Open repairs with biceps rerouting does not impact retears in large to massive rotator cuff tears compared to conventional open repair: a randomized clinical trial.

BACKGROUND: The treatment of large to massive rotator cuff tears leads to a high failure rate and several techniques have been proposed to improve these results. Rerouting the long head of the biceps tendon (LHBT) appears to be a biological reinforcement to increase tendon healing. Therefore, this study aimed to compare the clinical and radiological outcomes of open repair for large and massive rotator cuff tears reinforced with the LHBT to conventional open repair. METHODS: A prospective randomized study was conducted with patients diagnosed with large to massive rotator posterosuperior rota

Biceps tenodesis with 360° suture anchor versus self-locking tenodesis in the absence of full-thickness rotator cuff tears: protocol for a non-inferiority randomised controlled trial (BLAST-2).

INTRODUCTION: Pathology of the long head of the biceps tendon (LHBT) is commonly treated with either tenotomy or tenodesis, which provide comparable clinical outcomes but have distinct disadvantages. A novel self-locking biceps tenodesis aims to combine the benefits of both techniques. Comparative evidence in patients without full-thickness rotator cuff tears is limited. This study aims to compare the clinical outcomes and complications of self-locking biceps tenodesis and biceps tenodesis with 360° suture anchor in patients without full-thickness rotator cuff tears. METHODS AND ANALYSIS: Thi

Detailed Analysis of Intraretinal Layers and Their Correlation to Brain Structures in Normal Healthy Aging.

IMPORTANCE: Patients with head and neck cancer who undergo radiotherapy can develop chronic radiation-induced xerostomia. Prior acupuncture studies were single center and rated as having high risk of bias, making it difficult to know the benefits of acupuncture for treating radiation-induced xerostomia. OBJECTIVE: To compare true acupuncture (TA), sham acupuncture (SA), and standard oral hygiene (SOH) for treating radiation-induced xerostomia. DESIGN, SETTING, AND PARTICIPANTS: A randomized, blinded, 3-arm, placebo-controlled trial was conducted between July 29, 2013, and June 9, 2021. Data

Did the Implementation of a Bye Week Decrease Injury Rates in the National Hockey League?

BACKGROUND: In the 2016-2017 National Hockey League (NHL) season, a bye week was introduced to give players rest in the middle of the season. Bye weeks and midseason breaks are a common practice in other sports such as football and soccer. PURPOSE: To compare the rates of different types of injuries, games missed due to injury, and number of multiple injuries by players between previous uninterrupted seasons and the succeeding seasons with bye weeks scheduled. STUDY DESIGN: Descriptive epidemiology study. METHODS: This study retrospectively looked at NHL injuries from 2000 to 2023. Injured

Robotic-Assisted Spinal Instrumentation from C1 to S1 Experience of a UK Neurosurgical Tertiary Referral Centre.

Background: Robotic-assisted spinal instrumentation is increasingly used to support implant placement, reduce radiation exposure, and improve operative workflow. However, UK NHS data describing early programme experience across elective and emergency practice remain limited. This study aimed to evaluate the feasibility, safety profile, and early learning curve of robotic-assisted spinal instrumentation in an unselected UK NHS cohort. Methods: A retrospective, single-centre, single-arm observational cohort study was conducted, including the first 50 consecutive patients treated from programme i

Iatrogenic Nerve Injuries Affecting Upper Limb Function: Medicolegal Implications.

PURPOSE: Nerve injuries are among the commonest iatrogenic complications and most frequently are the result of direct intraoperative damage causing serious distress and long-term morbidity. Additionally, they can add a considerable financial burden to health care systems, while they constitute a well-known source of medicolegal implications. There are few published articles addressing the medicolegal consequences of the iatrogenic nerve injuries affecting upper limb function. Our aim was to identify malpractice claims related to iatrogenic nerve injuries affecting upper limb function and to de

Safety and Effectiveness of Placental Connective Tissue Matrix Allograft Versus Corticosteroid Injection in Adhesive Capsulitis: A Double-Blinded Prospective Randomized Study.

BACKGROUND: Adhesive capsulitis causes significant pain and functional limitation, with limited effective treatment options. Current standard-of-care treatments, such as corticosteroid injections, may offer short-term symptom relief but are associated with limited long-term efficacy and potential side effects. Regenerative therapies using extracellular matrix-based products derived from placental tissue have shown promise in treating musculoskeletal injuries due to their anti-inflammatory and tissue-healing properties. Clinical data supporting their use in adhesive capsulitis remains limited.

Locking plate fixation versus intramedullary nailing fixation in displaced proximal humerus fractures: A meta-analysis of randomized controlled trials.

BACKGROUND: Proximal humerus fractures (PHFs) are common injuries in older adults. However, the optimal approach to open reduction and internal fixation (ORIF) of PHFs remains a subject of debate. We conducted an updated systematic review and meta-analysis of randomized controlled trials (RCTs) comparing the clinical outcomes between locking plate (LP) and intramedullary nailing (IMN) fixation in ORIF of PHFs with a focus on fracture type. METHODS: We searched PubMed, EMBASE, the Cochrane Library, Scopus, and the ClinicalTrials.gov registry for RCTs published before December 2022. The effects

The Woodpecker variable-speed drilling protocol for robot-assisted subaxial cervical pedicle screw placement: technique and a consecutive series of 1027 screws.

Published descriptions of robot-assisted cervical pedicle screw placement remain limited, particularly for subaxial screws and for workflows integrating positioning, registration, and drilling. Small pedicle diameter, steep trajectories, and proximity to the vertebral artery magnify the consequences of minor technical error. This retrospective single-center case series included 117 consecutive patients undergoing robot-assisted subaxial cervical pedicle screw placement between November 2023 and June 2026. One workflow was applied across open, hybrid and percutaneous approaches, comprising rigi

Impact of Concomitant Meniscus Repair With Anterior Cruciate Ligament Reconstruction on Functional Outcomes at Short-Term Follow-Up: A Prospective Observational Study.

BACKGROUND: Anterior cruciate ligament (ACL) ruptures are among the most common knee injuries and impose a substantial health burden. ACL injuries are common, and meniscal tears frequently co-occur with acute ACL ruptures, most often involving the medial meniscus. Proper management of combined ACL and meniscus injuries is critical to restore knee function and prevent long-term degeneration. Although concurrent meniscal repair with ACL reconstruction (ACLR) is increasingly favoured over meniscectomy, reported patient-reported outcomes remain inconsistent across registries and are drawn largely

Intraoperative Zoledronic Acid for Arthroscopic Rotator Cuff Repair: Short-Term Results From a Prospective, Randomized, Placebo-Controlled Phase II Trial.

BACKGROUND: Despite technical innovations and surgical improvements, retear rates after arthroscopic rotator cuff (RC) repair remain high. Preclinical studies demonstrated improved outcomes using bisphosphonates after RC repair. PURPOSE/HYPOTHESIS: The purpose of this monocentric, prospective, randomized, placebo-controlled, triple-blinded phase II trial was to investigate retears in patients without osteoporosis undergoing arthroscopic RC repair with a systemic single-dose zoledronic acid. It was hypothesized that zoledronic acid would reduce retears after RC repair. STUDY DESIGN: Randomize

Proximal row carpectomy versus open reduction and internal fixation in late-presenting perilunate dislocations.

PURPOSE: Perilunate dislocations and fracture-dislocations are severe wrist injuries, frequently caused by high-energy trauma, that can lead to long-term complications if not promptly addressed. While early open reduction and internal fixation (ORIF) is the standard treatment for acute injuries, the optimal management of late presentations remains controversial. This study aims to compare the clinical outcomes of proximal row carpectomy (PRC) with those of ORIF in patients with late-presenting perilunate injuries. METHODS: This retrospective comparative study analyzed data from three hand tra

A Clinician's Guide to Evaluating and Managing Shoulder Overuse Injury in Softball Pitchers.

BACKGROUND: Approximately one-third of softball pitchers sustain a pitching-related injury across a single season, with the throwing shoulder being the most frequently injured site. Overuse is a commonly proposed mechanism given the repetitive and high-stress placed on the biceps-labral complex. CLINICAL QUESTION: How can clinicians evaluate and manage shoulder overuse injury in softball pitchers? KEY RESULTS: Clinical evaluation of softball pitchers should include a comprehensive patient history, physical examination, and appropriate imaging. Understanding injury mechanisms and risks will hel

Evaluating the efficacy of an SMS text message information and support programme for people who have experienced a motor vehicle-related injury: protocol for a double-blind randomised controlled trial.

INTRODUCTION: Road traffic injuries can be physically and emotionally challenging, and navigating compensation processes may increase uncertainty and distress. Early high distress is linked with poorer long-term recovery, including persistent pain and delayed return to work. Digital health tools, particularly brief, scalable SMS programmes, may offer timely information and emotional support during recovery. This protocol outlines a double-blind randomised controlled trial evaluating the efficacy, feasibility and acceptability of an SMS-delivered health information and support programme for peo
