Exploring Variations in Median Nerve Formation: Embryological Basis and a Pathway to Improved Clinical Outcomes.

Aim This study investigated the gross anatomical variability in the formation of the median nerve and evaluated its clinical and surgical implications. The research aimed to observe variations in median nerve root formation, compare differences between sides and genders, and to understand the embryological basis of these variations. Materials and methods A cadaveric investigation was conducted on 60 upper limbs (ULs) from 30 embalmed adult cadavers (15 males and 15 females). An infraclavicular approach was used for dissection, following standard dissection protocols. The number of roots formin

Balloon-Assisted and Snare-Facilitated Retrieval of a Kinked Radial Catheter Entrapped in the Axillary Artery.

BACKGROUND: Catheter kinking during transradial coronary angiography is rare but may result in fixed entrapment, where forceful manipulation risks vascular injury. CASE SUMMARY: A 67-year-old woman with non-ST-segment elevation myocardial infarction underwent diagnostic coronary angiography via right radial access using a 6-F FL3.5 catheter. During manipulation, the catheter became fixed and kinked in the axillary artery, with loss of rotational control. Standard wire-based maneuvers failed. Dual arterial access was obtained with femoral snare stabilization of the distal catheter tip and intr

The Unappreciated Morbidity of Brachial Plexus Injuries Following Low Energy Shoulder Trauma: Understanding Risk Factors and Natural History.

BACKGROUND: Brachial plexus injury (BPI) following shoulder trauma is an uncommon but potentially disabling complication that is often underrecognized, leading to delayed diagnosis. This study aimed to define the institutional incidence, nerve distribution, and recovery patterns of BPI after shoulder trauma and to identify factors associated with injury severity and recovery. METHODS: We performed a retrospective review of adult patients diagnosed with BPI following isolated shoulder trauma at a single academic center from January 2016 to July 2023. Patients with BPI were compared with a rand

Blunt trauma precipitating arterial thoracic outlet syndrome: a case report.

A 39-year-old male who sustained polytraumatic injuries in a motor vehicle collision, including a neck seatbelt sign and acute thrombosis of the left subclavian artery adjacent to an undiagnosed hypoplastic cervical rib, presented with a threatened left upper extremity. He underwent emergent surgical exploration through a supraclavicular incision, with cervical rib resection, resection of the contused subclavian artery segment, and end-to-end anastomosis. Through an infraclavicular incision, he had a thrombectomy of the axillary and brachial arteries. Postoperatively, he developed progressive

Concomitant Variations of the Scapular Region Musculature With an Aberrant Neurovascular Pattern.

The shoulder complex exhibits a distinctive amalgamation of muscular power, integration, and an extensive range of motion. The shoulder joint allows for a wide range of upper limb movements because the four small muscles that make up the musculotendinous rotator cuff work together to keep the joint stable. The four muscles that come from the scapula and form the rotator cuff are the subscapularis, supraspinatus, infraspinatus, and teres minor. Accurate diagnosis and proper management of shoulder-related injuries are essential, particularly for sportspersons and individuals engaged in heavy wei

Early post-operative nerve complications after primary open Latarjet: a single-surgeon series of 164 shoulders.

BACKGROUND: Post-operative nerve complications remain a concern following the Latarjet procedure due to the proximity of the musculocutaneous, axillary, and suprascapular nerves during key surgical steps. Reported incidence rates of nerve injuries with this procedure vary considerably in the literature. The aim of this study was to evaluate the incidence of early post-operative nerve complications in consecutive patients undergoing a primary open Latarjet procedure treated with a standardized surgical technique. METHOD: A single-center, single-surgeon retrospective case series of patients und

Early Outcomes of Proximal Humerus Fractures Treated With Proximal Humerus Locking Plate Fixation Using the Deltoid-Split Approach.

Introduction The deltopectoral approach is the workhorse approach used for plating of proximal humerus fractures. However, the less invasive and less commonly used deltoid-split approach has shown comparable outcomes in selected cases. Literature on its functional outcomes and complications remains limited, especially in the Indian subcontinent. This study evaluates the results of patients with proximal humeral fractures managed by open reduction and internal fixation (ORIF) with plating using proximal humerus locking plates (PHLPs) performed through the deltoid-split approach. Reverse total shoulder replacement after traumatic acute ischemia of the upper limb due to proximal humerus fracture.

Proximal humerus fractures represent approximately 5% of all fractures, predominantly affecting elderly patients with osteoporotic bone. While many cases are minimally displaced and treated conservatively, complex fracture patterns-particularly those involving multiple fragments or head-splitting components-pose significant diagnostic and therapeutic challenges. One of the rare but serious complications of these injuries is vascular involvement, most notably affecting the axillary artery due to its anatomical proximity. Although uncommon, acute ischemia of the upper limb secondary to such trau

Endosteal Button Fixation for Latissimus Dorsi Tendon Rupture in a Bodybuilding Athlete.

BACKGROUND: Bodybuilders are uniquely at risk of shoulder injuries given training intensity, frequency, and heavy overhead loads through a full arc of motion. Latissimus tears are rare injuries that have increased incidence in overhead and weightlifting athletes, following forceful hyperabduction/adduction during sport-specific activities. Humeral avulsions of the latissimus tendon are treated with endosteal button fixation. INDICATIONS: Endosteal button repair is indicated for acute or chronic latissimus tears up to 2 years after injury, in patients who are unable to return to their desired

Improving lives: Health-related quality of life after surgery for traumatic brachial plexus lesions in a single center retrospective study.

INTRODUCTION: Traumatic injuries of the brachial plexus are life-altering, associated with complex injury patterns and commonly affect young people. Since the introduction of brachial plexus microsurgery, studies have largely focused on improving motor outcomes without patient-reported outcome measures. RESEARCH QUESTION: To assess the development of quality of life after surgical treatment for traumatic injuries of the brachial plexus in addition to neurological outcome. MATERIAL AND METHODS: This retrospective study investigated neurological outcome and health-related quality of life in 98

Modified partial radial to axillary nerve transfer as the index procedure for isolated axillary nerve injury.

BACKGROUND: Axillary nerve injuries often produce profound functional deficits. Beyond expectant management or microsurgical repair, treatment is generally limited to salvage procedures. We present the results of partial radial-to-axillary nerve transfer as the index procedure for treatment of axillary nerve injury after failure of initial conservative management. METHODS: Minimum 2-year follow-up data were collected and retrospectively analyzed for this single-surgeon cohort of patients with isolated axillary nerve injury. The injury was confirmed with electromyography, and all patients unde

Subdeltoid ultrasound-guided brachial plexus block: A technical description with anatomical correlation and a preliminary case series.

Regional anaesthesia in the emergency department requires reliable techniques with minimal complications. We describe a subdeltoid ultrasound-guided brachial plexus approach targeting a potential "nerve funnel" at the surgical neck of the humerus, bridging infraclavicular and axillary approaches. Cadaveric dissection explored its anatomical basis and correlated it with clinical sonographic findings. This retrospective case series included six adult patients with traumatic upper-limb injuries. Cadaveric observations suggested terminal nerve clustering and proximity of the musculocutaneous and i

Anatomical appropriateness of intramuscular injection at a new deltoid intramuscular injection site: comparison of nerve and arterial courses among three arm positions in healthy adult volunteers.

A new deltoid intramuscular injection site has been proposed as an appropriate site for the intramuscular injection into the deltoid muscle. However, its anatomical appropriateness in the hand on ipsilateral hip position has not been established. This study aimed to compare the three arm positions in healthy adult volunteers and estimate positional changes in the nerves and accompanying arteries. Measurements were obtained in the anatomical position, the hand on the ipsilateral hip position with the thumbs positioned anteriorly, and the hand on the ipsilateral hip position with the thumbs posi

Supercharging of the axillary nerve with a nerve innervating the triceps in a partial traumatic brachial plexus injury of a 5-year-old child in India: a case report.

A 5-year-old girl presented with an inability to abduct her left shoulder and flex her left elbow following a fall 1.5 months earlier. Electrodiagnostic studies were suggestive of a partial upper trunk brachial plexus injury. The child was advised to attend monthly follow-up visits with regular physiotherapy and nerve stimulation. At 3 months after injury, elbow flexion had recovered completely, and shoulder abduction had improved to 30°. After an additional month of observation without further clinical improvement in shoulder abduction, a nerve-to-triceps transfer to the axillary nerve was pl

Limb Salvage After Grenade-Related Brachial Artery Injury: A Case Report.

Since war injuries are not predictable, each patient presents as a new and unique case. Before deciding on amputation for upper extremity injuries, it is necessary to evaluate all limb-sparing vascular interventions that will keep the amputation level as distal as possible, in order to improve the patient's quality of life and enable the use of more functional prostheses. While amputation may be a faster and more life-saving option for a surgeon, limb-sparing and mobile joint-preserving surgical procedures should be the preferred choice, provided the patient's overall condition permits. This c

Imaging Spectrum of Thoracic Outlet Syndrome: A Case Series Highlighting Arterial, Venous, and Neurogenic Subtypes.

INTRODUCTION: Thoracic outlet syndrome (TOS) encompasses a spectrum of neurovascular compression disorders involving the brachial plexus, subclavian artery, and subclavian vein. Diagnosis remains challenging due to variable clinical presentations and limited specificity of imaging findings, particularly in neurogenic TOS. This case series aims to demonstrate the multimodality imaging spectrum of TOS across arterial, venous, and neurogenic subtypes, compare imaging appearances among subtypes, and emphasise the importance of clinicoradiological correlation in diagnosis. METHODS: This retrospect

Mechanical thrombectomy as a safe alternative to catheter directed thrombolysis for upper extremity DVT.

BackgroundUpper extremity deep vein thrombosis (UEDVT) has been widely managed with anticoagulation and selectively with catheter directed thrombolysis (CDT) targeting prompt venous recanalization. CDT, however, comes with a bleeding risk and complex perioperative logistics. Percutaneous mechanical thrombectomy (MT) devices may simplify the procedure. However their safety and efficacy remain unknown.MethodsThe records of 14 consecutive patients with symptomatic UEDVT who underwent MT at two tertiary European centers were reviewed (2020-2025). Subclavian vein thrombosis was the principal anatom
