The evolution and clinical impact of single-port transaxillary robotic thyroidectomy: a comprehensive review.

Single-port (SP) transaxillary robotic thyroidectomy represents a significant innovation in the field of endocrine surgery, offering a minimally invasive approach that combines oncological efficacy with enhanced cosmetic outcomes. The introduction of the SP robotic system has enabled surgeons to perform thyroidectomy and lateral neck dissection through a single axillary incision, resulting in a scarless neck and reduced postoperative morbidity. This review explores the current practices and surgical techniques associated with SP transaxillary robotic thyroidectomy, emphasizing the benefits of

Nerve transfers for axillary nerve repair in brachial plexus injuries: results from 206 patients.

OBJECTIVE: Restoration of axillary nerve function is one of the main priorities of brachial plexus surgery. Neurotization, the transfer of a functional but less important donor nerve to a nonfunctional, more important recipient nerve, has become a leading treatment option. A variety of donor nerves, from different segmental levels of the spinal cord, have been used to reinnervate the axillary nerve. This study aimed to describe the clinical results of commonly used donor nerves. METHODS: A group of 206 patients with a minimum follow-up period of 24 months was analyzed. Axillary nerve injuries

Pain Management in Burned Patients Treated with Bromelain-Based Enzymatic Debridement.

Background/Objectives: Enzymatic debridement with bromelain is a treatment option for deep partial thickness and full thickness burns. This procedure is associated with significant pain, necessitating the use of anesthesia techniques. However, there is limited evidence on the optimal strategy to achieve effective pain control. To detail the anesthetic approach in patients undergoing bromelain-based enzymatic debridement for burn injuries. Methods: A retrospective observational study was conducted by analysing the medical records of burn patients treated with enzymatic debridement using bromela

The deltoid abduction lag test for assessment of axillary nerve injury and functional deltoid recovery.

BACKGROUND: Diagnosis and assessment of functional deltoid recovery following axillary nerve injury is challenging due to numerous other muscular contributions to shoulder movement - particularly supraspinatus, rhomboids and long head of triceps. We present the deltoid abduction lag test (DALT) which offers four clear benefits: 1) it successfully isolates deltoid in the presence of a functioning supraspinatus with intact tendon and attachment; 2) it is safe to perform early following anterior shoulder dislocation (the most common cause of axillary nerve injury); 3) it allows assessment of earl

MR neurography findings of brachial plexopathy following total shoulder arthroplasty.

OBJECTIVES: To characterize nerve injury patterns following total shoulder arthroplasty (TSA). METHODS: This retrospective study reviewed brachial plexus MR neurography (MRN) in patients with brachial plexopathy within 180 days post TSA from 2016 to 2023. Patients with pre-existing neurologic symptoms or without available electrodiagnostic testing (EDX) data were excluded. MRN findings were extracted from the original report and independently reviewed by a second, musculoskeletal fellowship trained radiologist. RESULTS: A total of 27 patients (15F/12 M, mean age 67) were included. MRN and ED

Real-World Evidence of Langer's Axillary Arch: Bridging Anatomical Variations and Surgical Practice for Safer Axillary Procedures.

Langer's axillary arch is an anatomical variant characterized by a fibromuscular band extending from the latissimus dorsi to the pectoralis major, humerus, or coracoid process, traversing the axillary region. This structure may alter expected anatomical relationships and has the potential to compress neurovascular elements, thereby complicating surgical procedures. Although extensively described in cadaveric studies, in vivo identification during surgery remains infrequently reported. Given its anatomical variability and potential impact on surgical outcomes, further intraoperative documentati

Transfer of the long head of triceps branch to the anterior division of the axillary nerve for deltoid reanimation: Anterior or posterior approach? An anatomical study.

Transferring the long head of the triceps nerve to the axillary nerve is a widely used technique for restoring abduction and elevation of the shoulder in adults with partial brachial plexus lesions. This procedure can be performed using either an anterior (axillary) or a posterior approach. This anatomical study aimed to compare the distance between the microsurgical suture of the nerve transfer and the axillary nerve's entry into the middle and anterior deltoid, to determine which approach provides the shortest nerve regrowth path. The anatomical study was carried out on 12 subjects (i.e. 24

Morphometric analysis of peripheral nerves in people with spinal cord injury.

The morphology of peripheral nerves sampled at the time of nerve transfer surgery was directly assessed in participants within 15 months of their SCI. Fresh nerve samples were fixed in glutaraldehyde, processed and embedded in Araldite Epon. Light and electron microscopy was used to conduct a qualitative analysis of morphology and a quantitative morphometric analysis of axon, fibre and myelin areas and diameters and g-ratios. Over two-thirds of both donor and recipient nerves showed abnormalities that differed in appearance and aetiology. These included myelin thickening and folding (tomacula)

Magnetic Resonance Neurography of Sports-Related Peripheral Nerve Injuries of the Shoulder Region.

CONTEXT: Although many sports-related injuries in the shoulder region are self-limiting, those involving the peripheral nerves can have a profound adverse effect on an athlete's career. While physical examination and electrodiagnostic testing are the mainstays for assessing such injuries, peripheral nerve magnetic resonance (MR) imaging, also known as MRN, can play an important and complementary role in helping further characterize injury extent and localization. This review explores the application of MRN in diagnosing and managing sports-related peripheral nerve injuries. EVIDENCE ACQUISITI

Minimal improvements in objective shoulder strength following shoulder arthroplasty: a systematic review.

BACKGROUND: Patients with degenerative shoulder conditions indicated for shoulder arthroplasty frequently present with weakness due to disuse, pain, or their underlying pathology. However, it is unclear whether shoulder arthroplasty results in improvements in strength postoperatively. The purpose of this systematic review was to evaluate changes in objective strength following both anatomic total shoulder arthroplasty (aTSA) and reverse total shoulder arthroplasty (rTSA). METHODS: A systematic review was conducted in April 2024 using the keywords "shoulder arthroplasty," "outcomes," and "stre

Different patterns of neurogenic quadrilateral space syndrome: a case series of undefined posterior shoulder pain.

BACKGROUND: Quadrilateral space syndrome is a painful disorder of the shoulder caused by static or dynamic entrapment of the axillary nerve and the posterior humeral circumflex artery. It was first described in 1983; however, it is an uncommon syndrome that initially presents with nonspecific shoulder pain or selective deltoid atrophy, and diagnosis is often delayed owing to its rarity. Young athletes of overhead sports are more commonly affected by this syndrome. Symptoms of quadrilateral space syndrome include silent deltoid atrophy, persistent posterior shoulder pain, paresthesias, and tend

Occult breast cancer management with axillary lymph node dissection versus sentinel lymph node biopsy: Case reports and literature review.

BACKGROUND: Breast cancer is the most common cancer worldwide. In patients with occult breast cancer, sentinel lymph node biopsy (SLNB) has been shown to reduce complications, shorter hospital stay, and improved quality of life compared to axillary lymph node dissection (ALND). This study presents two case reports comparing outcomes of ALND versus SLNB in the management of occult breast cancer. CASE 1: A 52-year-old female presented with a left axillary tail mass which showed invasive ductal carcinoma. Underwent left axillary lymph node dissection. She developed postoperative drain seroma. Sequential injury: Delayed-onset axillary artery thrombosis following brachial plexus paralysis from a traumatic shoulder injury.

The association of brachial plexus nerve injury with arterial involvement in the context of shoulder trauma is well documented in the literature. However, the clinical presentation can vary significantly from case to case. In severe trauma cases, there may be a simultaneous occurrence of both types of injuries. An arterial injury can lead to the formation of a hematoma, causing compression of the nerves that innervate the upper limb. Additionally, avulsion or stretching of the roots of the brachial plexus may also generate a peri‑arterial hematoma, which can result in arterial thrombosis. Thes

Nerve Grafting for Axillary Nerve Injuries Following Shoulder Trauma: A Systematic Review of Surgical Outcomes.

The axillary nerve is particularly susceptible to injury following shoulder trauma such as dislocation and fracture-dislocation, leading to loss of abduction, external rotation, and sensory deficits. Severe cases with loss of continuity require surgical intervention. Nerve grafting is the mainstay when direct repair is not feasible, though outcomes are influenced by timing, graft length, and patient selection. This review was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. A systematic search of PubMed, Embase, Scopus, a

Evaluating the Impact of Anesthesia Type on Finger Replantation Surgery Success.

OBJECTIVES: Replantation surgery is one of the most difficult areas of reconstructive surgery. The aim of finger replantation is to restore sensation and adequate function of the amputated part in addition to restoring circulation. It is very crucial to investigate prognostic factors to improve the outcomes of this surgery. The type and severity of the injury, along with the duration between the injury and surgery, are the most significant factors influencing the success of replantation. This study investigates the impact of the type of anesthesia used on the success of replantation and other

Surgical Outcomes in Non-Transected and Partially Transected Peripheral Nerve Injuries.

BACKGROUND: Non-transected and partially transected peripheral nerve injuries (neuromas-in-continuity) are relatively common but understudied. Their optimal surgical management and expected outcomes remain unclear. We conducted a literature review of surgical repairs in such lesions and illustrate a case to guide decision-making. Systematic searches of PubMed and Google Scholar identified 70 eligible reports (Level I = 2, Level II = 5, Level III = 37, Level IV = 20, Level V = 4). Across studies, neurolysis of NAP-positive lesions often restored antigravity strength, while direct repair or graf
