Controlled external compression was applied to the medium nerve of 16 volunteer subjects. Tissue fluid pressure in the carpal canal was monitored with a wick catheter and pressures of 30, 60 and 90 mm Hg were induced for periods varying from 30 to 90 minutes.l Sensory and motor conduction and two-point discrimination were continuously monitored. Tissue compression at 30 mm Hg caused mild neurophysiological changes and symptoms of hand paresthesias. Compression at both 60 and 90 mm Hg induced a rapid, complete sensory conduction block which consistently preceded a motor block by 10 to 30 minute

The status of 958 patients who underwent median sternotomy between January, 1978, and May, 1981, was analyzed. Fifty-four patients had an upper extremity neuropathy. Among 38 patients who underwent further evaluation, motor and sensory nerve conduction studies localized the injury to the level of the elbow in 13, to the brachial plexus in 10, and to both locations in 6. Ninety-two percent of these 38 patients were asymptomatic 3 months after operation.

Twenty-one consecutive hemiplegic patients underwent electrophysiological evaluation of their shoulder subluxation. The patients comprised 13 men amd 8 women, with a mean age of 57 years. The distal latency times of the suprascapular, axillary, musculocutaneous and radial nerves were 4.8, 6.2, 5.3 and 5.5 msec, respectively, and all nerves tested were statistically delayed. Ten out of 21 suprascapular nerves did not respond adequately. Based on needle EMG studies, about 75% of the supraspinatus and deltoid muscles revealed increased insertional activity and/or positive sharp waves. It is postu

Seven cases with tumours localised in the brachial plexus are described. The main clinical features were a progressive lameness in one forelimb with marked muscle atrophy and very obvious but non-localizable pain. A palpable lump in the axilla was present in less than half the cases. Ancillary aids contributed little in the diagnosis except for electrophysiology which gave evidence of neural damage at an early stage of the disease and as such may be the most useful aid to an early diagnosis. Two different pathological entities were observed, the first where the tumour, primarily of neural orig

In six cases of penetrating trauma to the subclavian or axillary arteries without primary coincident injury to the brachial plexus, the vascular injury was not initially recognized as there were no suggestive clinical signs. The first clinical sign of vascular injury in all cases was delayed onset of brachial plexus palsy due to compression by an expanding false aneurysm. Following vascular repair, neurological recovery occurred only in two of six cases. Since brachial plexus injuries are associated with a poor prognosis, and the functional impairment of the upper extremity is serious, an aggr

A female drug abuser inadvertently injected an intravenous injection of an oral anorexiant, phentaramine (Preludin), into her own brachial artery. Severe hand pain, vasospasm, and ischemia occurred. Treatment with regional axillary block, intravenous dextran, intra-arterial priscoline, and salicylate anticoagulation may have influenced a favorable outcome. Intimal damage, vasospasm, and various additive effects after intra-arterial injection contribute to the potential for serious sequelae. Prompt diagnosis, vasodilatation, elevation and compression, pain relief and, in some cases, surgery app

The authors report a series of 100 brachial plexus injuries between April 1974 and January 1979. The frequency of these lesions, knowledge of the anatomy and pathology, and trends in prognosis are correlated to give the surgical indications for nerve graft repair. Correlation between preoperative myelography and the anatomy and type of pathology demonstrated at the time of operation, have been of special interest and have consolidated tha operative indications. Out of the 100 cases operated on between April 1974 and January 1979, neurolysis without plexus repair was performed in 25, nerve graf

In 52 patients with traumatic injuries of the brachial plexus plastic procedures were carried out on the nerves in order to improve function: primary suture of the torn portion of the plexus (one case), external and internal neurolysis with division of the epineurium (four times), anastomosis of the secondary branches or the main arm nerves, with the intercostal nerves by insertion of sural transplants (47 times). Seventy per cent showed an improvement in protective sensation and in 48% a useful gain in motor function was achieved. The results are dependent on the age of the patient (better re

Forty-two patients with unilateral brachial plexus traction lesions were investigated by recording sensory nerve action potentials (SNAPs) from the lower arm and somatosensory evoked potentials (SEPs) from the clavicle, the cervical spine and the scalp overlying the contralateral somatosensory cortex, in response to electrical stimulation of peripheral nerves. The median and radial nerves were assumed to derive principally from the C6 and C7 roots, and the ulnar nerve from the C8 and T1 roots. Combination of SEP and SNAP findings suggested a location for the lesion (preganglionic, postganglion

The pneumatic tourniquet is used routinely in extremity surgery, not only to prevent unnecessary blood loss, but also to increase the safety of the operative procedure. The mechanical part of this study shows that the system can fail in either of two major ways, each causing serious problems. The tourniquet may be inadequate if the pressure delivered to the cuff is not accurately displayed on the gauge or if the pre-set pressure is not maintained for the duration of the operative procedure. In our hospital, three of seven tourniquets (43%) in daily use showed significant errors in this regard.

The relative importance of ischaemic and direct mechanical injury to nerves compressed by a tourniquet, in the pathogenesis of tourniquet paralysis in man has not been established. To investigate this question, conduction in ulnar or median nerve fibres has been measured in healthy subjects both at the level of the pneumatic tourniquet and distal to the tourniquet. Measurement was prior to, for the period of tourniquet inflation, and following release of the tourniquet. The earliest conduction delays and block were observed at the level of the tourniquet, particularly across the proximal tourn

Here we report 42 cases of postirradiation lesions of the brachial nerve plexus in patients treated with radiotherapy after radical mastectomy. These lesions usually appeared at least 1 year after treatment, and motorial disturbances always developed, even if initially they may have been absent. A comparison of the case material indicates that a reduction in the dose administered to the supraclavicular region and exclusion of the axillary region from the treatment resulted in a significant reduction in the incidence of these lesions. In fact, in a group of 490 patients treated by the old radio

Fractures of the scapula are rare injuries, and fractures of the acromion comprise only a small percentage of this group. An unusual fracture through the base of the acromion with an acromioclavicular separation is described in this paper. The segmental acromial injury had an associated isolated axillary nerve deficit to deltoid muscle. The above-described 51-year-old patient is presented to make the clinician aware of potential neurologic complications associated with acromial fractures.

Electromyogram studies were conducted in 55 patients with the thoracic outlet syndrome, together with the motor nerve conduction velocity (M.N.C.V) in the median cubital nerves, between the Erb's point and axilla and in the upper limb. Distal S.N.C.V. was measured in 19 cases. Proximal nerve compression was confirmed in 29% of cases, with associated carpal canal compression in 22%. The proximal N.C.V. of the median and cubital in the neurological forms, and of the median in the neurovascular forms, were significantly different from normal values in 18 control subjects, but were not so in the v

The charts of twenty-nine patients who had undergone thirty musculocutaneous neurectomies for acquired spasticity of the elbow in a non-functional upper extremity were reviewed. The most common causes of the spasticity were cerebrovascular accident (59 per cent) and head injury (24 per cent). The aims of the operation were to increase the patient's capacity for self-care and to improve ambulation, personal hygiene, and appearance. Patients who had 30-degree flexion contractures preoperatively did not require a cast postoperatively; those who had 30 to 75-degree flexion contractures preoperativ

The vast majority of severe traumatic lesions of the brachial plexus are due to road accidents. Surgical intervention is indicated in cases of persistent motor and sensory loss. The operation should be performed one to three months after the traumatic event. Indications and results of neurolysis, autologous nerve grafts, and neurotization are reported. Neurolysis was employed 43 times on 36 patients primarily to relieve pain rather than to restore function. Autologous nerve grafting was employed in 100 patients and can give good results when the lesion involves the three upper roots. Neurotiza

Thirty-three peripheral arterial injuries were observed in 29 patients following blunt trauma. Motor vehicle accidents were the most common initiating event. The mechanism of injury was fracture or dislocation in 21 patients, acute traction on the extremity in five patients, and contusion in three patients. Pulses were absent distal to the site of arterial injury in all patients. Complete arterial disruption was documented in 19 arteries; intimal or intimal and medial tears were observed in 10 arteries. Four arteries not surgically explored. If possible, debridement and primary anastomosis was

The shoulder is the most mobile joint in the body. Because it serves as a way station for the nerves supplying the upper limb, it creates a potential for nerve lesions that may be caused or significantly influenced by the complex dynamics of the shoulder girdle. This article presents the most commonly encountered lesions as well as an algorithm for their diagnosis and treatment.

Five years ago a follow-up study of first rib resections disclosed a recurrence rate of over 15%. Many patients were reexplored supraclavicularly, and in every case the anterior scalene muslce was found to be reattached to the bed of the first rib. Scalenectomy invariably was successful, which led to this study of scalenctomy as the first operation for all cases of persistent thoracic outlet syndrome (TOS). The study revealed that most patients with TOS gave a history of neck trauma and had symptoms not only of paraesthesias of the hands and weakness of the arms, but also of neck pains and hea
