Isolated avulsion of the short head of the biceps tendon from the coracoid insertion: A case report and narrative review of the literature.

Isolated injuries to the short head of the biceps (SHB) are uncommon, and the existing literature mainly includes case reports documenting tears at the muscle belly or myotendinous junction. This case report presents the first reported SHB tendon injury at the coracoid insertion in a 34-year-old healthy male patient after trauma while playing with his daughter. The injury occurred with the arm forced into hyperextension, abduction, and external rotation, with the elbow fully extended. Diagnosis was confirmed by MRI, showing an isolated insertional tear of the SHB tendon with intact coracobrach

Axillary microaxial flow pump support for stepwise source control and bridge to durable left ventricular assist device therapy in cardiac device-related infective endocarditis.

Cardiac device-related infective endocarditis complicated by end-stage heart failure poses a major therapeutic challenge because complete hardware removal is essential, whereas transvenous lead extraction may be poorly tolerated in patients with severe ventricular dysfunction and shock. We report a 53-year-old man with long-standing dilated cardiomyopathy and cardiac resynchronization therapy-defibrillator infection who presented with methicillin-sensitive Staphylococcus aureus bacteremia, pocket infection, respiratory failure, acute kidney injury, and a clinical presentation consistent with m

Morphometric cadaveric assessment of suprascapular and axillary nerve trajectories in relation to glenohumeral landmarks relevant to reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is increasingly used for complex shoulder pathology but carries a risk of iatrogenic nerve injury, particularly to the suprascapular (SSN) and axillary nerves (AXN). Detailed morphometric data on their relationship to glenoid landmarks remain limited. METHODS: Forty embalmed adult cadavers (60 shoulders) were dissected at PGIMER, Chandigarh, after ethics approval (PGI/IEC/2024/EIC000563). The SSN and AXN were exposed, and distances from nerves to glenoid "clock-face" positions and to osseous landmarks were measured with digital calipers.

Morphometric analysis of the axillary nerve in relation to the deltopectoral and deltoid-splitting approaches: implications for surgical safe zones.

PURPOSE: The deltopectoral splitting approach is commonly used in shoulder surgery; however, injury to the axillary nerve remains a major concern due to potential motor and sensory deficits. This study aimed to define the anatomical relationships of the axillary nerve to surrounding structures in order to optimize surgical access and minimize the risk of iatrogenic injury. METHODS: Thirty formalin-fixed cadavers (15 males, 15 females) were dissected. Quantitative measurements were obtained to determine the position of the axillary nerve relative to the deltopectoral interval and deltoid muscl

A Multicenter Matched Cohort Study of Processed Nerve Allograft and Autograft in Mixed and Motor Nerve Reconstruction.

PURPOSE: Nerve transections often require surgical intervention with a bridging material to achieve meaningful recovery (MR). We explored similar cohorts with nerve reconstruction using nerve autograft or nerve allograft. We hypothesized that motor recovery in mixed and motor nerve injuries repaired with nerve allograft is noninferior to repairs with nerve autograft. METHODS: Subjects had mixed or motor nerve transections in the upper extremity distal to the axilla surgically repaired with nerve autograft or nerve allograft. Motor recovery outcomes were analyzed using the British Medical Rese

Efficacy of Pull-In Suture for Tendon Avulsion Injury Associated With Severe Forearm Open Fracture, Forearm Amputation, and Hand Amputation.

BACKGROUND: The pull-in suture technique is a method of primary tendon repair that involves pulling the distal stump of the ruptured tendon into the proximal muscle belly and suturing it to the intact muscle fascia for avulsion injuries at the musculotendinous junctions associated with severe open forearm trauma. The aim of this study is to report the clinical outcomes of the pull-in suture technique for musculotendinous avulsion injuries associated with severe open forearm trauma and to evaluate its effectiveness. METHODS: This study included patients who underwent pull-in suture repair for

High-dose rifampicin and clarithromycin for 4 weeks versus standard-dose rifampicin and clarithromycin for 8 weeks to treat Buruli ulcer: An open-label, individually randomized, controlled trial in Ghana.

BACKGROUND: The current 8-week regimen of rifampicin and clarithromycin for Buruli ulcer (BU) is suboptimal. High-dose rifampicin could shorten treatment and improve outcomes. We evaluated whether a 4-week high-dose regimen could improve time to clearance of viable Mycobacterium ulcerans. METHODOLOGY: In this open-label, individually randomised trial conducted in Ghana, participants with PCR-confirmed BU were assigned (1:1) to either high-dose oral rifampicin (20mg/kg) with clarithromycin (15mg/kg) for 4 weeks (HR) or standard-dose rifampicin (10mg/kg) with clarithromycin (15mg/kg) for 8 week

Novel anatomical mapping of the anterior branch of the axillary nerve across the anterior and middle part of the deltoid: a fresh-cadaver study defining safe zones.

PURPOSE: To map the surface projection of the anterior branch of the axillary nerve (ABAN) over the anterior and middle part of the deltoid and to define safe and high-risk zones for proximal humeral approaches. METHODS: Fourteen whole adult fresh-frozen cadavers (28 paired shoulders) were dissected in this study. Acromial landmarks were defined as point A (anterolateral corner of acromion), point B (middle point of AC), point C (posterolateral corner of acromion), and insertion of middle part of the deltoid (point D) in a modified anatomical position. A delto-pectoral landmark connected D to

Overuse Injuries of the Shoulder.

Shoulder overuse injuries represent a broad and often overlapping spectrum shaped by repetitive loading, soft tissue adaptation, and changes in bone. Rather than distinct entities, they are better viewed along a continuum, where repeated microtrauma gradually leads to structural and functional alterations in tendons, capsulolabral structures, bursae, and bone. This narrative review covers the broad anatomical spectrum of shoulder overuse injuries, including rotator cuff disorders, impingement syndromes, labral and capsular abnormalities, biceps tendon pathology, bursal disease, and osseous and

True brachial artery aneurysm secondary to arterial wall infiltration by Langerhans cell histiocytosis.

Langerhans cell histiocytosis is a rare histologic disorder that can affect and damage any tissues and organ in the body. A 40-year-old man with multisystem Langerhans cell histiocytosis presented with a painful distal arm mass. Imaging demonstrated a 3.6-cm true distal brachial artery aneurysm and progressive hypermetabolic activity of the affected artery on the retrospective review of serial positron emission tomography/computed tomography scans. The patient underwent aneurysm resection and arterial reconstruction using a reversed great saphenous vein bypass. Histopathology confirmed infiltr

Medial Distal Arm Perforator Flap for elbow Soft Tissue Defects: A Cadaveric Study.

A cadaveric anatomical study was performed on seven fresh adult upper limbs to assess the technical feasibility and reproducibility of a medial distal arm perforator flap for coverage of elbow soft-tissue defects. A theoretical 5-cm-diameter circle, corresponding to the flap skin-paddle width, was centered 7.5 cm proximal to the medial epicondyle on an axis joining the epicondyle to the mid-axillary fossa; this central point defined the flap pivot, and flap length was adjusted to match simulated elbow defects. Between two and five skin perforators were identified within the circle per limb. Pe

Transosseous Double-Row Triceps Repair in a Professional Athlete.

Distal triceps rupture is a relatively rare pathology among muscle-tendon injuries and upper extremity trauma. The most common mechanisms include a fall onto an outstretched arm or direct trauma to the extended elbow, both of which can lead to disruption of the distal triceps tendon. The available literature on treatment outcomes remains limited; however, recent reports suggest that surgical management offers superior results compared with conservative approaches. Among surgical techniques, both suture anchors and transosseous suturing are established options. In this report, we describe the u

Neural Regeneration After Supercharged End-to-Side Nerve Transfer: Which Way to Go?

BACKGROUND AND OBJECTIVE: The exact regeneration pattern and directional fiber distribution of donor axons within the recipient nerve after supercharged end-to-side nerve transfer (SETS NT) over time still remains elusive. This study aimed to investigate whether axons preferably regenerate toward an end organ and how the regenerative condition of the recipient nerve influences axonal ingrowth and directional preference. METHODS: SETS NTs were performed in an upper-limb rat model divided into 3 groups: musculocutaneous nerve (MCN) transection and repair (Group A), SETS NT to intact MCN (Group

Pulsed Dye Laser for Sensory Dysesthesias Associated With Chronic Radiation Dermatitis.

Chronic radiation dermatitis (CRD) is a long-term complication of radiation therapy, typically manifesting as scarred atrophic plaques with telangiectasias. In some cases, patients report persistent sensory dysesthesias such as burning, stinging, and pruritus. These symptoms are underrecognized and likely underreported but can significantly impair quality of life and are often refractory to conventional therapies. Pulsed dye laser (PDL) has demonstrated efficacy in treating CRD-associated telangiectasias, but its role in alleviating sensory symptoms has not been reported. We report 2 breast ca

The Interscalene Triangle: A Cadaveric Dissection of a Critical Neurovascular Corridor at the Root of the Neck.

The interscalene triangle is a key cervicothoracic gateway bounded by the anterior and middle scalene muscles and the first rib. It transmits the roots and trunks of the brachial plexus (C5-T1) and the subclavian artery (SA), while the subclavian vein (SV) courses anterior to the anterior scalene muscle and the phrenic nerve (PN) descends along its anterior surface. Cadaveric dissection highlights these critical topographic relationships, providing an anatomical basis for understanding thoracic outlet syndrome, optimizing ultrasound-guided regional anesthesia, and minimizing the risk of iatrog

Analysis of factors associated with persistent night pain after locking plate fixation of proximal humerus fractures and development of a prediction model.

BACKGROUND: Proximal humerus fractures are common fragility fractures in older adults. Open reduction and locking plate internal fixation can restore fracture alignment and facilitate early rehabilitation; however, some patients still develop persistent night pain after surgery, potentially impairing sleep and tolerance of rehabilitation. This study aimed to analyze factors associated with persistent night pain 3 months after locking plate fixation for proximal humerus fractures and to construct a preliminary preoperative risk model and a complete risk stratification model after immediate post
