largely restricted to the ampulla of non-injured mice, they exhibited a marked increase in the ampulla and a progressive appearance in the intrapancreatic biliary duct system after injury. The incre

Innovations to Improve Survival in Acute Liver Failure.

Acute liver failure (ALF) is a medical emergency with high mortality rates. Liver transplantation is the optimal treatment for eligible patients. However, it necessitates access to transplantation services, significant financial resources, and frequently poses challenges in ensuring safe transportation. Advances in intensive care have improved survival rates from 20% to over 60%. Key factors include early prognostication, dynamic modeling, neurocritical monitoring, and liver support systems. There is a significant need for better public health services for preventing and managing ALF in India,

Unraveling the gastrointestinal tract's response to alcohol binges: Neutrophil recruitment, neutrophil extracellular traps, and intestinal injury.

BACKGROUND: Excessive alcohol consumption results in gastrointestinal (GI) tract dysfunction, including disruption of the intestinal barrier and exposure of the liver to microbes and gut-derived pathogen-associated molecular patterns (PAMPs). The upper GI tract is exposed to the highest amount of alcohol, but little is known about alcohol's impact on GI inflammation. This study aimed to evaluate the initial effects of alcohol binges on GI inflammation. METHODS: A murine model of binge drinking was established with daily oral gavages of alcohol (3.5 g/kg) for 3 days in C57BL/6J female mice. Th

Primary extra-gastrointestinal stromal tumor of the gallbladder: A case report.

BACKGROUND: Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract, predominantly arising in the stomach (approximately 60%) and small intestine (approximately 30%), and accounting for 1%-3% of all gastrointestinal malignancies. In most cases, GISTs originate within the gastrointestinal tract; However, in rare instances, they may develop in extra-GISTs (EGISTs). Among these, gallbladder-derived EGISTs are exceedingly uncommon, with only nine cases reported to date. CASE SUMMARY: We present the case of a 66-year-old woman who presented w

FAK-Activated Mucosal Healing Promotes Resistance to Reinjury.

BACKGROUND: Gastrointestinal (GI) mucosal injury is a frequent complication of long-term nonsteroidal anti-inflammatory drug (NSAID) use. Effective mucosal healing requires coordinated epithelial migration, proliferation, and angiogenesis, which may be influenced by focal adhesion kinase (FAK). This study aimed to determine whether our newly developed FAK activators promote intestinal mucosal healing by enhancing angiogenesis and whether FAK activation increases resistance to reinjury. METHODS: Ischemic jejunal ulcers were induced in C57BL/6 mice. After 24 h, mice received intraperitoneal inj

Meta-Analysis of the Efficacy of Spirulina Intervention in Mitigating the Negative Impact of Heat Stress on Production Physiology and Health Indices of Broilers.

There is an increasing number of published studies on the effect of spirulina (an aquatic plant known for its high nutritional value and potential health benefits) intervention on productivity and health of heat-stressed broilers. However, the effect of spirulina intervention on the performance of broilers exposed to heat stress is poorly understood. A better understanding of the productivity of heat-stressed broilers on spirulina intervention will assist in utilizing these data in decision-support systems in the poultry industry. Therefore, this study aimed to determine the effectiveness of s

Documentation and Follow-Up of Incidental Findings Found on Trauma CT Scan Imaging: A Scoping Review.

INTRODUCTION: Incidental findings (IFs) on computed tomography (CT) scans are increasingly detected during trauma workup, with prevalence rates exceeding 50% in some studies. While early detection of potentially serious conditions is critical, less than half of IFs are documented, resulting in poor follow-up. We aimed to characterize IFs, describe IF documentation and follow-up frequency, and recognize interventions to improve IF reporting and follow-up. METHODS: A scoping review was conducted using Medline and PubMed, adhering to Preferred Reporting Items for Systematic Reviews and Meta-Anal

Protective Effects of Velvet Antler Polypeptides on Cyclophosphamide-Induced Myelosuppression in Mouse and Bone Marrow Mesenchymal Stem Cells.

Background: Myelosuppression is one of the most common chemotherapy side effects, seriously threatening the quality of life of cancer patients. Studies have shown that velvet antler polypeptides (VAPs) could enhance immunity and anti-aging and also have a hematopoietic-promoting effect. However, there are relatively few studies on the treatment of myelosuppression with VAPs, and the therapeutic mechanism remains unclear. Methods: This study employed both in vitro and in vivo models to explore the mechanism of VAPs against myelosuppression. In this study, the cyclophosphamide (CTX)-induced bone

A Review of the Endoscopic Treatment for Bile Leak Following Cholecystectomy and Hepatic Surgery.

Bile leak occurs in 2-25% of liver transplant, 3-27% of hepatic resection, and 0.1-4% of cholecystectomy cases. The clinical course of bile leak varies depending on the type of surgery that caused the fistula, as well as the type, severity, and timing of bile duct injury. Although infections resulting from bile leak can be life-threatening, the introduction of endoscopic treatment has enabled some patients to avoid reoperation and has reduced the negative impact on quality of life associated with external fistulas for percutaneous drainage. Endoscopic interventions, such as sphincterotomy and

Advances in EUS-Guided Biliary Drainage for the Management of Pancreatic Cancer.

The indications for biliary drainage in cases of pancreatic head tumors with biliary obstruction are well established. ERCP with stent placement has long been the gold standard technique, outperforming surgery or percutaneous drainage. However, in cases of distal malignant biliary obstruction, ERCP becomes more complex, increasing the risk of complications. The advent of therapeutic endoscopic ultrasound (EUS), particularly EUS-choledochoduodenostomy (EUS-CDS) and EUS-hepaticogastrostomy (EUS-HGS), has transformed the management of distal malignant biliary obstruction in the case of pancreatic

The anatomy of the common hepatic artery: a computed tomography angiography analysis.

BACKGROUND: The common hepatic artery (CHA) is a major vessel supplying the liver, stomach, pancreas, and duodenum. While typically arising from the celiac trunk, its origin, course, and morphometry exhibit considerable variability, which may complicate hepatobiliary and pancreatic surgery, liver transplantation, and interventional procedures. This study aimed to comprehensively assess the anatomy and morphometry of the CHA using computed tomography angiography (CTA), with attention to clinically relevant variations. MATERIALS AND METHODS: A retrospective analysis was performed on 73 consecut

Histopathological Scoring of Renal Injury in an Experimental Model of Acute Pancreatitis Caused by Karwinskia humboldtiana Fruit: Exploring Its Potential Utility in the Treatment of Pancreatic Cancer.

Background: Karwinskia humboldtiana (Kh) is a toxic plant that produces a fruit that, when ingested in large quantities, causes damage to the lungs, liver, kidneys, pancreas, and duodenum. Pancreatic damage was measured using a semiquantitative score, revealing that it is progressive and characterized by selective apoptosis and necrosis of the exocrine portion. However, renal injury has not been evaluated using injury scales and has only been reported descriptively. Pancreatic adenocarcinoma is one of the top five causes of cancer death in the United States, and options for its treatment are l
