Brachial Plexus Nerve Transfer Under Wide Awake Local Anesthesia No Tourniquet.

Brachial plexus injuries commonly require nerve transfers, traditionally performed under general anesthesia. We propose the wide awake local anesthesia no tourniquet (WALANT) technique, utilizing lidocaine with epinephrine to achieve anesthesia and maintain a bloodless surgical field without general anesthesia. Our initial experience with nerve transfers (eg, ulnar-to-musculocutaneous for elbow flexion) entirely under WALANT demonstrates excellent patient tolerance, minimal pain, and high satisfaction. Intraoperative nerve identification was enhanced by patient feedback and nerve stimulation w

[Operations in patients with giant false aneurysms of upper-limb arteries (clinical case reports)].

False aneurysms of upper-limb arteries are rare pathological conditions in vascular surgery. They are typically caused by various injuries, more often iatrogenic, after endovascular interventions. These pseudoaneurysms can be complicated by ruptures, compression of nerves and nerve plexuses, as well as embolization of the distal arterial bed with the development of limb ischemia. In this article, we summarize the experience of surgical treatment of false aneurysms of upper-limb arteries and describe two clinical case reports of successful surgical management of giant pseudoaneurysms of the bra

Variable Innervation of the Triceps Brachii: Anatomical and Clinical Perspectives in a Narrative Review.

Traditional anatomical texts describe the triceps brachii as receiving motor innervation exclusively from branches of the radial nerve. However, growing morphological, electrophysiological, and clinical evidence demonstrates notable variability in its neural supply, particularly to the long and medial heads of the muscle. To clarify these inconsistencies, a narrative review was conducted using PubMed, Scopus, and Google Scholar with search terms related to triceps innervation and nerve variations. After screening 930 records, 27 studies met the inclusion criteria. The articles were reviewed an

Misventure in the Treacherous Terrain of Subscapularis during Latarjet Procedure: A Case Report.

Latarjet procedure is one of the commonly performed surgeries for recurrent anterior dislocation of the shoulder and has been modified by many surgeons over the past eight decades. This procedure mandates detailed understanding of the anatomy of the shoulder, axillary region, and brachial plexus, as the subscapularis muscle will be split open during the procedure, which forms the bed for axillary artery and cords and branches of the brachial plexus. Any deviation from the procedure could lead to untoward complications. We present an extremely rare case of injury to the lateral cord of brachial

Clinical outcomes of reverse shoulder arthroplasty in obstetric brachial plexus palsy with suprascapular nerve injury and glenohumeral osteoarthritis.

BACKGROUND: Untreated obstetric brachial plexus palsies (OBPP) present with various upper limb impairments, including muscle weakness, soft tissue contractures, and osteoarticular deformities. Reverse shoulder arthroplasty (RSA) medializes the center of rotation, optimizing deltoid function to compensate for deficits caused by OBPP. This study evaluates the effectiveness of RSA in patients with untreated OBPP involving the suprascapular nerve and end-stage glenohumeral osteoarthritis (OA), focusing on functional and quality-of-life improvements. METHODS: A retrospective analysis was performed

Low-Level Laser Therapy in Maxillofacial Trauma: A Prospective Single-Arm Observational Study.

Background: Surgical management of trauma in the maxillofacial complex can result in iatrogenic nerve injuries, particularly involving the infraorbital, inferior alveolar, and mental nerves. Paresthesia is a common postoperative complication, often attributed to the anatomical positioning of these nerve structures, making them vulnerable to injury. Among current therapeutic options for nerve injuries, low-level laser therapy (LLLT) has shown promising results in published studies. Objectives: This prospective observational study evaluated the effects of LLLT on nerve recovery following maxillo

A Preliminary, Three-Arm, Randomized, Double-Blinded, Placebo-Controlled Trial Comparing the Efficacy of Adductor Canal Pain Catheters Following Total Knee Arthroplasty.

INTRODUCTION: The United States has experienced a notable increase in opioid abuse over the past several years, with orthopaedic surgeons reported as the third-highest prescribers by specialty. Several studies have addressed opioid use after major orthopaedic procedures but largely focused on prescribing practices. There is limited data on the utility of adding adductor canal pain catheters to multimodal regimens following total knee arthroplasty for improved pain control and reduced opioid dependence. The purpose of this preliminary study was to compare single-shot adductor canal blocks to co

The fascial plane blocks in the upper arm surgery and trauma: A narrative review.

Fascial plane blocks are becoming popular as useful and safe methods to provide pain relief. While their use in thoracic and abdominal surgery is well known, their application in upper arm surgery and trauma is underscored. This review explores research from the last 10 years focusing the use of fascial plane blocks to relieve pain after surgery or trauma in the upper limb. Studies include randomized controlled trials, observational studies, and case reports. They look at techniques like the cervical and high-thoracic erector spinae plane (ESP) block, the clavipectoral fascia plane (CFP) block

Presence of Martin-Gruber Anastomosis among a Cohort of Mexican Origin; a Corpse Dissection Study.

BACKGROUND: At the forearm level, Martin-Gruber anastomosis is described as the most common anatomic variation. Early recognition of this anastomosis in patients with peripheral nerve lesions and preserved function of the hand musculature lowers the rate of misdiagnosis and prevents unnecessary treatment approaches. We aimed to determine the prevalence and characteristics of Martin-Gruber anastomosis in a Mexican cohort and discuss its impact on clinical diagnostics and approach. METHODS: We conducted a cross-sectional dissection study on 40 inert anatomical models at the National Forensic Sc

Fully Arthroscopic Anterior Latissimus Dorsi Transfer for Irreparable Subscapularis Tears.

Latissimus dorsi (LD) transfer can be used for the treatment of irreparable subscapularis tears. This article describes a fully arthroscopic LD transfer procedure to manage irreparable subscapularis tears. The LD tendon and muscle are dissected and released arthroscopically, and care must be taken to avoid injury to the axillary and radial nerves. With the use of a portal superior to the upper margin of the pectoralis major (PM) and a portal inferior to the lower margin of the PM, detachment of the proximal PM tendon is not necessary for access to the insertion of the LD tendon. The posterior

Axillary to Radial Nerve Transfer for Elbow Extension in the Tetraplegic Patient: A Case Series.

BACKGROUND AND OBJECTIVES: Recovery of upper extremity function is considered the top priority in the tetraplegic population. Midcervical injury often results in paralysis of the hand and loss of elbow extension. Elbow extension is essential for extending one's workspace and facilitating mobility (ie, self-transfer, wheelchair propulsion). Tendon transfers have traditionally been used to restore elbow extension, but the axillary to radial nerve transfer (ARNT) has emerged as a promising alternative. Our objective was to review a case series of single-surgeon outcomes for recovery of elbow exte

A meta-analysis and systematic review: association of timing and muscle strength after nerve transfer in upper trunk palsy.

A systematic review following PRISMA guidelines was conducted to address the PICO question: Does early (< 6 months) nerve transfer improve muscle strength outcomes when compared to late (≥ 6 months) nerve transfer for upper trunk palsy, as measured by the Medical Research Council (MRC) scale? The association between age and successful muscle function recovery was assessed as a secondary outcome. The quality of included studies was evaluated using the Methodological Index for Non-Randomized Studies (MINORS) tool. A total of 349 studies were screened, with 27 studies meeting criteria for systema

The 'teres major first' approach for deltoid reinnervation in brachial plexus injuries with shoulder fracture-dislocations.

BACKGROUND: In upper brachial plexus (C5-C6) avulsion injuries, deltoid reinnervation via radial-to-axillary nerve transfer is standard. However, fracture-dislocations around the shoulder complicate surgical exposure due to fibrosis and distorted anatomy. METHODS: Fifteen male patients (mean age: 26.1 years) with C5-C6 avulsion injuries and concomitant shoulder fractures were treated between October 2019 and April 2022 using a modified posterior approach. The "teres major first" technique utilized the inferior border of the teres major muscle as a consistent landmark to identify and mobilize

Upper brachial plexus nerve transfers under wide awake anesthesia (WALANT)- surgical technique.

BACKGROUND: Upper brachial plexus injuries often necessitate complex nerve transfers, which are traditionally performed under general anesthesia (GA). In patients with polytrauma or significant comorbidities, GA may present additional perioperative risks. The Wide Awake Local Anesthesia No Tourniquet (WALANT) technique offers a potential alternative by enabling nerve transfers under local anesthesia with real-time patient feedback and without the need for sedation or airway management. METHODS: We describe the application of the WALANT technique in a 15-year-old male patient with a C5-C6 brac

Evaluation of lower trapezius function after transfer of axillary nerve to suprascapular nerve in patients with ERB's palsy.

BACKGROUND: It is expected that transfer of spinal accessory nerve to suprascapular nerve, which is widely used in the restoration of the shoulder function in brachial plexus birth injury (BPBI), impairs the trapezius function. AIM: To hypothesize that the lower trapezius muscle remains functional after this neve transfer. METHODS: In a retrospective cross-sectional study, patients with BPBI who underwent nerve transfer from accessory nerve to supraclavicular were followed for at least six months following the operation and demographic data were extracted from the database. A Cadaveric Study on the Variations in the Infraclavicular Part of Brachial Plexus in Axillary Region and Upper Arm.

BACKGROUND: The brachial plexus shows frequent anatomical variations that can complicate diagnosis, surgical planning, and anesthetic procedures. Detailed knowledge of these variations is essential to prevent iatrogenic injury and improve clinical outcomes. MATERIALS AND METHODS: Eighteen donors (36 upper limbs) were dissected at Mercer School of Medicine, Columbus, Georgia, United States. The sample included 10 males and 8 females, aged 36-89 years (mean age: 67.67 years).
