Clinical picture of the amyloid arthropathy in patients with chronic renal failure maintained on haemodialysis using cellulose membranes.

The clinical picture of 15 patients (10 male, five female) with amyloid arthropathy secondary to chronic renal failure treated with haemodialysis has been studied. The average period of haemodialysis was 10.8 years. Joint symptoms appeared between three and 13 years after starting haemodialysis. No patient had renal amyloidosis. Early symptoms were varied and often overlapped: knee swelling (seven patients), painful and stiff shoulders (seven), and carpal tunnel syndrome (six) were the most prominent. Follow up showed extension to other joints. Joint effusions were generally of the non-inflamm

Sports medicine for the internist.

Because of increasing participation in leisure sports and exercise rehabilitation programs, many patients are developing nonsurgical, soft tissue overuse syndromes. Many of these musculoskeletal problems arise because of underlying biomechanical difficulties, training errors, or improper use of equipment. Epidemiologic observations reveal that these individuals frequently visit a general medical practitioner before seeking the advice of an orthopedic specialist. Prompt diagnosis simply requires an appreciation of anatomy and an analytic understanding of sports biomechanics. Although compulsive

Epiphyseal separation of the coracoid process associated with acromioclavicular sprain. A case report and review of the literature.

A rare injury of traumatic separation of the epiphysis of the coracoid process associated with Grade I acromioclavicular sprain occurred in a nine-year-old girl. The accident occurred when she was asleep. The mechanism of injury was unknown. Tenderness and mild swelling around the coracoid process were seen. The anteroposterior radiograph of the shoulder with the arm abducted 150 degrees (abduction view) clearly demonstrated the epiphyseal separation with subtle displacement at the base of the coracoid process. She was treated by simple immobilization and recovered fully with normal shoulder a

Orthopedic problems in athletes.

Physicians and patients must recognize the demands of the sport involved, as well as the individual's ability to meet those demands. Careful analysis of the sport and examination of the patient to assess recovery and chances of reinjury are mandatory, if he is to enjoy sports after injury. Physicians should become familiar with padding, shoes, warm-up programs, preparticipation examinations, and protective equipment and devices, all of which are helpful in preventing sports injuries. We have touched only briefly on the epidemic problem of quality of life trauma that is part of our society. Bec

Shoulder strength following acromioclavicular injury.

The acromioclavicular (AC) joint enjoys the dubious distinction of being one of the few joints in the body whose total dislocation is routinely treated by simply leaving the joint dislocated. Adherents of both conservative and operative treatment have presented reasons for their viewpoints. Residual shoulder weakness has been offered as a sequela of untreated acromioclavicular injury and a reason for repairing the joint. An objective evaluation of shoulder strength would be valuable in determining the optimum treatment for this injury. The purpose of our study was to quantitate, using the Cybe

Cervicobrachial disorders in certain occupations, with special reference to compression in the thoracic outlet.

One hundred ninety-one workers in three different occupations were examined regarding presence of symptoms from the cervicobrachial region. Eighteen percent of the workers (27% of the female and 11% of the male workers) had symptoms of compression in the thoracic outlet. However, only 2% of all workers had pronounced symptoms of compression in the thoracic outlet. In addition, 27% of the workers (33% of the female and 23% of the male workers) had symptoms of other cervicobrachial disorders. Thus, 45% of the workers had symptoms from the cervicobrachial region. The workers with symptoms of comp

Open radiocarpal fracture-dislocations.

Ten open radiocarpal fracture-dislocations in nine patients were treated by wound debridement, open reduction, and maintenance of reduction by casting supplemented with internal or external fixation. Eight patients had associated fractures or dislocations of the ipsilateral extremity: elbow fracture or dislocation (three patients), shoulder fracture (one patient), fractured humerus (two patients), and phalangeal fractures (three patients). In seven of ten cases the median and/or ulnar nerve was severely contused and required immediate decompression. Six patients were followed at least 15 month

Overuse syndromes in baseball, tennis, gymnastics, and swimming.

The shoulder is the "center of action" for most sports including those discussed in this article. Overuse problems of the shoulder are the most common group of injuries affecting the athlete involved in baseball, tennis, swimming, and gymnastics. Most of the injuries can be directly related to the mechanics of throwing, swimming, or gymnastics. After diagnosis, treatment is directed at "selective rest" of the injured shoulder and modalities and medications intended to decrease the inflammatory response of the body to stress.

Surfboard-riding injuries.

The results of a study aimed at determining the nature, rate, and cause of traumatic surfing injuries by gathering injury data directly from surfers rather than by retrospective analysis of hospital or first-aid station records are reported. Three hundred and forty-six surfers of varying ages, experience, and competence reported 337 injuries sustained over a two-year period. The most common injuries requiring medical attention or resulting in inability to surf were lacerations (41%) and soft-tissue injuries (35%). The high incidence of back and shoulder sprains and strains has not previously b

Impingement syndrome in the athletic shoulder.

The repetitive overhead use of the arm required by many sports may lead to impingement in the vulnerable avascular region of the supraspinatus and biceps tendons. The impingement syndrome may spill over at any time to involve the adjacent biceps tendon, subacromial bursa, and acromioclavicular joint and, as a continuum, with the passage of time, may eventuate in degeneration and partial, even complete thickness, rotator cuff tears later in life. Physical findings, particularly a positive impingement sign, confirm the diagnosis. In the young competing athlete, therapeutic measures consist of ca

Subluxation of the shoulder in athletes.

Shoulder subluxation is an extremely common problem confronting the sports medicine physician and may present in a variety of ways, with the patient complaining of pain of a sense of instability. In throwing spots, tendinitis secondary to impingement is common, but it may also be secondary to instability as a result of traction or compression of the rotator cuff. In addition, subluxation may develop initially, then progress to dislocation, or the converse may occur. However, some patients may have subluxation over an extended period of time. The physician must consider the entity and take a ca

[Neurologic syndromes affecting the shoulder (author's transl)].

A number of locally defined as well as generalised diseases of the peripheral nervous system is manifested in the shoulderarm region besides lesions of individual nerves through traumas or as a result of pressure. Radicular syndromes are particularly frequent. These are based on prolapsed disks or degenerative changes of the cervical vertebral column. We must differentiate between these syndromes and the often rather unknown neuralgic amytrophy of the shoulder. Our experience has shown that in too many cases, paralysis of the plexus cervico-brachialis is assumed, although this occurs almost ex

Rheumatic diseases. 1. Differential diagnosis.

In a case of rheumatic disease, the patient's history and a careful physical examination should yield most of the information needed to identify the specific disorder present. A convenient classification is based on four differentiating features: number of joints affected, acuteness or chronicity of disease, absence of joint involvement, and anatomic distribution.

Carpal tunnel syndrome, humeral epicondylitis, and the cervical spine: a study of clinical and dimensional relations.

Forty-three patients with idiopathic carpal tunnel syndrome, confirmed by nerve conduction studies and treated by surgery, were compared clinically and radiologically with 43 age- and sex-matched control patients. Patients with carpal tunnel syndrome had a significantly greater prevalence of lateral humeral epicondylitis (tennis elbow) (33%) than controls (7%). Randomised reading of the cervical spine radiographs in ignorance of the groups to which they belonged showed no significant difference in the prevalence of either intervertebral disc degeneration or intraforaminal osteophyte protruion
