A case of open scapulothoracic dissociation with intact neurovascular status in an 8-year-old girl is reported. Scapulothoracic dissociation is a rare injury. Most cases present with significant soft tissue disruption in the shoulder region secondary to separation of the scapula from the thorax, and involve major vascular and brachial plexus injuries. This case report illustrates that an obvious roentgenographic scapulothoracic dissociation may be seen without any neurovascular deficit in a child. Successful treatment, with anatomical repair of the muscles and stabilization of the scapula, is

13 humeral shaft fractures were treated by Sarmiento-brace. Fracture-bracing is a special conservative traumatological management, that permits early remedial gymnastics. Light weight, optimal cosmetic results and quickly rehabilitation of the injured arm are favorable. Cooperation in gymnastics and controls within short times by the doctor are necessary. All our patients of ages under 45 years achieved nearly normal articular function, that increases 95% in comparison to the opposite arm. External rotation and abduction of the shoulder decreased in elder persons. Loss of function could not be

External fixation of forearm and arm fractures is an excellent treatment option, particularly in compound, comminuted fractures that result from high-energy trauma. Limb salvage and good function have been achieved in 75% of cases by applying the principles outlined. Problems with delayed bone union have been recognized and emphasize the importance of supplemental bone grafting. A number of external fixation systems are applicable to these upper limb fractures, and the authors have not found one system superior to the others. Open pin insertion is preferred, and pins (three proximal and three

An experience with the treatment of 547 patients with open fractures of long tubular bones of extremities is described. The method used for the treatment of the wounds and osteosynthesis gave less amount of complications and better results of the treatment.

We treated sixteen patients, all of whom had a large segmental defect of bone in an extremity, with transfer of a vascularized graft of the iliac crest. Thirteen patients had an open defect that required an osteomuscular or osteocutaneous graft; the other three had a closed segmental osseous defect and the graft of the iliac crest was transferred without soft tissue. The average length of the osseous defect was seven centimeters. For the defects of the lower extremity, the average time to osseous union was 8.8 months. For the defects of the upper extremity, it was four months. Three patients h

The treatment of fractures in growing children, especially shaft fractures, is subject to special rules compared to the treatment of such injuries in adult persons. Osteosynthesis is only rarely indicated. Marrow nailing involves great risks at the point where the nail is driven in. Therefore it is not indicated in case of open epiphyseal cartilages.

This is the case of a thirty-year-old athlete suffering from a painful right shoulder while moving and working with his right arm and showing a massive atrophy of his right infraspinatus muscle. After careful examination an isolated compression of the inferior branch of the suprascapular nerve was suspected. The entrapment of the nerve at the spinoglenoid notch by a large and hypertrophied ligament was demonstrated by an open revision.

We treated twenty-two consecutive patients for an open fracture and segmental loss of bone in the upper extremity by delayed insertion of a graft of iliac corticocancellous bone. All of the wounds were left open and healed by secondary intention. Sixteen patients (nineteen grafts) had long-term follow-up. Ten injuries were secondary to a gunshot wound. The ten patients (twelve grafts) who had an injury to the hand were followed for an average of 24.1 months. No patient in this group had an infection, and all had primary union after an average of 13.3 weeks. Nine had a satisfactory result. The

Wound seromas and lymphedema are continuing problems after mastectomy for carcinoma of the breast. This study was done to determine whether or not the volume of postoperative wound drainage was related to the frequency of seromas and lymphedema of the arm. The daily wound drainage and the total hospital drainage (THD) were recorded for 49 consecutive patients who had undergone a mastectomy and the results were correlated with the frequency of seromas and lymphedema. All of the patients had modified mastectomies and no irradiation. All had closed suction drainage and none had flap necrosis or i

Four patients with the crush syndrome due to prolonged limb compression were treated at Cook County Hospital, Chicago. Limb injury was caused when the obtunded patient fell asleep lying on the involved extremity. Prolonged limb compression may cause an acute compartment syndrome with ischemic muscle injury. Continued muscle ischemia may lead to myonecrosis resulting in shock or renal failure. A history of prolonged limb compression with a swollen limb should suggest the diagnosis of crush syndrome. Prompt therapy, including rapid correction of volume and metabolic derangements, extensive open

In the time from 1972 through 1985 173 children with supracondylar fractures of the humerus were treated according to the fracture type with two upper arm plaster splints, nail extension from von Ekesparre and in special cases with open reduction and fixation with Kirschner wires-osteosynthese. As primary complications due to accidents we discovered 4% lesions of the arm nerves and in 1.5% disturbance of the blood circulation, of which 0.5% were due to vessel lesions. All nerve lesions could be treated conservatively, however, one child had to be operated because of ischemia of the lower arm a

This is a case report of a 15-year-old boy with an unusual fracture of the glenoid. The mechanism of injury was a fall on the partially extended, abducted arm. The fall produced a fracture with displacement of the inferior half of the glenoid. The technique of open reduction and exposure used differed from the standard approach in that the long head of the triceps was sectioned at its glenoid origin to facilitate the reduction. At one-year follow-up, the patient demonstrated a full range of motion and excellent triceps strength.

The management of ten patients with fracture of the humeral diaphysis and concomitant injury to the brachial artery was analyzed retrospectively for examination of the factors contributing to preservation of limb viability and function. Of ten treated fractures, eight were rigidly stabilized with either a plate or an external fixation device. Seven of the vascular injuries required an interpositional graft. Three arteries were repaired with an end-to-end anastomosis. Two of these repairs failed in patients whose fractures were not rigidly stabilized, and one of these patients required amputati
