Neuroarthropathy: an overuse injury of the shoulder in quadriplegia.

The shoulder joint of the spinal cord injured patient is the primary joint used for transfer and wheelchair propulsion. In addition to ordinary use, the shoulder is subject to overuse injury with resultant functional impairment. Frequent overuse injuries of the shoulder include subdeltoid bursitis, bicipital tendonitis, rotator cuff tear and secondary degenerative arthritis. Neuroarthropathy of the shoulder, an overuse injury of an insensate shoulder girdle, has been rarely described in the literature. The case presented is that of an active 47 year old, right hand dominant, C7 complete quadri

Peripheral nerve entrapment, occupation-related syndromes, sports injuries, bursitis, and soft-tissue problems of the shoulder.

In this review, three areas are highlighted: knee injuries due to athletic events, carpal tunnel syndrome, and shoulder problems, especially glenoid labral tears. In patients with chronic anterior cruciate ligament insufficiency, an increasing incidence of meniscal tears was seen. A Finnish study showed that athletes from all types of competitive sports are at a slightly increased risk of requiring hospital care because of osteoarthritis of the hip, knee, or ankle. A number of studies on the electrodiagnosis of carpal tunnel syndrome were published, and some of these are reviewed. The shoulder

Pathophysiology of overload injuries around the elbow.

The most common elbow injuries appear to have a pathophysiology that is microtrauma based but with multifactorial origin. This pathophysiology is a process of gradual degeneration on a cellular and tissue level, with resulting alterations that predispose to more overt injury as the athlete responds to the inherent demands of the sport. This process is similar to injury patterns described at the foot, knee, and shoulder. Evaluation frameworks can be used to describe the clinical changes that are detectable, and to evaluate all of the various components that may play a role in the causes of thes

A modified internal rotation stretching technique for overhand and throwing athletes.

Strength, stretching, and rehabilitation methods for the shoulder have been previously described and have been universally applied; nevertheless, many throwing athletes continue to develop overuse injuries. The pitching process tends to increase external rotation and limit internal rotation of the shoulder joint. Our technique involves a modified stretching method of the posterior shoulder musculature. The athlete lies prone with the elbow flexed 90 degrees. With the shoulder abducted 90 degrees, in neutral flexion/extension, and 90 degrees or more of internal rotation, the scapula protrudes p

Posture in patients with shoulder overuse injuries and healthy individuals.

Assessment of posture is an integral component of patient evaluation with shoulder overuse injuries. However, the professional literature contains relatively few studies that have assessed the relationship between posture, function, and injury. The purpose of this study was to determine the relationship and differences in postural variables within and between subjects with overuse injuries to the shoulder of healthy subjects. Thirty patient subjects and 30 healthy subjects matched for age and gender were recruited. Scapular protraction and rotation, forward head position, midthoracic curvature

Posterior superior glenoid impingement: expanded spectrum.

Posterior superior glenoid impingement is a recently recognized mechanism of injury producing rotator cuff injury in athletes. Usually the mechanism is repetitive overhand activity such as throwing. A survey of the author's practice was undertaken to show a wider spectrum of this mechanism both in the activity that caused it and the number of structures at risk of injury from this mechanism. The survey revealed 11 patients who had a clear recollection of their mechanism of injury and an objective documentation of the injury by arthroscopy or imaging studies. The majority of shoulders had damag

Atraumatic osteolysis of the distal clavicle: MR findings.

OBJECTIVE: The purpose of this study was to describe the MRI appearance in atraumatic osteolysis of the distal clavicle (AODC). MATERIALS AND METHODS: We retrospectively evaluated MRI, medical records, ancillary diagnostic imaging studies and clinical course in five men and two women (mean age, 39 years) in whom the final clinical diagnosis of AODC was established. None of the patients had significant shoulder injury, but all participated in activities involving repetitive strain of the acromioclavicular (AC) joint. In three of these patients, we performed follow-up MRI (ranging from 5 1/2 to

Acromioclavicular joint injuries.

Acromioclavicular joint injuries, including sprains and dislocations, are common shoulder problems treated by orthopaedic surgeons. The classification of acromioclavicular joint injuries comprises six grades, according to the degree of ligamentous disruption and displacement. Grades I and II are generally treated nonoperatively, whereas types IV, V, and VI are treated surgically. Treatment for grade III injuries remains controversial. Fractures of the distal aspect of the clavicle have been divided into three types. Type I fractures are stable and require brief immobilization. Type II fracture

Review of typical ice hockey injuries. Survey of the North American NHL and Hockey Canada versus European leagues.

Ice hockey is considered to be one of the fastest and roughest of all sports. Prospective injury reports of the North American National Hockey League, the Canadian Amateur Hockey Association and of several European teams (UdSSR, CSSR, Sweden and Switzerland) are reviewed to evaluate the patterns, anatomic locations, circumstances and sequelae of ice hockey-related injuries. Although different injury reporting systems are used in North America and Europe, knee injuries (sprains of the collateral ligaments) accounted for the majority of games missed (40%), followed by injuries to the shoulder (d

Injuries in Junior A ice hockey. A three-year prospective study.

This 3-year prospective cohort observational analysis of elite amateur hockey players ranging in age from 17 to 20 years on a United States Hockey League team describes ice hockey injuries using a strict definition of injury, standardized reporting strategies, and diagnosis by a team physician. One hundred forty-two injuries were recorded for an on-ice injury rate of 9.4 per 1000 player hours. A player was 25 times more likely to be injured in a game (96.1 per 1000 player-game hours) than in practice (3.9 per 1000 player-practice hours). Game-related injuries were more frequent in the third pe

Trampoline-related injuries.

Two hundred and seventeen patients who had sustained an injury during the recreational use of a trampoline were managed in the emergency room of Logan Regional Hospital in Logan, Utah, from January 1991 through December 1992. We retrospectively reviewed the charts and radiographs of these patients to categorize the injuries. Additional details regarding the injuries of seventy-two patients (33 per cent) were obtained by means of a telephone interview with use of a questionnaire. The injuries occurred from February through November, with the peak incidence in July. The patients were eighteen mo

Clinical implications of secondary impingement of the shoulder in freestyle swimmers.

Swimming has become a popular recreational activity as well as a highly competitive sport in the United States. The repetitive nature of swimming can predispose the shoulder to mechanical impingement and microtrauma, which may lead to laxity, rotator cuff fatigue, and subsequent secondary impingement. Improper stroke mechanics can place the swimmer's shoulder at further risk. The purpose of this paper is to describe the pathology of secondary impingement in freestyle swimmers and to discuss the clinical implications for rehabilitation of swimmers with the pathology. A thorough subjective and o

[Stress-related clinical and ultrasound changes in shoulder joints of handball players].

This prospective clinical and ultrasound study was performed to show specific overuse injuries or changes in the periarticular structures of shoulder joints. Therefore, 11 male handball players with an average age of 23 years and 6 months were evaluated via a standardised shoulder exam sheet and an ultrasound examination before, one hour after, 10-12 hours after, 16-20 hours after and 22-24 hours after specific handball training. Characteristic changes were found in respect of bursitis subacromialis and tendinitis of biceps tendon up to 16-20 hours after training, which were fully reversible 2

Diagnosis and treatment of carpal tunnel syndrome.

Secondary amyloidosis is a complication typical for patients on long-term hemodialysis. The first clinical signs are usually shoulder joint pain and carpal tunnel syndrome (CTS). We have questioned 74 patients who were on regular hemodialysis (HD) treatment and divided them into 3 groups according to the length of HD. Group 1--on HD for 1-4 years: There were 35 patients in this group, 15 of them (i.e., 43%) had shoulder joint pain and/or CTS. None of these patients had symptoms that would require surgical treatment. Group II--on HD for 5-9 years: There were 22 patients in this group, 15 of the

Current concepts in the rehabilitation of the athletic shoulder.

The rehabilitative process of the overhead athlete represents a significant challenge to the clinician. Overhead athletes (thrower, tennis player, swimmer) repetitively subject their shoulder joints to high microtraumatic stresses that, due to the accumulative effects, may lead to a variety of shoulder injuries. This type of athletic patient exhibits uniquely specific physical characteristics, such as hypermobility of the anterior shoulder capsule, excessive external rotation, hypomobility of the posterior capsule, limited internal rotation, and generalized ligamentous laxity of the glenohumer
