Fractures of the clavicle in the middle third usually undergo closed treatment by external fixation, because they heal spontaneously in most cases. However, a correct reposition and fixation by external manner is sometimes impossible, and consolidation in malposition with functional and esthetic disturbance occurs. External fixation can be painful and inconvenient to the patient, and congestion and paresthesia of the upper limbs may occur. Therefore the indication for internal fixation has been extended on fractures in persisting severe malposition despite external fixation and painful congest

During a seven-year prospective study, 158 humeral shaft fractures in 157 patients were treated with closed intramedullary nailing. Only humeral fractures not responsive to closed reduction and immobilization and those in noncomplaint patients were eligible. No immobilization was used postoperatively. Seven patients were lost to follow-up. A total of 148 fractures healed; there were three nonunions. The average time to clinical union was 7.2 weeks. There were no infections or malunions. Eight of 11 preoperative and three postoperative radial nerve palsies were lesions in continuity and healed

A prospective study of closed intramedullary fixation of fractures of the shaft of the humerus, using Ender nails, was performed over a six-year period. Eighty-nine fractures in eighty-eight patients were treated with no immobilization postoperatively. Three patients were lost to follow-up. Eighty-five of the remaining eighty-six fractures healed, the average time to clinical union being 7.2 weeks. Non-union of one fracture occurred and there were no infections or malunions. Six of the nine preoperative and two postoperative radial-nerve palsies were lesions in continuity and healed spontaneou

This is the case of a thirty-year-old athlete suffering from a painful right shoulder while moving and working with his right arm and showing a massive atrophy of his right infraspinatus muscle. After careful examination an isolated compression of the inferior branch of the suprascapular nerve was suspected. The entrapment of the nerve at the spinoglenoid notch by a large and hypertrophied ligament was demonstrated by an open revision.

167 patients with fracture dislocations of the proximal humerus--operated upon between 1970 and 1980--were followed up on an average 43.2 months after the accident. Fractures were classified according to Neer in two-part-(n = 24), three-part-(n = 60) and four-part-fractures (n = 61) as well as fractures of the articular surface (n = 21). 20.4% of all fractures were complicated by a neurological lesion. In 31% of all cases a good or excellent result was obtained. The functional result and the rate of avascular head necrosis were dependent on the type of fracture, that means the number of displa

In a series of 27 patients it is shown that percutaneous K-wiring in fractures of the head of the humerus can be regarded complementary to the conservative treatment. It can as well compete with open reduction and internal fixation by means of plate-or screw-osteosynthesis. The advantages of this method are as follows: simple procedure, quick operation, sufficient stability and easy removal of the implants particularly in elderly patients.

Results after conservative or surgical treatment of comminuted fractures of the head of the humerus are often unsatisfactory and complicated by pain and a severely decreased range of motion. In these patients good results have been obtained by replacement of the humeral head with the Isoelastic Humeral Head Prosthesis. Fourteen cases are reported and their results discussed.

Surgical intervention is indicated in the vast majority of humerus head fracture dislocations. In the present paper, we report on the results of 24 such interventions performed between 1975 and 1984. Implantation of a T-plate is considered the standard procedure to obtain osteosynthetic stability during exercise. However, in elderly patients or in cases of multiple fragment fracture, this procedure is inadequate to achieve the required stability. In such cases, development of a humerus head necrosis would rather be promoted. If a sufficiently stable osteosynthesis without the risk of necrosis

A new operative approach is presented for treatment of fractured distal end of clavicle associated with recurrent anterior shoulder subluxations. One case is presented in which open reduction of the fractured clavicle and stabilizing the anterior shoulder joint were accomplished by transfer of the coracoid process with its attached muscles under the subcapularis muscle onto the clavicle. The operation was followed by complete reduction of the fracture and a stable shoulder joint.

We have reviewed the notes and radiographs of 57 patients with fractures of the proximal humeral epiphysis and examined 30 of them at 2 to 8 years after injury. Regardless of treatment the maximum shortening of the humerus was 2 cm and residual varus angulation was insignificant. Manipulation of a displaced, fresh fracture did not improve the final outcome with respect to humeral growth or function; and open reduction is very rarely indicated.

Displaced intra-articular glenoid fractures are extremely rare. Anatomic reduction, rigid internal fixation and early mobilization restored a full, painless range of motion in our two cases.

Five patients with humeral nonunions were treated by open reduction and internal fixation with compression plating and adjunctive methylmethacrylate inserted by use of several techniques to gain secure screw purchase in markedly osteoporotic bone. The rigid fixation was supplemented in each case with cortical-cancellous bone grafting. Postoperative abduction splinting was applied in each case. The average follow-up examination was 24 months, with all five nonunions having healed at an average of five months postoperation. The shoulder arc of motion averaged 120 degrees, and the elbow arc of mo

Thirty-six patients with an acute fracture of the humeral shaft were treated by open reduction and internal fixation using AO plating techniques. In thirty-two patients an open fracture or multiple injuries, or both, were the indications for internal fixation. Four patients were treated after non-operative treatment failed to maintain a satisfactory reduction. Follow-up was possible for thirty-four patients and showed that thirty-three fractures united primarily and one failed to unite, necessitating two subsequent procedures. Two superficial wound infections in patients with an open fracture

Understanding neck injuries in football players starts with understanding how the game is played--not the strategy, but the training techniques and requirements of the game. Neck injuries are an unavoidable part of the game. What can be improved are the preventative measures, the treatment techniques, and some standardization of risk factors in playing after a neck injury. Prevention starts with decreasing the use of the head as an offensive weapon and with proper shoulder pads. Treatment begins with proper on-field evaluation and transportation techniques and is completed by good consultation

An elbow arthrodesis was performed as a salvage procedure in seven patients with an open elbow arthrosis resulting from open trauma and the multiple surgeries that had followed. Patients had undergone an average of four previous elbow operations since their initial injury. Patients had pain, limited motion and instability that seriously compromised ipsilateral upper extremity function. Hardware was placed posteriorly in five patients, and in two patients with predominantly posterior pathology and infection, it was placed anteriorly. All but one case was fused at 90 degrees of elbow flexion. Wo

The clinical and radiological features of fractures of the shoulder region in 16 horses are described. High quality radiographic views were essential for an accurate diagnosis. This sometimes necessitated general anaesthesia. The limitations of radiography are discussed. Nine horses had articular fractures of the supraglenoid tuberosity and eight of these were destroyed because of persistent lameness. The possibility of surgical removal of the fracture fragment(s) is discussed and the importance of early diagnosis emphasised. Seven other fractures of the shoulder region are described. The clin

Between January of 1978 and December of 1983, 41 patients developed deep sternal infections with mediastinitis after cardiac operations. Between January of 1978 and December of 1981, 19 of these patients were treated with débridement, primary wound closure, and mediastinal antibiotic irrigation (Group I). Between January of 1982 and December of 1983, 22 patients were treated with débridement, open "clean" packing, and delayed wound closure by the technique of pectoral muscle flap mobilization, which preserves the thoracoacromial pedicles and the pectoral humeral attachments (Group II). Ten consecutive patients with clavicular non-union were treated at the University of California, Davis, Medical Center and Kaiser Permanente Hospital, Sacramento, with compression-plating and iliac bone-grafting. Seven patients were male and three were female, and their ages ranged from sixteen to sixty years. All patients achieved clinical union by ten weeks postoperatively, with no operative or postoperative complications, and had a full, painless range of motion of the ipsilateral shoulder, with an acceptable cosmetic result. In our experience, patients with symptomatic clavicular non-union

In this paper late results of 65 proximal humeral fractures, operated upon between 1970 and 1980 are reported. Fractures were classified according to Neer in two-part-(28), three-part-(15), four-part-fractures (21) and fractures of the articular surface (1). In 34 cases internal fixation was performed by a T-plate. In 15 cases either open reduction without internal fixation or open reduction and minimal internal fixation was done using screws and K-wires. Primary prosthetic replacement of the head was done in 8 patients with four-part-fractures. The follow-up period ranged from 2.5 years up to

We report about two patients with posterior fracture-dislocation of the shoulder following convulsive seizures, one case unilateral, one case bilateral. The radiological findings are typical and similar to the cases described in literature: Impression or head splitting fracture of the ventro-medial part of the humeral head associated with posterior dislocation. The mechanism of injury, methods of treatment and late results are discussed.
