From the experience with one-stage osteosynthesis of two segments in 71 patients with multiple fractures of the extremities and their sequelae the author considers that up-to-date achievements of anesthesiology favour such interventions. Traumatic dislocations of the sternoclavicular joint may be anterosternal or retrosternal. Anterior dislocation is due to forces which retract and depress the clavicle. Posterior dislocation is due to either direct force on the medial end of the clavicle or to a force acting on the posterolateral aspect of the shoulder. From 1950 to 1974 we treated 16 patients with traumatic complete sternoclavicular dislocations. Twelve patients were followed and their cases are discussed. Treatment may be closed or open. In some cases we did not attempt reduction because it may be very difficult to maintain

Posterior dislocation of the shoulder is a rare lesion. We have seen six of these dislocations in recent years. Review of these six cases indicated that posterior dislocations of the shoulder have consistent causes and physical and roentgenographic findings. Recent dislocations can be treated conservatively with closed reduction. Recurrent or unreduced dislocations can be treated successfully with the McLaughlin technic of open reduction and suturing of the subscapularis tendon into the defect in the humeral head. Our results were satisfactory with either closed or open treatment.

Following traumatic retrosternal dislocation of the clavicle, the patient presented with thoracic outlet syndrome 6 months postinjury with swelling and cyanosis of the right upper extremity, and cramps of the right arm and forearm after heavy manual work. When open reduction of the dislocated clavicle failed, the medial half of the clavicle was resected. Four years postoperation, the patient is doing well, and is asymptomatic. A review of the literature, anatomy, mode of injury, mechanism of injury, pathology, clinical picture, diagnosis and treatment are discussed.

The records of 146 consecutive patients with mastectomy operations over a three year period were reviewed. Of these, ninety-three (63.6 per cent) had one or more complications secondary to surgery. There were 175 complications, of which complications, of which 107 (73.2 per cent) were wound-related. Seroma formation occurred most frequently but was of minor consequence. In forty patients treated at The University of Texas Medical Branch Hospitals for fifty-two fractures of forty-one scapulae from 1961 through 1973, most of the fractures were the result of vehicular accidents. The results are reported in 21 patients who had acute acromioclavicular joint dislocation treated by open reduction and Bosworth screw fixation, with an average follow-up period of ten years. Nineteen patients had a good or excellent functional result. Good cosmetic results were obtained. Full recovery of shoulder movement occurred in all patients but one, although coracoclavicular calcification was a frequent finding. Complications following this technique were few.

An open dislocation of the shoulder is a very uncommon and serious injury. The sequelae of open glenohumeral dislocation include infection, avascular necrosis, and limited motion. The patient with such complications will have serious compromise in shoulder function, as illustrated in this case report and reference to another report in the literature.

An analysis of 144 sports accidents of 275 tennis players in Switzerland (203 men, 72 women) showed fractures of bones in 5,5% open wounds in 14,5%, torsions in 33% and other injuries in 47%. Vandeput and J. Tanner's method for the reconstruction of the reticular grafting skin has been used and is recommended by the authors for the treatment of not only the thermic--, but of the mechanis lesions. The author has obtained very good results. The method is discussed and its advantages are pointed out.

Internal fixation is considered for the treatment of 10 percent at most of all fractures that occur in childhood. Only minimal fixation should be applied, to avoid the induction of an epiphyseal lesion in addition to the original fracture. Internal fixation allowing stability during weight-bearing or exercise immediately is not necessary in a child. Concomitant soft-tissue damage hardly affects internal fixation in childhood, except in the case of unstable reduction when there is extensive soft-tissue damage.

The surgical treatment of a severe injury in the cubital region of a bus-driver, aged 47, is reported. Because of the extended contamination and the splintered fracture radical wound excision - involving also the chondral surfaces - has been performed and hereupon humero-radial arthrodesis was carried out. The skin defect has been successfully treated secondarily by insert of a flap. After uneventful recovery the patient could resume his work 6 months after the injury again.

In treatment of 50 patients with injuries of large vessels of limbs the authors have employed all kinds of vascular plasty, including lateral and circular sutures, putting of a patch on the vascular wall, replacement of an arterial defect with an autovenous graft. Autovenous plasty employed in various modifications in 21 cases is felt to be the method of choice in operations on injured arteries in cases of an extensive damage of soft tissues of limbs and open fractures. The results of the treatment are described.
