BACKGROUND: Allograft reconstruction is indicated for chronic pectoralis major tears because such tears will have fibrotic changes, retraction, and stiffness that prevent primary repair1-3. Various reconstruction techniques have been described, particularly in cases in which the myotendinous unit is degenerated. Autografts (such as hamstring tendons and bone-patellar tendon-bone grafts) and allografts (such as Achilles tendon, fascia lata, and dermal grafts) have both been utilized4,5. In the present video article, we describe a technique utilizing a doubled Achilles tendon allograft, which pr

BACKGROUND: Reoperative sternotomy often necessitates peripheral arterial cannulation to initiate systemic or cerebral perfusion. In patients with prior right axillary cannulation, the site can be reused by anastomosing a new Dacron graft at, proximal to, or distal to the original anastomosis. Axillary cannulation provides reliable systemic perfusion while avoiding the embolic risks of femoral access and technical challenges of central cannulation in the redo setting. However, outcomes after redo axillary cannulation are sparsely reported, and the safety of this approach is not well defined.

BACKGROUND: Peripheral nerve regeneration via electrical stimulation on nerve repair has been shown to potentiate axonal growth and functional recovery. However, it remains unknown whether increasing the frequency of electrical stimulation improves, diminishes, or has no effect on the quality of nerve regeneration after repair. Additionally, few animal studies have investigated the potential beneficial effects of serial stimulations on accelerating and improving end-organ functionality over time. We therefore sought to evaluate the additive effect of weekly versus biweekly serial electrical st

PURPOSE: Factors influencing the results of radial nerve to triceps to axillary nerve transfers remain poorly defined. Therefore, the goal of this study is to compare radial to axillary nerve transfer outcomes by age, and to determine if age is a reliable predictor of outcome.
METHODS: A single center retrospective cohort study was conducted between 2015 and 2025. Exclusions included patients younger than 18 years, less than 1 year follow-up, or lack of MRC strength documented after surgery. Patients were subanalyzed by age: 18-39 years, 40-59 years, and ≥60 years. The primary outcome was rec

BACKGROUND: Recovery of shoulder external rotation after upper brachial plexus injury remains limited. This study investigated the anatomical feasibility of selective neurotizations using radial nerve branches to the triceps brachii as donors for the motor branches to the infraspinatus (bIS) and teres minor (bTm) muscles, aiming to enhance external rotation outcomes.
METHODS: Twenty fresh adult cadavers underwent posterior dissections to expose the suprascapular, axillary, and radial nerves. Radial nerve branches to the triceps were identified, measured, and mobilized to simulate transfers to

Major injuries to the infraclavicular or axillary region requiring axillary artery repair or reconstruction, a replantation carried out at the proximal third of the arm, or an infraclavicular brachial plexus palsy with failed nerve reconstruction pose a challenge to a reconstructive surgeon with only the shoulder remaining functional. We observed that patients with these injuries exhibited a 'trick' movement causing elbow flexion by abducting the shoulder beyond 90 degrees, which led us to use the anterior fibers of the deltoid extended with fascia lata graft as a donor to restore elbow flexio

BACKGROUND/OBJECTIVES: Patients suffering from Worker's Compensation (WC) injuries typically demonstrate poorer outcomes despite optimal surgical management. In the occupational setting, shoulder pathology is the most implicated musculoskeletal disease. We recently described the Human Disharmony Loop (HDL), a clinical model of chronic, historically intractable pain of the neck, upper back, shoulder, and arm (Sharma and Friedman in Clin Med 14(5):1769, 2025. https://doi.org/10.3390/jcm14051769 ; Sharma et al. in J Clin Med 14(16):5650, 2025. https://doi.org/10.3390/jcm14165650 ; Friedman et al.

The management of irreparable posterosuperior massive rotator cuff tears remains challenging. With advances in arthroscopic techniques, minimally invasive arthroscopic latissimus dorsi tendon (LDT) transfer has been increasingly used for irreparable posterosuperior massive rotator cuff tears. The present article describes arthroscopic LDT transfer in the lateral decubitus position for irreparable posterosuperior massive rotator cuff tears. The dissection and release of the LDT were performed through the interval between long head of the biceps tendon and humeral shaft. When releasing the LDT,

Neurovascular injuries associated with proximal humerus fractures represent a rare yet clinically significant complication with potential for devastating functional outcomes. Their multifactorial etiology encompasses direct trauma from displaced fracture fragments, indirect stretch or compression mechanisms, and iatrogenic causes during surgical management. The close anatomic relationship between the proximal humerus, axillary artery, and brachial plexus predisposes these structures to combined injury patterns that can threaten limb viability. Diagnosis relies on early recognition through meti

Surgical repair can effectively treat rotator cuff tears (RCTs) and Bankart lesions after shoulder dislocation. Bankart repair has been associated with potential complications such as glenohumeral arthritis, reduced external rotation, and axillary nerve injury. This study evaluated arthroscopic rotator cuff repair (ARCR) outcomes in patients aged ≥ 60 years with RCTs and Bankart lesions after primary shoulder dislocation. Outcomes were compared with patients who had RCTss alone. This retrospective comparative study included patients who underwent ARCR without Bankart repair. Patients were segme

BACKGROUND: Thoracic outlet syndrome (TOS) is a neurovascular condition caused by compression of the brachial plexus or subclavian vessels as they pass through the thoracic outlet. Since the clavicle border the thoracic outlet, injury to the clavicle and potentially post-traumatic callus formation, malunion, or scarring may precipitate compression and TOS. This study aims to determine the incidence and key predictors of TOS after clavicle fracture.
METHODS: We present a representative case description illustrating the clinical course and diagnostic considerations of post-traumatic TOS. We als

BACKGROUND: Certain clinically selected proximal humerus fractures (PHFs) require operative treatment to preserve shoulder function. There is no consensus on the timing of surgery after PHFs, and guidelines are lacking. This study utilized the Delphi method to (1) find consensus on the preferred timing for multiple PHF patterns potentially requiring operative treatment and (2) assess statistical stability of consensus.
METHODS: A three-round electronic modified Delphi study was conducted between May and October 2025. Sixty-one shoulder experts from Australia, Europe, Asia, and North America p

BACKGROUND: Regional anesthesia is increasingly used for volar plate fixation of distal radius fractures and may offer advantages over general anesthesia. However, its association with radiographic reduction quality remains unclear. This study compared regional versus general anesthesia with respect to radiographic reduction quality and early clinical outcomes using an AO/OTA-stratified matched-pair design.
METHODS: This single-center, retrospective study included operatively treated distal radius fractures from January 2016 to December 2024. A strict 1:1 matched-pair cohort compared regional

INTRODUCTION: Point-of-care ultrasound (POCUS) is increasingly used for shoulder dislocation and provides rapid, radiation-free imaging. The posterior glenohumeral disruption view is the most commonly used and best-validated approach, but it can be limited in patients with pain, restricted mobility, large body habitus, or positioning constraints such as supine procedural sedation or spinal precautions. When the posterior window is not accessible, alternative scanning approaches can assist in diagnosis.
OBJECTIVE: This technical review presents a structured framework of ultrasound windows for

Acute traumatic shoulder injuries are often associated with severe pain that hinders examination, radiographic positioning, and timely orthopaedic management. Conventional systemic analgesia may be inadequate, while interscalene and superior trunk blocks can be limited by hemidiaphragmatic paresis. The ultrasound-guided infraspinatus teres minor (ITM) inter-fascial plane block is a recently described diaphragm-sparing technique that targets the suprascapular and axillary nerves through a single posterior injection and has shown promising anatomic and postoperative analgesic coverage in earlier

BACKGROUND: Intra-articular corticosteroid injection in combination with rehabilitation is a prevalent treatment modality for moderate-to-severe shoulder stiffness. However, this approach frequently results in delayed and incomplete recovery. Conversely, arthroscopic capsular release is associated with inherent surgical risks. Manipulation under anaesthesia (MUA) presents a minimally invasive alternative; however, concerns regarding intravenous anaesthesia and iatrogenic rotator cuff injury have served to limit its adoption. This randomised controlled trial aimed to evaluate the hypothesis tha
