Spontaneous Auto-Amputation of the Breast in a Patient With Advanced Malignant Fungating Wound and Distant Metastases Involving the Lungs and Left Upper Extremity: A Case Report.

Malignant fungating wounds in advanced breast cancer cause significant morbidity and psychological distress. Spontaneous auto-amputation of the breast is a rare clinical phenomenon, conventionally linked to neglected tumors rather than actively treated disease. We present a case of spontaneous auto-amputation in an extensively pretreated 60-year-old female with advanced human epidermal growth factor receptor 2-positive (HER2+) breast cancer. She presented to the palliative care unit with refractory left upper extremity pain, severe tactile and thermal allodynia due to malignant brachial plexop

Anatomical relationship between the terminal thoracic duct and brachial plexus: A cadaveric study.

OBJECTIVE: To study the anatomical relationship between the terminal portion of the thoracic duct (TD) and the brachial plexus (BP). METHODS: Anatomical study performed on 72 fresh cadavers, dissected only on the left side. Anatomical structures were preserved, and measurements were performed using an digital caliper. Statistical analysis was performed with a 95% confidence interval and a precision level set at 3 mm. RESULTS: The TD terminated on the left side in 100% of specimens. Three termination sites of the thoracic duct (TD) were identified: the internal jugular vein (50.0%), the junct

Venoarterial extracorporeal membrane oxygenation in adults with cardiogenic shock.

Cardiogenic shock in adults continues to carry a high mortality despite advances in cardiac care, interventions and critical care. A major advancement in cardiogenic shock care came with the advent of temporary mechanical circulatory support including the intra-aortic balloon pump, ventricular assist devices, and venoarterial extracorporeal membrane oxygenation (VA ECMO). VA ECMO is a type of temporary mechanical circulatory support used in selected patients with cardiogenic shock that have failed to be adequately supported by less invasive approaches including vasoactive medications, volume o

Diagnostic Musculoskeletal Ultrasound in the Evaluation of the Teres Minor.

Rotator cuff injuries are one of the more common injuries that occur in the shoulder. Four muscles and tendons make up the rotator cuff. These include the supraspinatus, infraspinatus, subscapularis, and the teres minor. The teres minor is the smallest of the rotator cuff muscles. It originates from the middle third of the lateral border of the scapula, just below the insertion of the teres major. It inserts onto the inferior facet of the greater tubercle of the humerus. The teres minor is innervated by the axillary nerve, and its muscular action is that of external rotation, and it can be a w

Complex Intratendinous Pectoralis Major Rupture After Heavy Bench Press.

A pectoralis major rupture is a rare injury with important strength and cosmetic consequences, most often described in young, active men after a high-load eccentric contraction during the heavy bench press. Magnetic resonance imaging (MRI) is commonly used to confirm the diagnosis and localize the tear, but the location described on MRI may not fully define tendon continuity, tissue quality, or the tendon available for fixation. We report the case of a 23-year-old male powerlifter who developed acute right chest pain, swelling, ecchymosis, deformity, and pectoralis retraction during a 500-lb b

Implementing Donor Activation Focused Rehabilitation Approach (DAFRA) following nerve transfers for children with brachial plexus birth injury: A case series.

BACKGROUND: Nerve transfers are increasingly used to restore upper limb function in infants and toddlers with brachial plexus birth injuries (BPBI). However, standardized, pediatric-specific rehabilitation protocols remain lacking. Adult-focused approaches are often developmentally inappropriate, and the absence of tailored rehabilitation may contribute to high failure rates, particularly for complex transfers. PURPOSE: To present a structured, milestone-based rehabilitation protocol adapted from the Donor Activation Focused Rehabilitation Approach (DAFRA) for infants and toddlers undergoing

Autologous Nerve Grafting Versus Multiple Nerve Transfers for Upper Trunk Brachial Plexus Injury: A Rat Model Study.

Upper trunk (C5-C6) injuries of the brachial plexus impair shoulder abduction and elbow flexion. For C5-C6 ruptures with preserved proximal stumps, nerve grafting (NG) remains the conventional approach. Nerve transfers (NT) represent the sole viable option in C5-C6 avulsions. The current global standard for NT involves triple transfer procedures: spinal accessory nerve (SAN) to suprascapular nerve(SSN), Oberlin procedure, and Leechavengvongs procedure. Recent clinical trends indicate a broadening of NT indications to include root rupture scenarios, driven by the rationale that distal NT signif

Efficacy and safety of right neck phrenic nerve pacing in patients undergoing Cryoablation for atrial fibrillation.

BACKGROUND: Balloon-based ablation (BBA) is a useful treatment strategy for atrial fibrillation (AF) despite the current trend of decreasing BBA volume due to pulsed-field ablation. However, during BBA for AF of the right-sided (RS) pulmonary veins (PVs) (RS-PVs), constant phrenic nerve (PN) pacing from the superior vena cava (SVC) is required to prevent right PN injury. The PN courses anterior to the scalenus anterior muscle and posterior to the sternocleidomastoid muscle in the neck via a branch of the C3-5 roots. This study determined the efficacy and safety of right PN pacing from the neck

Robotic-Assisted Latissimus Dorsi Transfers Around the Shoulder.

Robotic-assisted surgery has gained prominence in orthopedic practice due to its precision, enhanced visualization, and minimally invasive nature. While robotic applications for orthopedic surgery are well established in arthroplasties, their use in soft-tissue surgery remains limited. This study presents the indications, technique, and clinical outcomes of robotic-assisted latissimus dorsi tendon transfers for shoulder reconstruction. The robotic approach allows accurate identification of the axillary and radial nerves, minimal soft-tissue trauma, reduced bleeding, and excellent cosmetic outc

Quality of Life in Patients following distal nerve transfers for the Restoration of Shoulder abduction and external rotation - Single centre experience.

INTRODUCTION: Restoration of shoulder function is a key objective in brachial plexus surgery, as impaired abduction and external rotation substantially limit upper limb use and quality of life. Distal nerve transfers offer shorter regeneration distances and more predictable outcomes than nerve grafting. RESEARCH QUESTION: This study evaluated the effectiveness of distal nerve transfers in restoring shoulder function, reducing pain, and improving quality of life in patients with peripheral nerve injuries. MATERIAL AND METHODS: A prospective cohort of 36 patients with traumatic brachial plexus

Efficient purification and biochemical characterization of a thermostable and organic solvent-stable α-amylase from Bacillus licheniformis SO7.

UNLABELLED: Upper extremity deep vein thrombosis (UEDVT) is relatively rare, and much less as an initial presentation of systemic lupus erythematosus (SLE). Primary UEDVT should be considered in individuals with unilateral arm swelling where the brachial, axillary, and subclavian veins are frequently involved. SLE is a chronic autoimmune disease that predominantly affects women of childbearing age and of African descent. Patients present with clinical features ranging from arthritis and arthralgias (over 90% of patients with SLE) to life-threatening hematologic, or central nervous system invol

Natural history of Impella 5.5 in patients with cardiogenic shock and small left ventricular cavities.

The Impella 5.5 is an axillary mechanical circulatory support device used in patients with cardiogenic shock, but those with small left ventricular (LV) cavities may be at risk for suction events, hemolysis, malpositioning, hypoperfusion, and LV injury. Informally accepted guidelines, extrapolated from durable LVAD studies, suggest a minimum left ventricular internal dimension in diastole (LVIDd) of 58 mm, with some clinical practices using a more stringent ≤52 mm cutoff. We retrospectively analyzed 154 consecutive patients supported with Impella 5.5 between January 2020 and December 2024. Ove

The medial septum model as a pacemaker of theta rhythm: the key role of short-term synaptic plasticity.

The hippocampal theta rhythm is crucial for memory and navigation. It is paced by parvalbumin-expressing GABAergic neurons in the medial septum. These neurons segregate into two subpopulations-Teevra and Komal cells-which fire in anti-phase bursts at theta frequency, with a stable phase shift of approximately 150 degrees, and exhibit mutual inhibition. While essential, the network mechanism enabling this stable anti-phase synchronization remains unresolved, as previous models relying on intrinsic neuronal adaptation or external rhythmic inputs conflict with experimental evidence. Here, we prop

Variations in the Origin and Communication of the Intercostobrachial Nerve: Surgical Implications.

The intercostobrachial nerve (ICBN) supplies the upper part of the postero-medial region of the arm, lateral thoracic wall, and axilla. It is susceptible to being damaged during various axillary surgeries like axillary lymph node dissection, sentinel lymph node biopsy, and mastectomy because of its diverse origin and branching configuration. Safeguarding the ICBN during axillary surgical procedures can minimise postoperative discomfort and paraesthesia. During standard dissection of the right side axilla and pectoral region of a female cadaver, the ICBN was detected, and its path was followed.

Congenital Anatomical Variations of the Posterior Cord of the Brachial Plexus in Human Fetuses: Branching Patterns and Surgical Implications.

BACKGROUND: The posterior cord of the brachial plexus, formed by the posterior divisions of the upper, middle, and lower trunks (C5-T1), gives rise to the upper subscapular, thoracodorsal, lower subscapular, and axillary nerves and continues as the radial nerve. Variations in its formation and branching are clinically important for regional anesthesia (infraclavicular and axillary blocks), axillary surgery, and nerve-transfer procedures in brachial plexus palsy. Fetal dissection can reveal congenital patterns before postnatal or post-mortem distortion. OBJECTIVE: The single combined objective

Deltoid reanimation after axillary nerve injury: Comparing grafts and nerve transfers outcomes.

BACKGROUND: Loss of deltoid function following axillary nerve (AN) injury or upper brachial plexus injury (BPI) severely limits shoulder function. Both interposition nerve grafting and distal nerve transfer are established reconstructive strategies; however, comparative outcome data remain limited. METHODS: A retrospective single-center study included patients treated between 2006 and 2022 for isolated AN injury or C5-C6 BPI with deltoid paralysis. Exclusion criteria were graft repair performed >8 months after trauma, nerve gaps >7.5 cm, and nerve transfers to the posterior AN branch. Overall
