Differential functions of ROCK1 and ROCK2 in liver fibrosis and advances in targeted therapeutic strategies.

Liver fibrosis, driven by metabolic dysfunction-associated steatohepatitis (MASH) and other chronic injuries, represents a global health crisis without approved pharmacological therapies. The Rho-associated coiled-coil kinases (ROCK1 and ROCK2), the principal effectors of Rho GTPases, have emerged as central nodes in fibrogenesis, yet the isoform-specific mechanisms and their translational potential remain incompletely defined. This review systematically dissects the divergent roles of ROCK1 and ROCK2 across the hepatic cellular landscape. We synthesize current knowledge on isoform-specific fu

Posterior Hip Dislocation with Ipsilateral Subtrochanteric Femur Fracture: A Rare Case Report.

INTRODUCTION: Posterior dislocation of the hip associated with an ipsilateral subtrochanteric femur fracture is a rare and technically challenging injury pattern, most commonly seen after high-energy trauma such as road traffic accidents. The associated proximal femoral fracture disrupts the normal femoral lever arm, making closed reduction difficult or impossible and often necessitating open reduction. Delay in diagnosis or treatment may result in complications such as avascular necrosis of the femoral head, post-traumatic arthritis, sciatic nerve injury, and poor functional outcomes. CASE R

Open Posterolateral Elbow Dislocation in an Adult after Low-Energy Trauma: A Case Report Highlighting Diagnostic and Operative Considerations.

INTRODUCTION: Open elbow dislocations are rare injuries in adults, accounting for <2% of cases, and may occur even after low-energy mechanisms. Their presentation can be subtle, particularly when periarticular wounds appear benign, leading to delayed recognition of joint violation. Early identification of intra-articular air and associated soft-tissue injury is essential to guide timely operative management. This case highlights the diagnostic challenges and surgical considerations associated with an uncommon open posterolateral elbow dislocation following a low-energy fall. CASE REPORT: A mi

High-dose rifampicin and clarithromycin for 4 weeks versus standard-dose rifampicin and clarithromycin for 8 weeks to treat Buruli ulcer: An open-label, individually randomized, controlled trial in Ghana.

BACKGROUND: The current 8-week regimen of rifampicin and clarithromycin for Buruli ulcer (BU) is suboptimal. High-dose rifampicin could shorten treatment and improve outcomes. We evaluated whether a 4-week high-dose regimen could improve time to clearance of viable Mycobacterium ulcerans. METHODOLOGY: In this open-label, individually randomised trial conducted in Ghana, participants with PCR-confirmed BU were assigned (1:1) to either high-dose oral rifampicin (20mg/kg) with clarithromycin (15mg/kg) for 4 weeks (HR) or standard-dose rifampicin (10mg/kg) with clarithromycin (15mg/kg) for 8 week

MgSO4 alleviates anxiety-like behaviours in mice after traumatic brain injury by inhibiting astrocytic TRPM7 in the amygdala.

BACKGROUND: Reports indicate anxiety as one of the most prevalent mental modifications that occur following traumatic brain injury (TBI). Transient Receptor Potential Melastatin 7 (TRPM7) is extensively distributed throughout the nervous system. It regulates intracellular homeostasis of calcium and magnesium ions, regulates the functions of glial cells, including astrocytes in the central nervous system, influences the inflammatory cascade, and may play a significant role in anxiety pathogenesis. However, the potential of astrocytic TRPM7 in the treatment of TBI-induced anxiety remains largely

Preserving the Bone Envelope: A Prospective Observational Study of Closed Reduction and Ilizarov Circular Fixation for Open and Closed Distal Tibial Fractures.

Distal tibial fractures are challenging injuries because of limited soft-tissue coverage, frequent high-energy mechanisms and the potential for soft-tissue compromise. Extensive surgical exposure may further disturb the local soft-tissue and biological environment. Ilizarov circular external fixation permits stable fixation with limited exposure of the fracture site. This prospective observational study evaluated fracture union, bone and functional outcomes and complications following closed or indirect reduction and Ilizarov circular external fixation in patients with open and closed distal t

Oropharyngeal Free Flap Inset With a Single Port Robot: A Case Series.

INTRODUCTION: Traditional oropharyngeal resection may require lip-split mandibulotomy approach (LSMA) which risks high morbidity. Transoral robotic surgery (TORS) through the oral pharynx mitigates these risks. Free flap (FF) inset following resection is challenging and may require open approaches, negating the benefits of TORS resection. We present our case series utilizing the single port (SP) robot for FF inset, which has improved visualization and functionality compared with a multiport robot. METHODS: A retrospective review of robotic FF inset following TORS using the SP from 2021 to 202

Trends in Upper Extremity Injuries in Female Marines following the Addition of the Pull-up as a Physical Fitness Test Event.

INTRODUCTION: In November 2012, the U.S. Marine Corps announced a policy change to the female Physical Fitness Test (PFT), replacing the flexed arm hang with pull-ups. This change, which became effective on January 1, 2017, aimed to standardize fitness requirements and facilitate gender integration into combat roles. Musculoskeletal injuries (MSKIs) are a leading cause of medical encounters and non-deployability among service members. Upper extremity (UE) injuries are particularly relevant given the physical demands of pull-ups. MATERIALS AND METHODS: A retrospective open cohort study was con

Utility of RECELL for Traumatic Skin Defects: Protocol for a Prospective, Single-Arm Pilot Study.

BACKGROUND: Skin grafting is currently the gold standard for managing traumatic skin defects. However, split-thickness skin grafting, the most common technique, is challenging because it yields a limited amount of skin and creates a new wound at the healthy donor site. RECELL (Avita Medical), a noncultured skin cell suspension technique, yields healing outcomes with comparable duration and quality to skin grafting for burn trauma. Although RECELL is widely used for burn trauma, its use for severe traumatic skin defects is not yet prevalent in Japan. OBJECTIVE: This study aims to evaluate the

Acute Repair of Latissimus Dorsi and Teres Major Tendon Tears.

BACKGROUND: Latissimus dorsi and teres major tendon tears are rare, typically occurring in high-level throwers but also from sudden shoulder extension or hyperabduction. While nonoperative management was previously preferred, recent literature favors operative repair, especially for complete, retracted tears. INDICATIONS: Recent literature suggests partial-thickness tears can be managed nonoperatively, allowing return to competitive play, while repair is recommended for higher-grade tears to restore full performance. This approach can also apply to young, active individuals seeking full shoul

Liposomal glutathione outperforms plain glutathione in uptake, cell regeneration and systemic availability: evidence from cellular and human models.

Glutathione is a key intracellular antioxidant involved in redox balance, detoxification and immune support. However, oral glutathione supplementation is limited by poor bioavailability due to gastrointestinal degradation and low membrane permeability. Liposomal formulations offer a promising strategy to enhance delivery and therapeutic efficacy. This study aimed to compare the biological activity and systemic availability of a novel liposomal glutathione formulation (LipoDuo™ Glutathione) with plain glutathione using both in vitro cellular models and in vivo human pharmacokinetic evaluation.
