From 1980-1988 twenty patients with vascular trauma were treated at the Kantonsspital Basel, Switzerland. The incidence of 1% in open fractures is low. We reviewed 11 cases of free tissue transfer used to salvage limbs in young patients (age range 4-16 years, mean 8.9 years). The defects were the result of acute trauma in eight cases, osteomyelitis in one, unstable scar over bony prominence in one, and correction of a previously infected talipes equinovarus repair in another patient. Most humeral shaft fractures are treated by early functional mobilization (Sarmiento-Bracing). In some situations (short transverse fractures, old age of patients living alone, extreme obesity, social circumstances) operative fixation may be warranted in individual patients. There are, however, several absolute and urgent indications for operative fracture stabilization: fractures involving nerve or vessel injury, open fractures, multiple fractures on the ipsilateral arm, and delayed fracture healing. Osteosythesis with a dorsally placed femoral DC plate gives the best results.

The authors analyze the treatment of 54 patients with 65 limb stumps resultant from injuries to the limbs in 41 patients shortly exposed to traumatic agents and in 13 patients after a prolonged crushing of the limb during the earthquake in Armenia. Split-thickness skin excision can be used as a one-stage procedure for the accurate diagnosis of flap viability and the immediate treatment of friction-avulsion injuries in severe open fractures. A prospective series of 32 consecutive patients, with 33 long-bone fractures suffering from delayed- or non-union were treated by pulsed electromagnetic fields (PEMF) or by PEMF with surgery. We evaluated the results of treatment in six patients with unreduced chronic anterior dislocations of the shoulder. Using a prefabricated brace, we treated 233 patients who had a fracture of the humeral shaft. One hundred and seventy patients were available for follow-up, which ranged from five weeks to forty-eight months.
