Avoiding Iatrogenic Axillary Artery Injury in Reverse Total Shoulder for Fracture: The Use of a Separate Axillary Incision: A Case Report.

CASE: A 71-year-old woman sustained a left proximal humerus fracture dislocation. Imaging demonstrated displacement of the humeral head medial to the axillary artery without initial injury. To avoid iatrogenic injury, a separate medial axillary incision was successfully used to remove the humeral head and then reverse total shoulder arthroplasty was performed through a deltopectoral approach. At 1 year, the patient had excellent range of motion and PROMIS Physical Function CAT (v2.0) score was 46.2. CONCLUSIONS: A separate medial axillary approach can be used to safely remove anterior/inferio

Temporary Cement Prosthesis as a Bridge After Control of Axillary Artery Hemorrhage in Proximal Humeral Fracture-Dislocation: A Case Report.

CASE: A 64-year-old woman underwent open reduction and internal fixation for four-part proximal humeral fracture-dislocation with palpable distal pulses. Upon dislocated head removal, life-threatening hemorrhage occurred from posterior circumflex humeral artery tear. Bleeding was controlled through packing, resuscitation, and open vascular repair. Owing to unavailable shoulder prosthesis and prolonged operative time, a temporary cement prosthesis with Steinmann pins was used. At the 3-year follow-up, partial function was restored with no loosening. CONCLUSION: This case highlights axillary br

Transient Radial-Dominant Neuropathy Following the Use of Sterile Exsanguination Tourniquet During the Posterior Plating of a Humeral Shaft Fracture: A Case Report.

Tourniquet-related nerve injury is an uncommon but clinically important complication of extremity surgery. Postoperative radial nerve palsy after humeral shaft fixation is usually attributed to fracture-related injury or surgical manipulation, particularly when a posterior approach is used. We present a case of transient radial-dominant neuropathy following the use of a sterile exsanguination tourniquet during the posterior plating of a distal humeral shaft fracture. A 27-year-old male construction worker with an AO/OTA (Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Associatio

Post-Traumatic Subcutaneous Emphysema of the Upper Extremity Associated with an Open Scapular Fracture: A Case Report and Review of Literature.

INTRODUCTION: Subcutaneous emphysema (SE) of the upper extremity is a rare condition, typically associated with penetrating trauma, infections, or chest injuries. Its occurrence following blunt trauma with an isolated scapular fracture is exceedingly uncommon and may mimic more serious conditions such as necrotizing fasciitis. We present a rare case of upper-limb SE associated with an open scapular fracture and propose a diagnostic approach to distinguish benign SE from limb-threatening infections. CASE REPORT: A 65-year-old male presented with pain and swelling in the right upper limb follow

Individualized Deltoid Landmark and Needle Length for Safe Intramuscular Vaccination in Southeast Asian Adults: An Ultrasound Study.

Background/Objectives: An incorrect intradeltoid injection technique can cause shoulder injury related to vaccine administration, including bursitis, septic arthritis, and axillary nerve injury, particularly when Western-derived landmarks and needle-length tables are applied to smaller-framed Southeast Asian adults. We aimed to define an individualized deltoid injection landmark and needle length that avoid the axillary nerve while ensuring reliable intramuscular delivery in Southeast Asian adults. Methods: In this cross-sectional ultrasound study of adults aged ≥18 years, four vertical landma

Predictors of Primary Arteriovenous Fistula Failure in Patients With End-Stage Renal Disease: A Single-Centre Retrospective Cohort.

BACKGROUND: Arteriovenous fistula (AVF) is the preferred vascular access for haemodialysis, yet primary failure remains common and its determinants in Southeast Asian populations are incompletely characterised. METHODS: We retrospectively analysed 151 AVFs created in 125 adults with end-stage renal disease (ESRD) at a tertiary hospital (2020-2022). The primary outcome was AVF failure, defined as an access that never became usable for haemodialysis (primary non-function, early occlusion, or non-maturation). RESULTS: Primary failure occurred in 34/151 fistulas (22.5%). Failure rates differed b

Osteopathic Manipulative Treatment as an Adjunctive Therapy for Post-mastectomy Pain Syndrome: A Case Report.

Post-mastectomy pain syndrome (PMPS) is a common yet often under-treated complication of breast cancer therapy, affecting an estimated 20-60% of patients. Characterized by persistent pain lasting greater than three months following breast surgery, PMPS is multifactorial in etiology, involving neuropathic injury, post-surgical fibrosis, radiation-induced tissue changes, and musculoskeletal dysfunction. Current management strategies emphasize a multidisciplinary approach, including pharmacological therapies and non-pharmacological modalities. Despite this, treatment outcomes remain variable, and

Acute right brachial artery embolism after left upper division segmentectomy with suspected pulmonary vein stump thrombus.

Systemic arterial embolism is an uncommon but potentially serious complication after lung resection. A 63-year-old woman underwent thoracoscopic left upper division segmentectomy for lung adenocarcinoma. On postoperative day (POD) 2, she developed sudden pain, numbness, and weakness of the right upper limb. Bedside ultrasonography showed right brachial artery occlusion, and contrast-enhanced computed tomography (CT) confirmed a distal brachial artery filling defect. A small filling defect was also observed at the pulmonary vein stump, but the embolic source could not be established. Emergency

Total Hip Arthroplasty for Advanced Femoral Head Osteonecrosis in a 24-Year-Old Competitive Wrestler: A Case Report.

INTRODUCTION: In chronic pectoralis major (PM) tendon tears, muscle retraction and involution of the residual tendon usually preclude the use of direct repair, and augmentation methods are commonly required. Investigators have tried to find ways to maximize biomechanical fixation strength, with studies focusing mainly on methods of bony fixation and showing few differences among fixation techniques. Achilles allograft reconstruction of chronic pectoralis tendon ruptures is a viable treatment option. CASE REPORT: A 38-year-old male weightlifter presented with a 2-year-old left-sided PM tendon

Great saphenous vein arterialisation as a novel technique of vascular conduit preparation for an elective repair of a brachial artery aneurysm.

This paper describes a technique used to treat an end-stage kidney disease patient, a kidney transplant recipient, who was diagnosed with an advanced aneurysmal degeneration of the brachial artery. The current report presents a novel, staged technique of surgical vascular repair with a previously arterialized autologous great saphenous vein (GSV), along with a 12-month follow-up observation period. The patient was treated with an arterialised great saphenous vein bypass that was not primarily suitable for use as a conduit due to its small diameter. To achieve a satisfactory vessel size, an art

Mid-term outcomes of trans-axillary versus thoracotomy approaches in alternative-access tavr: a retrospective multicenter study.

Transcatheter aortic valve replacement (TAVR) has several alternative access routes when transfemoral access is unsuitable. This study compared perioperative and mid-term outcomes of transaxillary (TAx) TAVR with those of transapical (TA) and direct aortic (DA) approaches. Among 2,185 patients who underwent TAVR at our three centers between April 2015 and April 2024, 198 non-transfemoral cases were retrospectively analyzed. TAx-TAVR was classified as a non-thoracotomy approach, whereas TA/DA-TAVR were classified as thoracotomy approaches. Perioperative and mid-term outcomes were compared betwe

Robot-assisted Axillary Lymph Node Dissection for Axillary Lymph Node Metastasis in Breast Cancer.

Current experience with robotic-assisted axillary lymph node dissection (R-ALND) is limited. Axillary lymph node dissection is essential for patients with pathological N2-3 disease, but conventional open surgery carries a risk of injury to the intercostobrachial nerve, leading to sensory deficits, pain, and reduced quality of life. R-ALND may enhance precision through minimally invasive techniques, potentially reducing complications. Patients with breast cancer who underwent R-ALND at our institution between March 2024 and March 2025 were enrolled and analyzed. The procedure used the da Vinci

Comprehensive overview of management and risk assessment of breast cancer-related lymphedema: a multidisciplinary approach.

Breast cancer is one of the most commonly diagnosed cancers worldwide, with a particularly high incidence in women. Although many are cured of their malignancy, treatment options can present with significant long-term complications for the patient. Lymphedema is the accumulation of protein-rich fluid that occurs following a disturbance to the lymphatic system. Secondary lymphedema to the arm often results following breast surgery or axillary lymph node dissection, although risk and exacerbating factors include radiation and medical chemotherapy, both in the short- and long-term intervals. Preoperative corrected carotid flow time is associated with spinal anesthesia-induced hypotension in hypertensive patients: A prospective observational study.

BACKGROUND: Spinal anesthesia-induced hypotension (SAIH) is common in hypertensive patients, yet reliable preoperative markers remain limited. Corrected carotid flow time (CCFT) is a Doppler-derived index influenced by preload and ventricular ejection timing, but its association with SAIH in this population has not been established. OBJECTIVE: To evaluate whether preoperative CCFT is independently associated with SAIH in hypertensive patients undergoing spinal anesthesia. DESIGN: Single-center prospective observational study. PATIENTS: In total, 110 hypertensive adults scheduled for lower a
