Axillary artery occlusion with erect dislocation of the shoulder.

Occlusion of the axillary artery occurred in a man with erect dislocation (luxatio erecta) of the shoulder. This combination of injuries has not previously been reported. The arterial injury was demonstrated by arteriography and repaired by a vein graft. There was an associated fracture of the greater tuberosity of the humerus, which required open reduction and internal fixation.

Fracture and complete dislocation of the thoracic or lumbosacral spine: report of three cases.

Severe fracture/dislocation of the thoracic (T) or lumbar (L) spine is usually associated with complete neurological dysfunction below the level of injury. Three cases of severe spinal fracture/dislocation are presented in this report. Two of these patients suffered only partial neurological deficit, which improved after open reduction, internal fixation, and bone fusion. Severe fracture/dislocation of the T or L spine may be quite obvious on lateral roentgenograms. Occasionally, however, the fracture site may be obscured by the overlying shoulders, hips, or soft tissues. If no lateral displac

Posterior shoulder dislocation with ipsilateral humeral shaft fracture. A case report.

A 41-year-old man incurred the unusual combination of subacromial posterior dislocation of the shoulder and ipsilateral humeral shaft fracture. The importance of inspection of joints proximal to long bone fractures cannot be overemphasized. The transscapular view is easily adapted to the diagnosis of this injury. When recognized early, closed reduction is the preferred treatment; open reduction, as in the present case, is reserved for chronic dislocation.

An operative repair of acromioclavicular separation.

Sixteen patients underwent an open reduction for complete acromioclavicular separation with a loop of woven Dacron arterial graft. This technique of internal fixation permits an early range of shoulder motion and eliminates the need for a second procedure to remove wire or pins. Experience in vascular reconstruction suggests that fibroblastic ingrowth might occur to substitute for the disrupted ligament complex. All but two of the 16 patients showed excellent results: full restoration of shoulder function, satisfactory cosmesis, and complete relief of pain. One required reoperation for an impr

Pattern of bone injury in Ibadan.

A series of 1,329 patients with 1,327 fractures and 100 dislocations has been studied. Boys under 10 years of age predominated. The majority of the injuries resulted from falls or road traffic accidents. Upper limb fractures occurred more commonly than those of the lower limbs or axial skeleton. Femoral shaft fractures were 2-1/2 to 5 times more common while femoral neck fractures were three to five times less common than in Americans. Dislocations were most common at the elbow, followed by the shoulder. Only 11.5% of the patients had open operations; the others were treated conservatively. Th

The pattern of fractures and dislocations in western Nigeria.

A consecutive series of 2579 patients with 2571 fractures and 189 dislocations was studied prospectively. Males and children, especially under 10 years of age, predominated. The majority of the injuries resulted from falls either in the home or outdoors and from road traffic accidents. Fractures occurred more commonly in the upper limb than in the lower limb or the axial skeleton. Fractures of the shaft of the femur were three times more common, while fractures of the neck of the femur were less common by the same factor, in Africans than in Caucasians. Dislocations were commonest at the shoul

Traumatic sternoclavicular dislocation.

Traumatic dislocations of the sternoclavicular joint may be anterosternal or retrosternal. Anterior dislocation is due to forces which retract and depress the clavicle. Posterior dislocation is due to either direct force on the medial end of the clavicle or to a force acting on the posterolateral aspect of the shoulder. From 1950 to 1974 we treated 16 patients with traumatic complete sternoclavicular dislocations. Twelve patients were followed and their cases are discussed. Treatment may be closed or open. In some cases we did not attempt reduction because it may be very difficult to maintain

Posterior dislocation of the shoulder: report of six cases.

Posterior dislocation of the shoulder is a rare lesion. We have seen six of these dislocations in recent years. Review of these six cases indicated that posterior dislocations of the shoulder have consistent causes and physical and roentgenographic findings. Recent dislocations can be treated conservatively with closed reduction. Recurrent or unreduced dislocations can be treated successfully with the McLaughlin technic of open reduction and suturing of the subscapularis tendon into the defect in the humeral head. Our results were satisfactory with either closed or open treatment.

Fractures of the scapula. An analysis of forty cases and a review of the literature.

In forty patients treated at The University of Texas Medical Branch Hospitals for fifty-two fractures of forty-one scapulae from 1961 through 1973, most of the fractures were the result of vehicular accidents. The thirty-eight fractures involving only the body, neck, or spine of thirty scapulae (without involvement of the acromion, glenoid, or coracoid process) were successfully treated with a sling and early active motion. Three other fractures involving body were associated with three fractures involving the glenoid. The eleven fractures of the acromiom, glenoid, or coracoid process resulted

Acute acromioclavicular joint dislocation: results of operative treatment with the Bosworth screw.

The results are reported in 21 patients who had acute acromioclavicular joint dislocation treated by open reduction and Bosworth screw fixation, with an average follow-up period of ten years. Nineteen patients had a good or excellent functional result. Good cosmetic results were obtained. Full recovery of shoulder movement occurred in all patients but one, although coracoclavicular calcification was a frequent finding. Complications following this technique were few.

Open anterior dislocation of the shoulder.

An open dislocation of the shoulder is a very uncommon and serious injury. The sequelae of open glenohumeral dislocation include infection, avascular necrosis, and limited motion. The patient with such complications will have serious compromise in shoulder function, as illustrated in this case report and reference to another report in the literature.

Fractures of the proximal humeral epiphysis.

Displacements of the proximal humeral epiphysis at the epiphyseal plate result from extension injuries. The configuration of the epiphyseal plate and the thickness of the periosteum surrounding the epiphysis make slight to moderate displacements relatively stable injuries, which can be treated successfully with external support. In patients with more severe displacement, flexion, abduction, and slight external rotation of the distal fragment bring the metaphysis into alignment with the proximal fragment, correcting the anterior angulation. The thick periosteal sleeve attached to the physis thr

[Internal fixation in the presence of soft-tissue damage in childhood (author's transl)].

Internal fixation is considered for the treatment of 10 percent at most of all fractures that occur in childhood. Only minimal fixation should be applied, to avoid the induction of an epiphyseal lesion in addition to the original fracture. Internal fixation allowing stability during weight-bearing or exercise immediately is not necessary in a child. Concomitant soft-tissue damage hardly affects internal fixation in childhood, except in the case of unstable reduction when there is extensive soft-tissue damage.
