Prevalence of carpal tunnel syndrome and upper extremity tendinitis among dental hygienists.

PURPOSE: This study was undertaken to determine the prevalence of carpal tunnel syndrome (CTS) and upper extremity (UE) tendinitis among dental hygienists. METHODS: In a cross-sectional study dental hygienists (n = 305) were screened using sensory nerve conduction, a focused physical examination of the UE, and a symptom questionnaire. CTS was diagnosed if the subject had slowing of the median nerve at the wrist and symptoms of numbness, tingling of pain in the median distribution. Localized tendinitis of the UE was diagnosed if the subject had focal symptoms and associated findings on physica

Rotator cuff tendinosis in an animal model: role of extrinsic and overuse factors.

The rat shoulder animal model has been used previously to study the role of intrinsic injury (modeled as an acute insult to the tendon), extrinsic injury (modeled as external subacromial impingement), and overuse factors on rotator cuff tendinosis. These studies demonstrated that it is possible to produce rotator cuff tendinosis with any one of these factors in isolation. The current study uses the rat shoulder model to study the roles of extrinsic compression, overuse, and overuse in combination with extrinsic compression, on the development of rotator cuff tendinosis. The results of this stu

Neer Award 2001: nonrecoverable strain fields of the anteroinferior glenohumeral capsule under subluxation.

Although tears of the glenohumeral capsule have been observed in anteroinferior instability, prefailure, nonrecoverable deformation is suspected but has not been shown to exist after shoulder subluxation. The inferior glenohumeral ligament in the anteroinferior capsule (AIC) is a primary stabilizer in anteroinferior instability. The aim of this study was to examine the nonrecoverable strain field of the AIC due to shoulder subluxation. Nonrecoverable strains were calculated between a nominal strain state and a postsubluxed state. AIC marker coordinates were reconstructed from stereoradiographs

Intratendinous strain fields of the supraspinatus tendon: effect of a surgically created articular-surface rotator cuff tear.

Articular-surface partial-thickness rotator cuff tears play a significant role in shoulder pathology, but the role of the articular-surface tissue is poorly understood. This investigation assessed the effect of an articular-surface partial-thickness rotator cuff tear on intratendinous strain fields. A magnetic resonance imaging-based technique quantified intratendinous strains in healthy cadaveric shoulders at 15 degrees, 30 degrees, 45 degrees, and 60 degrees of glenohumeral abduction. A second set of magnetic resonance images was acquired after an articular-surface partial-thickness tear was

Computer terminal work and the benefit of microbreaks.

Microbreaks are scheduled rest breaks taken to prevent the onset or progression of cumulative trauma disorders in the computerized workstation environment. The authors examined the benefit of microbreaks by investigating myoelectric signal (MES) behavior, perceived discomfort, and worker productivity while individuals performed their usual keying work. Participants were randomly assigned to one of three experimental groups. Each participant provided data from working sessions where they took no breaks, and from working sessions where they took breaks according to their group assignment: microb

Medially based anterior capsular shift of the glenohumeral joint. Passive range of motion and posterior capsular strain.

The effect of a medially based anterior capsular shift on translational and rotational range of motion and posterior capsular strain was investigated in an in vitro model. Six cadaveric shoulders were tested in a robot-assisted shoulder simulator. Translational and rotational range of motion were reduced by the capsular shift, particularly with the shoulder at higher elevation angles. At 60 degrees of elevation, anterior translation was decreased 1.9 +/- 2.9 mm, and posterior translation was decreased 2.3 +/- 6.2 mm. External rotation was decreased 11.5 degrees +/- 10.2 degrees, and internal r

Overuse injuries of the upper extremity in baseball.

The shoulder and elbow are prone to many overuse injuries in baseball. Injury to the muscles or ligaments leads to pain and loss of effectiveness in competition. Although many of these disorders respond to conservative management, surgery is sometimes necessary to provide pain relief and restore function. Unfortunately, overuse injuries that require surgery still are career-threatening injuries. Correct diagnosis and treatment increase the probability of a throwing athlete returning to competition.

Effect of superior capsule and coracohumeral ligament release on strain in the repaired rotator cuff tendon. A cadaveric study.

Twelve cadaveric shoulders were used to determine the effects of release of the superior capsule and the coracohumeral ligament on the strain in the repaired rotator cuff tendon. A rotator cuff tear (2 cm wide and 1.5 cm long) was created and repaired under a 3-kg tensile force. The strain in the repaired tendon was measured with use of linear transducers with the arm in 50 different positions. Release of either the superior capsule or the coracohumeral ligament diminished the tension of the repaired rotator cuff by an average of 25% with the arm in adduction. Release of both of these structur

[Overuse injury syndromes in the shoulder area].

The shoulder is the most movable and the most unstable joint in a human body. The glenohumeral joint is particularly vulnerable because overhead activities put tremendous stress on its static stabilizers (ligamentous labral complex) and the dynamic stabilizers (rotator cuff muscles). Overhead activities may stretch or injure the static stabilizers. Minor aberrations in mechanisms controlling stability have a significant and cumulative effect on the shoulder biomechanics, and increase the risk of injury. Impingement syndrome and glenohumeral instability are predominant injuries affecting the sh

[Overuse injury syndromes of the hand, forearm and elbow].

The basic role of the shoulder, upper arm, elbow, forearm, and wrist is to place the hand in the appropriate position, so that it can fulfill its function. The upper extremity, especially the wrist and the hand, has a very complex anatomy with a large number of bones, muscles, tendons and nerves. They are essential to work and sport activities and are subject to acute and/or chronic mechanical injuries. This is why overuse injuries are the most common in these regions. They are often characterised by tendinitis, tenosynovitis, tunnel syndromes, or stress fractures. This paper gives an overview

Understanding work-related upper extremity disorders: clinical findings in 485 computer users, musicians, and others.

Four hundred eighty five patients whose chief complaints were work related pain and other symptoms received a comprehensive upper-body clinical evaluation to determine the extent of their illness. The group had a mean age of 38.5 years. Sixty-three percent of patients were females. Seventy percent were computer users, 28% were musicians, and 2% were others engaged in repetitive work. The time between the onset of symptoms and our initial visit ranged from 2 weeks to over 17 years. A majority sought care within 30 months with the greatest number of them seeking care before 12 months. Fifty nine

Biceps tendinitis and subluxation.

Since the 17th century, the long head of the biceps tendon as a source of shoulder pain and its functional significance has been a source of debate. Although the term tendinitis is commonly used, overuse tendon injuries infrequently demonstrate inflammatory cells; instead, degenerative changes resulting from the failure of self-repair usually are found. Bicipital tendinitis or bicipital tenosynovitis is most often secondary to impingement beneath the coracoacromical arch. Primary bicipital tendinitis and tendinitis secondary to instability are possible, however. Through a careful history, phys

[The most common overuse injury syndromes of the upper extremity associated with work activity].

Repetitive use of upper extremities, non-physiological body posture, especially of cervical spine, shoulders, lower arms, and fists are still present in modern technological processes. Innate deformities, weakness of connective tissue, accompanying morbidity (radicular symptoms, neuropathies, superior thoracic aperture syndrome, vasopathies and neuroses) are significant pathogenic overuse syndromes. Other causes of overuse syndromes are non-ergonomic tools, musical instruments, furniture, or keyboards. Lack of training for jobs involving repetitive movements, as well as job dissatisfaction are

Shoulder impingement in front-crawl swimming: I. A method to identify impingement.

PURPOSE: The impingement of subacromial structures has been proposed as a major cause of the shoulder problems experienced by athletes who use repetitive overhead actions. The purpose of this study was to develop a noninvasive method to identify instances at which the shoulder was experiencing impingement during front-crawl swimming. METHODS: Shoulder impingement has been reported to occur when an arm is: (a) elevated above shoulder height while being rotated internally; and (b) forcibly elevated at, or beyond, the maximum active elevation angle. In this study shoulder configurations that sat
