Temporal trends, diagnostics, and outcomes of pediatric nontuberculous mycobacterial disease in Slovakia (2017-2024).

BACKGROUND: Nontuberculous mycobacteria (NTM) are rare but emerging pathogens in pediatric populations, particularly in countries where BCG vaccination has been discontinued. Their diagnosis is often delayed due to nonspecific symptoms and limited microbiological sensitivity. METHODS: We conducted a retrospective cohort study of all pediatric patients with confirmed or clinically probable NTM disease, diagnosed at the National Institute for Pediatric Tuberculosis and Respiratory Diseases in Slovakia between 2017 and 2024. Medical records from 2012 to 2016 were screened, but no cases fulfillin

[Tuberculous pseudotumour, a challenging diagnosis].

INTRODUCTION: The pseudotumorous form of tuberculosis is a rare entity. Whatever its location, it can simulate neoplasia by its radiological and/or endoscopic appearances. We highlight the diagnostic difficulties associated with this type of presentation. METHODS: We performed a retrospective study of inpatient records from 2003 to 2016 in the pneumology department of La Rabta Hospital to identify cases of thoracic tuberculous pseudo-tumor. RESULTS: Seventeen patients were identified. The median age was 41 years and their symptomatology was dominated by cough and general debility. All had ab

Correlation of microbiological yield with radiographic activity on chest computed tomography in cases of suspected pulmonary tuberculosis.

BACKGROUND: Little is known about the correlation between microbiological yield and radiographic activity, on chest computed tomography (CT), in suspected pulmonary tuberculosis (PTB) cases, despite CT being widely used, clinically. METHODS: We used multicenter retrospective data, obtained from medical records, focusing on the diagnostic performance for definite PTB. We categorized patients into four groups, by radiographic activity: definitely active, probably active, indeterminate activity, and probably inactive. RESULTS: Of the 650 patients included, 316 had culture-confirmed PTB; 190 (29

Multimacronodular pulmonary tuberculosis (bacteriologically negative) confirmed histologically.

BACKGROUND: Pulmonary tuberculosis can be confirmed by positive bacteriology of sputum, bronchial aspirate or by biopsies (microscopy and/ or culture) or by histopathological examination highlighting specific tuberculous granulomas. When microscopy is repeatedly negative during noninvasive methods, lung biopsy by thoracoscopy is needed for confirmation and differential diagnosis. CASE PRESENTATION: A 40-year-old female patient (nonsmoker, diabetic, with previous exposure to chemicals) was admitted to the hospital for weight loss, dry cough, loss of appetite, pallor, and fatigue. Chest-X-ray a

Starry sky sign: A prevalent sonographic finding in mediastinal tuberculous lymph nodes.

BACKGROUND AND OBJECTIVES: We report a prevalent finding in tuberculous lymphadenitis (TL): Starry sky sign, hyperechoic foci without acoustic shadows over a hypoechoic background. MATERIALS AND METHODS: We retrospectively searched the database for a possible relationship of starry sky sign with a specific diagnosis and also the prevalence and accuracy of the finding. RESULTS: Starry sky sign was found in 16 of 31 tuberculous lymph nodes, while none of other lymph nodes (1,015 lymph nodes) exhibited this finding; giving a sensitivity, specificity, positive predictive value (PPV), negative pr

[Extrapulmonary tuberculosis].

Each year, there are more than eight million new cases of tuberculosis and 1.3 million deaths. There is a renewed interest in extrapulmonary forms of tuberculosis as its relative frequency increases. Among extrapulmonary organs, pleura and lymph nodes are the most common. Their diagnosis is often difficult and is based on clinical, radiological, bacteriological and histological findings. Extrapulmonary lesions are paucibacillary and samplings, in most cases, difficult to obtain, so diagnosis is often simply presumptive. Nucleic acid amplification tests, which are fast and specific, have greatl

Pulmonary botryomycosis on a lung cavity: a rare pulmonary infection mimicking cancer.

Lung botryomycosis is a rare disease. We report what is to our knowledge the first case occurring on a lung cavity. In a 42-year-old man suffering asthenia and cough, a chest radiograph revealed a right upper lobe opacity. Computed tomography scan showed a necrotic mass which was also spiculated. Repeated research for Mycobacterium tuberculosis was negative. The patient underwent a lobectomy. Histological and bacteriological examinations made the diagnosis of botryomycosis, because the cavity presented numerous colonies of pyogenic Fusobacterium nucleatum bacteria. Botryomycosis is a difficult

Staining for acid-fast bacilli in surgical pathology: practice patterns and variations.

Analysis of acid-fast bacilli stains on sputum smears for the diagnosis of tuberculosis has a long history, but quality control for acid-fast bacilli in histologic sections is not as well established. In tissues, necrotizing granulomas are closely linked to positive cultures for mycobacteria. However, the practices of pathologists examining acid-fast bacilli in surgical specimens are not well described in the literature. This study characterizes practice patterns related to the histologic interpretation of acid-fast bacilli stains. A survey invitation was sent to 1299 pathologists including me

[Diagnostic difficulty in bronchopulmonary tuberculous pseudotumor].

Pseudotumour is a rare presentation of bronchopulmonary tuberculosis, occurring in immunocompetent patients, which simulates lung cancer and may thus cause diagnostic difficulty. To assess the frequency and clinical features of tuberculous pseudotumour in immunocompetent patients, we analyzed all cases of pulmonary tuberculosis hospitalized in our department. Tuberculous pseudotumour was defined by the presence of a bronchial or pulmonary lesion suggestive of lung cancer. Over a period of 11 years, 12 cases of tuberculous pseudotumour were collected among 341 cases of pulmonary tuberculosis (3

Prolonged culture for mycobacteria in mediastinal lymph nodes from patients with pulmonary sarcoidosis. A negative study.

BACKGROUND: Histological similarity between sarcoidosis and tuberculosis, with epithelioid cell granulomas as the typical finding, suggests that sarcoidosis could be a specific manifestation of infection with mycobacteria. However, convincing evidence for a causal relationship between mycobacterial infection and sarcoidosis has not been produced. OBJECTIVE: To examine whether mycobacteria could be cultured from mediastinal lymph nodes from patients with newly diagnosed pulmonary sarcoidosis using a prolonged culture technique. METHODS: The series comprised 15 patients (10 men) with a median

[Measuring of the level of IGG antibodies against mycobacterial A60 antigen in the differential diagnosis of lung tumors].

In some cases it is difficult to define an etiology of lung tumour in the other way than thoracotomy. In these diagnostic difficulties patients with tuberculous tumour are also involved. The aim of the present study was to define if A60-ELISA test, a new valuable tool in the diagnosis of tuberculosis, might be useful in the differential diagnosing of lung tumours. The levels of IgG antibodies against A60 antigen of mycobacteria was measured with the use of the ELISA test. 228 persons were enrolled into the study: 58 healthy controls (Group I), 26 nonmalignant and nontuberculous patients (Group

Rapid mycobacterial tuberculosis detection in bronchoalveolar lavage samples by polymerase chain reaction in patients with upper lobe infiltrates and bronchiectasis.

In areas where tuberculosis is endemic, a positive sputum acid-fast bacilli (AFB) smear is frequently regarded as almost diagnostic of pulmonary tuberculosis (PTB). The main problem arises when the AFB smear is negative. The main aim of this study was to determine the clinical utility of rapid mycobacterial tuberculosis (MTB) detection in bronchoalveolar lavage (BAL) samples by polymerase chain reaction (PCR) in 52 patients who underwent diagnostic bronchoscopy for suspected PTB. These patients had either upper lobe infiltrates (n = 31) or bronchiectasis (n = 21). Mycobacterial culture is usua

Polymerase chain reaction to detect Mycobacterium tuberculosis in histologic specimens.

There is a need for rapid and sensitive detection of Mycobacterium tuberculosis in tissue specimens. A polymerase chain reaction (PCR)-based assay for the diagnosis of tuberculosis was evaluated in 60 formalin-fixed tissue specimens, the target for the amplification being a segment of IS6110 in the M. tuberculosis chromosome. Of the 60 formalin-fixed, paraffin-embedded tissue specimens studied, 57 showed granulomatous inflammation and 53 had been cultured for mycobacteria; 10 were positive for M. tuberculosis and three were positive for other mycobacteria. Of 60 samples, 15 showed acid-fast ba
