Spontaneous Tumour Lysis Syndrome in Metastatic Endometrial Carcinoma.

Tumour lysis syndrome (TLS) is a potentially fatal oncological emergency characterised by rapid cellular breakdown resulting in hyperkalaemia, hyperphosphataemia, hyperuricaemia, and hypocalcaemia, with subsequent acute kidney injury, cardiac dysrhythmias, and multi-organ dysfunction. It most commonly occurs following initiation of anti-cancer therapy in highly proliferative haematological malignancies. Spontaneous tumour lysis syndrome (STLS), occurring in the absence of a therapeutic trigger, is considerably rarer, particularly in solid tumours. We report a 55-year-old female presenting with

Comparative antibacterial, synergistic, and anti-biofilm efficacy of metal ions against multidrug-resistant pathogens on dairy equipment surfaces.

UNLABELLED: This study evaluated the antibacterial and anti-biofilm activities of selected metal ions (Al3+, Cu2+, Au3+, Ag+, and Zn2+) against extended-spectrum β-lactamase-producing Escherichia coli (ESBL-E. coli), Methicillin-Resistant Staphylococcus aureus (MRSA), and vancomycin-resistant Enterococcus faecalis (VRE). The rapid emergence of antimicrobial resistance (AMR) has intensified the threat posed by multidrug-resistant (MDR) pathogens, particularly biofilm-forming bacteria associated with dairy equipment surfaces. Among the tested metals, silver ions showed the strongest antibacteria

A Network Meta-Analysis of Resuscitative Endovascular Balloon Occlusion of the Aorta in the Treatment of Noncompressible Torso Hemorrhage: Evaluating Mortality and Acute Kidney Injury Outcomes Compared to Alternative Surgical Techniques.

INTRODUCTION: Noncompressible torso hemorrhage (NCTH) is a major cause of preventable death in both civilian and military trauma settings. It involves vascular disruptions within the thoracic cavity, major axial torso vessels, severe solid organ injuries, or pelvic ring fractures leading to hemodynamic instability resistant to direct pressure control. Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) is a minimally invasive technique used to temporarily occlude aortic blood flow, mitigating distal hemorrhage and preserving proximal perfusion until surgical intervention. This me

Profiling the T cell response to polypropylene mesh in a non-human primate sacrocolpopexy model.

Polypropylene mesh (PPM) improves anatomic outcomes in pelvic organ prolapse (POP) repair, yet complications-most commonly pain and mesh exposure-occur in ∼10 % of cases. Clinically, meshes that are implanted flat often demonstrate striking deformation at explanation, including pore collapse and wrinkling. Both computational models and in vivo studies confirm that mesh geometry changes substantially after tensioning during prolapse repair. Although T cells have been implicated in mesh-related complications, the specific impact of mesh deformation on adaptive immunity is not fully understood. T

Laparoscopic Management of a Migrated Intrauterine Device Causing a Partial-Thickness Rectal Injury Diagnosed Following Contraceptive Failure: A Case Report.

Intrauterine devices (IUDs) are widely used long-acting reversible contraceptives due to their efficacy and safety. Although generally well tolerated, uterine perforation with subsequent migration into adjacent pelvic or abdominal organs is a rare but potentially serious complication. Rectal involvement is exceptionally uncommon and often clinically silent. We report the case of a 32-year-old woman who presented with complete abortion of an unintended pregnancy 18 months after IUD insertion. On physical examination, the IUD strings could not be visualized, and transvaginal ultrasound failed to

Obstetric trauma and its association with race and immigration: a population-based cohort study in Ontario, Canada.

BACKGROUND: The factors involved in the association between obstetric trauma and the race of the birthing parent are uncertain. We sought to assess the association between race and obstetric trauma, and the impact of immigration-related factors on this association. METHODS: We performed a retrospective, population-based cohort study of people with singleton, cephalic, vaginal births at 20 weeks' gestation or later in Ontario, Canada, between 2012 and 2021. The race of the birthing parent was recorded by care providers in prespecified categories (Asian, Black, White, other, and missing). The p

Effects of non-invasive spinal cord stimulation on autonomic function in individuals with subacute spinal cord injury: A pilot clinical trial protocol.

INTRODUCTION: Spinal cord injury (SCI) at or above the thoracic sixth spinal cord level disrupts descending sympathetic and parasympathetic control, leading to severe autonomic dysfunctions including cardiovascular and pelvic organ function. These complications adversely affect the quality of life and are associated with increased morbidity and mortality after SCI. Transcutaneous spinal cord stimulation (tSCS) may offer therapeutic benefits for these functions. The safety of tSCS in subacute SCI, however, remains unknown. Therefore, this study aims to evaluate the feasibility of tSCS for auton

Is There a Difference in Hiatal Area on Straining Between Type I and II Levator Avulsions?

INTRODUCTION AND HYPOTHESIS: A distinction can be made between levator avulsions that preserve the appearance of a connection to the pelvic sidewall (type I), and those that do not (type II). We evaluated whether avulsion type affects hiatal area on straining. METHODS: This retrospective observational study used archived patient data from a tertiary urogynaecological service between 2/2019 and 12/2020. All patients underwent a standardized interview, POP-Q examination, and 4D pelvic floor ultrasound. On analysing the archived volume data, the first author assessed hiatal area on maximum strai

Criteria for the qualification of geriatric patients to the trauma center.

BACKGROUND: The growth of the geriatric population, resulting in an exponential increase in the number of injured older people, determines the necessity of ensuring adequate access to specialized units such as trauma centers (TCs). The specificity of pathophysiological processes that progress with age, worsening the body's response to trauma, makes it difficult to develop an optimal triage protocol for geriatric patients, reducing high undertriage, i.e., underestimation of injuries and referral to lower-reference units. AIM: To verify the TC qualification criteria for 65+ patients and analyze

High-Velocity Penetrating Pelvic Injury With Iron Rod: A Case Report and Literature Review.

Penetrating pelvic impalement injuries caused by foreign objects are uncommon and potentially life-threatening due to the risk of severe hemorrhage and multi-organ involvement. We report the case of a 16-year-old female who sustained a penetrating injury when an iron rod/metal rod (handle-like structure) pierced her right gluteal region following a high-velocity road traffic collision. Initial assessment revealed a protruding metal object without active bleeding, and the patient was hemodynamically stable. Preoperative imaging demonstrated the trajectory of the foreign body through the gluteal

Complex Management of Severe Open Pelvic Fracture in a Polytrauma Patient.

Open pelvic fractures are rare but present a significant challenge for trauma surgeons. High mortality rates are frequently associated with hemorrhage during the acute phase, while delayed mortality is typically due to sepsis and multiple organ failure. Effective management of these patients requires a complex, multidisciplinary approach. We report the management of an open pelvic fracture with severe soft tissue injury in a polytrauma patient. The patient sustained multiple injuries, including traumatic brain injury, closed thoracic lesion, and a type C2 open pelvic fracture with extensive so

Predictors of Massive Transfusion in Patients With Hollow Organ Injury After Blunt Multiple Trauma: A Cohort of Blunt Bowel Mesenteric Injury.

BACKGROUND: Massive transfusion (MT) is life-saving for patients with exsanguination, especially after blunt abdominopelvic trauma, due to subtle manifestations. Blunt bowel mesenteric injuries (BBMIs), besides their potential risk of peritonitis, are still one of the few indications for emergency laparotomy for hemorrhagic shock in the era of nonoperative management. Early prediction of the need for MT defined as using ≥ 10 units of packed red blood cells (PRBCs) in 24 h and activation of the MT protocol (MTP) is a critical aspect in resuscitation. Current scoring systems predicting MT are us
