[Isolated neuropraxy of the musculocutaneous nerve].

Authors describe a case of isolated posttraumatic neuropraxy of the musculocutaneous nerve. With the presentation of this very rare case attention is called, based on literary date, to the fact that the above injury can be mixed with the distal tendon rupture of the biceps.

Reinnervation of avulsed brachial plexus using the spinal accessory nerve.

The use of the accessory nerve as a donor is one of the possibilities for the reinnervation of the brachial plexus in cases of paralysis due to root avulsion. In this paper, an analysis of the reinnervation of the musculocutaneous or axillary nerve using the spinal accessory nerve is made on 13 cases, 8 of total and 5 of upper partial avulsion. In all cases, Allieu's technique was used, but in seven cases reinnervation was supplemented by upper intercostal nerves when there was total avulsion and/or by the medial pectoral nerve when there was partial avulsion. The methods are discussed and com

Nerve transposition for the restoration of elbow flexion following brachial plexus avulsion injuries.

Despite technical advances, the ability to restore motor function following a brachial plexus avulsion is limited. Twenty patients who suffered the loss of elbow flexion following a brachial plexus avulsion injury underwent a neurotization procedure in an attempt to restore that lost function. Of 16 patients who underwent intercostal to musculocutaneous nerve anastomosis, seven obtained good elbow flexion. Four patients who no longer had a viable biceps brachialis muscle underwent an anastomosis between transposed intercostal nerves and a free vascularized gracilis muscle grafted to the positi

Percutaneous phenol block of the musculocutaneous nerve to control elbow flexor spasticity.

Twenty-three extremities in 17 brain-injured adults were prospectively studied to evaluate the effectiveness of percutaneous phenol blocks of the musculocutaneous nerve in controlling spasticity of the biceps and brachialis muscles. Twenty-one (93%) of the extremities improved after the initial injection. The mean resting position decreased from 120 degrees of flexion to 69 degrees. Elbow range of motion increased an average of 53 degrees. There were no complications. Two patients did not respond to the initial injection and required repeat nerve blocks. Concomitant phenol motor point block of

Phrenic nerve transfer for treatment of root avulsion of the brachial plexus.

Phrenic nerve transfer was performed in 164 patients with root avulsion of the brachial plexus. The methods of operation consisted of phrenic nerve transfer to and anastomosis with the musculocutaneous nerve, the phrenic nerve bridging to the musculocutaneous nerve, phrenic nerve anastomosis with or bridging to the median nerve, and phrenic nerve anastomosis with other nerves. Follow-up of 65 patients for more than 2 years showed an effective rate of 84.6%. The result indicated that this operation has no deleterious effects on respiration, and the surgical effects are related to severity of in

Peripheral nerve injuries of the shoulder in the athlete.

Athletic injuries about the shoulder resulting in lesions of the suprascapular, axillary, long thoracic, musculocutaneous, and spinal accessory nerves are uncommon. Yet these lesions are well described in the literature, and they may occur as isolated injuries. Rarely do they occur following surgical procedures about the shoulder of the athlete. Furthermore, these injuries may mimic other more commonly occurring athletic injuries thus giving rise to diagnostic confusion. The clinical syndromes that result from these isolated lesions have been discussed, along with their mechanisms of injury. I

Transference of thoracodorsal nerve to musculocutaneous or axillary nerve in old traumatic injury.

On the basis of anatomic studies, we have devised a new method of transferring the thoracodorsal nerve to reinnervate the paralytic deltoid or biceps muscles in an old traumatic injury of the associated nerve. It has been applied to a case of musculocutaneous nerve injury and two cases of axillary nerve injury with good results. The muscle functions have returned to normal within 1 and 2 years, respectively, as shown by electromyography and the muscular contractility test.

Phrenic nerve transfer for brachial plexus motor neurotization.

We report a series of 164 patients who underwent phrenic neurotization to elements of the brachial plexus with root avulsion injuries. Recipient nerves included musculocutaneous nerve in 125 patients (78 direct neurotizations and 48 with intervening autograft), median nerve in 10 patients, and a variety of other nerves in 28 patients. Sixty-five patients presented a follow-up period of 2 or more years. Of this group, 55 patients (84.6%) achieved a recovery of M-3 or better. We observed no long-term deleterious effects on respiratory function.

A lesion of the musculocutaneous nerve. A rare complication of anterior shoulder dislocation.

A 47-year-old male patient with an anterior shoulder dislocation underwent reduction 3 days later. A residual neurologic injury remained, consisting primarily in weakness of shoulder and elbow flexion and sensory involvement of the ventral side of the forearm. Secondary sequelae included muscle atrophy with a concomitant frozen shoulder. The clinical examination and electromyography revealed a lesion of the musculocutaneous nerve, which is a very rare complication in shoulder dislocation.

[The use of axial-pattern cutaneous and musculocutaneous flap transposition in the reconstruction of deep burns].

From June 1979 to July 1987, 84 acute burned patients with localized deep wounds involving tendon, nerve, vessel, bone and joint were treated by utilising 26 types of island axial-pattern cutaneous or musculocutaneous flaps. All flaps survived completely with a primary healing rate of 91.4%. All patients were divided into 'urgent' (within 48 hours), 'early' (within 2 weeks), or 'infected' groups according to the time interval between the burn injury and the surgery conducted. The primary healing rates for each groups were 100%, 94%, and 86% respectively. No significant differences exist betwee

Microsurgical treatment of brachial plexus irreversible avulsion.

A new combined measure of nerve transfer, including antebrachial medial cutaneous nerve, and free musculocutaneous flap transfer was applied to the treatment of irreversible avulsion of the brachial plexus. 12 patients underwent this operation for restoring elbow and finger flexion. Good result was obtained in 71% for elbow and 60% for finger flexion restoration. The definition of irreversible avulsion and the methods, key points and value of the operation are described and discussed in detail.

[Relaxation and repair of axillary scar contracture and musculocutaneous nerve injury].

Relaxation of the scar contracture in the axillary region may lead to injure the musculocutaneous nerve, it hasn't been reported up to now. Through clinical observation and cadaveric measurement the degree of injury can be classified as Sunderland I...II. In general it can return to normal within 1-3 months. This article tries to explain the cause of injury and its clinical symptom and the method of the prevention.

Regional donor nerves in the reinnervation of brachial plexus palsy due to upper spinal roots avulsion.

The prognosis for surgical treatment in cases of upper brachial plexus palsy due to spinal roots avulsion is somewhat better than in cases of total palsy. The main reasons are better possibilities for surgical reinnervation using regional donors i.e. the medial pectoral and the thoracodorsal nerves, and a shorter time span for nerve regeneration. Regional donor nerves, alone or in combination with upper intercostals and/or the spinal accessory nerve, were used in 13 cases for the reinnervation of the musculocutaneous and/or the axillary nerves. The value of the regional donors is analysed and

[Experimental and clinical substantiation of laser therapy of wounds and trophic ulcers].

The authors have studied the results of 120 experiments on 60 guinea-pigs and 110 rabbits who received the treatment of modelled fenestrated musculocutaneous wounds and trophic ulcers respectively on the replanted extremity and on the back with helium and neon laser until they healed completely. The authors have demonstrated that the laser therapy accelerates the process of reparative regeneration. In 50 intact rabbits who received local irradiation with helium and neon laser there was an increase in the activity of lysosome enzymes and in the protein metabolism values in the blood serum. The

Direct nerve crossing with the intercostal nerve to treat avulsion injuries of the brachial plexus.

One hundred seventy-nine patients with root avulsion brachial plexus injuries were treated with direct nerve crossing with the intercostal nerve and 159 cases were followed more than 1 1/2 years after the operation. When suture was done to the musculocutaneous nerve, 90% of 10 children who had operation within 7 months of injury and 81.8% of 110 adults, younger than 40 years with operation within 6 months of injury regained grade 3 or 4 elbow flexion power. This direct method seems to produce better results than those of nerve crossing, which uses intermediary nerve grafts.

Medial brachial fascial compartment syndrome: anatomic basis of neuropathy after transaxillary arteriography.

The authors reviewed their experience with 320 transaxillary arteriograms, as well as the English-language literature on neuropathy complicating transaxillary arteriography. Three of their patients had median and ulnar motor and sensory nerve injury, and six others had only sensory involvement. The occurrence or severity of nerve injury did not correlate well with the size or presence of an observable axillary or arm hematoma. Dissection of the axillae and arms of 25 human cadavers revealed a tough medial brachial fascial compartment (MBFC) outside a thin axillary sheath. The median and ulnar

[Cross replantation in a case of bilateral amputation of the legs].

A case is reported of a patient aged 32 years who suffered bilateral traumatic amputation of the legs by a wagon. Neither of his two feet could be re-implanted to its original site. In spite of the severity of the lesions, reconstructive surgery was attempted by crossed re-implantation of the right foot onto the stump of the left leg. After bony shortening by 20 cm and internal fixation of the tibia, revascularisation was achieved by suture of the posterior tibial vascular pedicle. Primary repair of the posterior tibial nerve allowed protective sensibility to recover in the sole of the foot. S

Free functional musculocutaneous transfer: electrophysiological studies.

Transplanted autogenous muscles were electrophysiologically studied in 2 patients with forearm crush injury. Only fibrillation action potentials and positive sharp waves were seen at 1 month after the operation. Low-amplitude and short-duration muscular unit potentials (MUPs) appeared at 3 months and high-amplitude, long-duration and polyphasic MUPs were frequently seen at 6-7 months after the transplantation. The distal latencies of the evoked motor responses were gradually shortened from time to time and eventually became normal at about 1 year after the grafting. The mean consecutive differ
