Type 4 Acromioclavicular Joint Separation Reconstruction With Suspensory Fixation, Allograft, and Suture Augmentation.

BACKGROUND: This is a case of surgical treatment of a type 4 acromioclavicular (AC) joint separation via an arthroscopically assisted approach and reconstruction with combined suspensory fixation, allograft ligament reconstruction, and suture augmentation. AC joint injuries are relatively common, representing approximately 9% of shoulder injuries and approximately 50% of sport-related shoulder injuries. Most AC joint injuries do well when treated conservatively. However, the literature supports surgical treatment for high-grade injuries, failed conservative treatment, and open injuries or thos

Isolated Extra-articular Medial Dislocation of the Long Head of the Biceps Tendon in an Adolescent: A Case Report.

Isolated instability of the long head of the biceps tendon is uncommon and is usually associated with rotator cuff or bicipital pulley injuries. Isolated extra-articular medial dislocation in the absence of associated lesions is particularly rare in the adolescent population. We report the case of a 15-year-old male who presented with anterior shoulder pain after a recreational sports activity. Physical examination revealed localized anterior and anterolateral tenderness, with preserved active and passive range of motion and no clinical signs of instability. Magnetic resonance imaging using MR

Dual radiation-induced sarcomas after breast cancer in a TP53 variant carrier.

Li–Fraumeni syndrome (LFS) is a hereditary syndrome caused by TP53 variants, conferring a high risk of early-onset malignancies, particularly breast cancer. TP53 variants also increase susceptibility to radiation-induced tumors, complicating therapeutic strategies. This report illustrates a representative case and reviews current recommendations and related challenges. A literature review was conducted on patients with LFS who developed secondary malignancies within previously irradiated fields. We describe a 31-year-old woman with prior breast augmentation and left-sided breast cancer treated

[Treatment of acute, first-time posterior shoulder instability].

BACKGROUND: Posterior shoulder instability is a complex and frequently underdiagnosed condition. In the context of primary manifestation, first-time (sub-)luxation (Moroder type A1) are distinguished from first-time traumatic dislocation (Moroder type A2). Posterior shoulder dislocation is particularly difficult to diagnose in emergency settings and is often overlooked. DIAGNOSTICS: Clinical findings include a prominent coracoid and loss of active and passive external rotation. Radiographic features such as the lightbulb sign, vacant glenoid sign, or rim sign may be present, yet the diagnosis

Reconstruction of an arteriovenous graft using the transected end of a pre-existing stent graft: A case report.

Stent grafts have improved secondary patency in arteriovenous grafts (AVGs) by effectively treating venous outflow lesions and have contributed to prolonged secondary patency of AVGs. As AVGs remain functional for longer periods, degeneration of the prosthetic graft itself becomes more frequent, increasing the need for graft-side interventions. During surgical reconstruction of AVGs, surgeons may be required to manage previously implanted stent grafts. We report a case of AVG reconstruction using the transected end of a pre-existing stent graft as the venous anastomosis. A 78-year-old woman un

Do Pitching Restriction Policies Reduce Shoulder and Elbow Injuries in High School Baseball Players?

BACKGROUND: Shoulder and elbow injuries (SEIs) remain prevalent among high school (HS) baseball players. Pitching restriction (PR) policies were mandated by the National Federation of State High School Associations (NFHS) in 2016 as an injury prevention strategy to set workload limits and reduce overuse injuries. The effect of PR policies on HS baseball SEIs remains unknown. HYPOTHESES: (1) SEI rates in HS baseball players would be lower in the 3 seasons after PR policy implementation when compared with the 3 seasons before PR policy implementation and (2) specific components of PR policies w

Two-week postoperative duplex ultrasound as a clinical workflow tool for risk stratification of radiocephalic arteriovenous fistulas.

BACKGROUND: Radiocephalic arteriovenous fistulas (RCAVFs) are the preferred initial haemodialysis access but are limited by high non-maturation rates, particularly in populations with smaller vessels and high diabetic burden. Preoperative duplex ultrasound (DUS) guides vessel selection but inconsistently predicts postoperative haemodynamic adaptation. We evaluated whether a standardised 2-week postoperative DUS checkpoint could function as a clinical workflow tool for early risk stratification and long-term patency prediction. METHODS: In this prospective single-centre study, 57 patients unde

Predicting complications following proximal humerus fracture repair using the 6-item modified frailty index.

BACKGROUND: Frailty is a well-established predictor of poor outcomes after orthopedic surgery. The 6-item modified Frailty Index (mFI) 6, which includes hypoalbuminemia, may provide enhanced risk stratification compared to the widely used 5-item mFI-5. The predictive value of the mFI-6 vs. the mFI-5 for short-term complications after surgical management of proximal humerus fractures (PHFs) remains unclear. METHODS: A retrospective analysis of the American College of Surgeons National Surgical Quality Improvement Program database was conducted to identify patients who underwent open reduction

How POCUS uncovered Alagille syndrome in a non-mobile infant with bruising: a case report.

A bruise or haematoma in a non-mobile infant is a red flag prompting concern for non-accidental injury. Bruising does not always signify trauma however and may occur spontaneously due to an underlying medical cause. This case report describes a 3-month-old infant presenting with unexplained faint bruising to the axilla and an associated lump. Point of care ultrasound revealed a chest wall haematoma without any other evidence of trauma prompting blood tests, which demonstrated severely deranged coagulation and conjugated hyperbilirubinaemia. The child was subsequently diagnosed with Alagille Sy

A rare seizure: A case of seizure-induced central acetabular fracture-dislocation in a patient with chronic kidney disease and hepatitis-B.

UNLABELLED: Seizure induced fractures are uncommon and usually involve long bone fractures like proximal humerus, proximal femur, shoulder dislocations etc. These occur due to intense muscle contractions or may result from indirect trauma post-seizure episode in the patient. Acetabulum fracture with central head dislocation, as seen in this case, is very rare following a seizure episode. It was further complicated by the presence of Hepatitis-B and dialysis dependent CKD both of which impact the surgical outcomes as well as pose challenges in the management of patient in the perioperative peri

Post-assault brachial vein thrombosis in an adolescent female: Diagnosis by CT angiography.

Upper extremity deep venous thrombosis (UEDVT) is an uncommon clinical entity in adolescents, most often linked to central venous catheters or hypercoagulable states. Trauma-induced UEDVT is rare and challenging to diagnose due to nonspecific clinical signs that can mimic infection or soft tissue injury. We report an 18-year-old female who developed post-traumatic swelling and tenderness of the right forearm after being beaten by her school teacher. Initial non-contrast CT revealed subcutaneous edema without fracture. Subsequent CT angiography identified right brachial vein thrombosis with pro

Recurrence Rates and Functional Outcomes Following Arthroscopic ALPSA Repair With Concomitant Remplissage: A Prospective Cohort Study.

BACKGROUND: Anterior labroligamentous periosteal sleeve avulsion (ALPSA) lesions are often associated with higher failure rates after arthroscopic repair compared with non-ALPSA lesions. PURPOSE/HYPOTHESIS: The purpose of this study was to compare failure rates after arthroscopic ALPSA repair versus non-ALPSA (Bankart/Perthes) repair, with the effect of the Hill-Sachs lesions (HSLs) controlled by performing concomitant remplissage in all patients. It was hypothesized that addressing the Hill-Sachs defect would result in similar failure rates for both ALPSA and non-ALPSA repairs. STUDY DESIGN
