Meningitis - infection o f the meninges - can be attributed to a variety o f agents, including bacteria, protozoa and some helminths. In helminth infections, but rarely in protozoal infections, eosinophilia is a common sign. Thus eosinophilic meningitis (EOM) is typically associated with certain helminth infections in which nervous system involvement occurs. Among the most important of these are ongiostrongyliosis, gnathostomiasis, porogonimiasis and cysticercosis (see Fig. 1). Here, Nibha Jaroonvesoma discusses the key distinctions between such infections.

Specific IgE antibody levels in the serum of patients with proven gnathostomiasis and in those with intermittent cutaneous migratory swellings were determined by the enzyme-linked immunosorbent assay (ELISA) using aqueous extracts of Gnathostoma spinigerum infective larvae as antigen. There was not only an elevation of specific IgE antibody levels but also a marked increase of total IgE in the serum of these patients. The mean levels of specific IgE antibody in both groups of patients were significantly higher than that of healthy controls (P less than 0.01). Only one of the 50 serum specimens

A mass found in the stomach of a 10-year old cat contained a female worm of the genus Gnathostoma. The mass was described as a muscular pseudogranuloma induced by the worm. Although the worm species could not be ascertained, it was concluded that the cat served as an aberrant host for a Gnathostoma sp that usually infects wild mammals in the area.

Recognition of parasitic diseases by Western physicians is becoming increasingly important because of increased international travel and the recent influx of refugees from South east Asia. We describe two patients who presented with acute anterior uveitis and secondary glaucoma caused by intracameral third stage larvae of Gnathostoma spinigerum. The parasites were successfully removed, with preservation of good visual acuity. The initial infection may occur through consumption of contaminated water and/or raw or undercooked foods, while the eyes may be involved even years later. Accurate diagn

In order to diagnose gnathostomiasis immunologically, Gnathostoma doloresi was evaluated for the antigenicity in comparison with G. hispidum which was recently reported in Japan by using micro-ELISA. The study revealed that G. doloresi can be used as the alternate source of antigen in the test. A significant increase of specific IgG antibodies was seen in 22 (73.3%) out of 30 gnathostomiasis cases. Although double diffusion was slightly less sensitive than ELISA, it was considered more specific than the latter method.

Twenty five patients with nodular migratory eosinophilic panniculitis (gnathostomiasis) were studied, several samples of skin were taken in different periods of evolution; and histologic criteria are established for the first time in literature. The histological picture was different and changed according to the day the biopsy was taken. Three parasites were recovered from the skin material showing all the features of generic gnathostoma.

A sensitive enzyme-linked immunosorbent assay is described for the detection of immunoglobulin G antibody to Gnathostoma antigen in the sera of patients with intermittent cutaneous migratory swelling who were suspected of being infected by the tissue nematode Gnathostoma spinigerum. The antigen used was a crude somatic aqueous extract of the third larval stage obtained from naturally infected eels. The 1:320 dilution of all sera from 46 patients gave enzyme-linked immunosorbent assay values above the mean (+2 standard deviations) of normal healthy controls, which was arbitrarily used as a cuto

A case report of a cutaneous larva migrans caused by the nematode, Gnathostoma spinigerum, in which the worm was visualized at surgery and successfully removed, is presented. To the best of our knowledge, this is the second case of documented subcutaneous gnathostomiasis in the Western Hemisphere and the first in the upper extremity.

Parasitic infestation is a major health problem in tropical countries. Increasing tourism and the influx of refugees from Southeast Asia demand a greater awareness of ocular parasitology. In Thailand, cysticercosis, gnathostomiasis, and angiostrongyliasis are the three commonest ocular parasitic diseases. No antiparasitic drugs are yet available to treat ocular involvement, and therapeutic success depends upon early and complete surgical removal. Because the eye is the only site at which direct visualization and surgical removal are possible, ocular examination is crucial to diagnosis and trea

An ELISA for immunodiagnosis of human gnathostomiasis using a crude water extract of third-stage larvae of G. spinigerum as antigen, and alkaline phosphatase labelled goat antihuman IgG in the indicator system was developed and evaluated. At the titre of 1:400 and above positive results were observed in 100% of 4 parasitological confirmed and 10 eosinophilic meningo-encephalitis (EME) typical of gnathostomiasis cases, 56% of 160 cutaneous migratory swelling cases, 33% of 24 cases with EME typical of A. cantonensis infections, 23% of 92 cases with other parasitic infections and 1.5% of blood do

A young Thai woman was afflicted with aural gnathostomiasis. The only symptom she had was sudden intermittent otalgia without apparent hearing loss, tinnitus, vertigo or otorrhea. Presumptive diagnosis was made from the recent history of ingesting raw fish with subsequent migratory swellings. Definite diagnosis rested on identification of the worms, pertinent eosinophilia and positive skin test. This case was different from those previously reported because it was a primary aural gnathostomiasis without neurological involvement.

With the recent influx of southeast Asians into the United States, an increased incidence of human gnathostomiasis can be expected. A Laotian woman had had two weeks of pruritus associated with fleeting erythematous patches on her abdomen. A peripheral eosinophilia was present. She withdrew a Gnathostoma spinigerum from the skin of her abdomen. Gnathostomiasis is endemic in much of Asia and causes not only migratory cutaneous swellings with erythema but also serious visceral sequelae such as eosinophilic encephalomyelitis. Excision of the parasite is the treatment of choice when possible.

Gnathostomiasis, or larva migrans profundus, is a significant cause of morbidity in many parts of the world, especially the Far East. Over forty cases have recently been reported from South America, and some of those patients are seeking diagnostic evaluation and treatment in the United States. A clinical course of painless migratory recurrent urticarial skin lesions in a patient who has eaten raw or poorly cooked freshwater fish should alert a physician to the diagnosis of gnathostomiasis.

Fifteen patients with human gnathostomiasis are reported from Ecuador. The name "nodular migratory eosinphilic panniculitis" is proposed because of the histopathologic and clinical changes found in two cases in which the parasite was found on biopsy.

The larva migrans syndrome is superficial and deep. Superficial when it acts at the epidermis level and deep when the larva produces a hipodermic reaction. In the latter, are included the hypodermitis of the paragonimiasis and of the gnathostomiasis, subjects that have been discussed thoroughly in this paper.

A 51-year-old man had excruciating pains in the left arm and chest approximately four weeks after ingestion of live loaches. Eosinophilia, eosinophilic pleocytosis in the CSF, and a high serum IgE level were noted. Skin tests and antigen-antibody reactions were positive for Gnathostoma infection. His clinical signs and symptoms ameliorated with symptomatic treatment within six months. Only 34 cases of gnathostomiasis involving the CNS have been reported in the English literature, and ours is the first Japanese case, to the best of our knowledge, of eosinophilic meningoradiculomyelitis caused b
