An innovative 2D optical navigation workflow for percutaneous pedicle screw fixation in thoracolumbar fractures: comparison with O-arm 3D navigation.

OBJECTIVE: Thoracolumbar fractures (TLFs) are common spinal injuries caused by high-energy trauma, falls, or osteoporosis. Traditional open surgery may involve substantial blood loss, tissue damage, and prolonged recovery. Percutaneous pedicle screw fixation (PPSF) is minimally invasive and can accelerate rehabilitation, but it often depends on repeated fluoroscopy, increasing radiation exposure and potential neurovascular risk. This study compared the clinical outcomes of two-dimensional (2D) optical navigation and three-dimensional (3D) navigation systems for PPSF in TLFs, focusing on operat

Rebuilding the pelvis: advances in robotic-assisted management of complex pelvic fractures.

Complex pelvic fractures are infamously challenging to fix surgically because of their fine anatomy and proximity to vital neurovascular structures. Traditional open reduction and internal fixation (ORIF) improves stability but is complicated by excessive blood loss, longer operative time, and morbidity. Robotic-assisted surgical methods, i.e. Robot-Assisted Fracture Reduction (RAFR) and the TiRobot platform, provide a paradigm shift toward precise, minimally invasive fracture reduction and fixation. The RAFR system blends preoperative high-definition 3D CT imaging with intraoperative cone-bea

Outcomes of intra-articular distal humerus open reduction and internal fixation based on severity of articular comminution.

BACKGROUND: The advent of modern, precontoured locking plates has improved treatment of distal humerus fractures (DHFs) by providing an anatomic fit and a reliable locking mechanism with interdigitating screws. However, severe articular comminution may predispose surgical failure despite anatomic reduction and a biomechanically strong construct. METHODS: A retrospective review was performed to identify patients who underwent distal humerus open reduction internal fixation (ORIF) between 2015 and 2020. Fractures were classified using the AO/OTA classification and further categorized by anatomi

Medial Epicondyle Fractures Treated with Diverse Fixation Techniques: A Case Series.

INTRODUCTION: Medial epicondyle fractures of the humerus, account for ~12-20% of pediatric elbow fractures and are often associated with elbow dislocations. Management is debated, especially for moderately displaced injuries. Conventionally, displaced fractures were sometimes treated non-operatively, yielding acceptable function but high rates of fibrous non-union. Recent evidence shows surgical fixation provides ~9-fold higher union rates without significant differences in long-term pain or nerve function. We present five patients treated with different fixation techniques to highlight manage

Treatment of Perilunate Injuries With Radial-sided Fixation.

BACKGROUND: Perilunate dislocations and fracture-dislocations are high-energy wrist injuries. Standard treatment involves urgent closed reduction followed by open reduction and fixation of both radial (scapholunate) and ulnar (lunotriquetral) intervals. In our experience, fixation of the radial-sided injury alone (scaphoid fracture or scapholunate ligament tear) restores carpal alignment without separate lunotriquetral stabilization. We present the largest series to date using this approach. METHODS: Between 2004 and 2025, 36 patients with acute perilunate injuries were treated by a single su

Bilateral Displaced Midshaft Clavicle Fractures Following High-energy Road Traffic Trauma: A Case Report and Review of the Literature.

Bilateral clavicle fractures represent one of the rarest patterns of shoulder girdle trauma, occurring predominantly after high-energy mechanisms and posing unique challenges in diagnosis and management. Their simultaneous disruption compromises both clavicular struts, resulting in impaired shoulder biomechanics and necessitating timely stabilization for optimal functional recovery. A 25-year-old male sustained high-velocity trauma following a motorcycle accident and presented with severe bilateral shoulder pain, deformity, swelling, and markedly restricted arm elevation. Neurological and dist

Open versus arthroscopic management of acute and chronic distal radioulnar joint instability: a systematic review and meta-analysis.

BACKGROUND: Foveal triangular fibrocartilage complex (TFCC) repair can restore distal radioulnar joint (DRUJ) stability, but whether open or arthroscopic techniques provide superior clinical outcomes remains uncertain. This systematic review and meta-analysis compared functional outcomes, DRUJ stability, and complications between open versus arthroscopic foveal TFCC repair for acute and chronic post-traumatic DRUJ instability. METHODS: PubMed, Scopus, and Cochrane CENTRAL were searched initially in January 2026 and updated on 15 April 2026 to identify comparative studies of open versus arthro

Minimally invasive anatomical fixation of distal humeral fractures: Direct reduction through the anterior MIPO distal window.

Conventional surgical treatment for distal humeral shaft fractures consists of osteosynthesis through an open posterior approach. This technique requires a wide incision, extensive muscular dissection, and direct handling of the radial nerve, resulting in considerable surgical morbidity. Although the anterior MIPO technique represents a less invasive alternative and avoids these drawbacks, it has traditionally been associated with indirect reduction and relative stability alone. In distal humeral shaft fractures, the distal window of the anterior MIPO approach is located directly above the fra

Association Between Upper Limb Injury and Risk of Falls: A Nationwide Population-Based Cohort Study.

Background/Objectives: Falls and upper limb injuries (ULI) are prevalent in older adults, yet whether ULI independently predisposes to subsequent falls remains poorly characterized. This nationwide cohort study evaluated the association between ULI and future fall risk using Taiwan's National Health Insurance Research Database (2011-2019, follow-up through 2020). Methods: Adults aged ≥ 50 years with newly diagnosed ULI-defined as fractures (clavicle, scapula, humerus, radius, ulna, hand), sprains, strains, or open wounds of the shoulder, arm, elbow, forearm, wrist, or hand-were propensity scor

Operative Management of a Late-Presenting McKee Type IV Capitellum Coronal Shear Fracture via an Anterolateral Approach: A Rare Case Report.

INTRODUCTION: Coronal shear fractures of the distal humerus are rare, comprising <1% of elbow fractures. McKee Type IV fractures involve both the capitellum and lateral trochlea. These injuries are uncommon and may result in stiffness and functional impairment if diagnosis is delayed or treated conservatively. CASE REPORT: A 15-year-old boy presented 6 months after an untreated McKee Type IV coronal shear fracture of the distal humerus with pain, stiffness, and restricted elbow motion. Radiographs and computed tomography demonstrated a malunited shear fragment with a "double-arc" sign. Osteot

Association Between the Backhand Technique and the Prevalence of Dominant-Arm Elbow Pain and Low Back Pain in Tennis Players: A Cross-Sectional Study.

Background Tennis is a physically demanding sport characterized by explosive movements, repeated trunk rotations, and asymmetric loading of the dominant upper limb. Overuse injuries are common, with dominant-arm elbow pain (DEP) and low back pain (LBP) representing frequent musculoskeletal complaints among tennis players. Biomechanical differences between the one-handed backhand (1HB) and the two-handed backhand (2HB) may influence the distribution of mechanical loads on the upper limb and spine; however, their relationship with clinically relevant symptoms remains unclear. This cross-sectiona

Open Versus Arthroscopic Reduction for Perilunate and Lunate Dislocations: A Systematic Review and Network Meta-Analysis.

PURPOSE: Perilunate and lunate dislocations represent complex wrist injuries requiring urgent reduction and stabilization. While open reduction has been the gold standard, arthroscopic techniques have emerged as less invasive alternatives. However, comparative evidence remains limited, and the management strategies are controversial. METHODS: We conducted a systematic review and network meta-analysis following Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. Dedicated searches of major databases were performed up to June 16, 2025. Studies comparing open and

[Management of humeral shaft fractures in childhood and adolescence : Consensus report of the Pediatric Traumatology Section of the DGU].

Humeral shaft fractures are rare in childhood, accounting for less than 5% of all pediatric fractures. Indirect trauma predominates in younger children, whereas direct trauma is more common in older patients. To date, no uniform recommendations exist regarding diagnostic evaluation or treatment management. Therefore, during the 13th and 14th scientific meetings of the Pediatric Traumatology Section (SKT), the available evidence was reviewed, the management strategies were critically discussed within an expert panel and a consensus was formulated. The management remains predominantly conservativ
