Low-Volume Superior Trunk and Superficial Cervical Plexus Block With Sedation as the Sole Regional Anesthetic for Proximal Humerus Fracture Fixation in a Patient With Alzheimer Disease.

BACKGROUND Proximal humerus open reduction and internal fixation (ORIF) is usually performed under interscalene brachial plexus block (ISB) or general anesthesia (GA). ISB causes ipsilateral phrenic nerve paresis in nearly all patients, and GA may raise the risk of postoperative neurocognitive disorders in elderly patients with cognitive impairment. We report the feasibility of a low-volume regional technique, used as the sole regional anesthetic with sedation, in a patient in whom GA avoidance was desirable. CASE REPORT A 68-year-old woman (American Society of Anesthesiologists physical statu

Clinical validation of the DRoPH score: a preoperative tool for estimating reduction difficulty in proximal humerus fractures.

BACKGROUND: Proximal humerus fractures are common injuries that are frequently associated with considerable technical challenges during surgical reduction and fixation. Although anatomical reduction is crucial for fracture healing and functional recovery, the anticipated difficulty of achieving satisfactory reduction is difficult to estimate preoperatively. The Difficulty of Reduction of Proximal Humerus Fractures (DRoPH) score was recently developed through a structured expert consensus process to estimate anticipated reduction difficulty based on predefined fracture- and patient-related char

Donor site morbidity and patient satisfaction after arthroscopic superior capsule reconstruction using fascia lata autograft for irreparable rotator cuff tears.

BACKGROUND: Donor site morbidity-previously a major concern-remains a considerable issue in arthroscopic superior capsule reconstruction using a fascia lata autograft due to the limited scientific evidence. We aimed to evaluate donor site morbidity and patient satisfaction after arthroscopic superior capsule reconstruction using a fascia lata autograft for irreparable rotator cuff tears. We hypothesized that the procedure would result in certain donor site morbidities, would not have a significant impact on daily activities, and that improvements in shoulder function could compensate for donor

Arthroscopic and mini-open rotator cuff repair yield comparable clinical and structural outcomes in small to medium full-thickness tears: a systematic review and meta-analysis of randomized controlled trials' importance.

IMPORTANCE: Arthroscopic rotator cuff repair (ARR) and mini-open rotator cuff repair (MRR) are both widely used, but whether ARR offers clinical advantages over MRR remains uncertain. OBJECTIVE: The aim of this study was to compare the clinical and structural outcomes of ARR and MRR in adult patients with rotator cuff tears. EVIDENCE REVIEW: A systematic search of PubMed, Scopus, and the Cochrane Library was performed from inception to November 2025 for randomized controlled trials (RCTs) directly reporting functional score, shoulder range of motion (ROM), re-tear rate, pain, or complication

Proximal humerus fractures: A review of current management evidence and guidance on contemporary surgical strategies.

Proximal humerus fractures (PHFs) most commonly affect elderly, osteoporotic women, although younger patients sustain them through high-energy trauma. Both incidence and the associated healthcare burden continue to rise with demographic ageing. Non-operative management is preferred for undisplaced fractures and for many elderly patients, whereas a range of internal fixation methods and arthroplasty options are available for more complex injuries. Patient age, comorbidities, functional demands and fracture pattern are the primary determinants of treatment selection, implant choice and expected

Stress-Related Acromion Base Fracture after Clavicular Hook Plate Fixation: A Case Report.

INTRODUCTION: Clavicular hook plate fixation is widely used for the surgical management of high-grade acromioclavicular (AC) joint injuries and generally provides reliable clinical outcomes. Nevertheless, implant-related complications involving the acromion have been reported. Stress-related fractures at the base of the acromion following hook plate fixation are extremely rare and remain poorly described in the literature. CASE REPORT: A 48-year-old woman sustained a high-grade AC joint injury and underwent open reduction and internal fixation with a clavicular hook plate. Two months after su

Upper-extremity Injuries from Side-by-side All-terrain Vehicle Accidents: Experience and Outcomes in over 200 Patients.

BACKGROUND: Side-by-side all-terrain vehicle (ATV) crashes produce high-energy upper-extremity trauma requiring complex reconstruction. Despite the increasing prevalence of these injuries, characterization of injury patterns, management, and outcomes remains limited. METHODS: A multicenter retrospective review was performed across 9 level I trauma centers from 2019 to 2023. Adult patients with upper-extremity injuries from side-by-side ATV crashes who were evaluated by the hand surgery service were identified through a trauma registry and manual chart review. Demographics, injury characterist

Analysis of factors associated with persistent night pain after locking plate fixation of proximal humerus fractures and development of a prediction model.

BACKGROUND: Proximal humerus fractures are common fragility fractures in older adults. Open reduction and locking plate internal fixation can restore fracture alignment and facilitate early rehabilitation; however, some patients still develop persistent night pain after surgery, potentially impairing sleep and tolerance of rehabilitation. This study aimed to analyze factors associated with persistent night pain 3 months after locking plate fixation for proximal humerus fractures and to construct a preliminary preoperative risk model and a complete risk stratification model after immediate post

Surgical treatment of complex scapular fractures: a single centre prospective observational study.

BACKGROUND: Conservative management is traditionally preferred for scapular fractures, but recent studies suggest that surgical intervention may improve outcomes in selected patients. This study evaluates functional and imaging outcomes, complication rates, and under-reported factors, such as duration of surgery and time from injury to definitive fixation, in surgically treated patients. METHODS: This prospective observational study included 12 patients who underwent surgery for scapular fractures between 2016 and 2022. Inclusion criteria were complex scapular fractures, defined as displaced

Chronic Overuse Injuries in Young Athletes-Upper Extremity.

Young athletes are uniquely susceptible to overuse injuries of the upper extremity because their open physes and apophyses are structurally weaker than adjacent bone, ligament, and tendon. Repetitive sport-specific loading-torsional stress in throwers, axial compression in gymnasts and climbers-can outpace the repair capacity of growing tissues, producing a spectrum of physeal and apophyseal stress injuries from widening and sclerosis to frank epiphysiolysis. This review examines the mechanisms, clinical presentation, and imaging findings of chronic overuse injuries at key upper extremity site

Traumatic Serratus Anterior Muscle Tear Presenting as Isolated Posterior Shoulder Pain in a Professional Volleyball Athlete: A Case Report.

Serratus anterior muscle injuries are rare, most commonly reported in rowing and overhead throwing athletes. These injuries typically present with anterolateral chest wall pain and a palpable mass. Atypical presentations may lead to diagnostic challenges, particularly when symptoms mimic shoulder pathology. We report the case of a 26-year-old male professional volleyball player who presented with acute-onset posterior shoulder pain following a serving motion. The pain persisted for two weeks and was exacerbated by overhead movements, particularly during spiking. Physical examination revealed l

Shoulder Surgery Disasters: How to Manage Complications and Failures After Arthroscopic and Open Shoulder Surgery.

It is important to discuss the optimal treatment options for the common complications after arthroscopic and open shoulder surgery based on both personal experience and evidence available in the literature. Discussion of complications after shoulder stabilization, rotator cuff repair, proximal humerus fracture fixation, coracoclavicular ligament repair, and shoulder arthroplasty can highlight both the optimal treatment pathway and technical tricks along with postoperative outcomes. The orthopaedic surgeon should be informed about the decision-making process for the management of common complic

Periprosthetic Fractures Around Shoulder Arthroplasty: Tips and Tricks for Management of Challenging Injuries.

Periprosthetic fractures associated with shoulder arthroplasty are on the rise, which include humeral shaft fractures as well as scapular spine and acromion fractures. Although some of these fractures can be treated nonsurgically, surgery is indicated in certain situations. Because it is related to periprosthetic humeral shaft fractures, the Unified Classification System for Periprosthetic Fractures can be used to guide treatment. Type A fractures are stable and can be treated nonsurgically. Type B fractures at the tip of the stem have a high risk of failure with nonsurgical treatment, and ben

Proximal Humerus Fractures in Children: To Fix or Not to Fix?

Proximal humerus fractures are a common upper extremity injury that the pediatric orthopaedic surgeon will certainly encounter. Most of these fractures are managed with closed reduction with expectant remodeling. However, inevitably, some fractures are not well suited to minimal or nonsurgical management. Whether the fracture alignment would cause functional impingement or rotational impairment, if the fracture is irreducible, or if the patient is an adolescent with minimal remodeling potential, there are various treatment options. Closed reduction and creative immobilization techniques can ad

CT of Acute Shoulder Girdle Fractures in Adults: Biomechanics, Classification, and Management.

The shoulder girdle, which includes the clavicle, scapula, and proximal humerus, forms a dynamic scaffold for seamless motion and load transfer to the axial skeleton. CT with multiplanar reformatted and volume-rendered images allows precise characterization, measurement, and injury classification and is particularly useful for complex high-energy disruptions encountered in trauma centers. The spectrum of shoulder girdle injuries spans clavicle fractures, acromioclavicular joint separations, floating shoulder patterns, glenohumeral fracture-dislocations, glenoid fossa fractures, proximal humeru
