Fractures of the distal radius are common injuries and may result from high-energy or low-energy trauma. Treatment options include non-surgical and surgical management, and although high-level evidence does not support surgical management for all of these fractures, there is a clear increasing trend in plate fixation for fractures of the distal radius. Where there is an indication to manage these surgically, open reduction and internal fixation of distal radius fractures is a core surgical skill that most trauma surgeons possess. However, some of these fractures may present in complex or atypi

BACKGROUND: Many overhead athletes, both amateur and professional, experience an increasing number of injuries to the ulnar collateral ligament (UCL) as a result of overuse or trauma. As a result of this increasing frequency, multiple techniques have been developed to reconstruct or repair the UCL with success and faster return to sports (RTS) for athletes who are ideal candidates for a repair. However, a large proportion of UCL tear patients cannot undergo repair due to chronic injury, leading to poor tissue quality, midsubstance tears, or older age, which predispose them to slower healing an

Combined acromioclavicular dislocation and coracoid process fracture disrupts the superior shoulder suspensory complex, leading to shoulder instability that often requires surgical intervention. This rare injury, increasingly identified through advanced diagnostics, lacks consensus on the optimal treatment. We describe a minimalist arthroscopy-assisted technique using a single cannulated screw for simultaneous fixation of both injuries. The procedure involves lateral decubitus positioning, diagnostic arthroscopy with a 70° scope for fracture visualization and debridement, ACL drill guide-assis

Proximal humerus fractures are frequent injuries, representing the third most common fracture in geriatric patients. Due to the variety of fracture patterns, patient profiles, and subspecialty training of treating physicians, management of these fractures continues to garner debate. Nonoperative treatment remains the mainstay for most fractures; however, operative treatment has increased over the last decade. The purpose of this article is to provide a contemporary review of the clinical evaluation, classification, and management of proximal humerus fractures.

INTRODUCTION: Fatty infiltration (FI) of the rotator cuff (RC) muscles reflects chronic deterioration in muscle quality associated with tendon injury, disuse, ageing and nerve injury. Accurate and reliable assessment of FI based on imaging interpretation is essential for evaluating muscle degeneration, guiding rehabilitation planning and enabling meaningful comparisons across clinical and research settings. Several semi-qualitative grading systems and quantitative measurement approaches across imaging modalities, including CT, MRI, ultrasound and advanced techniques such as Dixon MRI have been

The shoulder complex exhibits a distinctive amalgamation of muscular power, integration, and an extensive range of motion. The shoulder joint allows for a wide range of upper limb movements because the four small muscles that make up the musculotendinous rotator cuff work together to keep the joint stable. The four muscles that come from the scapula and form the rotator cuff are the subscapularis, supraspinatus, infraspinatus, and teres minor. Accurate diagnosis and proper management of shoulder-related injuries are essential, particularly for sportspersons and individuals engaged in heavy wei

BACKGROUND: Post-operative nerve complications remain a concern following the Latarjet procedure due to the proximity of the musculocutaneous, axillary, and suprascapular nerves during key surgical steps. Reported incidence rates of nerve injuries with this procedure vary considerably in the literature. The aim of this study was to evaluate the incidence of early post-operative nerve complications in consecutive patients undergoing a primary open Latarjet procedure treated with a standardized surgical technique.
METHOD: A single-center, single-surgeon retrospective case series of patients und

INTRODUCTION: Humeral shaft fractures account for approximately 1-3% of all fractures and present a unique surgical challenge due to the proximity of the radial nerve. Retrograde titanium elastic nailing system (TENS) has gained increasing acceptance as a minimally invasive, load-sharing implant for managing such fractures, particularly in adolescents and young adults. This case series evaluates the functional and radiological outcomes of retrograde TENS in humeral shaft fractures.
MATERIALS AND METHODS: We present a series of 10 patients (age range 19-55 years; M:F = 7:3) treated with retrog

Introduction The deltopectoral approach is the workhorse approach used for plating of proximal humerus fractures. However, the less invasive and less commonly used deltoid-split approach has shown comparable outcomes in selected cases. Literature on its functional outcomes and complications remains limited, especially in the Indian subcontinent. This study evaluates the results of patients with proximal humeral fractures managed by open reduction and internal fixation (ORIF) with plating using proximal humerus locking plates (PHLPs) performed through the deltoid-split approach. PURPOSE: Surgical options are limited for distal humerus and proximal ulna injuries with articular bone loss in physiologically young patients. Osteochondral grafts are a potential treatment option between open reduction internal fixation and arthroplasty that does not impart lifelong activity restrictions and has longer longevity. Feasible graft harvest sites that match the surface geometry of the distal humerus and proximal ulna are unknown. This study assesses the surface geometry of potential grafts with known well-defined donor site morbidity for the reconstruction of distal humerus and p

AIMS: Reverse shoulder arthroplasty (RSA) after prior rotator cuff repair (RCR) is commonly performed for recurrence of pain and arthritic progression; however, the influence of prior RCR timing and technique on outcomes of RSA has not been adequately explored. Our primary aim was to evaluate the relationship between the postoperative American Shoulder and Elbow Surgeons (ASES) score, whether prior RCR was performed open or arthroscopic, and the time interval between RSA and prior RCR. Secondary outcomes were forward elevation (FE) and external rotation (ER).
METHODS: A prospectively collecte

BACKGROUND: Bodybuilders are uniquely at risk of shoulder injuries given training intensity, frequency, and heavy overhead loads through a full arc of motion. Latissimus tears are rare injuries that have increased incidence in overhead and weightlifting athletes, following forceful hyperabduction/adduction during sport-specific activities. Humeral avulsions of the latissimus tendon are treated with endosteal button fixation.
INDICATIONS: Endosteal button repair is indicated for acute or chronic latissimus tears up to 2 years after injury, in patients who are unable to return to their desired

Introduction Proximal humerus fractures (PHFs) represent the third most common adult fracture, increasingly affecting women and middle-aged individuals. While older patients typically sustain PHFs via low-energy trauma, younger populations more frequently incur these injuries through high-energy mechanisms. Surgical decision-making is influenced by patient-specific factors, with open reduction and internal fixation (ORIF) using locking plates considered a reliable technique for select cases. Despite demonstrated utility, limited data exist regarding functional outcomes of locking plate fixati

OBJECTIVE: Displaced scapular fractures are uncommon injuries, and the optimal surgical approach remains controversial. Muscle-preserving techniques may improve post-operative shoulder function by minimizing soft-tissue damage. The purpose of the study was to evaluate the clinical and functional outcomes of open reduction and internal fixation (ORIF) using a muscle-preserving modified Judet approach in displaced scapular fractures and to assess the effect of surgical timing on post-operative shoulder function.
METHODS: This retrospective study included 12 adult patients who underwent ORIF for

Pediatric lower extremity lawnmower injuries are high-energy traumas that often result in severe composite tissue loss and require advanced reconstruction. Compared with other mechanisms of pediatric lower extremity trauma, these injuries have a higher incidence of amputation and complex reconstruction. This case series describes 3 pediatric patients, aged 2.5-10 years, who sustained severe lower extremity lawnmower injuries that were managed with free tissue transfer (FTT). Reconstructions included a tensor fasciae latae perforator flap, a latissimus dorsi myocutaneous flap, and an anterolate
