Collagen-Matrix Cuff Reconstruction for Circumferential Lumbar Nerve Root Sleeve Defects: Technical Note and Single-Center Case Series With Case Illustration.

BACKGROUND AND OBJECTIVES: Circumferential defects of the dural sleeve of exiting lumbar nerve roots may occur after incidental durotomy, trauma, or deliberate opening of the root cuff during resection of nerve sheath tumors. Standard onlay duraplasty can control cerebrospinal fluid (CSF) leakage but does not restore the tubular dural-epineural conduit. We describe a collagen-matrix cuff technique for root sleeve reconstruction and report our initial institutional experience. METHODS: This single-center, retrospective case series includes 9 consecutive reconstructions performed between Januar

Evaluation of the effects of extremity elongation on brachial plexus nerves via intraoperative neuromonitoring in patients undergoing reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is one of the most common treatment method for irreparable rotator cuff tear. One of the significant complications of this surgery is nerve injury. This study aimed to identify the patterns of brachial plexus nerve involvement and to determine the surgical stages associated with increased nerve vulnerability during reverse total shoulder arthroplasty using intraoperative neuromonitoring (IONM). Additionally, the relationship between extremity lengthening, acromiohumeral distance (AHD) changes and intraoperative nerve events were evaluated.

The safety of percutaneous screw placement in reverse shoulder arthroplasty for fracture: A cadaveric study.

BACKGROUND: Cementless reverse shoulder arthroplasty (RSA) with percutaneous locking screw fixation is an increasingly utilized option for treatment of proximal humerus fractures. The safety of percutaneous screw placement in the humerus has not been well established. This study aims to evaluate the proximity of the axillary and radial nerves to percutaneous screw trajectories in RSA. METHODS: A humeral fracture stem with percutaneous locking screws was implanted into six male cadaveric specimens. Screw holes were over-drilled, and distances from trocars and drill bits to the axillary and rad

Ultrasound-guided axillary approach to the brachial plexus for ulnar osteosynthesis in a lesser anteater (Tamandua tetradactyla): a case report.

An adult lesser anteater (Tamandua tetradactyla), weighing 8.4 kg, was presented for surgical repair of a distal ulnar fracture following a road accident. The aim of this case report was to describe the use of an ultrasound-guided regional anesthetic block in this species, specifically an axillary approach to the brachial plexus, as part of a multimodal analgesic protocol. Premedication included dexmedetomidine (5 µg/kg), ketamine hydrochloride (2 mg/kg), midazolam (0.2 mg/kg), and morphine sulfate (0.2 mg/kg), administered intramuscularly in a single syringe. Anesthetic induction was achieved

Variations in Commencement of Median Nerve.

BACKGROUND: The median nerve, normally formed by two roots from the medial and lateral cords in relation to the axillary artery, but variations in its roots and unusual communications with other nerves are common. These anatomical differences can compromise anesthesia and are vulnerable to iatrogenic injury during surgical procedures of the upper limb. This study was conducted to determine the prevalence of such anatomical variations in axillary and arm regions. METHODS: A descriptive cross sectional study was conducted on 66 upper limbs from embalmed adult human cadavers, selected using a co

Two-point deep serratus anterior plane block for modified radical mastectomy - A case series assessing axillary coverage using structured sensory mapping.

Serratus anterior plane block (SAPB) provides chest wall analgesia in modified radical mastectomy (MRM) surgeries by targeting T2-T6 intercostal nerves, but conventional single-injection techniques show inconsistent T2 and axillary coverage, failing to reliably block the intercostobrachial nerve - a key contributor to postoperative axillary pain and postmastectomy pain syndrome. MRM patients experience significant axillary pain from dissection-related intercostobrachial nerve (ICBN) injury, necessitating multimodal analgesia that ensures reliable T2-axillary desensitisation. There is inadequat

Transcutaneous electrical acupoint stimulation for intercostobrachial nerve syndrome after breast cancer surgery: a randomized controlled trial protocol.

INTRODUCTION: Intercostobrachial nerve (ICBN) syndrome is a common peripheral sensory nerve injury following axillary surgery for breast cancer. It is characterized by persistent numbness and burning pain extending from the affected axilla to the medial aspect of the upper arm, as well as restricted shoulder joint mobility, which severely impacts postoperative recovery and quality of life. Current interventions primarily focus on symptomatic analgesia and functional exercises, lacking standardized external treatment protocols to promote nerve repair. Transcutaneous electrical acupoint stimulat

Intraoperative FCU CMAP Amplitude During Oberlin Nerve Transfer: Association with Reinnervation Timing and Functional Outcomes.

Background/Objectives: Selective transfer of an ulnar nerve fascicle to the motor branch of the musculocutaneous nerve (Oberlin technique) is widely used to restore elbow flexion following upper brachial plexus injury. Intraoperative neurophysiological mapping allows quantitative recording of compound muscle action potentials (CMAPs) during donor fascicle selection; however, its prognostic relevance remains unclear. This study evaluated whether intraoperative flexor carpi ulnaris (FCU) CMAP amplitude is associated with time to electromyographic reinnervation of the biceps brachii and with fina

Posterior Approach for Harvesting and Transferring an Ulnar Nerve Fascicle to Posterior Arm Muscles in C5-C7 Upper Brachial Plexus Injury: Surgical Technique and Case Series.

BACKGROUND AND OBJECTIVES: Nerve transfers are crucial to upper brachial plexus injury reconstruction, and we describe a posterior approach for harvesting an ulnar nerve fascicle to reanimate posterior targets in C5-C7 injuries. METHODS: Twelve patients with traumatic upper brachial plexus injuries underwent staged nerve reconstruction between 2018 and 2025, including transfer of a selected ulnar nerve fascicle harvested through a posterior arm approach. The fascicle was transferred to the anterior branch of the axillary nerve or, in selected cases, to the motor branch of the long head of the

Intraoperative Nerve Action Potential Amplitude and Functional Recovery After Selective Ulnar-to-Musculocutaneous Nerve Transfer (Oberlin Technique).

Background: Predicting functional recovery after selective nerve transfer remains challenging. Intraoperative nerve action potential (NAP) recording is widely used to confirm axonal continuity in peripheral nerve surgery; however, its quantitative prognostic value in selective nerve transfer has not been clearly established. This study evaluated whether intraoperative donor fascicle NAP amplitude predicts functional recovery following selective ulnar-to-musculocutaneous nerve transfer (Oberlin procedure) for restoration of elbow flexion. Methods: This retrospective exploratory observational st

Ultrasonographic evaluation of safer deltoid intramuscular injection sites to reduce the risk of SIRVA.

Intramuscular injection into the deltoid muscle has increased in Japan following coronavirus disease 2019 (COVID-19) vaccination programs and the introduction of vaccines such as Shingrix™. Limited experience with this technique may increase the risk of shoulder injury related to vaccine administration (SIRVA). Landmark-based techniques have not been sufficiently validated using ultrasonography. Thirty-four healthy volunteers were enrolled. Using ultrasound, we measured the distances and depths from the conventional injection site-defined as the intersection of a vertical line from the acromio

Anatomic feasibility of intercostal-to-femoral and ulnar-to-sciatic nerve transfers for lower extremity neurotization after traumatic spinal cord injury.

BACKGROUND: Traumatic spinal cord injury (SCI) is associated with functional morbidity and limited options for restoring lower extremity motor function. We explored the technical feasibility of intercostal-to-femoral and ulnar-to-sciatic nerve transfers as a potential approach to reinnervate the lower limb following SCI. METHODS: A cadaveric anatomical feasibility study was performed using a fresh-frozen adult human specimen to evaluate two novel nerve transfer strategies: posterior intercostal-to-femoral and long-segment ulnar-to-sciatic nerve transfers. Dissections assessed the ability to a

Early and Emergency Department Management of Infraclavicular Brachial Plexus Injuries Secondary to Shoulder Trauma.

Infraclavicular brachial plexus injury is an uncommon but potentially disabling complication of shoulder trauma, particularly glenohumeral dislocation and proximal humerus fracture or fracture-dislocation. Reported frequencies of neurological deficit vary widely because of heterogeneous definitions and ascertainment methods. Systematic reviews consistently identify the axillary nerve as the most commonly affected nerve following dislocation; however, multi-nerve deficits indicating cord-level involvement are clinically significant because recovery may be delayed or incomplete. Emergency care p

Elbow flexion recovery after intercostal nerve transfer in elderly patients: a clinical experience report.

INTRODUCTION: Restoration of elbow flexion is a primary goal in the surgical management of complete traumatic brachial plexus injuries (BPIs). When proximal donor nerves are unavailable, intercostal nerve (ICN) transfer to the musculocutaneous nerve (MCN) represents a well-established reconstructive option. However, elderly patients are markedly underrepresented in published series, and outcomes in this population remain poorly defined. The objective of this study was to evaluate clinical outcomes of ICN-to-MCN transfer in elderly patients and to identify perioperative factors associated with

Concomitant Variations of the Scapular Region Musculature With an Aberrant Neurovascular Pattern.

The shoulder complex exhibits a distinctive amalgamation of muscular power, integration, and an extensive range of motion. The shoulder joint allows for a wide range of upper limb movements because the four small muscles that make up the musculotendinous rotator cuff work together to keep the joint stable. The four muscles that come from the scapula and form the rotator cuff are the subscapularis, supraspinatus, infraspinatus, and teres minor. Accurate diagnosis and proper management of shoulder-related injuries are essential, particularly for sportspersons and individuals engaged in heavy wei
