Lateral antebrachial cutaneous nerve injury as a complication of phlebotomy.

The lateral antebrachial cutaneous nerve may be injured by attempts at cephalic vein venipuncture because of its anatomic location under the cephalic vein. Multiple attempts at venipuncture using plunging-type action should be avoided. Electric dysesthesias during venipuncture should alert the phlebotomist to possible nerve damage. Primary repair of the injured nerve or its fascicles may be hindered by tension across the antecubital fossa when the elbow is in extension or by the presence of the biceps tendon. As an alternative surgical solution, the neuroma may be resected and the proximal end

Forearm pain secondary to compression syndrome of the lateral cutaneous nerve of the forearm.

This report describes a syndrome of compression of the lateral cutaneous nerve of the forearm (LCNF), the distal termination of the musculocutaneous nerve. Three patients presented with pain or numbness along the radial aspect of the distal forearm. There was a history of vigorous upper extremity exercise with elbow extension and arm pronation or resisted elbow flexion. Signs included: decreased sensation to pin and light touch in the distal forearm over the cutaneous distribution of the nerve; tenderness to palpation over the nerve where it pierces the deep fascia of the arm lateral to the bi

Complications of breast reconstruction.

The authors discuss general complications of breast reconstruction, complications associated with reconstruction with subcutaneous or submuscular prostheses, complications of latissimus dorsi musculocutaneous flaps, and complications of transverse abdominal island flaps. Although no surgeon can expect to perform a series of complex yet delicate operations such as breast reconstructions and escape the complications described, it is hoped that by utilizing sound judgment in patient and procedure selection and having detailed knowledge of the procedure to be performed, such complications can be k

Shoulder defect correction with the island latissimus dorsi flap.

Reconstruction of normal shoulder contour is possible utilizing a latissimus dorsi musculocutaneous flap at the end of a long neurovascular pedicle. The thoracodorsal vessels and their lateral divisions form the basis of the pedicle. The nerve in the pedicle is left intact if maintenance of muscle bulk is desired and sectioned if atrophy is preferred. The amount of muscle taken in conjunction with the skin island is determined by the nature of the defect to be corrected. The twin goals of a single-stage reconstruction and a satisfactory aesthetic result are achieved with this method.

Isolated proximal musculocutaneous nerve palsy: case report.

Isolated musculocutaneous nerve injury is extremely rare. The few reported cases were associated with heavy exercises and violent extension of the forearm. The lesions in those cases were below the coracobrachialis muscle, involving the biceps, brachialis, and cutaneous nerves of the forearm. This patient's lesion was unique, located proximal to the coracobrachialis. There was no antecedent trauma. At follow-up six weeks and four months after onset of symptoms, significant improvement was noted both clinically and electrophysiologically. Musculocutaneous nerve palsy is important to distinguish

Nerve lesions associated with shoulder dislocation; an electrodiagnostic study of 11 cases.

Electrodiagnostic examination of 11 patients with shoulder dislocation revealed nerve damage not previously reported. Although axillary nerve lesions were most common, posterior cord and musculocutaneous nerve damage occurred each in five cases. The mechanism of injury was important. The most surprising patterns were associated with blunt injury or recurrent spontaneous dislocation.

Spastic hemiplegia in a quadriplegic patient: treatment with phenol nerve block.

A quadriplegic patient with a C6 lesion had a stroke with aphasia and right hemiplegia 20 years after his cervical cord injury. The combination of hemiplegia and quadriplegia created unusual rehabilitation problems, the most disabling of which was a painful flexion contracture of the right elbow that prevented any useful right arm function. A sequence of phenol nerve blocks produced almost complete resolution of this contracture and was a key factor in his successful return to independent living. Although phenol block is much less commonly performed in the upper than lower extremities, its jud

Electrodiagnostic localization of traumatic upper trunk brachial plexopathy.

Eighteen patients having traumatic upper trunk brachial plexopathy ("the stinger"), a common football injury, were investigated electrodiagnostically. Proximal nerve conduction was determined by stimulating the supraclavicular fossa and at the C5 root and recording from muscles supplied by the long thoracic, suprascapular, musculocutaneous, axillary, lateral pectoral and thoracodorsal nerves. The accessory nerve was stimulated in the lateral posterior triangle and the evoked potential recorded from the upper trapezius muscle. Median and ulnar nerves were also tested, sensory and motor fibers b

[Lesions of the circumflex nerve. Apropos of 19 cases].

The authors studied a series of 19 lesions of the circumflex nerve either isolated (14 cases) or in association with a lesion of the suprascapular or musculocutaneous nerves. All were treated surgically. One was caused by a gunshot wound, the others were due to traction on the axillary region, and associated with anterior dislocation of the shoulder in five cases, a fracture of the humerus in two cases and a fracture of the clavicle in one case. Despite complete paralysis of the deltoid, active abduction was often possible. The operation took place 4 to 6 months after injury. In the 14 cases o

Intercostal nerve transfer in brachial plexus injuries: an experimental study.

Eighteen adult mongrel dogs underwent unilateral surgical disruption of the brachial plexus. Twelve animals (Group I) had as a second-stage procedure transfer of T4 and T5 intercostal nerves and their accompanying vascular bundles to the distal musculocutaneous nerve stump. Six animals (Group II) had restoration of musculocutaneous nerve continuity with conventional interpositional sural nerve cable grafts. Group I animals demonstrated significantly better electromyographic evidence of reinnervation and, comparing appearance and weight of operated and unoperated biceps muscles at the time of a

Somatosensory evoked potentials from musculocutaneous nerve in the diagnosis of brachial plexus injuries.

A technique for recording somatosensory evoked potentials using stimulation of musculocutaneous sensory nerve fibers proximal to the wrist has been used in 10 normal subjects and in 8 patients with traumatic lesions of proximal parts of the brachial plexus. The technique gave satisfactory evoked potentials in all normal subjects and provided useful information in patients with avulsion of the 5th and 6th cervical roots. The results were similar to those obtained by radial nerve stimulation. The results in 3 patients with upper trunk injuries and in 1 patient with avulsion of the 5th cervical r

The investigation of traumatic lesions of the brachial plexus by electromyography and short latency somatosensory potentials evoked by stimulation of multiple peripheral nerves.

A study of 10 patients with brachial plexus trauma was performed to determine whether the diagnostic accuracy of sensory evoked potentials (SEPs) may be improved by using stimulation of multiple peripheral nerves (median, radial, musculocutaneous and ulnar). In addition, the relative advantages of SEPs and peripheral electrophysiological studies were considered. SEP patterns following most common brachial plexus lesions were predictable. Injuries to the upper trunk affected the musculocutaneous and radial SEPs predominantly. Lower trunk or medial cord lesions primarily affected ulnar SEPs. Dif

Studies on conversion of motor function in intercostal nerves crossing for complete brachial plexus injuries of root avulsion type.

The progress of the functional conversion in cases where the 3rd and 4th intercostal nerves were crossed to the musculocutaneous nerve to regain elbow flexion after a total avulsion type of brachial plexus injury was followed up by means of the electromyogram, goniogram and spirogram in 25 patients. The joint position sense was further studied. It was found that in the early stage of reinnervation, spontaneous activity which synchronized with respiration was found in the biceps brachii muscles, but this involuntary element disappeared gradually and followed this the volitional control and endu

The role of cutaneous afferents in position sense, kinaesthesia, and motor function of the hand.

In reconstructive surgery and rehabilitation work, for decades it has been a basic problem for the author to discover which of the peripheral receptor systems provide the primary information on position, motion and degree of load (proprioception). At one time, joint receptors were accepted as the dominant source and more recently musculocutaneous receptors, whereas little attention has been given to cutaneous factors. A number of clinical observations, however, have indicated the importance of cutaneous afferents and this prompted the present study. There are no means of isolating the joint re

Bristow-Latarjet procedure for recurrent anterior dislocation of the shoulder. A 2-5 year follow-up study on the results of 112 cases.

A follow-up study of 111 out of 112 patients operated on for shoulder joint dislocation according to the Bristow-Latarjet procedure during the years 1975 through 1979 in four Swedish hospitals is presented. The average follow-up time was 30 months (range 24-60 months). There were seven cases of significant recurrences (6 per cent). During follow-up, further surgery had been performed on four of these. Another eight patients (7 per cent) had experienced occasional insignificant subluxations. In one case neurolysis of the musculocutaneous nerve was undertaken because of postoperative paresis of

Reconstructive procedures for extremities with peripheral nerve defects.

Surgeons who undertake reconstruction of extremities with peripheral nerve loss should be experienced enough to select the appropriate procedures for the individual patient. To gain adequate strength for a specific function, a new muscle-tendon unit usually is added to the power-train rather than redistribute muscles that have related function. Precise sensibility required precise motion, which is very difficult to achieve following peripheral nerve loss. The anticipated result should be a balanced simplification of motor and sensory function.

Current uses of open phenol nerve block for adult acquired spasticity.

Forty-nine open phenol nerve injections were performed for acquired spasticity during a five-year period. The most common etiology of acquired spasticity was closed head injury (70%). Thirteen phenol injections to the musculocutaneous nerve were performed for excessive elbow flexion. The immediate results were absence of biceps and brachialis muscle activity, and an average gain of 43 degrees in extension. The effects of the nerve block subsided within six months. Nine nerve blocks were followed for more than one year. In two patients the upper extremities were almost normal. In the remaining

[Results of great nerve trunk grafts].

The authors describe the results of 60 cases of trunk repairs, carried out by the same surgeon, and with a follow-up period of 18 months or more. The grafts were autologous and in most cases the donor was the saphenous nerve. The grafts were placed under microscope. The recipient sites were the median nerve (19 cases), the ulnar nerve (16 cases), the radial nerve (10 cases), the sciatic nerve (8 cases), the musculo-cutaneous nerve (6 cases) and the spinal nerves (1 case). After a description of the technique used, the results are analyzed in accordance with the classical criteria of Highet and

[Direct treatment of nerve lesions in brachial plexus injuries in adults--a series of 100 operated cases].

The authors report a series of 100 brachial plexus injuries between April 1974 and January 1979. The frequency of these lesions, knowledge of the anatomy and pathology, and trends in prognosis are correlated to give the surgical indications for nerve graft repair. Correlation between preoperative myelography and the anatomy and type of pathology demonstrated at the time of operation, have been of special interest and have consolidated tha operative indications. Out of the 100 cases operated on between April 1974 and January 1979, neurolysis without plexus repair was performed in 25, nerve graf

[Nerve grafting and other plastic procedures in traumatic injuries of the brachial plexus (author's transl)].

In 52 patients with traumatic injuries of the brachial plexus plastic procedures were carried out on the nerves in order to improve function: primary suture of the torn portion of the plexus (one case), external and internal neurolysis with division of the epineurium (four times), anastomosis of the secondary branches or the main arm nerves, with the intercostal nerves by insertion of sural transplants (47 times). Seventy per cent showed an improvement in protective sensation and in 48% a useful gain in motor function was achieved. The results are dependent on the age of the patient (better re
