Diffuse peritonitis secondary to spontaneous perforation of pyometra in an elderly female.

An elderly female in her 80s with multiple comorbidities presented to our emergency department with a 1-day history of progressive abdominal pain. Physical examination revealed peritonitis on abdominal examination. Laboratory examinations were concerning for sepsis. Contrast-enhanced CT of abdomen and pelvis was suspicious for ischaemic colitis and intra-abdominal abscess. In the setting of peritonitis, exploratory laparotomy was performed and found to have a spontaneous rupture of the uterine fundus causing purulent peritonitis. This was managed with the total abdominal hysterectomy and bilat

Delayed Presentation of Penetrating Abdominal Trauma From a Nail Gun: A Case Report.

Penetrating abdominal trauma typically presents in an acute manner, and most require emergent intervention due to small bowel, large bowel, and liver injury. Here, we report a 53-year-old man who sustained multiple penetrating intra-abdominal injuries from a nail gun. The patient did not seek medical attention until four weeks after the incident. He presented with worsening abdominal pain and underwent exploratory laparotomy and removal of three intact nails. Fortunately, there were no injuries to the intra-abdominal organs, and the patient had an uneventful post-operative course and was seen

Pathophysiological Mechanisms Linking COVID-19 and Acute Surgical Abdomen: A Literature Review.

Acute surgical abdomen is characterized by intense, sudden abdominal pain due to intra-abdominal conditions requiring prompt surgical intervention. The coronavirus disease 2019 (COVID-19) pandemic has led to various complications related to the disease's complex pathophysiological mechanisms, hence the hypothesis of COVID-19-induced acute abdominal surgical pathologies. The connection between acute surgical abdomen and COVID-19 involves two primary mechanisms. First, there is the presence of angiotensin-converting enzyme 2 (ACE2) receptors in multiple abdominal organs. This facilitates the cyt

Primary repair versus resection for American Association for the Surgery of Trauma grades I and II colon injuries: Does the management approach really matter? An Eastern Association for the Surgery of Trauma multicenter trial.

BACKGROUND: The management of traumatic low-grade (American Association for the Surgery of Trauma [AAST] grades I and II) colon injuries has evolved. Recent data suggest that primary repair (PR) or resection over colostomy decreases morbidity and mortality. However, data comparing patients undergoing PR versus resection with anastomosis (RWA) are lacking. We hypothesized that patients presenting with low-grade colon injuries undergoing PR would have fewer postoperative complications than patients undergoing RWA. METHODS: This was a retrospective, multicenter analysis of all patients presentin

To validate the F-AST score as a CT decision tool in pediatric blunt abdominal trauma: A prospective diagnostic accuracy study.

INTRODUCTION: Focused assessment with sonography for trauma (FAST) is a valuable tool in managing adult trauma patients but has poor utility in children due to its poor sensitivity. Multiple studies have explored the addition of liver enzymes to improve the diagnostic accuracy of FAST in pediatric blunt abdominal trauma (BAT) patients. A retrospective study at our institute used FAST findings with liver enzymes (F-AST score) to rule out Intra-Abdominal Injury (IAI) in pediatric BAT patients, concluding that normal liver enzymes and a negative FAST scan can effectively rule out IAI. This study

Cumulative intra-abdominal pressure exposure and dynamic trajectories in ICU-admitted patients reveal prognostic determinants of severe acute pancreatitis.

BACKGROUND: Intra-abdominal pressure (IAP) critically drives organ failure progression in severe acute pancreatitis (SAP). However, traditional static IAP monitoring inadequately captures dynamic injury evolution. This study aimed to assess the impact of cumulative IAP exposure (CumIAP) and dynamic trajectories on the prognosis of SAP. METHODS: This retrospective cohort study analyzed 1,008 ICU-admitted SAP patients from the Jiangxi cohort (2005-2023) and 83 from MIMIC-IV (2008-2019). CumIAP was quantified via time-weighted integration of serial IAP measurements. Multivariate Cox regression m

Integrated trauma care successfully treats a severe case of combined steel bar penetration injury to the head, neck, chest, and abdomen: A case report and literature review.

RATIONALE: Penetrating injuries spanning multiple anatomical regions carry significant risks of morbidity and mortality. Successful management depends on coordinated prehospital care, expedited imaging, and a multidisciplinary surgical approach. This case report details the successful management of an exceptionally severe combined penetrating injury involving the head, neck, chest, and abdomen, emphasizing the critical role of an integrated trauma care system. It serves to highlight the importance of standardized trauma protocols for optimizing outcomes in such complex and life-threatening inj

Revolution in sepsis: a symptoms-based to a systems-based approach?

Severe infection and sepsis are medical emergencies. High morbidity and mortality are linked to CNS dysfunction, excessive inflammation, immune compromise, coagulopathy and multiple organ dysfunction. Males appear to have a higher risk of mortality than females. Currently, there are few or no effective drug therapies to protect the brain, maintain the blood brain barrier, resolve excessive inflammation and reduce secondary injury in other vital organs. We propose a major reason for lack of progress is a consequence of the treat-as-you-go, single-nodal target approach, rather than a more integr

Severe acute pancreatitis complicated with intra-abdominal infection secondary to trauma: A case report.

BACKGROUND: Pancreatic trauma (PT) is rare among traumatic injuries and has a low incidence, but it can still lead to severe infectious complications, resulting in a high mortality rate. Acute pancreatitis (AP) is a common complication after PT, and when combined with organ dysfunction and sepsis, it will result in a poorer prognosis. CASE SUMMARY: We report a 25-year-old patient with multiple organ injuries, including the pancreas, due to abdominal trauma, who developed necrotising pancreatitis secondary to emergency caesarean section, combined with intra-abdominal infection (IAI). [Therapeutic and diagnostic complex in intestinal failure syndrome in patients with closed abdominal trauma].

UNLABELLED: Currently, severe combined abdominal trauma ranks third among all causes of mortality In Russia, second only to cardiovascular and oncologic diseases. In the period from 2019 to 2020 in our country, a slight decrease in traumatism is noted due to a decrease in the number of traffic accidents as the main cause of combined and multiple trauma. The number of abdominal injuries from the total number of injuries In Russian regions ranges from 1.5 to 36.5% and is accompanied by a high level of disability (25-80% in combined trauma and 5-8% in isolated trauma). Despite modern medical adva

Emergency pancreaticoduodenectomy for complex pancreaticoduodenal damage with multiple organ injuries following blunt abdominal trauma: A case report and literature review.

INTRODUCTION: Pancreaticoduodenectomy is a complex surgical procedure with significant potential for complications such as pancreatic fistula, bile leakage, intra-abdominal abscesses, and hemorrhage. Emergency pancreaticoduodenectomy (EPD) performed for traumatic injuries carries even greater risks due to the patient's severely unstable condition upon admission. While the literature recommends that EPD be reserved for hemodynamically stable trauma patients, there are scenarios where it may be the last resort to save the patient's life. CASE PRESENTATION: A 49-year-old male presented in the em

Anterior retroperitoneal approach in constructing thymokidney organs in swine for xenotransplantation.

INTRODUCTION: Thymokidneys (TK) have been constructed to transplant life-supporting kidney grafts containing donor thymic tissue to induce transplant tolerance. Historically, TKs were constructed by inserting pieces of thymus tissue under the kidney capsule using an intra-abdominal or posterior retroperitoneal (lateral/flank) approach. The intra-abdominal approach is technically easier but causes intra-abdominal adhesions and makes kidney procurement more challenging. The posterior retroperitoneal approach causes fewer complications, but thymus tissue implantation is technically demanding due

Compact Arterial Monitoring Device Use in Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA): A Simple Validation Study in Swine.

Introduction Hemorrhage is the leading cause of preventable death in trauma in both the military and civilian settings worldwide. Medical studies from Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) informed change in military prehospital medicine by influencing widespread tourniquet distribution and training on their use to stop life-threatening extremity hemorrhage. In the military setting, there has been a significant reduction in preventable death due to extremity exsanguination since the widespread implementation of tourniquets within the Department of Defense. However,

An Uncommon Manifestation of Systemic Lupus Erythematosus as Lupus Enteritis With Intestinal Pseudo-Obstruction and Invasive Candidiasis.

Systemic lupus erythematosus (SLE) is a multifaceted autoimmune disorder, occasionally presenting with rare complications like lupus enteritis (LE) and intestinal pseudo-obstruction (IPO). We present a unique case of a 32-year-old woman with LE and IPO, complicated by invasive candidiasis, as an initial manifestation of SLE. The patient presented with a 15-day history of abdominal pain, vomiting, and poor oral intake, and was initially misdiagnosed with infective enterocolitis. Examination revealed abdominal distension and absent bowel sounds due to IPO, alongside severe hypokalemia and signs

Polymicrobial infection presenting as non-clostridial gas gangrene in a patient with an open pelvic ring fracture accompanied by abdominal evisceration: a case report.

BACKGROUND: Open pelvic fractures are rare but represent a serious clinical problem with high mortality rates. Acute mortality is often associated with hemorrhage, whereas delayed mortality is most often associated with sepsis and multiple organ failure. We report a case of Wang's classification of type II open pelvic ring fracture with hemorrhagic shock and septic shock from gas gangrene. CASE DESCRIPTION: A healthy 41-year-old man presented with an open pelvic ring fracture and a laceration in the lower abdomen sustained at an ironworks. His hemodynamic status was unstable. circFLNA promotes intestinal injury during abdominal sepsis through Fas-mediated apoptosis pathway by sponging miR-766-3p.

BACKGROUND: Intra-abdominal infections are the second most common cause of sepsis in the intensive care unit. Intestinal epithelial injury due to abdominal sepsis results in a variety of pathological changes, such as intestinal bacteria and toxins entering the blood, leading to persistent systemic inflammation and multiple organ dysfunction. The increased apoptosis of intestinal epithelial cells induced by sepsis further exacerbates the progression of sepsis. Although several studies have revealed that circRNAs are involved in intestinal epithelial injury in sepsis, few studies have identified

Efficacy and safety of CounterFlow in animal models of hemorrhage.

INTRODUCTION: Hemorrhage is a major cause of battlefield mortality, contributing to 91% of potentially survivable combat-related deaths. Death from hemorrhage often occurs in pre-hospital settings, and more efficacious hemorrhage control interventions are needed to extend survival and enable casualties to reach definitive surgery. CounterFlow is a novel topical hemostatic agent that uses self-propelling particles to deliver thrombin and tranexamic acid against active bleeding. It achieves and maintains hemostasis at the site of injury through a combination of mechanical, biological, and chemic
