Quality of Life in Patients following distal nerve transfers for the Restoration of Shoulder abduction and external rotation - Single centre experience.

INTRODUCTION: Restoration of shoulder function is a key objective in brachial plexus surgery, as impaired abduction and external rotation substantially limit upper limb use and quality of life. Distal nerve transfers offer shorter regeneration distances and more predictable outcomes than nerve grafting. RESEARCH QUESTION: This study evaluated the effectiveness of distal nerve transfers in restoring shoulder function, reducing pain, and improving quality of life in patients with peripheral nerve injuries. MATERIAL AND METHODS: A prospective cohort of 36 patients with traumatic brachial plexus

Variations in the Origin and Communication of the Intercostobrachial Nerve: Surgical Implications.

The intercostobrachial nerve (ICBN) supplies the upper part of the postero-medial region of the arm, lateral thoracic wall, and axilla. It is susceptible to being damaged during various axillary surgeries like axillary lymph node dissection, sentinel lymph node biopsy, and mastectomy because of its diverse origin and branching configuration. Safeguarding the ICBN during axillary surgical procedures can minimise postoperative discomfort and paraesthesia. During standard dissection of the right side axilla and pectoral region of a female cadaver, the ICBN was detected, and its path was followed.

Congenital Anatomical Variations of the Posterior Cord of the Brachial Plexus in Human Fetuses: Branching Patterns and Surgical Implications.

BACKGROUND: The posterior cord of the brachial plexus, formed by the posterior divisions of the upper, middle, and lower trunks (C5-T1), gives rise to the upper subscapular, thoracodorsal, lower subscapular, and axillary nerves and continues as the radial nerve. Variations in its formation and branching are clinically important for regional anesthesia (infraclavicular and axillary blocks), axillary surgery, and nerve-transfer procedures in brachial plexus palsy. Fetal dissection can reveal congenital patterns before postnatal or post-mortem distortion. OBJECTIVE: The single combined objective

Deltoid reanimation after axillary nerve injury: Comparing grafts and nerve transfers outcomes.

BACKGROUND: Loss of deltoid function following axillary nerve (AN) injury or upper brachial plexus injury (BPI) severely limits shoulder function. Both interposition nerve grafting and distal nerve transfer are established reconstructive strategies; however, comparative outcome data remain limited. METHODS: A retrospective single-center study included patients treated between 2006 and 2022 for isolated AN injury or C5-C6 BPI with deltoid paralysis. Exclusion criteria were graft repair performed >8 months after trauma, nerve gaps >7.5 cm, and nerve transfers to the posterior AN branch. Overall

More Frequent Electrical Stimulation Impairs Nerve Regeneration After Median Nerve Repair in a Rat Model.

BACKGROUND: Peripheral nerve regeneration via electrical stimulation on nerve repair has been shown to potentiate axonal growth and functional recovery. However, it remains unknown whether increasing the frequency of electrical stimulation improves, diminishes, or has no effect on the quality of nerve regeneration after repair. Additionally, few animal studies have investigated the potential beneficial effects of serial stimulations on accelerating and improving end-organ functionality over time. We therefore sought to evaluate the additive effect of weekly versus biweekly serial electrical st

Does Age Predict Outcomes of Triceps to Axillary Nerve Transfer?

PURPOSE: Factors influencing the results of radial nerve to triceps to axillary nerve transfers remain poorly defined. Therefore, the goal of this study is to compare radial to axillary nerve transfer outcomes by age, and to determine if age is a reliable predictor of outcome. METHODS: A single center retrospective cohort study was conducted between 2015 and 2025. Exclusions included patients younger than 18 years, less than 1 year follow-up, or lack of MRC strength documented after surgery. Patients were subanalyzed by age: 18-39 years, 40-59 years, and ≥60 years. The primary outcome was rec

Triceps Motor Branch Transfers to the Infraspinatus and Teres Minor Motor Branches for Restoration of Shoulder External Rotation: A Cadaver Study.

BACKGROUND: Recovery of shoulder external rotation after upper brachial plexus injury remains limited. This study investigated the anatomical feasibility of selective neurotizations using radial nerve branches to the triceps brachii as donors for the motor branches to the infraspinatus (bIS) and teres minor (bTm) muscles, aiming to enhance external rotation outcomes. METHODS: Twenty fresh adult cadavers underwent posterior dissections to expose the suprascapular, axillary, and radial nerves. Radial nerve branches to the triceps were identified, measured, and mobilized to simulate transfers to

Anterior Deltoid to Biceps Transfer for Restoration of Elbow Flexion.

Major injuries to the infraclavicular or axillary region requiring axillary artery repair or reconstruction, a replantation carried out at the proximal third of the arm, or an infraclavicular brachial plexus palsy with failed nerve reconstruction pose a challenge to a reconstructive surgeon with only the shoulder remaining functional. We observed that patients with these injuries exhibited a 'trick' movement causing elbow flexion by abducting the shoulder beyond 90 degrees, which led us to use the anterior fibers of the deltoid extended with fascia lata graft as a donor to restore elbow flexio

The human disharmony loop: high rates of return to work in intractable workers' compensation patients undergoing surgical release of the pectoralis minor with infraclavicular neurolysis.

BACKGROUND/OBJECTIVES: Patients suffering from Worker's Compensation (WC) injuries typically demonstrate poorer outcomes despite optimal surgical management. In the occupational setting, shoulder pathology is the most implicated musculoskeletal disease. We recently described the Human Disharmony Loop (HDL), a clinical model of chronic, historically intractable pain of the neck, upper back, shoulder, and arm (Sharma and Friedman in Clin Med 14(5):1769, 2025. https://doi.org/10.3390/jcm14051769 ; Sharma et al. in J Clin Med 14(16):5650, 2025. https://doi.org/10.3390/jcm14165650 ; Friedman et al.

Reinnervating the Median and Musculocutaneous nerve in Total Brachial Plexus Injury: Free Ulnar Nerve (FUN) flap for C5 grafting.

Median nerve (MN) reinnervation in total brachial plexus injury (TBPI) is often neglected due to inconsistent outcomes with sural grafting or use of extra-plexus donors for nerve transfer. 1 Here we describe the novel use of the vascularized nerve flap concept to improve grafting from the C5 root to the MN, using the Free Ulnar Nerve (FUN) flap to improve reliability of perfusion along the entire nerve length. Targeting the MN allows movement of the finger flexors, wrist flexors, and forearm pronators and provides sensation in the hand. Magnetic Resonance Neurography of C5 ventral and dorsal r

Brachialis Branch to Radial Nerve Transfer for Reanimation of Wrist and Finger Extension: An Anatomical Feasibility Study.

PURPOSE: The motor branch to the brachialis muscle has been described as a versatile donor in upper limb nerve reconstruction. This includes patients with peripheral nerve injuries and tetraplegia. However the anatomic feasibility of transfers from the musculocutaneous nerve to radial nerve branches around the elbow has not been systematically evaluated. This study aims to assess this feasibility. Such a transfer may be particularly relevant in patients with high-level tetraplegia or in patients with spastic upper limb involvement, in which selective denervation of spastic elbow flexors could

Arthroscopic Latissimus Dorsi Transfer in the Lateral Decubitus Position for Irreparable Posterosuperior Massive Rotator Cuff Tears.

The management of irreparable posterosuperior massive rotator cuff tears remains challenging. With advances in arthroscopic techniques, minimally invasive arthroscopic latissimus dorsi tendon (LDT) transfer has been increasingly used for irreparable posterosuperior massive rotator cuff tears. The present article describes arthroscopic LDT transfer in the lateral decubitus position for irreparable posterosuperior massive rotator cuff tears. The dissection and release of the LDT were performed through the interval between long head of the biceps tendon and humeral shaft. When releasing the LDT,

Neurovascular injuries associated with proximal humerus fractures: a review of the current literature.

Neurovascular injuries associated with proximal humerus fractures represent a rare yet clinically significant complication with potential for devastating functional outcomes. Their multifactorial etiology encompasses direct trauma from displaced fracture fragments, indirect stretch or compression mechanisms, and iatrogenic causes during surgical management. The close anatomic relationship between the proximal humerus, axillary artery, and brachial plexus predisposes these structures to combined injury patterns that can threaten limb viability. Diagnosis relies on early recognition through meti

Similar outcomes after arthroscopic rotator cuff repair alone in elderly patients with or without primary shoulder dislocation and Bankart lesion.

Surgical repair can effectively treat rotator cuff tears (RCTs) and Bankart lesions after shoulder dislocation. Bankart repair has been associated with potential complications such as glenohumeral arthritis, reduced external rotation, and axillary nerve injury. This study evaluated arthroscopic rotator cuff repair (ARCR) outcomes in patients aged ≥ 60 years with RCTs and Bankart lesions after primary shoulder dislocation. Outcomes were compared with patients who had RCTss alone. This retrospective comparative study included patients who underwent ARCR without Bankart repair. Patients were segme

Surgical fixation of distal diaphyseal radial and ulnar fractures in a macaque (Macaca fascicularis) using dual titanium locking plates: a case report.

Antebrachial fractures in captive non-human primates are challenging because unpredictable forelimb use and limited tolerance of external coaptation increase torsional loading across the radius-ulna unit. A 10-year-old, 7.8-kg male long-tailed macaque (Macaca fascicularis) presented with a closed distal diaphyseal oblique radial fracture and a concurrent transverse distal diaphyseal ulnar fracture of the right forelimb. No radiographic evidence of underlying osseous pathology was found. After initial medical management, open reduction and internal fixation of both bones was performed. A low-co

Ultrasound-guided infraspinatus-teres minor plane block for acute post traumatic shoulder pain in the emergency department: A case series.

Acute traumatic shoulder injuries are often associated with severe pain that hinders examination, radiographic positioning, and timely orthopaedic management. Conventional systemic analgesia may be inadequate, while interscalene and superior trunk blocks can be limited by hemidiaphragmatic paresis. The ultrasound-guided infraspinatus teres minor (ITM) inter-fascial plane block is a recently described diaphragm-sparing technique that targets the suprascapular and axillary nerves through a single posterior injection and has shown promising anatomic and postoperative analgesic coverage in earlier

A randomized controlled trial of brachial plexus block manipulation versus steroid injection in conscious patients for moderate to severe shoulder stiffness.

BACKGROUND: Intra-articular corticosteroid injection in combination with rehabilitation is a prevalent treatment modality for moderate-to-severe shoulder stiffness. However, this approach frequently results in delayed and incomplete recovery. Conversely, arthroscopic capsular release is associated with inherent surgical risks. Manipulation under anaesthesia (MUA) presents a minimally invasive alternative; however, concerns regarding intravenous anaesthesia and iatrogenic rotator cuff injury have served to limit its adoption. This randomised controlled trial aimed to evaluate the hypothesis tha
