The work presents results of the investigation of the content of sulfhydryl groups in the blood of patients with fractures of long tubular bones. The content of sulfhydryl groups of blood was studied in 168 patients with open fractures. The author comes to a conclusion that the investigation of sulfhydryl groups of blood allows not only facilitating the evaluation of severity of patients in the pre- and postoperative period and detecting complications at early terms but also enables a timely correction of the disorders to be fulfilled.

In 7 cases of fracture of the humerus occurring in arm wrestlers, each patient sustained a simple spiral fracture of the distal third of the humerus. No patient had any neural or vascular compromise. Three patients underwent open reduction and internal fixation with 2 screws, at which time, muscle was found to be interposed between the fracture fragments. The results of treatment of 158 patients with open fractures of bones were analyzed. They depended on different factors: time of admission of the patient, the degree of the injury (according to Kaplan--Markova), choice of the radical surgical treatment of the wound, rational application of antibiotics, intraosseous perfusion, enzymotherapy etc. Treatment of the wound after Syzganov--Tkachenko has been shown to be very effective.

Internal fixation of open fractures can be carried out according to the same indications employed in closed fractures in the presence of a type I wound. In type II and type III wounds early internal fixation may be indicated in victims of multiple trauma, in the elderly, in intra-articular fractures, in mutilated limbs, and in some fractures with associated vascular injuries. 99 cases of primary traumatic shoulder dislocation were evaluated in regard of the influence of the time of immobilization on recurrency. 3 weeks of strict immobilization gave a recurrency rate as low as 6.4% whereas a shorter immobilization resulted in 12% of recurrency within 1--6 years. A survey from 46 other clinics as well as the literature has given no evidence of clear rules of treatment and thus the discussion of treatment remains open.

A 91-year-old woman presented with an acute anterior dislocation of the shoulder which could not be reduced by closed manipulation. Open reduction demonstrated a fresh dislocation which could not be reduced until the subscapularis muscle was divided. Only 2 previous cases of irreducible anterior dislocation of the shoulder seem to have been reported in the recent or readily available literature.

Free vascularized fibular grafts were employed in five patients with segmental bone defects following trauma or resection of tumors of the upper extremity with excellent results in three patients and satisfactory results in two.
