BACKGROUND: The supraclavicular (SC) nerve block is a procedure utilized by anesthesiologists with the goal of providing analgesia to the upper extremity at and below the shoulder. We present a case report of an SC continuous peripheral nerve block (CPNB) complicated by isolated musculocutaneous nerve palsy despite cessation of infusion of local anesthetic.
CASE REPORT: Patient was a 39-year-old woman with chronic ischemic left-hand pain for which an SC CPNB was placed. Following sympathectomy of the radial and ulnar digital arteries, she reported inability to flex/extend her elbow or shoulde

Brachial plexus injuries commonly require nerve transfers, traditionally performed under general anesthesia. We propose the wide awake local anesthesia no tourniquet (WALANT) technique, utilizing lidocaine with epinephrine to achieve anesthesia and maintain a bloodless surgical field without general anesthesia. Our initial experience with nerve transfers (eg, ulnar-to-musculocutaneous for elbow flexion) entirely under WALANT demonstrates excellent patient tolerance, minimal pain, and high satisfaction. Intraoperative nerve identification was enhanced by patient feedback and nerve stimulation w

Traditional anatomical texts describe the triceps brachii as receiving motor innervation exclusively from branches of the radial nerve. However, growing morphological, electrophysiological, and clinical evidence demonstrates notable variability in its neural supply, particularly to the long and medial heads of the muscle. To clarify these inconsistencies, a narrative review was conducted using PubMed, Scopus, and Google Scholar with search terms related to triceps innervation and nerve variations. After screening 930 records, 27 studies met the inclusion criteria. The articles were reviewed an

Latarjet procedure is one of the commonly performed surgeries for recurrent anterior dislocation of the shoulder and has been modified by many surgeons over the past eight decades. This procedure mandates detailed understanding of the anatomy of the shoulder, axillary region, and brachial plexus, as the subscapularis muscle will be split open during the procedure, which forms the bed for axillary artery and cords and branches of the brachial plexus. Any deviation from the procedure could lead to untoward complications. We present an extremely rare case of injury to the lateral cord of brachial

PURPOSE: Brachial plexus traction injuries have conventionally been categorized as involving the C5-C6, C5-C7, C5-T1, and C8-T1 roots. In this article, we report a distinct clinical presentation of brachial plexus injury characterized by intact finger flexion with signs of complete brachial plexus injury.
METHODS: From 2010 to 2022, 989 patients who sustained brachial plexus injuries were examined and underwent surgery. Prior to surgery, brachial plexus computed tomography myelography was obtained. A total of 16 patients (1.6%) with preserved finger flexion and panplexus injury were identifie

BACKGROUND: Upper brachial plexus injuries often necessitate complex nerve transfers, which are traditionally performed under general anesthesia (GA). In patients with polytrauma or significant comorbidities, GA may present additional perioperative risks. The Wide Awake Local Anesthesia No Tourniquet (WALANT) technique offers a potential alternative by enabling nerve transfers under local anesthesia with real-time patient feedback and without the need for sedation or airway management.
METHODS: We describe the application of the WALANT technique in a 15-year-old male patient with a C5-C6 brac

BACKGROUND: The brachial plexus shows frequent anatomical variations that can complicate diagnosis, surgical planning, and anesthetic procedures. Detailed knowledge of these variations is essential to prevent iatrogenic injury and improve clinical outcomes.
MATERIALS AND METHODS: Eighteen donors (36 upper limbs) were dissected at Mercer School of Medicine, Columbus, Georgia, United States. The sample included 10 males and 8 females, aged 36-89 years (mean age: 67.67 years). Dissections followed Grant's Dissector, 17th edition, with all variations in the infraclavicular brachial plexus and its

A variety of approaches have been described to obtain a rudimentary grasp following traumatic pan-plexus injury in the adult. The aim of this report is to present a novel reconstructive algorithm based on the combination of reconstructive surgery and the fitting of a myoelectric orthosis (Myoelectric Orthosis Bionic Approach [MOBA]), to gain grasp function after a pan-plexus injury. A 44-year-old patient presented with a pan-plexus injury after a heavy branch fell on his right shoulder during forestry operations. After 5 months without spontaneous recovery, the patient underwent a brachial ple

BACKGROUND: Nerve transfers are a cornerstone in the surgical management of traumatic brachial plexus injuries (BPIs) to restore elbow flexion. Common donor nerves include intraplexal sources like the ulnar and median nerves (fascicular transfers) and extraplexal sources like the intercostal nerves (ICNs). Despite the widespread use of both techniques, the optimal donor nerve remains a subject of debate. This systematic review and meta-analysis aims to compare these techniques for restoring elbow flexion in BPIs.
METHODS: A systematic search was conducted across PubMed, Embase, Cochrane Libra

BACKGROUND: Targeted Muscle Reinnervation (TMR) improves real-time control of EMG-based prostheses by connecting severed nerves to adjacent muscles, creating new EMG signals. However, TMR requires cutting original nerve connections, which can cause denervation atrophy and limit functional recovery. As an alternative, Targeted Nerve Function Replacement (TNFR) offers a fundamentally different approach by establishing a direct end-to-end anastomosis between an intact donor nerve and the original nerve of a target muscle, preserving existing neural pathways while providing supplementary neural in

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is widely used for cuff-deficient and degenerative shoulder conditions in older adults. While stemmed implants offer long-term success, stemless designs have gained attention due to bone preservation and ease of revision, but evidence remains limited.
METHODS: A systematic search was performed in PubMed, Scopus, Cochrane Library, and Google Scholar till June 2025. Five studies met the inclusion criteria. Key outcomes included satisfaction, Visual Analog Scale for pain, functional scores, shoulder range of motion (ROM), fracture-related co

UNLABELLED: Auxin response factors (ARFs) are key transcription factors involved in auxin signaling and play important roles in plant growth and floral development. However, their evolutionary history and potential roles in sex-related floral development in spinach remain unclear. Here, we performed a comparative genomics analysis of ARF gene families across Chenopodium species and Beta vulgaris, together with Spinacia species, to investigate their evolutionary characteristics and potential functional divergence. A total of 269 non-redundant ARFs were identified across 20 genomes and classifie

INTRODUCTION: Adhesive capsulitis, or frozen shoulder, is a painful and disabling condition characterized by progressive restriction of shoulder movement. Hydrodilatation, a minimally invasive procedure involving joint capsule distension, has been proposed as an effective treatment option. This study evaluates the functional outcomes of hydrodilatation in patients with adhesive capsulitis using the Constant-Murley score (CMS) and American Shoulder and Elbow Surgeons (ASES) score, along with pre- and post-procedure range of motion (ROM) assessments.

Neuropathic pain is a common complication following radial forearm free flap (RFFF) reconstruction, often associated with damage to the superficial branch of the radial nerve (SBRN) and lateral antebrachial cutaneous nerve (LACN). Traditional treatment approaches, including pharmacologic and non-pharmacologic interventions, frequently provide incomplete pain relief. This case report describes the innovative use of ultrasound-guided musculocutaneous and SBRN blocks combined with pulsed radiofrequency (PRF) neuromodulation in a 70-year-old male with severe neuropathic pain following RFFF for ton

Brachial plexus injuries are rare but highly disabling, with major implications for upper limb function and quality of life. Nerve transfers have emerged as a key reconstructive technique, particularly valuable in cases where primary repair or grafting is unfeasible or delayed. This retrospective study analysed functional outcomes following nerve transfers in 37 patients with brachial plexus injury. Motor recovery was assessed using the Medical Research Council scale. Patients were stratified by age, timing of surgery, injury severity, and type of nerve transfer performed. The majority of our

BACKGROUND: Spinal accessory nerve (SAN) transfer is commonly used to reanimate the suprascapular nerve (SSN) in traumatic brachial plexus injuries (BPI). Despite its technical simplicity and satisfactory results, this transfer has two disadvantages: the inadequacy of the number of nerve fibers between donor and recipient and the weakening of the trapezius muscle. To avoid this, we transferred a fascicle from the C7 root for the pectoralis major muscle to the suprascapular nerve for shoulder abduction.
METHODS: Patients with traumatic upper trunk brachial plexus injury presenting within 6 mon

BACKGROUND: Brachial plexus injuries (BPI) can be devastating for patients, often resulting in notable physical, psychological, and socioeconomic distress1. Violent accidents that torque the head away from the shoulder frequently damage the upper brachial plexus roots, with varying severity of damage to the lower roots1. Patients having pan-plexus injuries typically present with a flail extremity, loss of sensory function, and generalized atrophy. To improve activities of daily living, the treatment of pan-plexus injuries focuses on restoring antigravity motion of the upper extremity, with elb

STUDY DESIGN: International survey.
OBJECTIVES: C5 palsy (C5P) is a neurological complication affecting 5-10% of patients after cervical decompression surgery. Most cases improve with conservative treatment; however, nearly 20% of patients may be left with residual deficits. Guidelines are lacking on C5P management and timing of surgical intervention. Therefore, we sought to survey peripheral nerve surgeons on their management of C5P.
METHODS: An online survey was distributed centered around a patient with C5P after posterior cervical decompression and fusion. Questions included surgeon demo

INTRODUCTION: Anatomical variations of the brachial plexus and its branches are frequently encountered. Absent musculocutaneous nerve with complete or partial takeover of function by median nerve is known. Such variation creates confusion in the interpretation of a clinical or electro diagnostic test for evaluation of peripheral nerve injury.
METHODS: Institutional guidelines for the use of human cadaver were followed. Routine dissection of the upper limbs for undergraduate medical teaching was performed in a 67-years-old male cadaver following standard methods. Relevant gross anatomical feat

Brachial plexus reconstruction (BPR) consists of the complex surgical restoration of nerve structures. To further understand the underlying motor cortex changes and evaluate neuroplasticity after a successful surgery, we performed a navigated transcranial magnetic stimulation (nTMS) study mapping the postoperative motor representation of the formerly plegic arm. We conducted a prospective nTMS study mapping the musculocutaneous nerve as a representative, prominent target of BPR including a patient (n = 8) and a control group (n = 10). Measurements like resting motor threshold (RMT), cortical m
