Duodenal Stenosis With Significant Edema Due to Complication From an Enteral Nutrition Tube: A Case Report.

Enteral nutrition (EN) is a well-established feeding method for individuals with dysphagia or those with intubation. Although EN tube placement is generally well tolerated, complications can sometimes occur. A 60-year-old male was brought to our hospital after being struck by a container that had slid off the trailer bed. He sustained severe traumatic brain injury, pelvic fracture, and open fracture of the lower leg, for which appropriate treatment was initiated. No abnormalities were identified in other organs on CT. As mechanical ventilation management was continued, EN was administered via

Application of Surgical Adhesive Reduces Ureteroureterostomy Leakage.

Their close proximity to abdominal structures and organs places the ureters at risk of iatrogenic injury during abdominal and pelvic surgical procedures. Ureteroureterostomy is the mainstay intervention for the repair of the proximal and middle ureter. However, the anastomosis may be subject to leakage, leading to potentially serious complications, including urinary tract infection, peritonitis, sepsis, and kidney damage. This study addressed the hypothesis that topical application of surgical adhesive reduces acute anastomotic leakage without increasing ureteric resistance. Nine ureters from

Complications of Robotic Pelvic Lymph Node Dissection for Prostate Cancer: An Analysis of the National Surgical Quality Improvement Program Targeted Prostatectomy Database.

INTRODUCTION/BACKGROUND: Treatment of localized prostate cancer includes radical prostatectomy (RP) with or without pelvic lymph node dissection (PLND). While multiple guidelines recommend PLND for staging purposes, recent data has shown questionable therapeutic benefit. Thus, understanding the morbidity associated with PLND is important for counseling patients. We used the American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) targeted prostatectomy database to quantify real-world 30-day postoperative outcomes of patients undergoing contemporary robot-assisted

Perineal structural changes on 3-dimensional endovaginal ultrasound 6 months after difficult vaginal birth: the Hourglass-Band pattern and how it relates to levator ani avulsion and hiatus size.

BACKGROUND: Current evidence indicates that pelvic organ prolapse development after childbirth is strongly associated with an enlarged hiatus. However, few childbirth-related injuries beyond anal sphincter lacerations and levator ani avulsion-which explain less than a quarter of hiatal enlargement-have been investigated. Other types of injuries that lead to an enlarged hiatus and contribute to prolapse development remain incompletely explored. The perineal complex components (perineal membrane, perineal body, and levator ani) are responsible for hiatal closure and form a distinct anatomical an

Grade V AAST Intestinal Vascular Injury Secondary to Blunt Abdominal Trauma: A Case Report From a University Hospital in Saltillo, Mexico.

A 65-year-old male with no relevant medical history presented to the emergency department with generalized abdominal pain following blunt trauma caused by a bicycle handlebar impact to the mesogastrium 48 hours prior. The patient initially self-medicated with analgesics without improvement. Upon evaluation, he had stable vital signs, epigastric ecchymosis, decreased peristalsis, and signs of generalized peritonism. Laboratory results showed leukocytosis (17.6 x 109/L). Plain abdominal radiographs showed intestinal loops with diameters within normal limits and a few short air-fluid levels in th

Internal Pudendal Artery Injury and Bleeding Into the Corpora Cavernosum Due to Complex Open Book Pelvic Fracture.

Open book fractures frequently occur in road traffic accidents and are often associated with organ injuries, soft tissue damage, and urogenital trauma. Pelvic fractures can vary in severity, with some being stable and others involving significant structural disruption. A 32-year-old man presented with severe lower abdominal and right lower limb pain following a road traffic accident. He remained alert and denied loss of consciousness but exhibited tachycardia. Examination revealed lower abdominal tenderness, pelvic swelling, bruising, and a scrotal hematoma. CT imaging showed extensive pelvic

Under the Radar: Back Pain and Acute Kidney Injury as Harbingers of Type A Aortic Dissection.

Aortic dissection is a catastrophic vascular emergency with a high mortality rate if not diagnosed and managed in a timely manner. The classic presentation of thoracic aortic dissection includes the sudden onset of severe chest pain radiating to the back. Still, it may also present with atypical symptoms and signs of end-organ vascular compromise. We present a case of a 57-year-old female with a medical history of hypertension with poor medication adherence, major depressive disorder, chronic low back pain, and cocaine use disorder, who initially presented to the ED with worsening chronic low

A propensity score-matched analysis of intra- and postoperative penile prosthetic complications in the solid organ transplant population.

INTRODUCTION: Solid organ transplant (SOT) has an anticipated higher risk of penile prosthesis (PP) complications related to immunosuppression and surgical approach post-SOT. It is still not determined if PP surgery in the SOT population incurs these same higher risks. OBJECTIVES: To observe differences in intra- and postoperative PP complications between SOT and non-SOT cases from the TriNetX US Collaborative Network, a large real-world database of deidentified patient data from 56 health care organizations within the United States. METHODS: We used the TriNetX database to perform a propens

Enhancement of Levator Ani Muscle Strength in Postpartum Women: The Impact of Pelvic Floor Muscle Training.

BACKGROUND Levator ani muscle injuries during vaginal childbirth can lead to pelvic organ prolapse (POP). Pelvic floor muscle training (PFMT) is an effective conservative approach to alleviate these symptoms. This study aimed to compare outcomes with and without 3 months of PFMT in 34 women with levator ani muscle injury following vaginal delivery. MATERIAL AND METHODS In a quasi-experimental study, 34 postpartum women were divided into 2 groups: one received PFMT along with home-based materials and regular follow-ups, while the other served as the control. We measured basal tone and maximal l

Damage-control orthopedics or early total care: What you need to know.

Patients with multisystem injuries are defined as multiply injured patients and may need multiple surgical procedures from more than one specialty. The importance of evaluating and understanding the resuscitation status of a multiple-injury patient is critical. Orthopedic strategies when caring for these patients include temporary stabilization or definitive early fixation of fractures while preventing further insult to other organ systems. This article will define multiple injuries and discuss specific markers used in assessing patients' hemodynamic and resuscitation status. The decision to u

Transvaginal mesh or grafts or native tissue repair for vaginal prolapse.

BACKGROUND: Pelvic organ prolapse is the descent of one or more of the pelvic organs (uterus, vaginal apex, bladder, or bowel) into the vagina. In recent years, surgeons have increasingly used grafts in transvaginal repairs. Graft material can be synthetic or biological. The aim is to reduce prolapse recurrence and surpass the effectiveness of traditional native tissue repair (colporrhaphy) for vaginal prolapse. This is a review update; the previous version was published in 2016. OBJECTIVES: To determine the safety and effectiveness of transvaginal mesh or biological grafts compared to native

Evaluation of Deep Learning Model Architectures for Point-of-Care Ultrasound Diagnostics.

Point-of-care ultrasound imaging is a critical tool for patient triage during trauma for diagnosing injuries and prioritizing limited medical evacuation resources. Specifically, an eFAST exam evaluates if there are free fluids in the chest or abdomen but this is only possible if ultrasound scans can be accurately interpreted, a challenge in the pre-hospital setting. In this effort, we evaluated the use of artificial intelligent eFAST image interpretation models. Widely used deep learning model architectures were evaluated as well as Bayesian models optimized for six different diagnostic models

Vaginal Bleeding Due to Iatrogenic Uterine Perforation - A Case Report.

UNLABELLED: Uterine perforation is a rare but potentially life-threatening complication of gynecologic procedures. Serious complications include hemorrhage, infection, and injury to surrounding organ systems (eg, gastrointestinal, urological, vascular, etc.). Risk factors include advanced maternal age, prior gynecologic surgeries, and other anatomical features that impact the difficulty of accessing the uterine cavity. In this case report, we discuss a patient who presented to the emergency department (ED) with diffuse abdominal pain and vaginal bleeding that occurred after an elective dilatio

Structural, functional, and dysfunctional pelvic anatomy.

The structural basis of the Integral Theory is holistic. Four main pelvic muscles interact holistically with five main pelvic ligaments to maintain pelvic organ structure and function. The vagina is structurally weak. The support it provides to the bladder base is contingent on being stretched by opposite pelvic muscle forces, much like a trampoline. Its main role is to transmit muscle forces to facilitate continence, evacuation and control of urgency. Therefore, as an organ that cannot regenerate, the vagina should be conserved, and not excised. The ligaments provide the main structural suppo

Exosomes: A Cellular Communication Medium That Has Multiple Effects On Brain Diseases.

Exosomes, as membranous vesicles generated by multiple cell types and secreted to extracellular space, play a crucial role in a range of brain injury-related brain disorders by transporting diverse proteins, RNA, DNA fragments, and other functional substances. The nervous system's pathogenic mechanisms are complicated, involving pathological processes like as inflammation, apoptosis, oxidative stress, and autophagy, all of which result in blood-brain barrier damage, cognitive impairment, and even loss of normal motor function. Exosomes have been linked to the incidence and progression of brain
