Bilateral posterior shoulder dislocations with reverse Hill-Sachs lesions: a report of two cases and a literature review.

BACKGROUND: Posterior shoulder dislocation accompanied by reverse Hill-Sachs lesions is relatively rare, whereas bilateral posterior shoulder dislocation is exceedingly rare. CASE PRESENTATION: We report two cases of bilateral posterior shoulder dislocations associated with reverse Hill-Sachs lesions. In Case 1, the lesions were managed using the McLaughlin procedure and the modified McLaughlin procedure. In Case 2, open reduction and internal fixation of the left shoulder was unsuccessful; consequently, reconstruction of the right humeral head was performed using bone cement. CONCLUSIONS: Bil

Fewer major complications but more pain in reverse shoulder arthroplasty compare with open reduction internal fixation in closed proximal humerus fracture: a National database study.

BACKGROUND: The incidence of closed proximal humerus fractures (CPHF) is steadily increasing worldwide. Open reduction internal fixation (ORIF) remains the most common surgical technique while reverse shoulder arthroplasty (RSA) is becoming a noteworthy approach. The optimal management of CPHF (ORIF vs. RSA) remains complex and continually evolving. METHODS: This retrospective cohort study utilized data from the National Inpatient Sample (2016-2019). Patients with proximal humerus fractures (PHF) treated with ORIF or RSA were included. Demographics, fracture characteristics, comorbidities, an

Beyond Conventional Donor Tendons: Flexor Tendon Reconstruction with Fascia Lata-A Narrative Review and Case Illustration.

BACKGROUND: Hand flexor tendon injuries can be treated with primary repair by direct tenorrhaphy or, when this option is not feasible, with secondary repair by reconstruction with tendon grafts. Commonly used autologous grafts include palmaris longus and plantaris; however, these may be congenitally absent or unavailable. Fascia lata (FL) is widely used for ligament and tendon reconstruction in other anatomical regions, but its use in hand surgery is rarely reported. METHODS: A narrative review of the literature was conducted using the PubMed and Scopus databases to identify clinical and expe

Subscapularis Avulsion in a 12-Year-Old Wrestler.

BACKGROUND: Traumatic rotator cuff injuries are infrequently encountered among pediatric patients. Literature describing isolated subscapularis injuries, specifically with an associated avulsion fracture of the lesser tuberosity, is limited to case reports, with most patients undergoing operative management. INDICATIONS: In this video submission, we present the case of a 12-year-old male patient who presented to the clinic with right shoulder pain following a traumatic injury during competitive wrestling. The patient's examination was notable for pain and weakness with testing of subscapulari

Role of Elastic Intramedullary Nails in the Stabilisation of Displaced Midshaft Clavicular Fractures in Adolescents: A Case Series.

The aim of this study is to investigate the post-operative functional outcome following the use of titanium elastic intramedullary nailing for non-comminuted, displaced midshaft clavicle fractures in adolescents with greater than 20 mm shortening. Clavicular fractures are common acute shoulder injuries. In children and adolescents, mid-shaft clavicular fractures are managed non-operatively due to the better healing tendency of the bone. However, adolescents are commonly involved in high-energy sports and are poorly compliant with non-operative measures, which could affect healing. Studies have

Outcomes After Arthroscopic Bankart Repair With and Without Biceps Tenodesis in a Military Population.

INTRODUCTION: Active duty U.S. Military service members experience rates of anterior shoulder instability up to twenty times greater than the general population. Arthroscopic Bankart repair is the primary treatment for anterior shoulder instability without bone loss, though coexistent pathology involving the long head of the biceps tendon (LHBT) or biceps labral complex (BLC) may necessitate additional intervention. Biceps tenodesis (BT) is frequently employed in such cases. Although BT has been studied extensively in the context of superior labrum anterior to posterior (SLAP) tears and rotato

Primary Stability of a New TendoClip Fixation for Tuberosity Refixation in Total Shoulder Arthroplasty.

Sufficient refixation and healing is crucial for the function of a reverse shoulder arthroplasty within the treatment of proximal humeral fractures. Conventional refixation with suture cerclage is time-consuming and leads to healing in only 60-70% of cases. This study used a biomechanical test to investigate two different refixation procedures for treating 4-fragment fractures of the proximal humerus. Eight paired humerus specimens were randomly assigned to two groups and treated with reverse shoulder prostheses. One group received conventional suture cerclage fixation for 4-fragment fractures

Shoulder morphology and throwing shoulder pain: Associations with humeral retroversion angle and posterior glenohumeral distance.

PURPOSE: To examine the association between shoulder joint morphology and a history of throwing shoulder pain. METHODS: Male collegiate and corporate league baseball players (n = 21; 20.2 ± 1.5 years) underwent bilateral shoulder imaging using open magnetic resonance imaging (MRI) in simulated abduction-external rotation positions replicating the late cocking phase. The posterior glenohumeral distance (PGHD) and humeral retroversion angle were measured. Side-to-side measures were calculated as the %PGHD and the difference in the humeral retroversion angle; higher values indicated greater morp

A Novel Methodology for Establishing Best Values for MCID, SCB, and PASS Thresholds for Rotator Cuff Repair.

BACKGROUND: The heterogeneity of threshold values for the minimal clinically important difference (MCID), substantial clinical benefit (SCB), and Patient Acceptable Symptom State (PASS) as reported in the rotator cuff repair literature undermines the utility of these concepts. This systematic review identifies studies with published threshold values and proposes a methodologic framework for determining which values should be used for rotator cuff repair moving forward. PURPOSE: To provide recommendations for the MCID, SCB, and PASS thresholds of commonly utilized patient-reported outcome meas

Structural Augmentation in Rotator Cuff Repair Decreases the Risk of Retear: A Systematic Review and Meta-analysis.

BACKGROUND: Rotator cuff tears are a common orthopaedic injury often resulting in shoulder pain and functional impairment. Despite the prevalence of rotator cuff tears, the incidence of retear after rotator cuff repair (RCR) remains high. There is growing interest in the use of structural augmentation in RCR to decrease retear rates and improve patient outcomes. PURPOSE: The purpose of this systematic review and meta-analysis was to compare the clinical and imaging outcomes of structural augmentation versus standard RCR in patients with full-thickness rotator cuff tears. The effectiveness of

Effect of Subacromial Bursa Augmentation on Self-Reported and Magnetic Resonance Imaging Outcomes in Arthroscopic Rotator Cuff Repair: A Prospective, Blinded Randomized Clinical Trial.

BACKGROUND: Rotator cuff repair (RCR) failure remains a persistent clinical challenge despite advances in surgical techniques. Although the subacromial bursa-historically excised during repair-is recognized for its regenerative potential due to its rich population of connective tissue progenitor cells, high-quality clinical evidence supporting its utility as an autologous biological adjunct remains lacking. HYPOTHESIS: Subacromial bursa augmentation during arthroscopic RCR improves patient-reported outcomes and magnetic resonance imaging (MRI)-based tendon healing compared with standard repai

Type 4 Acromioclavicular Joint Separation Reconstruction With Suspensory Fixation, Allograft, and Suture Augmentation.

BACKGROUND: This is a case of surgical treatment of a type 4 acromioclavicular (AC) joint separation via an arthroscopically assisted approach and reconstruction with combined suspensory fixation, allograft ligament reconstruction, and suture augmentation. AC joint injuries are relatively common, representing approximately 9% of shoulder injuries and approximately 50% of sport-related shoulder injuries. Most AC joint injuries do well when treated conservatively. However, the literature supports surgical treatment for high-grade injuries, failed conservative treatment, and open injuries or thos

Isolated Extra-articular Medial Dislocation of the Long Head of the Biceps Tendon in an Adolescent: A Case Report.

Isolated instability of the long head of the biceps tendon is uncommon and is usually associated with rotator cuff or bicipital pulley injuries. Isolated extra-articular medial dislocation in the absence of associated lesions is particularly rare in the adolescent population. We report the case of a 15-year-old male who presented with anterior shoulder pain after a recreational sports activity. Physical examination revealed localized anterior and anterolateral tenderness, with preserved active and passive range of motion and no clinical signs of instability. Magnetic resonance imaging using MR

[Treatment of acute, first-time posterior shoulder instability].

BACKGROUND: Posterior shoulder instability is a complex and frequently underdiagnosed condition. In the context of primary manifestation, first-time (sub-)luxation (Moroder type A1) are distinguished from first-time traumatic dislocation (Moroder type A2). Posterior shoulder dislocation is particularly difficult to diagnose in emergency settings and is often overlooked. DIAGNOSTICS: Clinical findings include a prominent coracoid and loss of active and passive external rotation. Radiographic features such as the lightbulb sign, vacant glenoid sign, or rim sign may be present, yet the diagnosis
