Pelvic ring fracture and erectile dysfunction (PERFECD) - 3 year follow-up cross sectional study.

INTRODUCTION: Pelvic ring fractures are known to be associated with complications associated with adjacent organ injuries, such as the urogenital tract (e.g. erectile dysfunction (ED), which are sometimes diagnosed in a delayed fashion. Therefore, we assessed the quality of life (QoL) and the rate of erectile dysfunction (ED) following pelvic ring fractures at a minimum of 3 years after pelvic ring injury. METHODS: Between January 1, 2016, and December 31, 2020, adult male patients (≥ 18 years) with pelvic ring injuries were included in the study. Fractures were classified according to the Yo

Vaginoplasty in female bladder exstrophy-epispadias complex: Analysis of operative technique, outcomes, and complications.

INTRODUCTION AND OBJECTIVE: Bladder exstrophy-epispadias complex (BEEC) is a rare, but serious congenital malformation. After achieving urinary continence and upper tract preservation, genital reconstruction is performed for function and cosmesis. We evaluated our institutional experience with vaginoplasty, including technical considerations and post-operative outcomes. PATIENTS AND METHODS: An IRB-approved database was used to conduct an institutional retrospective review of BEEC patients undergoing vaginoplasty from 2000 to 2024. Patients with XY karyotype, primary vaginoplasty during BEEC

Abdominal bleeding secondary to isolated mesenteric injury following blunt abdominal trauma: A case report.

RATIONALE: Isolated mesenteric injury often results from traffic accidents causing blunt abdominal trauma, exhibit nonspecific symptoms and signs. Most mesenteric injuries occur concurrently with injuries to other organs, such as the liver and spleen. As a result, the incidence of isolated mesenteric injury is very low and often misdiagnosed. PATIENT CONCERNS: A 49-year-old male patient presented with abdominal pain following a traffic accident more than 5 hours before admission. Upon admission, his blood pressure was normal but dropped sharply in a short time and presented with shock. Anemia

Enterocervical Fistula Six Weeks After Laparoscopic Mesh Sacral Colposuspension: A Case Report.

Laparoscopic colposuspension with mesh is a common procedure performed to treat apical pelvic organ prolapse refractory to non-surgical interventions. Mesh complications involving a fistula are an uncommon event that typically occurs months or years later. We report a case complicated by enterocervical fistula that occurred six weeks after mesh placement. This was the case of a 67-year-old female with no surgical history who underwent a laparoscopic supracervical hysterectomy, bilateral salpingo-oopherectomy, laparoscopic colposuspension with mesh, and midurethral sling for treatment of stage

Rapid, moderate, or slow bleeding? CT analysis of abdominopelvic active vascular contrast extravasation classes and mortality outcomes.

OBJECTIVES: Building on prior findings that active vascular contrast extravasation (AVCE) size is an independent predictor of in-hospital mortality in abdominopelvic hemorrhages, this study aimed to categorize AVCEs using latent profile analysis (LPA) and examine differences in patient characteristics, treatments, and outcomes. METHODS: We retrospectively included consecutive adults with CT-detected AVCE between January 2019 and May 2022. LPA was applied to classify AVCEs based on size-related features, optimizing the number of classes predictive of 24-h and in-hospital mortality. These class

Radiation-induced injury and the gut microbiota: insights from a microbial perspective.

Although radiotherapy is the second most effective cancer treatment, radiation injuries limit its use. About 80% of abdominal-pelvic radiotherapy patients develop acute radiation enteritis, with 20% discontinuing radiotherapy. The lack of effective mitigation measures restricts its clinical application. Recent studies have proposed gut microbiota as a potential biomarker for radiation injuries. However, the interaction between gut microbiota and radiation injuries remains poorly understood. This review summarizes two forms of interaction between gut microbiota and radiation injuries based on t

Reference line lengthening on resting-state magnetic resonance imaging in patients with pelvic organ prolapse seeking surgical treatment.

AIM: Injury to and laxity of the pelvic floor muscles are highly important factors in the etiology of pelvic organ prolapse. When women with pelvic organ prolapse perform the Valsalva maneuver, progressive descent and widening of the levator ani muscle are observed on dynamic magnetic resonance images. However, physical examination of such women often reveals pelvic floor laxity, even in a relaxed state. Therefore, we aimed to verify the hypothesis that sagging of the pelvic floor can be detected on resting-state magnetic resonance images in the supine position. METHODS: We retrospectively ev

Association between serum phosphorus levels and pelvic bone mineral density in U.S. adults aged 18-59 years.

BACKGROUND: Although the role of serum phosphorus in bone metabolism is well-established, its precise connection to bone density in different anatomical regions remains uncertain. The pelvis, a vital load-bearing structure crucial for movement and stability, has received limited attention because of its relationship with serum phosphorus. This study investigated the potential association between serum phosphorus levels and pelvic bone mineral density (BMD) in adults aged 18 to 59 years. METHODS: Using data from the 2015-2018 National Health and Nutrition Examination Surveys NHANES, we applied

Urogenital Hiatus Closure: Facts, Fallacies, and Why a Unified Theory of Hiatal Failure is Needed.

An enlarged urogenital hiatus is as important as apical support or fascial attachment failures in the development of prolapse and is strongly related to operative failure, yet we lack a conceptual model for factors responsible for hiatal failure. For a conceptual model to be valid, it cannot be proven false by empirical observation. We present six clinical observations with which future model development must be consistent. (1) Perineal body damage alone does not explain an enlarged urogenital hiatus. Three women have complete 4th degree lacerations but small hiatuses. (2) Levator damage is no

A rare case of the coexistence of injuries typical of a fall from height.

Falls from heights constitute the second most common cause of suicides in Poland, often resulting in multiple organ injuries. This paper describes a rare case of a 32-year-old man who died after jumping from a four-story building. The autopsy and post-mortem computed tomography analysis revealed a unique coexistence of injuries characteristic of a fall onto extended lower limbs, such as a circular fracture of the base of the skull as well as multiple fractures of the spine, pelvis, and lower limbs. The geodetic analysis enabled the determination of the fall height and flight range, which facil

Approach and Management of Multiple Victims of Civilian Bombing Events With Abdominopelvic Injuries: A 20-Year Institutional Experience in Pakistan.

PurposeBomb blast injuries in non-combat settings have seen a surge in the last two to three decades. Third-world countries like Pakistan have been at the receiving end of these attacks. However, the extent of the damage inflicted in these regions is not fully understood due to gross underreporting. We aim to assist radiologists in identifying common abdominopelvic injuries in bomb blast victims and highlighting specific injury patterns to guide more effective management.MethodsThis was a retrospective observational study designed to analyze abdominopelvic injuries among bomb blast victims tre

Incidence and impact of urogenital sequelae in women following pelvic-ring injuries: a retrospective cohort study.

PURPOSE: Pelvic-ring injuries in women often result in urinary dysfunction owing to the proximity of pelvic organs to the urinary tract, significantly affecting quality of life. However, detailed research on urinary sequelae remains limited. This study aimed to assess the incidence of urinary dysfunction in women after pelvic-ring injuries and to identify risk factors influencing urinary function. METHODS: We conducted a retrospective cohort study of women who underwent osteosynthesis for pelvic-ring injuries between January 2022 and June 2023 with ≥ 12 months of follow-up. Urinary dysfunctio

Transperineal combined two- and three-dimensional ultrasound evaluation of the effects of different delivery methods on levator ani muscles and pelvic organ prolapse.

BACKGROUND: The levator ani muscle (LAM) comprises the main component of female pelvic floor support structure and plays an important role in maintaining the normal position of pelvic organs. Early detection of its injury is critical to ensure the health of the pelvic floor. Vaginal delivery exacerbates LAM injury, and LAM injury changes the normal pelvic floor structure. Therefore, this study used ultrasound to measure the degree of pelvic organ decline enabling qualitative assessment of the severity of prolapse. To investigate whether vaginal delivery leads to greater LAM trauma and pelvic o

A protocolised approach provides a favourable outcome in patients with severe pelvic trauma.

Background Pelvic fractures are often a result of injuries caused by a major force and lead to considerable morbidity and mortality. Despite advances in acute management protocols involving angioembolization as a prime intervention, mortality in complex pelvic fractures remains high. We analysed our experience with the outcomes of a pelvic trauma management protocol using surgical haemostatic techniques over 5 years. Methods We did a retrospective analysis of all patients admitted with pelvic factures from January 2014 to December 2018. Demographic profile, clinical details, imaging findings,

The Use of Glue as an Embolic Agent in Trauma.

Nonoperative management is becoming more common for hemodynamically stable patients with hemorrhagic traumatic injuries. N-butyl 2-cyanoacrylate (NBCA) glue is a safe and effective embolic agent in cases of solid organ traumatic injury, hemorrhage associated with pelvic fractures, as well as deep and superficial trauma. Although glue can be effectively used off-label in these cases (either alone or in combination with other embolization methods, such as microcoils), operators should first understand the mechanism of action, advantages and disadvantages, common applications, and proper preparat
