Lung-Protective Effect of Alveolar Recruitment Maneuvers in Children with Right Vertical Infra-Axillary Thoracotomy for Repair of Congenital Heart Disease.

BACKGROUND/OBJECTIVES: Right vertical infra-axillary thoracotomy to repair ventricular septal defects (VSDs) and atrial septal defects (ASDs) is less invasive than conventional surgical repair via median sternotomy. However, right vertical infra-axillary thoracotomy (RVIAT) may result in unilateral lung injury, a serious postoperative complication requiring extracorporeal circulation and unilateral lung collapse. The aim of this study was to evaluate whether repeated lung recruitment provides enhanced respiratory compliance and lung oxygenation in children who have undergone right vertical inf

Salvage of a thrombosed radio-cephalic fistula by anastomosis to brachial comitans vein: A case report.

Late thrombosis of radiocephalic arteriovenous fistulas (RCAVFs) is most commonly associated with inadequate venous outflow, and when superficial veins are unavailable or unsuitable, surgical salvage options remain limited. We report a case of complete RCAVF thrombosis caused by perforating vein occlusion in the setting of absent effective central venous drainage due to a sclerotic, non-patent cephalic vein of the arm. Surgical salvage was achieved by thrombectomy of the efferent vein followed by creation of a latero-lateral anastomosis with a brachial comitant vein. The fistula was immediatel

Blood flow rate of brachial artery anastomosis and outflow vein as predictor for brachiocephalic arteriovenous fistula maturation in end-stage renal disease: A prospective cohort study.

OBJECTIVE: To evaluate whether blood flow rate (BFR) changes in the proximal and distal brachial artery and outflow vein (OV) can predict brachiocephalic arteriovenous fistula (AVF) maturation. METHODS: A multicenter prospective cohort study involved end-stage renal disease (ESRD) patients undergoing brachiocephalic AVF surgery. BFR was measured using ultrasound preoperatively, immediately post-operative, and postoperatively on days 7 and 14. Maturation was assessed at week 6 using Rule of 6 criteria. RESULTS: Of 45 patients, 36 achieved maturation. A ≥167.23% increase in proximal brachial a

Pseudoaneurysm and arteriovenous fistula of the brachial artery presenting 25 years after elbow fracture: a case report.

INTRODUCTION: Post-traumatic pseudoaneurysms (PSAs) are uncommon vascular complications resulting from partial disruption of the arterial wall, leading to a contained hematoma that maintains communication with the arterial lumen. Although most PSAs develop shortly after trauma or vascular intervention, markedly delayed presentations are exceedingly rare and may pose significant diagnostic and therapeutic challenges. When associated with arteriovenous fistula (AVF) formation, these lesions carry an increased risk of progressive enlargement and sudden hemorrhage. CASE PRESENTATION: We report a

Limb salvage with brachial-to-radial bypass using a saphenous vein graft following severe crush injury: A case report.

BACKGROUND: Severe crush injuries to the upper limb can result in complex arterial damage and acute limb ischaemia, which often precludes primary repair. In such challenging settings, distal-target bypass using an autologous conduit may be required to achieve timely revascularisation and limb salvage. CASE PRESENTATION: A 21-year-old male patient presented with acute ischaemia of the upper limb following a work-related crush injury. Computed tomography angiography revealed a long-segment occlusion of the distal brachial artery and absent distal flow. Emergency surgical exploration revealed ex

A novel approach to the treatment of functional popliteal artery entrapment syndrome in college athletes.

BACKGROUND: Functional popliteal artery entrapment syndrome (FPAES) is a rare disorder that results from hypertrophy and overuse injury of calf muscles, which thereby compress neurovascular structures in the popliteal fossa. The majority of FPAES cases are found in young athletes who present with symptoms during exercise. Most commonly, FPAES is surgically treated with muscle debulking utilizing a posterior S-shaped incision, but wound healing and neuropathy remain problematic in this approach. Here, we present our case series of college athletes undergoing FPAES repair using a novel medial ca

Cross-sectional study of mechanism of arterial pulsation amplitude modulation under ultrasound probe compression: Similarities and differences compared with the oscillometric blood pressure measurement principle.

Arterial pulsation is used to differentiate arteries from veins on ultrasound imaging. However, the mechanism by which the arterial pulsation amplitude changes when an ultrasound probe compresses the artery has not been reported. Here, we aimed to investigate whether this mechanism resembles the oscillometric maximum amplitude algorithm. An automatic blood pressure cuff was placed on the upper left arm of 7 healthy participants (four men and 3 women; aged 27-46 years). An ultrasound probe attached to a force sensor was gradually applied to the distal brachial artery using an automated test sta

Peripheral endothelial and coronary microvascular responses to lipophilic versus hydrophilic ACE inhibitors in newly diagnosed hypertension.

BACKGROUND: The clinical relevance of tissue-selectivity among angiotensin-converting enzyme (ACE) inhibitors is debated. This study aimed to compare the effects of a lipophilic (quinapril) and a hydrophilic (lisinopril) ACE inhibitor on peripheral endothelial function and coronary microvascular function in patients with de novo hypertension. METHODS: In a controlled crossover trial, 33 hypertensive patients received eight weeks of quinapril (10 mg/day) and lisinopril (10 mg/day), separated by a washout period. Brachial artery flow-mediated dilation (FMD) and coronary flow reserve (CFR) were

Development and validation of a risk prediction model for rotator cuff tears following acute anterior shoulder dislocation.

PURPOSE: Following shoulder dislocation, injuries such as rotator cuff tear (RCT), axillary nerve injury, greater tuberosity (GT) fracture, Hill-Sachs lesion, and Bankart lesion frequently occur. This study aimed to develop and validate a clinical prediction model for the occurrence of RCT after acute anterior shoulder dislocation. METHODS: This study included all patients with acute anterior shoulder dislocation who presented to the emergency orthopedics department of the same institution from July 2021 to March 2025. Lasso regression was used to select independent variables for the model. A

Cephalic Venous Aneurysm and Intravascular Papillary Endothelial Hyperplasia in a Blood Donor.

Venous aneurysms (VAs) and pseudoaneurysms (VPAs) are uncommon vascular lesions with variable etiology and location. We report a 52-year-old male blood donor who developed thrombophlebitis presenting as a right antecubital mass after multiple venipunctures. Initial duplex ultrasound revealed a saccular dilation of the right cephalic vein (15.6 × 7.8 × 25.5 mm) with thrombus, as well as thrombosis of the right brachial vein. The patient was treated with anticoagulation, followed by definitive surgical management. Histopathological examination revealed a VA, as well as intravascular papillary en

Reverse End-to-Side Triceps to Axillary Nerve Transfer for Treatment of Axillary Nerve Palsy After Reverse Total Shoulder Arthroplasty.

Nerve injuries are uncommon, yet significant complications following reverse total shoulder arthroplasty (rTSA), with the axillary nerve being most frequently injured. We report a case of a 67-year-old female who developed persistent motor dysfunction and decreased sensation in the axillary distribution following rTSA for a comminuted proximal humerus fracture. Electrodiagnostic studies confirmed right axillary nerve injury with reduced motor unit recruitment. After no improvement in deltoid function at three months following rTSA, we discussed treatment options for the patient, who opted for

Latissimus Dorsi Transfer With Interposed Autograft for Axillary Nerve Palsy After Reverse Shoulder Arthroplasty: A Case Report.

Persistent axillary nerve palsy after reverse shoulder arthroplasty (RSA) results in profound deltoid dysfunction and loss of active shoulder elevation. Traditional reconstructive options such as nerve repair or transfer may not be feasible in chronic cases. Latissimus dorsi tendon transfer augmented with an interposed graft has been described to restore shoulder abduction in deltoid-deficient shoulders, but its application after RSA is not well documented. We present the case of a 73-year-old female patient who developed chronic axillary nerve palsy following RSA performed for complex proxima

A Perplexing Plexopathy After Pembrolizumab Therapy in Early-Stage Triple-Negative Breast Cancer.

Background: In triple-negative breast cancer (TNBC), the addition of immunotherapy has significantly improved outcomes. Immune-related adverse events (irAEs) can be accelerated in patients with pre-existing autoimmune (AI) conditions. The treatment-response standardized protocol used in clinical care raises concerns about the need for right-sizing strategies. As the use of immunotherapy expands, recognizing toxicity from recurrence and optimizing response-adapted approaches are essential to balance cure with quality of survival. Case Presentation: A 38-year-old pregnant woman with a distant hi

Classification Systems & Clinical Outcomes of Axillary Burn Contracture: A Systematic Review & Meta Analysis.

Axillary burn contracture is a common and debilitating complication of burn injuries, impairing mobility and quality of life. Multiple classification systems and reconstructive techniques exist, but optimal management remains unclear due to limited systematic data comparing outcomes across techniques and classifications. A systematic review was performed. PubMed, Embase, and Web of Science were searched for full-text studies reporting surgical treatment of axillary contractures. Data extraction and quality assessment were performed independently by two reviewers. Outcomes were synthesized qual

Resolution of Glenohumeral Pseudo-Subluxation: An Illustrative Case Report on Relative Clinical Success in Pan-Brachial Plexus Injury.

Brachial plexus injuries (BPIs) are burdensome to patients. These often involve the axillary nerve, which may lead to pseudo-subluxation of the humeral head because of deltoid atony. While advances in nerve reconstruction have improved outcomes, these procedures still have markedly high failure rates, which may lead to persistent pain, dysfunction and poor satisfaction. We present a case of a 51-year-old male with a traumatic BPI in the context of a motorcycle accident who experienced pseudo-subluxation of his humeral head and later underwent surgical reconstruction for his BPI. Despite a lack
