Limb Salvage After Grenade-Related Brachial Artery Injury: A Case Report.

Since war injuries are not predictable, each patient presents as a new and unique case. Before deciding on amputation for upper extremity injuries, it is necessary to evaluate all limb-sparing vascular interventions that will keep the amputation level as distal as possible, in order to improve the patient's quality of life and enable the use of more functional prostheses. While amputation may be a faster and more life-saving option for a surgeon, limb-sparing and mobile joint-preserving surgical procedures should be the preferred choice, provided the patient's overall condition permits. Natural history of Impella 5.5 in patients with cardiogenic shock and small left ventricular cavities.

The Impella 5.5 is an axillary mechanical circulatory support device used in patients with cardiogenic shock, but those with small left ventricular (LV) cavities may be at risk for suction events, hemolysis, malpositioning, hypoperfusion, and LV injury. Informally accepted guidelines, extrapolated from durable LVAD studies, suggest a minimum left ventricular internal dimension in diastole (LVIDd) of 58 mm, with some clinical practices using a more stringent ≤52 mm cutoff. We retrospectively analyzed 154 consecutive patients supported with Impella 5.5 between January 2020 and December 2024. Ove

Redo Axillary Artery Cannulation in Reoperative Cardiac Surgery: Outcomes and Technique.

BACKGROUND: Reoperative sternotomy often necessitates peripheral arterial cannulation to initiate systemic or cerebral perfusion. In patients with prior right axillary cannulation, the site can be reused by anastomosing a new Dacron graft at, proximal to, or distal to the original anastomosis. Axillary cannulation provides reliable systemic perfusion while avoiding the embolic risks of femoral access and technical challenges of central cannulation in the redo setting. However, outcomes after redo axillary cannulation are sparsely reported, and the safety of this approach is not well defined.

VIABAHN stent grafts for traumatic arterial injury: A case series.

OBJECTIVE: To describe a single-center experience using VIABAHN stent grafts (W. L. Gore & Associates) for endovascular repair of traumatic arterial injuries. METHODS: We performed a retrospective case series of adults who underwent repair of acute traumatic arterial injury using VIABAHN stent grafts at a single tertiary center from 2014 to 2025. Demographics, injury characteristics, procedural details, and outcomes were collected from the electronic medical record. Outcomes included technical success, limb salvage, complications, reintervention, mortality, and primary patency at last follow-

Anterior Deltoid to Biceps Transfer for Restoration of Elbow Flexion.

Major injuries to the infraclavicular or axillary region requiring axillary artery repair or reconstruction, a replantation carried out at the proximal third of the arm, or an infraclavicular brachial plexus palsy with failed nerve reconstruction pose a challenge to a reconstructive surgeon with only the shoulder remaining functional. We observed that patients with these injuries exhibited a 'trick' movement causing elbow flexion by abducting the shoulder beyond 90 degrees, which led us to use the anterior fibers of the deltoid extended with fascia lata graft as a donor to restore elbow flexio

Arthroscopic Latissimus Dorsi Transfer in the Lateral Decubitus Position for Irreparable Posterosuperior Massive Rotator Cuff Tears.

The management of irreparable posterosuperior massive rotator cuff tears remains challenging. With advances in arthroscopic techniques, minimally invasive arthroscopic latissimus dorsi tendon (LDT) transfer has been increasingly used for irreparable posterosuperior massive rotator cuff tears. The present article describes arthroscopic LDT transfer in the lateral decubitus position for irreparable posterosuperior massive rotator cuff tears. The dissection and release of the LDT were performed through the interval between long head of the biceps tendon and humeral shaft. When releasing the LDT,

Neurovascular injuries associated with proximal humerus fractures: a review of the current literature.

Neurovascular injuries associated with proximal humerus fractures represent a rare yet clinically significant complication with potential for devastating functional outcomes. Their multifactorial etiology encompasses direct trauma from displaced fracture fragments, indirect stretch or compression mechanisms, and iatrogenic causes during surgical management. The close anatomic relationship between the proximal humerus, axillary artery, and brachial plexus predisposes these structures to combined injury patterns that can threaten limb viability. Diagnosis relies on early recognition through meti

Avoiding Iatrogenic Axillary Artery Injury in Reverse Total Shoulder for Fracture: The Use of a Separate Axillary Incision: A Case Report.

CASE: A 71-year-old woman sustained a left proximal humerus fracture dislocation. Imaging demonstrated displacement of the humeral head medial to the axillary artery without initial injury. To avoid iatrogenic injury, a separate medial axillary incision was successfully used to remove the humeral head and then reverse total shoulder arthroplasty was performed through a deltopectoral approach. At 1 year, the patient had excellent range of motion and PROMIS Physical Function CAT (v2.0) score was 46.2. CONCLUSIONS: A separate medial axillary approach can be used to safely remove anterior/inferio

Temporary Cement Prosthesis as a Bridge After Control of Axillary Artery Hemorrhage in Proximal Humeral Fracture-Dislocation: A Case Report.

CASE: A 64-year-old woman underwent open reduction and internal fixation for four-part proximal humeral fracture-dislocation with palpable distal pulses. Upon dislocated head removal, life-threatening hemorrhage occurred from posterior circumflex humeral artery tear. Bleeding was controlled through packing, resuscitation, and open vascular repair. Owing to unavailable shoulder prosthesis and prolonged operative time, a temporary cement prosthesis with Steinmann pins was used. At the 3-year follow-up, partial function was restored with no loosening. CONCLUSION: This case highlights axillary br

Mid-term outcomes of trans-axillary versus thoracotomy approaches in alternative-access tavr: a retrospective multicenter study.

Transcatheter aortic valve replacement (TAVR) has several alternative access routes when transfemoral access is unsuitable. This study compared perioperative and mid-term outcomes of transaxillary (TAx) TAVR with those of transapical (TA) and direct aortic (DA) approaches. Among 2,185 patients who underwent TAVR at our three centers between April 2015 and April 2024, 198 non-transfemoral cases were retrospectively analyzed. TAx-TAVR was classified as a non-thoracotomy approach, whereas TA/DA-TAVR were classified as thoracotomy approaches. Perioperative and mid-term outcomes were compared betwe

Ascending aortic versus axillary artery cannulation in Stanford type A aortic dissection repair: a propensity score-weighted cohort study.

BACKGROUND: Axillary artery cannulation is widely regarded as the preferred arterial inflow strategy for surgical repair of Stanford type A aortic dissection (TAAD). Ascending aortic cannulation has emerged as a practical alternative, yet comparative evidence on early outcomes and long-term survival remains limited. This study aimed to compare the early outcomes and long-term prognosis between these two arterial cannulation strategies. METHODS: We retrospectively analyzed 160 patients who underwent surgery for TAAD at a single center between February 2017 and January 2025. Of these, 64 patien

A Heart Under Pressure: Anesthetic Management of a Large Ascending Aortic Aneurysm Mimicking Mediastinal Mass Physiology.

Large ascending aortic aneurysms (AscAAs), defined as exceeding 8 cm, are exceedingly rare and carry high mortality due to the combined risks of rupture and mediastinal compression. Their management demands precise anesthetic, surgical, and perfusion coordination to prevent hemodynamic collapse and neurologic injury. We report an 83-year-old woman with an 8.9 × 7.7 cm AscAA producing marked right ventricular and pulmonary artery compression. Perioperative management emphasized advanced monitoring, preservation of venous return during induction, and readiness for immediate cardiopulmonary bypas

Endovascular removal of a balloon-expandable uncovered superior mesenteric artery stent.

No defined strategy exists for the endovascular removal of chronically indwelling stents. Any attempt carries risks including extraction failure, stent disruption, arterial injury, and embolization. We report the successful endovascular removal of a balloon-expandable, cobalt-chromium, uncovered stent from the superior mesenteric artery. Using bilateral axillary and unilateral femoral access, the stent was retrieved under continuous control with biopsy forceps, a distal angioplasty balloon, a body-floss wire, and a large-bore femoral sheath. We describe the extraction technique and discuss key

Atraumatic Compartment Syndrome in Patients on Extracorporeal Membrane Oxygenation: A Case Series.

INTRODUCTION: Extracorporeal membrane oxygenation (ECMO) is a lifesaving procedure used to treat cardiopulmonary failure. Atraumatic extremity compartment syndrome is a known complication of limb ischemia related to thrombi from ECMO cannulation, prolonged hypotension, and pressor use. The purpose of this study is to describe a series of patients at a single institution who developed atraumatic compartment syndrome on ECMO. METHODS: A retrospective review was done at a single academic center between January 1, 2005, and October 30, 2023. The inclusion criteria were clinical diagnosis of compa

Does Thoracodorsal Pedicle Dissection Affect Upper-Extremity Lymphatic Drainage? Arm Circumference After Free Latissimus Dorsi Muscle Flap Harvest in Patients Without Axillary Lymphadenectomy.

Background: The free latissimus dorsi (LD) muscle flap remains a workhorse for large soft-tissue defects. Its pedicle, however, must be followed proximally into the axilla-into the immediate vicinity of the lymphatics that drain the arm-and whether this compromises upper-extremity drainage has never been settled. The few studies that examined lymphoedema after LD surgery relied on questionnaires and were confined to breast reconstruction, where axillary clearance and irradiation offer a sufficient explanation for anything they found; the harvest itself has never been isolated. Methods: We iden

Axillary Artery Injury in Proximal Humeral Fractures: Displacement Medial to the Coracoid on X-ray as a Risk Sign of Vascular Injury.

Axillary artery injury is a rare but serious complication of proximal humeral fractures that can threaten both life and limb. The artery lies in close proximity to the glenoid and humerus, making it susceptible to laceration, thrombosis, or pseudoaneurysm during trauma or surgical fixation. Medial translation of the humeral shaft under the coracoid on a radiograph may help surgeons predict arterial injury and prepare accordingly. A retrospective review of cases in a large UK shoulder unit covering a million patients identified five cases over the last three years where proximal humeral fractur

Building an Endovascular Skeleton: Prophylactic Stenting in a Hybrid Approach to a Thymic Carcinoma with Supra-aortic Trunk Encasement.

INTRODUCTION: Locally advanced thymic carcinomas with supra-aortic vessel involvement pose significant surgical challenges and are historically often deemed unresectable. Neoadjuvant radiotherapy further complicates surgical resection by inducing severe tissue fibrosis. The integration of endovascular techniques with open surgery can expand the boundaries of oncological radicality; however, literature on such hybrid approaches for mediastinal tumours remains limited. REPORT: A 45 year old man presented with a 6 × 9 cm locally advanced thymic squamous cell carcinoma causing tracheal compressio

Late radiation-induced cardiovascular disease after childhood rhabdomyosarcoma: a case report.

BACKGROUND: Radiation-induced cardiovascular disease may appear decades after cancer therapy and can involve multiple vascular, valvular, myocardial, and conduction structures of the heart. Management is particularly challenging when prior irradiation results in a hostile chest environment and impaired wound healing. CASE SUMMARY: A 31-year-old woman with a history of childhood right forearm rhabdomyosarcoma treated with resection, chemotherapy, and radiotherapy presented with recurrent syncope. Computed tomography angiography revealed asymmetric growth of the right-sided arteries, leading to

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Different patterns of neurogenic quadrilateral space syndrome: a case series of undefined posterior shoulder pain.

BACKGROUND: Quadrilateral space syndrome is a painful disorder of the shoulder caused by static or dynamic entrapment of the axillary nerve and the posterior humeral circumflex artery. It was first described in 1983; however, it is an uncommon syndrome that initially presents with nonspecific shoulder pain or selective deltoid atrophy, and diagnosis is often delayed owing to its rarity. Young athletes of overhead sports are more commonly affected by this syndrome. Symptoms of quadrilateral space syndrome include silent deltoid atrophy, persistent posterior shoulder pain, paresthesias, and tend
