Morphometric cadaveric assessment of suprascapular and axillary nerve trajectories in relation to glenohumeral landmarks relevant to reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is increasingly used for complex shoulder pathology but carries a risk of iatrogenic nerve injury, particularly to the suprascapular (SSN) and axillary nerves (AXN). Detailed morphometric data on their relationship to glenoid landmarks remain limited. METHODS: Forty embalmed adult cadavers (60 shoulders) were dissected at PGIMER, Chandigarh, after ethics approval (PGI/IEC/2024/EIC000563). The SSN and AXN were exposed, and distances from nerves to glenoid "clock-face" positions and to osseous landmarks were measured with digital calipers.

Morphometric analysis of the axillary nerve in relation to the deltopectoral and deltoid-splitting approaches: implications for surgical safe zones.

PURPOSE: The deltopectoral splitting approach is commonly used in shoulder surgery; however, injury to the axillary nerve remains a major concern due to potential motor and sensory deficits. This study aimed to define the anatomical relationships of the axillary nerve to surrounding structures in order to optimize surgical access and minimize the risk of iatrogenic injury. METHODS: Thirty formalin-fixed cadavers (15 males, 15 females) were dissected. Quantitative measurements were obtained to determine the position of the axillary nerve relative to the deltopectoral interval and deltoid muscl

A Multicenter Matched Cohort Study of Processed Nerve Allograft and Autograft in Mixed and Motor Nerve Reconstruction.

PURPOSE: Nerve transections often require surgical intervention with a bridging material to achieve meaningful recovery (MR). We explored similar cohorts with nerve reconstruction using nerve autograft or nerve allograft. We hypothesized that motor recovery in mixed and motor nerve injuries repaired with nerve allograft is noninferior to repairs with nerve autograft. METHODS: Subjects had mixed or motor nerve transections in the upper extremity distal to the axilla surgically repaired with nerve autograft or nerve allograft. Motor recovery outcomes were analyzed using the British Medical Rese

Novel anatomical mapping of the anterior branch of the axillary nerve across the anterior and middle part of the deltoid: a fresh-cadaver study defining safe zones.

PURPOSE: To map the surface projection of the anterior branch of the axillary nerve (ABAN) over the anterior and middle part of the deltoid and to define safe and high-risk zones for proximal humeral approaches. METHODS: Fourteen whole adult fresh-frozen cadavers (28 paired shoulders) were dissected in this study. Acromial landmarks were defined as point A (anterolateral corner of acromion), point B (middle point of AC), point C (posterolateral corner of acromion), and insertion of middle part of the deltoid (point D) in a modified anatomical position. A delto-pectoral landmark connected D to

Development and validation of a risk prediction model for rotator cuff tears following acute anterior shoulder dislocation.

PURPOSE: Following shoulder dislocation, injuries such as rotator cuff tear (RCT), axillary nerve injury, greater tuberosity (GT) fracture, Hill-Sachs lesion, and Bankart lesion frequently occur. This study aimed to develop and validate a clinical prediction model for the occurrence of RCT after acute anterior shoulder dislocation. METHODS: This study included all patients with acute anterior shoulder dislocation who presented to the emergency orthopedics department of the same institution from July 2021 to March 2025. Lasso regression was used to select independent variables for the model. A

Reverse End-to-Side Triceps to Axillary Nerve Transfer for Treatment of Axillary Nerve Palsy After Reverse Total Shoulder Arthroplasty.

Nerve injuries are uncommon, yet significant complications following reverse total shoulder arthroplasty (rTSA), with the axillary nerve being most frequently injured. We report a case of a 67-year-old female who developed persistent motor dysfunction and decreased sensation in the axillary distribution following rTSA for a comminuted proximal humerus fracture. Electrodiagnostic studies confirmed right axillary nerve injury with reduced motor unit recruitment. After no improvement in deltoid function at three months following rTSA, we discussed treatment options for the patient, who opted for

Latissimus Dorsi Transfer With Interposed Autograft for Axillary Nerve Palsy After Reverse Shoulder Arthroplasty: A Case Report.

Persistent axillary nerve palsy after reverse shoulder arthroplasty (RSA) results in profound deltoid dysfunction and loss of active shoulder elevation. Traditional reconstructive options such as nerve repair or transfer may not be feasible in chronic cases. Latissimus dorsi tendon transfer augmented with an interposed graft has been described to restore shoulder abduction in deltoid-deficient shoulders, but its application after RSA is not well documented. We present the case of a 73-year-old female patient who developed chronic axillary nerve palsy following RSA performed for complex proxima

A Perplexing Plexopathy After Pembrolizumab Therapy in Early-Stage Triple-Negative Breast Cancer.

Background: In triple-negative breast cancer (TNBC), the addition of immunotherapy has significantly improved outcomes. Immune-related adverse events (irAEs) can be accelerated in patients with pre-existing autoimmune (AI) conditions. The treatment-response standardized protocol used in clinical care raises concerns about the need for right-sizing strategies. As the use of immunotherapy expands, recognizing toxicity from recurrence and optimizing response-adapted approaches are essential to balance cure with quality of survival. Case Presentation: A 38-year-old pregnant woman with a distant hi

Resolution of Glenohumeral Pseudo-Subluxation: An Illustrative Case Report on Relative Clinical Success in Pan-Brachial Plexus Injury.

Brachial plexus injuries (BPIs) are burdensome to patients. These often involve the axillary nerve, which may lead to pseudo-subluxation of the humeral head because of deltoid atony. While advances in nerve reconstruction have improved outcomes, these procedures still have markedly high failure rates, which may lead to persistent pain, dysfunction and poor satisfaction. We present a case of a 51-year-old male with a traumatic BPI in the context of a motorcycle accident who experienced pseudo-subluxation of his humeral head and later underwent surgical reconstruction for his BPI. Despite a lack

Regional anesthesia strategies for proximal humerus fracture surgery: anatomical considerations, diaphragm-sparing techniques, and expert perspectives-a narrative review.

BACKGROUND: Proximal Humerus Fractures (PHFs) are increasingly common, particularly in elderly patients with osteoporotic bone. Surgical and anesthetic management significantly influence outcomes, yet evidence guiding optimal strategies remains limited. Regional Anesthesia (RA) offers effective analgesia, opioid sparing, and the potential to avoid general anesthesia in frail or high-risk patients. This narrative review summarizes current knowledge, highlights diaphragm-sparing approaches, and integrates expert clinical insights to provide practical recommendations for anesthesiologists managin

Is there still a role for the medial pectoral nerve as a donor for axillary reinnervation in partial brachial plexus injury?

OBJECTIVE: to assess axillary nerve reinnervation when utilizing either a radial nerve branch to triceps muscle (RB) or through using the medial pectoral nerve (MP) as donors for patients with partial brachial plexus injuries. METHODS: We performed a retrospective study with 55 patients who received treatment for partial brachial plexus injuries. The patients were divided into RB (n = 29) or MP (n = 26) groups based on donor nerve used to reinnervate the axillary nerve. The shoulder abduction and its external rotation were assessed after a minimum of 18 months after surgery by using the Medic

Traditional wisdom through scientific evidence: a review of Marma manipulation in Amsa sandhimokśa (shoulder joint dislocation) with a special focus on the Milch method.

INTRODUCTION: Shoulder dislocations represent 50 percent of all major joint dislocations, with anterior dislocation being most common accounting for up to 97% of all shoulder dislocations. The shoulder is inherently unstable because the shallow glenoid fossa articulates with only a small portion of the humeral head. Various techniques are available for reduction, with Kocher's being widely practiced. However, both the Kocher and Hippocratic techniques are linked to complications like axillary nerve injury and humeral fractures. Traditional Vaidyas utilize several techniques, notably one simila

A "Common Sense" Surgical Approach to Motor Shoulder Dysfunction Diagnosed as CRPS I/II Following Projectile Injuries to the Regio Deltoidea in the Late Stages of Ballistic Trauma.

BACKGROUND: High-velocity projectile injuries to the regio deltoidea (RD) can result in persistent pain and motor shoulder dysfunction (MSD). The underlying mechanisms may involve direct axillary nerve (Ax) trauma or remote nociceptor irritation caused by cavitation effects. OBJECTIVE: To characterize the anatomical basis of MSD and to establish a reliable diagnostic and surgical strategy for its management. METHODS: Twenty-seven patients presenting with painful MSD following projectile trauma to the RD were included. Eight patients exhibited deltoid palsy associated with Ax injury, of whom

Axillary nerve palsy incidence after total shoulder arthroplasty with and without neuromuscular blockade.

BACKGROUND: Axillary nerve palsy is a rare but significant complication of shoulder arthroplasty. The axillary nerve lies just beneath the inferior glenoid, increasing susceptibility to traction and compression injures during surgery. Neuromuscular blocking agents are commonly used to facilitate exposure and dissection but may reduce the surgeon's ability to perceive excessive traction or tension on the nerve. The aims of this study are twofold, to (1) determine whether intraoperative paralytic use is associated with post-operative axillary nerve palsy and (2) assess its relationship with oper

Challenges and Opportunities of Early Brachial Plexus Reconstruction in Polytrauma: Case Report and Review of the Literature.

Background: Assessment and treatment of brachial plexus injury in polytrauma patients is often challenging due to concomitant injuries requiring life-saving interventions. Furthermore, the role of immediate nerve exploration in closed postganglionic nerve injuries remains debated. Case presentation: We present the case of a 21-year-old male with an infraclavicular brachial plexus injury, floating shoulder and axillary artery rupture following a motorcycle accident. Early multidisciplinary intervention included vascular repair, bone stabilization and brachial plexus exploration. Nerve reconstru

Collagen-Matrix Cuff Reconstruction for Circumferential Lumbar Nerve Root Sleeve Defects: Technical Note and Single-Center Case Series With Case Illustration.

BACKGROUND AND OBJECTIVES: Circumferential defects of the dural sleeve of exiting lumbar nerve roots may occur after incidental durotomy, trauma, or deliberate opening of the root cuff during resection of nerve sheath tumors. Standard onlay duraplasty can control cerebrospinal fluid (CSF) leakage but does not restore the tubular dural-epineural conduit. We describe a collagen-matrix cuff technique for root sleeve reconstruction and report our initial institutional experience. METHODS: This single-center, retrospective case series includes 9 consecutive reconstructions performed between Januar

Mid-term outcomes of the Latarjet procedure for shoulder instability: A retrospective study of 187 shoulders with an eight-year follow-up.

PURPOSE: While the Latarjet procedure is a recognized intervention for glenohumeral instability, mid-term outcomes data, particularly regarding recurrence and complication rates, remain uncharacterized within the specified national context. This study aimed to retrospectively evaluate the mid-term clinical and functional results of the open Latarjet procedure for the management of recurrent anterior shoulder instability. METHODS: A retrospective, descriptive cohort study was conducted on patients undergoing the open Latarjet procedure for shoulder instability between 2009 and 2017 at a single

The safety of percutaneous screw placement in reverse shoulder arthroplasty for fracture: A cadaveric study.

BACKGROUND: Cementless reverse shoulder arthroplasty (RSA) with percutaneous locking screw fixation is an increasingly utilized option for treatment of proximal humerus fractures. The safety of percutaneous screw placement in the humerus has not been well established. This study aims to evaluate the proximity of the axillary and radial nerves to percutaneous screw trajectories in RSA. METHODS: A humeral fracture stem with percutaneous locking screws was implanted into six male cadaveric specimens. Screw holes were over-drilled, and distances from trocars and drill bits to the axillary and rad
