[Neurovascular autoregulation of the regeneration of nerves in the upper extremities].

Functional features of tissue microcirculation in injury and regeneration of nerves were revealed from analysis of 440 polarograms and capillarograms recorded in 30 healthy individuals and 70 patients with sequelae of injury to the median and ulnar nerves. The initial regeneration of the nerve trophic function proper increases the level of microcirculation and oxygenation of the tissues and thus prepares them for the subsequent reinnervation and is conducive to the trophics of the penetrating axons. "Neurovascular autoregulation" of nerve regeneration takes place. A microcirculatory approach t

Results of primary nerve repair in the upper extremity.

The results of primary repair of peripheral nerve injury in the upper extremity are reported for 143 nerves in 120 patients, with a mean follow-up of 24 months. Normal values for static and moving two-point discrimination were established and related to the person's age. Sensory reeducation was employed routinely in the postoperative rehabilitation program. Moving two-point discrimination recovered to a better level in 61%, the same level in 38%, and a worse level in 1% than static two-point discrimination after nerve repair. Results for digital, median, ulnar, and radial nerve repairs are rep

Medial brachial fascial compartment syndrome: anatomic basis of neuropathy after transaxillary arteriography.

The authors reviewed their experience with 320 transaxillary arteriograms, as well as the English-language literature on neuropathy complicating transaxillary arteriography. Three of their patients had median and ulnar motor and sensory nerve injury, and six others had only sensory involvement. The occurrence or severity of nerve injury did not correlate well with the size or presence of an observable axillary or arm hematoma. Dissection of the axillae and arms of 25 human cadavers revealed a tough medial brachial fascial compartment (MBFC) outside a thin axillary sheath. The median and ulnar

Contemporary treatment of peripheral nerve and brachial plexus lesions.

The paper outlines modern microsurgical techniques utilized in the repair of injured peripheral motor and sensory neurons. The diagnostic evaluation and its timing, which depend on the level and the extent of the lesion, are proposed. The author stresses the need during the operation for close monitoring, which is a prerequisite of proper coaptation of the severed nerve structures. A technically perfect microsurgical repair provides optimal conditions for regeneration of the divided peripheral nerves and/or brachial plexus. The repair of avulsion injuries of the brachial plexus still poses man

[Restoration of discrimination sensitivity after suturing nerves of the upper extremity].

Having examined the discriminational sensitivity with Weber's dividers in 244 patients 1-10 and more years after epineural suture of the median, ulnar and palmar digital nerves, the authors established that this type of sensitivity restored slowly, incompletely and rarely. Discrimination of two points was better restored after suture of the palmar digital nerves, it was worse after suture of the median nerve and was the worst after suture of the ulnar nerve. The factors adversely affecting the restoration of discriminational sensitivity included a high level of damage (upper arm, the upper thi

Ulnar compression neuropathy due to an occult post-traumatic synovial cyst.

A patient presented with an ulnar nerve palsy which developed some time after minimal trauma to the arm. It was thought that the paralysis was possibly caused by leprosy, but because of doubt about the diagnosis, biopsy of the nerve was advised. At operation, it was found that there was nerve compression by a slowly developing occult synovial cyst of the elbow joint, which had tracked down the tight confines of the cubital tunnel. If there is doubt as to whether an isolated ulnar palsy is caused by leprosy, it may be reasonable to explore the nerve at the level of the cubital tunnel. If compre

Management of arm arterial injuries.

Management, morbidity, and mortality of arterial injuries of the arm are dependent upon site of injury, promptness and accuracy of diagnosis, and involvement of contiguous structures. To determine results and establish principles of management, a retrospective analysis was undertaken of 109 arm arterial injuries from the thoracic inlet to the wrist in 102 patients. Iatrogenic injuries were excluded. Arterial injuries were classified into those affecting the upper arm (subclavian, axillary, and brachial) and those affecting the lower arm (radial, ulnar, and interosseous). This proved to be a us

Peripheral nerve entrapment syndromes in chronic hemodialysis patients.

15 of 271 patients (6%) treated with chronic hemodialysis developed peripheral nerve entrapment syndrome of the median or the ulnar nerve. The majority of these patients were female (p less than 0.03). Fistulas located in arms with nerve entrapment tended to have higher flow rates than fistulas located in arms without nerve entrapment (57%) vs. 4.4%, p less than 0.001). There was no correlation with the renal diagnosis, previous access surgery in the involved arm, the type of vascular access used, the duration of hemodialysis, the adequacy of dialysis, calcium or phosphate homeostasis, plasma

Upper extremity neuropathies following median sternotomy.

The status of 958 patients who underwent median sternotomy between January, 1978, and May, 1981, was analyzed. Fifty-four patients had an upper extremity neuropathy. Among 38 patients who underwent further evaluation, motor and sensory nerve conduction studies localized the injury to the level of the elbow in 13, to the brachial plexus in 10, and to both locations in 6. Ninety-two percent of these 38 patients were asymptomatic 3 months after operation.

The pneumatic tourniquet: a biomechanical and electrophysiological study.

The pneumatic tourniquet is used routinely in extremity surgery, not only to prevent unnecessary blood loss, but also to increase the safety of the operative procedure. The mechanical part of this study shows that the system can fail in either of two major ways, each causing serious problems. The tourniquet may be inadequate if the pressure delivered to the cuff is not accurately displayed on the gauge or if the pre-set pressure is not maintained for the duration of the operative procedure. In our hospital, three of seven tourniquets (43%) in daily use showed significant errors in this regard.

The pathogenesis of pneumatic tourniquet paralysis in man.

The relative importance of ischaemic and direct mechanical injury to nerves compressed by a tourniquet, in the pathogenesis of tourniquet paralysis in man has not been established. To investigate this question, conduction in ulnar or median nerve fibres has been measured in healthy subjects both at the level of the pneumatic tourniquet and distal to the tourniquet. Measurement was prior to, for the period of tourniquet inflation, and following release of the tourniquet. The earliest conduction delays and block were observed at the level of the tourniquet, particularly across the proximal tourn

Somatosensory cerebral evoked potentials in diagnosing brachial plexus injuries.

Seven patients, 5 months up to 7 years after closed injury of the brachial plexus, were stimulated in the wrist, elbow and axilla of the extremity affected in order to invistigate average somatosensory cerebral evoked potentials (SCEP). Avulsion of most of the roots C5-Thl from the spinal cord could be objectively proved. All available clinical, contrast X-ray, and electrophysiological methods were used as well as surgical approach in the supraclavicular region or laminectomy. Sensation was objectivized by the galvanic skin reflex. Sensory nerve action potentials (NAP) were registered from the
